Objective To explore the correlation between arterial iodine fraction ( AIF) obtained with energy spectrum CT and Child-Pugh staging of liver cirrhosis.Methods Retrospectively analyzed the Child-Turcotte-Pugh of hospitalized 80 patients diagnosed as liver cirrhosis (15 cases of Child -A,40 cases of Child -B,and 25 cases of Child-C) and CT examination results.HD750 spectral three-stage CT scanning was conducted in all patients.AIF was calculated with energy spectrum analysis software,and the correlation between AIF and Child-Pugh staging of liver cirrhosis was explored.Results There was no statistically significant difference in the iodine content of hepatic artery and portal vein among different groups(t=1.786,P>0.05).The hepatic artery iodine score pair-wise analysis among different groups showed that the differences were statistically significant(F=30.45,P<0.05). The hepatic artery iodine score was highest in Child-Pugh C group,secondary in Child-Pugh B group and lowest in Child-Pugh A group.AIF and Child-Pugh staging of liver cirrhosis was significantly correlated.Conclusion AIF of liver cirrhosis reveals the state of liver function and is correlated with Child-Pugh staging,thus can be adopted as a supplement in clinical Child-Pugh staging.

Objective To investigate the response of cryopreserved ovarian tissue after autologous implantation in mouse stimulated with gonadotrophin.Methods Thirty six female mice were randomly divided into three groups,with 12 mice in each group.In group of fresh ovarian tissue,fresh ovarian tissue was implanted into kidney capsule of mice:in group of cryopreserved ovarian tissue,ovarian tissue was implanted into kidney capsule of mice after cryopreserved by vitrification for two weeks.We investigated the response of ovarian tissue two weeks later after autologous implantation stimulated with gonadotrophin.Immunohistochemistry staining method was used to observe the expression of follicle stimulating hormone receptor.Results Before and after stimularian with gonadotrophin,the mature follicle rate of group of fresh ovarian tissue was 2.3%and 4.2%.that of group of cryopreserved ovarian tissue was 2.3%and 4.0%,and that of group of control was 2.6%and 5.8%.Regarding the percentages of mature follicle.there were significant differences after stimulation with gonadotrophin(P＜0.05).After stimulating with gonadotrophin the percentages of mature follicle were the same in the fresh tissue group,cryopreserved tissue group and control group(P＞0.05).The integrated optical density of follicle stimulating hormone receptor of fresh ovarian tissue in antrofollicle and pre-antrofollicle were 9408±2777 and 4531±1903.that of cryopreserved ovarian tissue were 9175±3093 and 4808±1386.and that of the control ovarian tissue were 8838±2064and 5516±1136 respectively.There was no significant difference between any two groups(P＞0.05).Conclusion The follicle stimulating hormone receptor is preserved by cryopreservation and transplantation,small pieces of ovarian tissue response to gonadotropin stimulation is normal.

Objective To observe the imaging features of ultra-wide field short wave fundus autofluorescence (SW-FAF) in eyes with multiple evanescent white dot syndrome (MEWDS),and analysis the correspondence to conventional images.Methods It was a retrospective case series study.Thirteen patients (14 eyes) diagnosed with MEWDS were enrolled.There were 12 females and 1 male,aged from 22 to 57 years,mean age was 34.5 years.All the eyes underwent fundus color photography,optical coherence tomography (OCT) and ultra-wide field autofluorescence (FAF).Simultaneous fundus fluorescein angiography (FFA) and indocyanine green angiography (ICGA) were performed in 6 eyes.The characteristic changes of SW-FAF in studied eyes were observed and compared with the images of FFA and ICGA.All the eyes were followed up every 1 to 2 weeks,with an average of 16.7 weeks.The characteristic images of SW-FAF and corresponding OCT were studied during follow up.Results MEWDS presented with numerous multiple hyper-autofluorescent spots,sized from 50-500 μm,with a vague boundary in ultra-wide field SW-FAF.These spots located mainly at the peripapillary area and the posterior pole with a confluent pattern.The lesions extended to the mid-peripheral retina as well and became more scattered.The distribution of the hyper-autofluorescent lesions in SW-FAF corresponded roughly to that of the greyish-white spots seen in color photograph and the hyper-fluorescent spots detected by FFA.It was consistent with the distribution of hypo-fluorescent spots in late-phase ICGA as well.But the number of the spot showed in FAF is much more than that in FFA,and slightly less than that in ICGA.The OCT scans through the hyper-autofluorescent lesions in SW-FAF showed impairment of outer retina.After the recovery,the hyper-autofluorescent spots disappeared with the outer retina structure repaired completely.Conclusions MEWDS presented with numerous multiple hyper-autofluorescent spots which located mainly at the peripapillary area in ultra-wide field SW-FAF.The distribution of the hyper-autofluorescent lesions in SW-FAF corresponded roughly to color photograph,FFA and ICGA in late-phase.The OCT scans through the hyper-autofluorescent lesions in SW-FAF showed impairment of outer retina.

