1.Pathogen Detection in Food Poison Episodes

OBJECTIVE TO improve the detection leval of bacterial pathogens caused food poison episodes. METHODS The baterial pathogens were cultured and identified in two food posison episodes.The antibiotic susceptibiity test was undertaken,retrospetirely. RESULTS Five enterotoxin-producing S.aureus strains were isolated from 55 samples collected in one event,and five enterotoxin-producing salmonella enteritidis strains were isolated from 66 samples collected in another event. CONCLUSIONS The two outbreak events are caused by bacteria contaminated foods.
2.Assessment of Attention Deficit Following Brain Injury

ObjectiveTo evaluate the attentional ability following brain injury.Methods26 patients with brain injury including traumatic brain injury(TBI) and stroke, and 30 normal volunteers were randomly selected. Different attention tests(Continuous Performance Test, Stroop Color-Word Interference Test, Trail Making Test and Paced Auditory Serial Addition Task) were applied to assess the attentional ability of the patients and normal volunteesrs. Then the data were sorted out before statistical analysis was performed.ResultsThere was significant difference between the normal and brain injury groups in scores(P<0.05). There was no significant difference between TBI and stroke in scores(P>0.05).ConclusionThe multiple aspects of attention deficit existed in the patients with brain injury, including sustained attention, selective attention, divided attention and attentional switching. Clinical manifestations of attention deficit after traumatic brain injury and stroke is closely related with the injury site.

This paper briefly introduces the structure and function of α7nAChR, and mainly reviews the relevant researches about the relationship between a7nAChR and cognition of schizophrenia in the last ten years from molecule genetics and new targets of medication two aspects. It suggests that the abnormal α7AChR is involved in cognition impairment of schizophrenia, and the a7nAChR agonist is probably able to improve the impaired cognition in patients with schizophrenia. Furthermore, KAT-2 as a key enzyme acting on its endogenous antagonist KYNA is also promising to become a new drug target
4.Life quality of patients after cholecystectomy.

OBJECTIVETo evaluate the quality of life in patients who underwent laparoscopic and open cholecystectomy for chronic cholecystolithiasis.
METHODSA prospective survey was made on 25 patients receiving laparoscopic cholecystectomy (LC group) and 26 patients receiving open cholecystectomy (OC group). The quality of life was measured with the gastrointestinal quality of life index (GLQI) preoperatively, and at 2, 5, 10 and 16 weeks after the operation.
RESULTSThe mean preoperative GLQI scores of all dimensions of the quality of life were 112.5 and 110.3 in the LC and OC groups respectively. In the LC group, the quality of life was not considerably reduced at 2 weeks after operation, with a mean GLQI score of 110. There was a significant improvement both in total mean score and in the aspects of symptomatology, emotional and physiological status from 5 to 16 weeks after LC operation. In the OC group, the GLQI score reduced to 102.0 at 2 weeks after surgery (P < 0.05). There were significant reductions in the aspects of symptomatology, physiological and social status as well. The GLQI scores reached to the preoperative level of 115.6 at 10 weeks after the operation (P > 0.05). The patients experienced significant improvements of GLQI at 16 weeks after OC operation (P < 0.01 or P < 0.05). The LC group showed better GLQI scores than did the OC group for up to 10 weeks postoperatively (P < 0.05).
CONCLUSIONSLC is betler or more rapidly than OC is improving the quality of life postoperatively. The assessment of the quality of life is valuable for measuring the outcome of surgical treatment.

