1.Effect of Octreotide on prophylaxis of post ERCP pancreatitis and hyperamylasemia: a multicenter, randomized clinical trial

Objective To study on the efficacy of Octreotide prophylaxis of post ERCP pancreatitis (PEP) and hyperamylasemia. Methods The study was conducted in 12 digestive endoscopic units in China. Patients were randomized into two groups. Octreotide group: Octreotide (0. 3mg) were dissolved in 500 ml of 0.9% saline solution and administrated by continues intravenous infusion, beginning 1 hr before the endoscopic examination and continuing for 6 hr afterward, 0. 1 mg Octreotide were injected subcutaneously at 6h, 12h after the intravenous injection stopped. Control group was given a placebo (saline solution) intravenously without subcutaneous injection. Results A total of 961 patients were accepted in the study, 832 patients were enrolled in the final analysis, Octreotide group 414 cases, and control group 418 cases. The overall incidence of acute pancreatitis was 3.85% (32/832) .which includec 2.42% (10/414) in Octreotide group and 5. 26% (22/418) in control group (P =0. 046). Incidence of hyperamylasemia was 14. 9% (124/ 832) which included 12. 32% (51/414) in Octreotide group and 17. 46% (73/418) in controlled group (P = 0. 041). The two groups were matched in many basic aspects, such as sex, age, contrast agent, indication of ERCP, the times of visualization of pancreatic duct and bile duct, etc. There was no side effect associated with Octreotide found. Conclusion The results of this trial indicate that Octreotide can prevent post ERCP pancreatitis and hyperamylasemia.
2.ANTI-INFLAMMATORY EFFECT OF TOTAL SAPONINS OF ROOT OF PANAX NOTOGINSENG ( TSPR )

The anti-inflammatory effect of total saponins of Panax notoginseng (TSPR ) were reported. After intramuscular injection of TSPR (200 mg/kg, rats and mice) and hydrocortisone ( 300mg/kg, rats ; 200mg/ kg, mice) the ear inflammation induced by croton oil decreased. Thirty minutes after intramuscular injection of TSPR ( 200mg/kg ) and hydrocortisone ( 200mg/kg ) in normal mice, the inflammation induced by acetici ( ip) shown by the infiltration of Even's blue decreased significantly.It is suggested antiinflammatory effect of TSPR was not related to the hypothalamic adrenal system.In chronic inflammatory experiments, TSPR as well as hydrocortisone could inhibit the proliferation of granuloma by the implantation of cotton pellet in rats and mice with inhibitory rates of 27.6% ( P
3.Control Hospital Infection to Realizet Potency Feeling among Malignant Tumor Patients

OBJECTIVE To strengthen the hospital infection control to realize the potency feeling among malignant tumor patients under the conduct of psychological nurses and other medical staff.METHODS The related hospital infection control system and the psychological countermeasure of nursing were formulated,we used two groups of comparisons methods around implements,counting and analyzing the effect of the hospital infection control among malignant tumor patients.RESULTS The psychologically healthy instruction enabled the patient positively to coordinate the treatment,the ratio of hospital infection from the original 21.33% dropped to 15.41%,and make the certain dynamic function to the self-realization of the patients' potency feeling.CONCLUSIONS The hospital infection control is an essential and important link to the nursing of the malignant tumor patients;the psychologically healthy instruction is an effective way to promote the self-realization of patients potency feelings.

5.Expression of urotensinⅡreceptor GPR14 in cardiovasculature and brain of rats

Objective: To observe the expression of the G-protein-coupled-receptor 14 (GPR14), urotensinⅡreceptor, in the cardiovascular system and brain of SD rats. Methods: Semi-quantitative reverse transcription-polymerase chain reaction (RT-PCR) was used to detect the GPR14 mRNA. Results: In cardiovascular system, GPR14 mRNA was detected in the left ventricle, left atrium, thoratic aorta and carotid aorta. The highest level of expression was found in the left ventricle. In the brain, GPR14 mRNA was detected in cortex, hippocampus, hypothalamus and cerebellum, and higher level of expression was found in the cerebellum. Conclusion: GPR14 mRNA expression is found in the cardiovascular and neural tissues of tested rat, suggesting that urotensinⅡ may play an important role in cardiovasculature and central nervous activity.
6.Control study of Beraprost Sodium and atorvastatin in treatment with TIA combined carotid plaques

Objective To investigate the effect of Beraprost Sodium and atorvastatin in the treatment of TIA combined carotid plaques. Method 60 cases in our hospital with TIA and carotid artery plaques were randomly divided into observation group and control group, 30 cases in each group. The observation group was received beraprost natriuretic peptide and atorvastatin calcium therapy, the control group was treated with atorvastatin calcium. 12 months later,two groups were compared with carotid plaque area change and coagulation conditions. Results Carotid plaque area in observed group was significantly less than the control group (P<0.05). The differences of platelet agglutination test(PAgT), fibrinogen(Fg) , hypersensieive 3 C-reaction protein, total cholesterol(TC), high density lipoprotein cholesterol (HDL), low density lipoprotein cholesterol (LDL), TIA recurrence rate and incidence of ischemic stroke between two groups after treatment were statistically significant (P<0.05). Conclusion Beraprost Sodium and atorvastatin has a good effect in reducing carotid plaques area, adjusting blood fat and preventing TIA and ischemic stroke, It is worthy to clinical popularization and application.