Objective To observe the clinical efficacy and adverse reactions of docetaxel and carboplatin 21 day regimen (TC)as second -or third -line in treatment of patients with advanced triple -negative breast cancer (ATNBC).Methods 37 ATNBC patients with anthracyclines resistance were given docetaxel 70mg/m2 ,the first day of intravenous drip,carboplatin AUC =5,through intravenous drip on the first day,21 days for a course of treatment, until disease progression or not tolerated or most received six cycles of chemotherapy.Results 147 cycles of chemo-therapy were completed in 37 patients,and the median chemotherapy cycle was 4 cycles.The efficacy and adverse reactions could be evaluated in all patients.Among them,complete remission (CR)in 0 patient,partial remission (PR)in 12 patients (32.43%),stable (SD)in 11 patients (29.73%),and progression (PD)in 14 patients (41.18%).The effective rate(CR +PR)was 32.43%.The disease control rate (CR +PR +SD)was 62.15%,median TTP was 3.9 months (95%CI:4 -8months),and median OS was 10 months.Until the end of follow -up,there were 3 patients with no progress.The main adverse reactions were hematological toxicity (51.53%)and gastrointestinal reaction (56.76%).Conclusion TC regimen is effective and safe in the treatment of patients with ATNBC.

Objective To compare differences of CSF biochemical index in optic myelitis combined with abnormal signal in craniocerebral MRI with no significant abnormalities in brain MRI.Methods The data of 39 cases who meet the diagnostic criteria of Wingerchuck were collected.The brain MRI and cerebrospinal fluid,blood bio-chemical indexes were retrospectively analyzed.Results There were no statistical differences between two groups of patients in onset age,average age,hospitalization days,the number of red blood cells,white blood cell,hemoglobin, cerebrospinal fluid leukocyte and CSF synthetic index,the total number of cells,cerebrospinal fluid IgG,synthetic rate index of sheath,but the index of blood brain barrier(CSF:serum albumin ratio,AR)of NMO patients with craniocere-bral MRI abnormalities was higher than normal group(P <0.05).Conclusion NMO patients with brain abnormali-ties are common,and cerebral MRI with signs of abnormalities shows a higher AR value,speculated that abnormal brain MRI signal in NMO patients is associated with the damage of blood -brain barrier,investigation of NMO brain MRI performance can improve the diagnosis of the NMO and identification with multiple sclerosis.

Objective To investigate the risk factors and prognosis of perioperative myocardial infarction in the patients undergoing noncardiac surgery. Methods Clinical data of 562 patients who had accepted non-cardiac surgery was collected and retrospectively analyzed. The risk factors, treatments and outcomes of all these patients were recorded and analyzed. Results A total of 19 out of the 562 patients had perioperative myocardial infarction ( PMI) . The incidence was 3. 4% . The mean occurrence time was (43. 5 ± 12. 7)h after operation. Eleven PMI patients (11 ∕ 19) were non-ST-segment elevation myocardial infarction and eight patients (8 ∕ 19) were ST-segment elevation myocardial infarction. Thirteen PMI patients were left coronary artery occlusion and six patients were right coronary artery occlusion. Advanced age, history of myocardial infarction, unstable angina, change of ST-T segment on electrocardiography (ECG), multivessel diseases, diabetes,hypertension,and high risk non-cardiac surgery were the risk factors of PMI and positively correlated to PMI. Sixteen PMI (16 ∕ 19) patients accepted PCI treatment and three patients (3 ∕ 19) accepted drug conservative treatment. Two patients had unstable angina attack after treatment and one patient had arrhythmia. The heart function in two patients decreased by one or more than one class within the follow up of 1 year. No patient had recurrent acute myocardial infarction or deceased during follow-up. Conclusions Many factors could lead to PMI. Making preoperative assessment, recognizing patients of high risks and dealing with patients who had PMI in time was necessary.

