1.Effect of XueBiJing injection on the expression of high mobility group box-1 protein in lung of rats with burn injury

Objective To investigate the effect of XueBiJing injection(a concoction of Chinese herbs)on the expression of high mobility group box-1 protein(HMGB1)in the lung with acute lung injury of rats following delayed resuscitation after burn injury.Methods 78 male rats were randomly divided into sham scald group(n=18),scald group(n=30)and XueBiJing treatment group(n=30).In the scald group and XueBiJing treatment group,4ml/kg normal saline or XueBiJing was intravenously injected 2 hours after scald injury.In the latter two groups,rats were subjected to 30% TBSA full-thickness scald injury,and fluid resuscitation was delayed.The animals were sacrificed at 8h,24h and 72h after injury,respectively.Lung tissue samples were collected to determine the expression of HMGB1 mRNA and protein,and the activity of pulmonary myeloperoxidase(MPO).HMGB1 mRNA level was semi-quantitatively measured by RT-PCR using GAPDH as an internal standard,and protein expression of HMGB1 was determined by Western blot.Results Compared with sham scald controls,both mRNA and protein expressions of HMGB1 were significantly enhanced in the lung at 8-72h after scald injury(P

3.Clinical experience of early surgical treatment to acute biliary pancreatitis

Objective To investigate the opportunity and methods of acute biliary pancreatitis.Methods A retrospective review was performed in 85 consecutive patients from June 2008 to October 2014 with acute biliary pancreatitis in Beijing Yanqing County Hospital.Results Eighty-five patients were performed operation.Sisty-two patients underwent laparoscopic cholecystectomy and laparoscopic transcystic common bile duct exploration.Fifyfourcases were found biliary calculi and take away the stones.All patients were no complications such as infection,bile leakage,hemorrhage,bile duct injury,cicatrical stenosis of papilla.After three months of follow-up without biliary residual bile stones and recurrent pancreatitis.Conclusions The early surgical treatment for no severe acute biliary pancreatitis is necessary,safe and feasible.Actively biliary tract exploration can avoid bile duct residual stones.
4.Establishment and evaluation of a bacteriemia model in mice after infection with an isolated methicillin-resistant Staphylococcus aureus( MRSA )

Objective To established a bacteriemia model of BALB/c mice after infection with a ST-239 methicillin-resistant staphylococcus aureus(MRSA) strain, which was isolated and identified from Shanghai Huashan Hospital. Methods We monitored the clinical signs and gross observations of MRSA-infected mice, and examined the histopathology among different groups. Results This isolated MRSA strain ST-239 can induced a typical bacteriemia in BALB/c mice, including the severe mortality and extensive histopathologic injury. However, higher survival rate and slight inflammatory injury were observed in vancomycin-treated mices. Conclusion The solid results obatined in this model will benefit us to study the pathogenic characteristics and patholgenesis in MRSA-induced bacteriemia, and propeled us to seek a safety cure approaches in the future.
5.Comparison of acute Mycobacterium tuberculosis infection mouse models established by different infection routes