5.Simultaneous Determination of 8 Non-anthraquinone Constituents in Rheum palmatum by HPLC

OBJECTIVE: To establish a method for simultaneous determination of 8 non-anthraquinone constituents in Rheum palmatum. METHODS: HPLC method was adopted. The determination was performed on Symmetry C18 column with mobile phase consisted of methanol-0.1% phosphoric acid (gradient elution) at the flow rate of 1.0 mL/min. The column temperature was 30 ℃ and detection wavelength was 280 nm. Sample size was 30 μL. RESULTS: The linear range of gallic acid, catechin, epicatechin, resveratrol 4′-O-glucopyranoside, epicatechin gallate, resveratrol 4′-O-β-D-(6″-O-galloyl)-glucopyranoside, sennoside A, 4′-hydroxyphenyl-2-butanone-4′-O-β-D-(2″-O-galloyl-6″-O-p-hydroxy cinnamyl)-glucopyranoside were 6.16-2 464 ng(r=0.999 9), 37.4-14 960 ng(r=0.999 9), 7.635-3 054 ng(r=0.999 7), 7.63-3 052 ng(r=0.999 9), 8.32-3 328 ng(r=0.999 9), 11.5-4 600 ng(r=0.999 9), 16.08-6 432 ng(r=0.999 9), 29.3-11 720 ng(r=0.999 9), respectively. The limits of quantitation were 3.48, 4.30, 6.40, 4.40, 3.39, 2.87, 8.40 and 4.95 ng, respectively. The limits of detection were 2.32, 2.58, 2.40, 2.64, 2.26, 1.23, 4.20, 2.97 ng, respectively. RSDs of precision, stability and reproducibility tests were all lower than 5%. Recoveries were 94.32%- 100.54%(RSD=2.78%,n=6), 91.15%-99.36%(RSD=3.72%,n=6), 92.16%-98.04%(RSD=2.39%,n=6), 93.41%-100.73%(RSD=3.17%,n=6), 93.89%-98.40%(RSD=1.99%,n=6), 92.61%-101.74%(RSD=3.71%,n=6), 92.66%-103.40%(RSD=3.76%,n=6), 95.45%-102.70%(RSD=3.06%,n=6), respectively. CONCLUSIONS: The established method is simple, accurate and specific, and can be used for the simultaneous determination of 8 non-anthraquinone constituents in R. palmatum.
6.Recombinant human tumor necrosis factor-α receptorⅡ: IgG Fc fusion protein for the treatment of drug-induced toxic epidermal necrolysis: a multicenter clinical observation

Objective:To evaluate the efficacy and safety of recombinant human tumor necrosis factor-α receptorⅡ: IgG Fc fusion protein (rhTNFR:Fc) in the treatment of drug-induced toxic epidermal necrolysis (TEN) .Methods:From 2009 to 2018, 22 patients with TEN were enrolled from 8 centers such as the Second Affiliated Hospital of Soochow University, including 10 males and 12 females, whose age ranged from 22 to 75 years. These patients were subcutaneously injected with rhTNFR:Fc at a dose of 25 mg once every 3 days for 6 - 8 consecutive sessions, and the initial dose was doubled. The drug eruption area and severity index (DASI) score and DASI improvement indices (DASI50, DASI75 and DASI90) were assessed before treatment and on days 4, 7, 10, 13, 16, 19, 22 and 25 after treatment; cytometric bead array (CBA) technology was used to detect the level of tumor necrosis factor (TNF) -α in peripheral blood and blister fluid samples. During the treatment, body temperature, rash changes, liver and kidney function of patients were monitored, and adverse reactions were recorded. Statistical analysis was carried out by using repeated measures analysis of variance, paired t test and Pearson correlation analysis. Results:Of the 22 patients, the temperature stopped rising in 20 patients without infections 24 - 72 hours after the first treatment, and returned to normal after 48 - 120 hours. Among the 22 patients, new blisters stopped appearing 24 - 48 hours after the first treatment, the skin color changed from bright red to dark purple after 48 - 96 hours, and most skin lesions subsided after 2 weeks. After 2 - 4 weeks of treatment, levels of alanine aminotransferase and aspartate aminotransferase returned to normal in 19 patients with abnormal liver function. After 4 - 13 days of treatment, levels of creatinine and urea nitrogen stopped rising in 7 patients with abnormal renal function. During the treatment, the DASI score of the 22 patients gradually decreased ( F = 532.81, P < 0.01) , from 53.64 ± 8.67 before treatment to 2.05 ± 1.21 on day 25 after treatment ( t = 26.60, P < 0.001) . On day 10 after treatment, 22 patients (100%) achieved DASI50; on day 19, 22 (100%) achieved DASI75; on day 25, 20 (90.90%) achieved DASI90. The level of TNF-α in peripheral blood of the 22 patients gradually decreased along with the extension of treatment duration, from 33.95 ± 27.90 ng/L before treatment to 2.38 ± 0.79 ng/L on day 25. Before treatment, the level of TNF-α in blister fluid of 15 patients was 111.99 ± 99.41 ng/L, and the ratio of blister-fluid TNF-α level to peripheral blood TNF-α level was 1.83 - 28.21. Before treatment, no correlation was observed between the serum level of TNF-α and DASI score in the 22 patients ( P = 0.10) , while the blister-fluid TNF-α level was positively correlated with DASI score in the 15 patients ( r = 0.59, P = 0.02) . No acute adverse reactions were observed during the treatment. All the 22 patients completed the treatment and were discharged with complete recovery. During 6 months of follow-up after discharge, no recurrence or any complication was observed. Conclusion:rhTNFR:Fc is effective and safe for the treatment of drug-induced TEN.
7.Clinical application of a prefabricated interocclusal recording cap for making interocclusal records of implant-supported fixed prostheses