OBJECTIVETo explore the clinical efficacy of acupoint catgut embedding therapy on chronic pelvic cavity pain syndrome differentiated as kidney deficiency and stagnation of damp heat, and explore the impacts on plasma P substance (SP), plasma beta-endorphin (β-EP).

METHODSOne hundred and eighty cases were randomly divided into a catgut embedding group (90 cases) and a western medication group (90 cases). In the western medication group, tamsulosin capsules 0. 2 mg were prescribed for oral administration, once a day; indometacin sustained release tablets, 25 mg, three times a day. Totally, the oral administration for 8 weeks was required. In the catgut embedding group, the acupoint catgut embedding therapy was applied to Qugu (CV 2), Shenshu (BL 23), Zhibian (BL 54), Huiyin (CV 1) and Sanyinjiao (SP 6), once every two weeks; the treatment of 4 weeks made one session, and two sessions were required. Before and after treatment, TCM symptom score, NIH-CPSI (the National Institute of Health Chronic Prostatitis Symptom Index) score, lecithin body numbers in prostatic fluid, score in SAS (self-rating anxiety scale), score in SDS (self-rating depression scale), the levels of SP and β-EP, etc. were observed in the two groups, and the clinical efficacy was assessed in the two groups.

RESULTS(1) Ten cases were dropped in either group. The total effective rate was 91. 25% (73/80) in the catgut embedding group, higher than 78. 75% (63/80) in the western medication group (P<0. 05). (2) After treatment, TCM symptom score, total score in NIH-CPSI, pain score and the scores in SAS and SDS were all reduced as compared with those before treatment in the two groups (all P<0. 05). After treatment, TCM symptom score, total score and pain score in NIH-CPSI, and the scores in SAS and SDS in the catgut embedding group were both lower than those in the western medication group (all P<0. 05). (3) After treatment, the lecithin body numbers were both increased as compared with those before treatment in the two groups (both P<0. 05), and the result in the catgut embedding group was higher than that in the western medication group (P<0. 05). (4)After treatment, the SP level was lower than that before treatment in the two groups (both P<0. 05); the level of p-EP was increased as compared with that before treatment (both P<0. 05). The SP level in the catgut embedding group was lower than that in the western medication group (P<0. 05); the level of β-EP was higher than that in the western medication group (P<0. 05).

CONCLUSIONThe acupoint catgut embedding therapy apparently relieves the clinical symptoms of chronic pelvic cavity pain syndrome differentiated as kidney deficiency and stagnation of damp heat as well as the condition of anxiety and depression, increases lecithin body numbers in prostatic fluid and β-EP level and reduces SP level.

Objective To evaluate the potential association of CD4+CD25+ T cells with alopecia areata.Methods Totally,this study enrolled 23 patients with progressive alopecia areata,25 patients with stable alopecia areata,and 25 healthy controls.Peripheral blood was isolated from these subjects followed by isolation of CD4+ CD25+ regulatory T cells,which were then cuhured with the presence of anti-CD3 and-CD28 monoclonal antibodies for four days.Subsequently,enzyme-linked immunosorbent assay was performed to measure the levels of interleukin (IL)-10 and transforming growth factor (TGF)-β1 in the culture supematant of these T cells.Results The levels of IL-10 and TGF-β1 were (31.68 ± 6.78) pg/ml and (32.29 ± 6.8) pg/ml respectively in the culture supernatant of CD4+CD25+ regulatory T cells from patients with progressive alopecia areata,significantly lower than those from the healthy controls ((57.34 ± 14.15) pg/ml and (57.43 ± 15.16) pg/ml,both P ＜ 0.05) and patients with stable alopecia areata ((52.56 ± 13.02) pg/ml and (61.75 ± 14.10) pg/ml,both P ＜ 0.05).However,no significant difference was observed in the supernatant levels of IL-10 or TGF-β1 between the healthy controls and patients with stable alopecia areata.Conclusions The secretion of IL-10 and TGF-β1 by CD4+CD25+ T cells is decreased in patients with progressive alopecia areata,which may contribute to the pathogenesis of alopecia areata.
