Objective To investigate and compare the effects of pretreatment with different concentrations of trarnadol and morphine on the differentiation of hunan helper T cells (Th) in vitro. Methods Fifteen ASA Ⅰ patients of both sexes (8 males, 7 females) aged 23-59 yrs scheduled for minor day-care surgery were enrolled in this study. The patients had never taken morphine and tramadol before. Blood samples were taken from peripheral vein. Whole blood and mononuclear cells isolated from peripheral blood (PBMCs) were incubated with morphine 10, 100 or 1000 ng?ml-1(group M1 , M2 , M3) or tramadol 50, 500, 5000 ng?ml-1(group Q1, Q2, Q3) for 24 h. Then irrntants was added and incubated for another 5 or 24 h. In the whole blood the level of IL-2 and IFN-?(as markers of Th1 cells) and IL-4 and IL-10 (as markers of Th2 cells) were determined using three-color flow cytometry. In PBMCs the expression of CD4+ and CCR5+ (as markers of Thl cells) and CD4+ and CCR3+ (as markers of Th2 cells) were determined. The Th1/Th2 ratio was calculated. Results Pretreatment with morphine or tramadol significantly increased the number of Th2 cells while the number of Thl cells significantly decreased both in a dose-dependent manner leading to a significant decrease in Th1/Th2 ratio. Conclusion Tramadol and morphine can inhabit in vitro the cellular immune function in dose-dependent manner. The immunosuppressive effect of tramadol is less than that of morphine.

Objective To evaluate the clinical characteristics and surgical risks for elderly patients with gastric cancer. Methods The clinical data from 607 patients, who received surgical therapy for gastric cancer between January 2006 and December 2009, were retrospectively analyzed.Among them, 267 patients were over 65 years of age, and the other 340 patients (≤65 years of age)were served as control. Complete surgical resection was done by doctors in same group. The preoperative complications such as cardiopulmonary disease and post-operative complications as well as pathologic patterns were compared between two groups. Results Coexisted diseases were found more in elderly patients than in controls (68.91% vs 31.63 %, P<0.01). Whereas there was no significant difference between two groups with respect to post-operative complication [28. 19% (75/267)vs 25.59 % (87/340)]. However, the incidence of cardiopulmonary complications, especially pulmonary infection, was higher in elderly patients than in controls (17.23% vs 5.29% ,P<0.01). The digestive tract complications were related to the pattern of radical operation. Post-operative complication occurred less in patients treated with D1 radical operation than those with D2 and D3 radical operation. Conclusions The incidence of post-operative complication in elderly patients with gastric cancer was closely related to the surgical method. The elderly patients may has less complications if they were operated with D1 radical method which can reduce risk of complication and improve the quality of life.

Objective To evaluate the efficacy of mydriasis induced by intracameral injection of 1% lidocaine instead of topical mydriatics and postoperative recovery of pupil diameter in phacoemulsification surgery through a prospectively controlled clinical trial. Design Randomized clinical trial. Participants 34 patients (36 eyes) in study group,46 patients (50 eyes) in control group. Methods The study group comprised 34 patients (36 eyes),who were given intracamerally 0.5 ml unpreserved lidocaine 1% without using mydriatic eyedrops preoperatively. 46 patients (50 eyes),pupil dilated with topical mydriatics,Mydrin-p ( Tropicamid,Phenylephrine Eye Drops),were studied for comparison. As for the study group, baseline pupil diameters were recorded before the surgery, and 90 seconds after the injection of 1% lidocaine. As for the control group pupil, diameters were recorded when the surgery began. Pupil diameters at 1h,2h,4h,8h,12h,and 24h postoperatively were recorded respectively for both groups. Corneal edema and vision recovery were recorded postoperatively. Main outcome measures Pupil diameter. Results The mean baseline pupil size of the study group was 2.0611?0.1016mm,and maximum mean pupil size obtained at 90 seconds after the injection was 6.0972?0.1566mm. At the beginning of the surgery the mean pupil diameter of the control group was 7.6100?0.0799mm which was larger than 6.0972?0.1566mm (P=0.000). 75.0% eyes in the lidocaine group returned to normal size with brisk light reflex, while only 22.0% in the control group did. The time required for return to normal pupil size was significantly different between the two groups (P=0.000). Conclusions Intracameral injection of 1% unpreserved lidocaine can provide adequate pupil dilation and get rapid recovery of pupil size after surgery, though not as large as that of topical mydriatics for phacoemulsification. Influences of the clock resolution of bandwidth estimator on the accuracy and stability of the packet pair algorithm was analyzed. A mathematic model has been established to reveal the relationship between the result deviation coefficient and the packet size, clock resolution and real bandwidth(value)of the measured route. A bandwidth self-adapting packet pair algorithm was presented based on the mathematic model to reduce the estimation error resulting from the clock resolution and to improve the accuracy and stability of measurement by adjusting the deviation coefficient. Experimental results have verified the validity and stability of the algorithm.
