High resolution melting ( HRM) analysis is a real-time PCR based technique which can detect DNA sequence polymorphism .Immediately after routine PCR with saturated fluorescent dye , melting transition of the amplicon was monitored and fluorescence was collected .According to the different melting curve, single base differences can be distinguished .HRM is a low -cost, single -step, closed -tube, accurate and rapid method , which has aroused general concern .Rapid and sensitive detection of the clinical pathogens is critical to diagnosis of infectious diseases .However , clinical research on application of HRM in infectious disease pathogen identification and drug resistance , especially melting curve analysis based bedside technology integration will provide strong support for the diagnosis and treatment of infectious diseases.

Objective Toinvestigatetheeffectoffolicacidonplasmahomocysteine(Hcy)levels and vascular endothelial function in elderly cerebral infarction patients with hyperhomocysteinemia. Methods Thisstudywasaprospectivecontrolledtrial.Atotalof112elderlylacunarinfarction patients with high hyperhomocysteinemia admitted to the Department of Neurology,Nanjing Municipal Government Hospital from November 2012 to October 2013 were enrolled. The patients were divided into two groups according to the method of random number table. The patients of both groups received the conventional drug treatment of ischemic stroke at the same time. Fifty-six patients were in group A,and they were treated with folic acid 20 mg,once a day;56 patients were in group B,and they did not treat with folic acid. The Hcy levels on admission and after 24 weeks treatment were measured. Brachial artery flow-mediated endothelium-dependent dilation (FMD)was measured by high resolution ultrasound,and nitric oxide (NO), nitricoxidesynthase(NOS),andendothelin(ET)weredetectedatthesametime.Results Twenty-four weeks after treatment,the plasma Hcy level in group A was significantly decreased as compared with prior treatment (12. 2 ± 4. 2 vs. 22. 6 ± 3. 7μmol/L;P<0. 05),moreover,it was lower than the level of group B at the same period (12. 2 ± 4. 2 vs. 21. 8 ± 4. 0μmol/L;P <0. 05 ). The FMD level of group A was significantly higher than that of prior treatment and group B (11. 2 ± 3. 6 vs. 9. 2 ± 3. 3,9. 3 ± 3. 3;P<0. 05). The NO and NOS levels of group A at 24 weeks after treatment were significantly higher than those of prior treatment and group B at the same period (76. 2 ± 9. 4 vs. 49. 2 ± 9. 0,50. 0 ± 9. 1μmol/L,47. 6 ± 9. 5 vs.38. 4 ± 7. 5,37. 8 ± 8. 8 kU/L;P<0.05). The ET level of group A was decreased as compared with that of prior treatment and group B at the same period (81. 3 ± 20. 5 vs. 105. 6 ± 25. 2,105. 3 ± 21. 8 ng/L;P<0.05).NoadversereactionwasfoundduringthetreatmentingroupA.Conclusion Alargerdoseof folic acid can significantly reduce the plasma Hcy level,decrease the FMD level,and ameliorate vascular endothelial function in elderly cerebral infarction patients with hyperhomocysteinemia.

Purpose To compare the wireless flat-panel detector with gadolinium oxide coating in X-ray photography in the small joints of the limbs compared with conventional flat panel detector with cesium iodide coating. Materials and Methods Comparison was randomly performed between gadolinium oxide coated sulfur wireless flat-panel detector and cesium iodide coated conventional flat-panel detector for 80 patients who received routine X-ray photography, and scoring data were applied with statistical analysis. Results There was no significant difference between the scoring data from two film-reading people (P>0.05). The resolution of anatomical structure of small joint of limbs and tube current showed no significant difference for two types of imaging systems (P>0.05). Conclusion Wireless flat panel detector sulfur with gadolinium oxide coatings can obtain satisfactory image quality at a reasonable inspection doses in the X-ray photography of small joint of limbs. The imaging quality and inspection doses are similar to conventional flat panel detector device with cesium iodide coating, but the posture is more convenient.

