1.Detection of myocardial coronary flow reserve of syndrome X by real-time myocardial contrast echocardiography

Objective To evaluate myocardial coronary flow reserve of syndrom X by real-time myocardial contrast echocardiography(MCE). Methods Ten patients with syndrome X and seven normal subjects were involved in the study. Real-time MCE was performed with acoustic contrast SonoVue,and the peak video density (A), and re-turgor velocity of microvessel (β), and the product of A ×β of quiescent condition and after adenosin loading were detected,and also the coronary flow reserve (CFR, the ratio of A xβ circa-adenosin loading). Results There was no significant difference of A between syndrom X and contrast group in quiescent condition, the β and product of A x β of patients with syndrome X were lower than those of contrast group, the CFR of syndrome X was obviously less than that of contrast group.Conclusions Myocardial microvessel function is abnormal in syndrome X, real-time MCE is useful for evaluate myocardial coronary flow reserve.
2.Effect of carbon dioxide pneumoperitoneum on serum levels of IL-6, E-selectin and P-selectin and pancreatic pathology in severe acute pancreatitis

Objective To examine the effect of carbon dioxide pneumoperitoneum on local pancreas pathological changes, serum levels of amylase, IL-6, E-selectin and P-selectin in rats with severe acute pancreatitis (SAP). Methods Fifty healthy male SD rats were randomly divided into 3 groups: CO2 experiment group ( n =20): SAP was induced by injecting 5% sodium taurocholate through retrogradely common biliopancreatic ducts via duodenal papilla,and then CO2 pneumoperitoneum was established at a pressure of 12 mmHg for 30 min; SAP group ( n = 20): The rats were treated as same as CO2 experiment group, excepting CO2 pneumoperitoneum; Control group (n = 10): Abdomen was incised and nothing was done to duodenum and pancreas except for moving them softly. The blood samples were gathered for examining serum levels of amylase, IL-6, E-selectin and P-selectin, and histopathologic examination of pancreas was done.Results Compared with control group, the pancreatic pathologic histology score, serum levels of amylase,IL-6, E-selectin and P-selectin were significantly higher in the CO2 experiment group and SAP group (P =0.000). The levels of IL-6 were significantly lower in CO2 experiment group compared to SAP group (P =0. 000). There was no significant difference between CO2 experiment group and SAP group in pancreatic pathologic histology score( P =- 0.294), the levels of serum amylase ( P = 0.073 ), E-selectin and P-selectin. Conclusions CO2 pneumoperitoneum has no adverse effect on the changes of pathological histopathology and the levels of IL-6, E-selectin and P-selectin in SD rats with SAP.

Huazhong University of Science and Technology is one of the five schools that trial implementing eight-year program medical education of first batch.Eight-year program medical students in Tongji college came into obstetrics and gynecology clinical contact in 2010.According to the characteristics of students and discipline,obstetrics and gynecology department used multiple teaching methods and means as PBL teaching,integrated case management,typical clinical case conference,clinical diagnosis and treatment,academic lecture,research report,experimental design,scientific research and experi-ment,etc.Available practice mode was reformed.Those students accomplished clinical practice,and achieved expected effect.
5.Effect of positive end expiratory pressure on pulmonary function of one-lung ventilation patient during perioperation

Objective To explore the effect of positive end expiratory pressure (PEEP) on arterial oxygenation and intrapulmonary shunt during one-lung ventilation (OLV) and pulmonary function during perioperation. Methods Forty patients with normal pulmonary function,ASA I - II :scheduled for pulmonary lobectomy, were divided into control group and PEEP group by random digits table with 20 cases each. Patients were induced by double-lumen tubes under intravenous anesthesia and were received 10 ml/kg tidal volume, 12 frequents/min breathing rate during the two-lung ventilation (TLV), secondary reduced to 6 ml/kg tidal volume, 16-18 frequents/min breathing rate without PEEP (control group) or with 5 cm H2O cm H2O =0.098 kPa) PEEP (PEEP group) during OLV.Hemodynamics and respiratory mechanical parameters were continuously monitored, lung function before operation and at 72 h after operation was detected. Results Compared to before OLV,arterial oxygen tension (PaO2), arterial oxygen saturation (SpO2), oxygenation index (OI) were decreased and intrapulmonary shunt ratio (Qs/Qt) was increased in control group and PEEP group at 30 min after OLV (P < 0.01 or < 0.05). However,PaO2 and SpO2 and OI were higher and Qs/Qt was lower in PEEP group than that in control group at the same time point (P<0.05). In addition, FEV1%, FVC% and FEV1/FVC were (121.8 ± 25.0% ,(117.2 ± 24.3)% , (87.6 ± 15.7)%before operation and (84.9 ± 21.6)%, (77.2 ± 18.3)% , (70.5 ± 12.5)% at 72 h after operation respectively in control group, (116.9 ±24.5)% , (112.1 ±23.6)% , (85.3 ± 13.8)% before operation and (96.3 ± 20.4)%, (88.1 ± 19.8)% , (78.4 ± 10.2)% at 72 h after operation respectively in PEEP group. Although decreased in control group and PEEP group at 72 h after operation comparing with preoperation (P< 0.01 or < 0.05 ), FEV1%, FVC% and FEV1/FVC were higher in PEEP group than those in control group at 72 h after operation (P<0.05). Conclusion Appropriate PEEP increases arterial oxygenation,reduces Qs/Qt and improves pulmonary function during OLV,reduces the risk of hypoxernia and lung injury induced by OLV during perioperation.