ObjectiveTo compare three types of acute Mycobacterium tuberculosis infection mouse models established through different infection routes and to set up the theoretical basis for further developing,selecting and applying these animal model in the tuberculosis-related research.MethodsStandard strain of Tubercle bacillus H37Rv was diluted to 1 × 106 colony forming unit (cfu)/mL.The mice were infected with the bacteria through different routes including intravenous injection,intranasal administration and inhalation of bacteria aerosol.Six weeks after the infection,the mice were euthaniz ed and necropsied. The lung tissues were collected and gross changes were observed.The colony counting was performed and the lung tissues were assessed by HE staining,acid fast staining.The e xpression level of tumor necrosis factor (TNF)-α per unit area in lung tissue was detected by immunohistochemistry. The data were analyzed by t test. Results The amounts of Mycobacterium tuberculosis in lung tissues of mice in inhalation group,intranasal administration group and intravenous injection group were (6.290±0.028),(6.150±0.021) and (6.120±0.008) lg cfu/mL,respectively; while no Mycobacterium tuberculosis was detected in control group. The difference between infection group and control group was statistically significant (t =3.762,P<0.01),while there were no significant differences among infection groups with different infection routes (P>0.05).According to the results of gross observations and histological assessment,the pathological changes were observed and red tubercle bacillus was detected by acid-fast staining in the lung tissues of all the mice in infection group.The results of immunohistochemistry showed that the expression levels of TNF-α per unit area were as follows:intravenous injection group (0.049 × 106 )<intranasal administration group(0.759×106) < inhalationgroup(1.042×106), whichwere statistically different (t =2.504,P< 0.05).ConclusionInhalation of bacteria aerosol may be the most efficient method to establish tuberculosis infection mouse model compared to intravenous injection and intranasal administration.
6.Clinical Study on Shiwei Longdanhua Capsule Combined with Western Medicine for Treatment of Acute Exacerbation of Chronic Obstructive Pulmonary Disease

Objective To observe the clinical efficacy of Shiwei Longdanhua Capsule combined with western medicine on patients with mild acute exacerbation of chronic obstructive pulmonary disease (COPD) with phlegm-heat obstructing lung syndrome. Methods Totally 192 cases of COPD with mild acute exacerbation (phlegm-heat obstructing lung syndrome) were recruited. They were randomly divided into the treatment group (96 cases) and the control group (96 cases). Patients in the control group were treated in line with the regular Western treatment, while patients in the treatment group were additionally treated by Shiwei Longdanhua Capsule. After 10-day treatment, the changes of TCM syndrome score, symptom integral and the lung function before and after treatment were compared. Results After treatment, 14 patients withdrew from the treatment group and 15 patients withdrew from the control group. The total efficiency rate of TCM syndrome was 80.49% (66/82) in the treatment group, while it was 56.79% (46/81) in the control group, with statistical difference between the two groups (P<0.05). The severity and frequency of cough and expectoration of the treatment group were better than the control group (P<0.05). The breathing symptom and pulmonary function of patients in the two groups improved, with similar efficacy. Conclusion The efficiency of Shiwei Longdanhua Capsule combined with Western medicine in the treatment of acute exacerbation of COPD is superior to that of simple Western medicine treatment, especially in terms of relieving cough and expectoration.
7.The cardiac auscultation proficiency in residents before and after training

The study is designed to investigate the progress in cardiac auscultation proficiency of residents after training program.Thirty eight residents were selected to test their ability of cardiac auscultation.There were 13 cardiac events(or murmurs)in 15 patients,and the auscultation proficiency was expressed as the percentage of recognizing cardiac events accurately.The results showed that the highest auscultation proficiency was whole systolic and diastolic murmurs(up to 79 percent and 61 percent respectively)before training.The percentage of recognizing continuous murmurs,click,mitral stenosis and regurgitation,aortic stenosis increased significantly after training program(P<0.05).