OBJECTIVE:To optimize the formula of piribedil hydrochloride orally disintegrating tablets and to investigate their related indexes.METHODS:Taking the contents of crospovidone(PVP XL)(disintegrating agent),amylum pregelatinisatum(loading agent) and Gum Acacia(glidant) as factors,the disintegrating time(td),the wet time(t) and the suspend stability(?A) as the evaluate indexes to carry on the orthogonal experimental to optimize the formula.The dissolution curve of the optimized formula was drawn.RESULTS:The optimized formula for the piribedil hydrochloride orally disintegrating tablets were as follows:the contents of crospovidone(PVP XL),amylum pregelatinisatum and Gum Acacia were 15%,20%,and 15% respectively.The prepared orally disintegrating tablets disintegrated completely within 30s and dissolved basically within 2min.CONCLUSION:The prepared preparation can meet the related standards specified of China Pharmacopeia.

Objective To study the treatment and control status of patients with diabetes in urban communities in our country. Method structured questionnaire survey with physical examination were conducted by well-trained investigator, the participants were patient with diabetes diagnosed at least one year ago in four communities from four cities of China. Result 1047 adults with diabetes were investigated, predominantly type 2 diabetes. 50.9% of participants had hypertension, 32.7% of participants had hyperlipidemia. 70.8% of patients were treated with oral medication alone, 10.2% with insulin alone, 6.3% with oral medication plus insulin, and 9.8% without any medication. So, total about 16.5% of patients were treated with insulin. The well-performed proportion of medication, diet, physical activity, self-management education and blood sugar monitoring were 81.2%, 69.4%, 80.6%, 69.8% and 66.9%, respectively. Participants who well-practised all the five treatment strategies accounted for 34.9% of all participants. Out of all participants, 63% participants had poor FBG control and 50% had poor BP control. Conclusion Most patients with diabetes in urban community receive nonstandard treatment and most of them had poor FBG control and BP control.

Objective To investigate the survival situation of thalassemia cases treated with Bushen Yisui Method. Methods The survival situation of 197 cases were investigated by telephone. Results The total survival rate of 197 cases is 87.3%, among which the thalassemia major is 65.71%, the thalassemia intermedia is 99.2%, the ?-thalassemia is 100%, the ?-thalassemia is 85.0%. 158 of 197 cases have been analyzed genotype, among which 63.16% is the survival rate of homozygote, 89.16% of the heterogenic duplex heterozygote, 100% of the heterozygote. OBJECTIVES:The effect of water-soluble polymers polyvinylpyrrolidone(PVP) and polyethylene glycol(PEG)on the complexation of drugs with β-cyclodextrin were investigated.METHODS:the thermodynamic properties of drugs in aqueous β-CD solution and in aqueous solut.RESULTS:The maximum solubilization concentration of PVP or PEG appeared from 1.5g.L－1 to 3.0g.L－1.At this low concentration,PVP or PEG had insignificant effect on the viscosity of the aqueous β-CD solution.The solubilising effect ofβ-CD on chloramphenicol was improved by 15% when containing 2.5g.L－1 polyethylene glycol in the solution and on vitamininum B2by 98%when containing 2.5g.L－1 polyvinylpyrrolidone.In aqueous solutions water-soluble polymers were shown to increase the solubilising effect of β-CD on drugs.Additing of PEG or PVP to the aqueous complexation medium resulted in an increased negative enthalpy change,together with an increased negative entropy change.CONCLUSION：Water-soluble polymers were shown to increase the solubilising effect of β-CD on drugs.

Microfluidic chip exhibit a great promising development in clinical diagnosis and disease screening due to their advantages of precise controlling of fluid flow,requirement of mini amount sample,rapid reaction speed and convenient integration.A lot of demonstrations on the diagnostic applications related to genes,proteins,and cells have been reported because of their advantages associated with miniaturization and automation.Here,the applications and developments of on-chip nucleic acid amplification and analysis,protein analysis and detection,cell selection and cell drug screening were discussed.Microfluidic chip can provide an easy integration platform for biomarkers in a high throughput and accurate detection.