Objective To discuss the relationship between the substype of peripheral blood CD8 lymphocyte in patients with chronic hepatitis virus B and clinic condition and the replication of HBV. Methods CD45RO, CD45RA and CD28 of peripheral blood CD8 lymphocyte were detected by flow cytometry with multicolor fluorescence technology in 107 patients with chronic hepatitis virus B and 20 healthy volunteers. Results The total quantity of CD8 lymphocyte in patientswith chronic hepatitis virus B was not different from that in controls. Objective On the basis of the investigation and test on professionals and technologists,to put forward the allotment-related encouragement mechanism.correlative principals and measures according to the above-mentioned indexes.to provides policies and measures for the questions generally reflected by professionals and technologists.and to establishes the allotment-related encouragement mechanism with reference of social contribution.Methotis 300 professionals and technologists of provincial academies (institutes) of medical research were surveyed by the questionnaire about following items including:fundomental personal information comprehensive strength of institute:advices of the innovative talent cultivation and soon.The data of the questioaire survey were analyzed and summarized.The valid suggestions were proposed.Results In the provincial academies(institutes)of medical research,57.9% of the professionals and technologists investigated hope to improve their present salary.The anuual income of a research is 30 000 Yuan.which is only one third of the ideal income of 90 200 yuan.Conclusion It is supposed to take regular examination and dynamic management of the professionals and technologists.and then to give performance-related salary treatment ia light of difierent examining results.
8.Diagnostic value of digital subtraction CT angiography in small intracranial aneurysms

Objective To evaluate the value of digital subtraction computed tomography angiography(DSCTA)in the diagnosis of intracranial small aneurysms.Methods 92 patients with intracranial aneurysms confirmed by digital subtraction angiography(DSA) or surgery were selected as the subjects of this study.Their clinical and CT imaging data were analyzed retrospectively.The diagnos-tic accuracy of DSCTA and conventional CTA for intracranial aneurysms was compared.Results A total of 102 aneurysms were con-firmed in the 92 patients,100 of them were detected by DSCTA and 89 by conventional CTA (χ2 =8.707,P =0.003).Two small aneurysms of less than 5.0 mm located respectively at the cavernous sinus and beneath the clinoid segments were missed by DSCTA, while 13 by conventional CTA.When aneurysms of less than 3.0 mm and 3.0-5.0 mm were combined with calculation,the sensi-tivity of DSCTA in detecting aneurysms of less than 5.0 mm was significantly higher than that of conventional CTA (χ2 =8.393,P =0.004).Conclusion DSCTA has more advantages than conventional CTA in the diagnosis of intracranial aneurysms,especially for the small aneurysms adjacent to the skull base.DSCTA can be used as a preferred technique in the screening and diagnosis of in-tracranial aneurysms.
9.Changes of endothelin and atrial natriuretic peptide in rats of chronic hypoxic pulmonary hypertension

To observe the changes of endothelin(ET) and atrial natriuretic peptide(ANP) lev-els in chronic hypoxic pulmonary hypertensive rats and discuss their mechanism. Methods: We duplicatedthe models of chronic hyp0xic pulmonary hypertensive rats and tested the concentrations of ET and ANP ofplasma and cardiac tissue by radioimmunoassey. Results: In pulmonary hypertension group, plasma ET,plasma ANP and myocardial ET levels increased more significantly than those of controls,except that my-ocardial ANP concentration decreased. There were positive c0rrelati0ns between plasma ET level and plas-ma ANP level and between plasma ET level and myocardial ET level, but a negative correlation betweenplasma ANP level and myocardial ANP level. C0nclusion: It is suggested that the synthesis and secretionof ET and ANP increase in pulmonary hypertension,and therefore provides reference to clinical diagnosisand therapy.
10.Chemical constituents from stems and leaves of Micromelum integerrimum.

A new benzene derivative microintegerrin C (1) and a new norsesquiterpenoid microintegerrin D (2), along with six known compounds (3-8), were isolated and identified from stems and leaves of Micromelum integerrimum by various chromatographies such as silica gel, Sephadex LH-20, RP-18 column chromatography and HPLC. Their structures were mainly identified based on the spectral data analysis such as 1D-, 2D-NMR and HR-EI-MS. All known compounds were isolated from this plant for the first time.