Background and objective Pre-operative assessment of mitral valve(MV)anatomy is essential to surgical design in patientsundergoing MV repair.Although 2-dimensional(2D)echocardiography provides precise information regarding MV anatomy,RT-3DTEE could increase the understanding of MV apparatus and individual scallop identification.We aimed to investigate the value of RT-3DTEE in MV repair. Methods RT-3DTEE was performed in six patients with mitral valve prolapse(MVP) by using Philips IE33with X7-2t probe.Preoperative RT-3DTEE studies were compared with surgical findings in patients undergoing surgical mitral valverepair,and quantitative evaluation was performed by QLab 6.0 software before and after surgicalmitralvalve repair.Results RT-3DTEE could display dynamic morphology of MV,the location of prolapse,and spatial relation to the surrounding tissue.It couldprovide surgical views of the valves and the valvular apparatus.These resuIts were consistent with surgical findings.The quantitativeevahuation before and after surgical MV repair indicated that anterolateral to posteromedial diameter of annalus.anterior to posteriordiameter of annulus,perimeter of annullus.,and area of annalus in projectionplane were significantlv smaller after operation comparedwith those before operation(P<0.05).The length of posterior leaflet,,the area of anterior and posterior leaflet,the maximal prolapseheight,the volume of leaflet prolapse and the length of coaptation in projection planewere significantly reduced after operation(P<0.05).Conclusion RT-3DTEE is a unique new medality for rapid and accurate evaluation of mitral valve prolapse and miwal valverepair.
9.Analysis of hospitalization costs of patients with diabetic foot ulcer and influencing factors

Objective To explore the hospitalization costs of patients with diabetic foot ulcer and analyze key influencing factors.Methods A total of 570 diabetic foot inpatients were recruited from January 2008 to March 2012 for analysis of the relationship between their hospitalization costs and clinical symptoms.The multifactor regression analysis model was used to identify main influencing factors for their hospitalization costs.Results The medical costs of such inpatients amounted to 1 9 825.6 yuan (12 476.6~32 745.9)yuan,while average hospitalization costs of the 3 401 diabetic patients admitted over the same period were but 9 757.1 yuan (6 282.1 ~ 1 5 723.7 )yuan,a difference of statistic significance.The single-factor analysis found statistical differences from the differences of hospitalization among various age,occupation,dorsal artery of foot,B ultrasound of lower extremity arteries,wagner classification,amputation,transfer,leucocyte,hemoglobin,serum albumin,serum creatinine and high density lipoprotein.Multifactor analysis found that the significant factors influencing the medical costs of inpatients were days of stay,albumin,wagner classification,dorsal artery of foot,leucocyte and age. Conclusion Hypoalbuminemia,high stage of wagner classification,weak or absent pulsation of foot dorsal artery,severe infection and aging were significant factors contributing to higher medical costs of hospitalization for patients with diabetic foot ulcer.
10.Clinic features and echocardiographic evaluation of patients with idopathic submitral left ventricular aneurysm

Objective To study cinical features of patients with idopathic submitral left ventricular aneurysm(ISLVA) and evaluate their global and segmental systolic function as well as diastolic function through echocardiography.Methods Clinical features of eight patients with ISLVA were analysed retrospectively.Standard 2-dimentional and real-time 3-dimentional echocardiography were performed in all the eight cases and other twenty subjects with normal left ventricular(LV) function (defined as control group).Results Two patients were diagnosed as ISLVA due to ventricular arrythmia and the other six case sowing to congestive heart failure.Coronary angiography was normal in all patients.Apical systolic murmur was audible in 6 cases.Four patients received mitral valve surgical repair,one underwent aneurysm resection and radiofrequency ablation,implantable cardioverter defibrillator was implanted in one case,one was only treated by medication.One case died.Patients with ISLVA demonstrated significanly larger left atrium(LA) and LV diameter (both end-diastolic and end systolic),thinner LV posterior wall,and lower LV ejection fraction (LVEF) than controls (P <0.05).Indexes of the LV 17 segments time-volume curves including the time to minimal systolic volume(Tmsv) 16-SD,Tmsv 12 SD,Tmsv 6-SD,Tmsv 16-Dif,Tmsv 12-Dif,Tmsv 6-Dif,Tmsv 16-SD%,Tmsv 12-SD%,Tmsv 6-SD%,Tmsv 16-Dif%,Tmsv 12-Dif%,Tmsv 6-Dif% were significantly higher in patients with ISLVA than those in controls(all P <0.05).All patients with ISLVA showed mitral regurgitation and decreased LV diastolic function in varying degrees,five patients accompanied by elevated LA pressure.Conclusions Clinical features of patients with ISLVA are nonspecific.Echocardiography can evaluate systematically their functional and structural abnormalities.