Objective To study the effect of sodium ?-aescin on the matrix metalloproteinase-9 (MMP-9) and laminin (LN) expression in rats after focal cerebral ischemia and reperfusion, and provide experimental evidence for the relationship of MMP-9 and LN expression change with the neuroprotection of sodium ?-aescin. Methods The Spraque-Dawley (SD) rat model of focal cerebral ischemia and reperfusion of cerebral middle artery was prepared. Fifty-six rats were randomly divided into the normal, sham, ischemia and reperfusion, as well as ischemia and reperfusion-treated with sodium ?-aescin groups. The latter two were further divided into six subgroups according to reperfusion time interval of 3h, 6h, 12h, 24h, 72h and 7d after brain ischemia, with 4 rats in each subgroup. Immunohistochemical staining and image analysis were used to measure the expression of MMP-9 and LN in the ischemic region. Results The expression of MMP-9 started to increase at 6h, reached the peak at 12h, decreased in 24h, and reached the lowest on 7d in cerebral ischemia and reperfusion group. Objective To explore the mechanism of plasma circulating miRNA-126 and miRNA-28-3p in diabetes mellitus (DM) patients,and to identify the related bioinformatics analysis.Methods Randomly selected 80 DM patients as the observation group and 80 non-DM patients as the control group.The plasma circulating miRNA-126 and miRNA-28-3p were analyzed by qRT-PCR,and its target genes,biological information,related lncRNA and circRNA were predicted.Results The circulating miRNA-126(0.115 0±0.014 4 vs.0.0019±0.000 6) and miRNA-28-3p(0.1386±0.01724 vs.0.000 6±0.000 05) levels in the observation group were significantly higher than those in the control group,and the differences were statistically significant (P<0.01).Pearson correlation coefficient of miRNA-126 and miRNA-28-3p was 0.433 5(P<0.01).ROC curve analysis of miRNA-126 and miRNA-28-3p showed that the differences of the area under curve were statistically significant between the two groups (P<0.01).Bioinformatics prediction showed that miRNA-126 and miRNA-28-3p may be involved in regulation of the insulin signaling pathway,insulin receptor signaling pathway,insulin/insulin growth factor signaling pathway,mitogen-activated protein kinase (MAPK) signaling pathway and angiogenesis.And it may be associated with a variety of lncRNA and circRNA.Conclusion Circulating miRNA-126 and miRNA-28-3p can be a novel biomarker of DM as it may participate in the mechanism of DM by regulating insulin and insulin growth factor related signaling pathways and be associated with some related lncRNA and circRNA.
10.Preliminary study on the role of novel LysR family gene kp05372 in Klebsiella pneumoniae of forest musk deer.

LysR-type transcriptional regulators are involved in the regulation of numerous cellular metabolic processes in Klebsiella pneumoniae, leading to severe infection. Earlier, we found a novel LysR family gene, named kp05372, in a strain of K. pneumoniae (designated GPKP) isolated from forest musk deer. To study the function of this gene in relation to the biological characteristics of GPKP, we used the suicide plasmid and conjugative transfer methods to construct deletion mutant strain GPKP-Δkp05372; moreover, we also constructed the GPKP-Δkp05372+ complemented strain. The role of this gene was determined by comparing the following characteristics of three strains: growth curves, biofilm formation, drug resistance, stress resistance, median lethal dose (LD50), organ colonization ability, and the histopathology of GPKP. Real-time polymerase chain reaction (RT-PCR) was used to test the expression level of seven genes upstream of kp05372. There was no significant difference in the growth rates when comparing the three bacterial strains, and no significant difference was recorded at different osmotic pressures, temperatures, salt contents, or hydrogen peroxide concentrations. The GPKP-Δkp05372 mutant formed a weak biofilm, and the other two strains formed medium biofilm. The drug resistance of the GPKP-Δkp05372 mutant toward cephalothin, cotrimoxazole, and polymyxin B was changed. The acid tolerance of the deletion strain was stronger than that of the other two strains. The LD50 values of the wild-type and complemented strains were 174-fold and 77-fold higher than that of the GPKP-Δkp05372 mutant, respectively. The colonization ability of the GPKP-Δkp05372 mutant in the heart, liver, spleen, kidney, and intestine was the weakest. The three strains caused different histopathological changes in the liver and lungs. In the GPKP-Δkp05372 mutant, the relative expression levels of kp05374 and kp05379 were increased to 1.32-fold and 1.42-fold, respectively, while the level of kp05378 was decreased by 42%. Overall, the deletion of kp05372 gene leads to changes in the following: drug resistance and acid tolerance; decreases in virulence, biofilm formation, and colonization ability of GPKP; and regulation of the upstream region of adjacent genes.