Objective:To use a self-developed, prefabricated cap for making interocclusal records in implant-supported fixed prosthetic treatment, and to evaluate its effect of clinical application and accuracy of transferring intra-oral intercuspal position.Methods:Series of prefabricated caps for occlusal recording of implant-supported fixed prostheses were designed based on the healing abutments, and fabricated with three-dimensional (3D) printing. According to the inclusion and exclusion criteria, 12 partially edentulous patients who visited the Department of Prosthodontics, Peking University School and Hospital of Stomatology between November 2020 and September 2021, had lost no fewer than 2 contiguous teeth in distal extension (Kennedy Class Ⅰ or Ⅱ), and had received implant placement 3 months ago were enrolled. Self-control study design was used. Two occlusal records of each included case were obtained by the following 2methods:for the test group, appropriate prefabricated caps were used; and for the control group, polyvinyl siloxane occlusal record was directly set on the healing abutments. The working casts were mounted on the mechanical articulator using the 2 groups of occlusal records successively. Accuracy of occlusal relationship of the mounted casts was evaluated. Diagnostics test was performed to obtain the sensitivity and positive predictive value, which were determined in photographs by comparing the intra-oral occlusal contact points with those in the mounted casts, point-by-point. Virtual casts were taken by intraoral and extraoral scans in intercuspal position and imported to Geomagic Studio 2014. Then the root mean square values of occlusal clearance space between the upper and lower occlusal surfaces of remaining posterior teeth, and the deviations of the interocclusal position of the occlusal surfaces of the remaining teeth in the mandibular arches when superimposing the maxillary arches of the intraoral and extraoral scans were calculated. As a self-control design, paired t test was used. Results:Twelve participants were enrolled, including 3 men and 9 women aged (52.6±12.1) years, and 36 missing teeth. The prefabricated caps could fit on the healing abutments with good retention and stability. The sensitivity of the test group (0.73±0.14) was significantly better than that of control group (0.63±0.12, P<0.01), with no significant difference in positive predictive value between the 2 groups ( P>0.05). The deviations of the interocclusal position of the occlusal surfaces of the remaining teeth were (357.0±140.2) μm for the test group, and (399.4±206.3) μm for the control group, with no significant difference between them ( P>0.05). Conclusions:Interocclusal position record based on prefabricated cap in this study for implant-supported fixed prosthetic treatment can improve the consistency between the intra-oral occlusion and the occlusion in dental casts. This technique has good accuracy, clinical convenience and usability.
