1.Effects of budesonide suspension combined with terbutaline on the VAP and rehabilitation of thoracic surgery

Objective To investigate the effect of budesonide suspension combined with terbutaline on the ventilator-associated pneumonia (VAP) and rehabilitation in patients undergoing thoracic surgery. Methods Fifty-five patients underwent thoracic surgery from June 2006 to January 2017 in our hospital. The patients were treated with budesonide suspension combined with terbutaline atomization solution for inhalation. Forty patients with thoracic surgery were selected as the control group. Comparison of the two groups after the blood gas indicators, the incidence of VAP, mechanical ventilation and hospital stay. Results There was no significant difference in white blood cells, neutrophils and CRP between the two groups.The oxygen saturation of the observation group was higher than that of the control group, the arterial oxygen partial pressure was higher than the control group, PaCO2 was lower than the control group, the difference was statistically significant (P<0.05). The VAP of the observation group was lower than that of the control group. The time of mechanical ventilation was shorter than that of the control group. The hospitalization time was shorter than that of the control group, the difference was statistically significant (P<0.05). Conclusion The combination of budesonide suspension and terbutaline atomization can improve the postoperative effect of thoracic surgery and reduce the incidence of postoperative VAP and promote the early rehabilitation of patients.
2.Surgical treatment of pancreatic cancer: present status and future

The incidence of pancreatic cancer has steadily risen in the past decades. Radical resection is still the best option for treatment of pancreatic cancer. Great progress has been achieved in pancreatic surgery;the mortality and morbidity have decreased dramatically, but long term survival after resection is still dismal. Standard pancreaticoduodenectomy is recommended now for pancreatic head cancer. Extended lymphadenectomy could not improve the prognosis of pancreatic cancer patients. Total pancreatectomy is suitable for some IPMNs patients, rather than a routine for pancreatic cancer patients. The current status and future of surgical treatment for pancreatic cancer will be summarized here.
3.The effect of anterior fusion with the internal fixation in treatment of fracture-dislocation complicated with spinal cord injure

Objective To study the the effect and rate of complications of anterior fusion with the internal fixation in treatment of fracture-dislocation complicated with spinal cord injure. Methods 46 patients with fracturedislocation complicated with spinal cord injure were selected as group A 23 cases and group B 23 cases according to the methods of treatment,group A was treated with anterior fusion with the internal fixation,group B was treated with cervical posterior, then the total effective rate, surgical time, the rate of complications, ASIA scores and SF-36 scores before and after the treatment of the two groups were studied. Results The total effective rate of group A(91.3% )was higher than that (73.9%) of group B ( 17/23 ) ( x2 = 6. 38, P < 0. 05 ), surgical time ( 83.6 ± 23.5 ) min of group A was shorter than that of group B( 106.8 ±25. 1)min(t =5.37,P <0. 05) ,the rate[13.0% (3/23)]of complications of group A was lower than that of group B[26. 1% (6/23)] ,ASIA scores and SF-36 scores of group A after the treatment were better than those of group B(P <0. 05) ,there were significant differences. Conclusion The effect of anterior fusion with the internal fixation in treatment of fracture-dislocation complicated with spinal cord injure was better,and the the rate of complications was lower,and it was worthy of exploitation and promotion.
5.Short-term and Long-term Efficacy of Arthroscopic Debridement,Lateral Patellar Decompression,and Rehabilitation Therapy for Osteoarthritis of the Knee Joint

Objective To study the short-term efficacy of arthroscopic debridement,lateral patellar decompression,and rehabilitation thearpy for osteoarthritis of the knee joint,and to discuss the long-term outcomes of the treatments.Methods A total of 85 cases of osteoarthritis that were diagnosed according to the clinical diagnostic criteria of ACR were enrolled in this study.The patients were randomly divided into groups A(39 cases) and B(46 cases) to underwent arthroscopic debridement and arthroscopic debridement combined with lateral patellar decompression respectivley.All the operations were performed by a same surgeon.Postoperative rehabilitation thearpies were carried out by one group of therapist.The Lysholm,HSS,and PPI scores were evaluated before the operation,as well as patellar tilt test and patellar gliding test.The examinations were repeated in 3 and 6 months respectivley after the surgery.Results In group A,the scores of Lysholm,HSS,and PPI determined at month 3 were significantly higher than those before the operation,but not changed markedly at month 6.Whereas,in group B,the scores kept being increased after the surgery.At month 3,the scores in group A were higher than those in group B,however,at month 6,the Lysholm score in group B was higher than that in group A.Conclusions Arthroscopic debridement and postoperative rehabilitation are effective for imroving the symtoms and knee function in patients with osteoarthritis and limited ROM of the patella.Combining with lateral patellar decompression,the treatments can achieve better outcomes in 6 months.

BACKGROUND:The main factors that influence revision of acetabular bone defects include evaluation of acetabular bone defects,reconstruction of acetabulum and appropriate acetabular cups used in revision.OBJECTIVE:The review from the three aspects will help to choose an appropriate cup in revision and to formulate the best revision protocols.METHODS:A computer-based online search of PubMed was performed for English articles published from January 1900 to June 2009 with the key words of acetabulum,revision,bone defect,in English and Chinese.Clinical studies published in core periodicals of the latest twenty years were reviewed.Articles regarding primary joint replacement,femoral prosthesis revision and animal experiments were excluded.RESULTS AND CONCLUSION:A total of 61 articles were analyzed,31 were excluded,and 30 mainly published in 5 years,were included.Reconstruction of acetabular bone structure,restoration of original rotation center of hip joint are key factors for repair.Moreover,appropriate prosthesis further benefits the repair effect.Bone transplantation for bone defect and cementedless prosthesis have become favorable methods for revision of acetabular bone defects.
8.Vocal Fold Augmentation by Transplantation of Autologous Fat and Fascia: An Experimental Study

Objective To compare the histological fates and the effects of autologous fat and fascia used as augmentative materials to treat unilateral vocal fold paralysis (UVFP). Methods Vocal fold augmentation with autologous fat and fascia was performed on 2 dogs (group A) to cure glottic insufficiency caused by left vocal fold paralysis. Autologous fat and fascia 0.4 ml were injected respectively into left and right vocal folds of 9 dogs (group B). The dogs of group B were killed after 1,3 and 5 months and the dogs of group A were killed after 8 months. The histology of both substances and the effect of treatment were evaluated. Results Surviving autologous fat grafts was found in the canine vocal fold after 8 months, but the absorption was too rapid to maintain the bulk of grafts, the effect could last no more than 5 months. Autologous fascia also survived after 8 months, Conclusion The results showed that these two augmentative materials were both effective in the treatment of IJVFP. Autologous fat can only be used as short - term augmentative material. However, autologous fascia has a potential use for long - term application to correct glottic insufficiency caused by UVFP and other diseases.
9.Application of iPad and its Software in Rehabilitation of Children with Autism:Four Cases Report

Objective To explore the application of iPad-based software Let'S Talk in the rehabilitation of children with autism. Meth-ods From March to September, 2015, iPad-based software Let'S Talk was applied in four children with autism. The language ability, emo-tion and behavior control ability, self-consciousness and eye contact were observed. The Psycho-educational Profile-3rd (PEP-3), Autism Be-havior Checklist (ABC) and Childhood Autism Rating Scale (CARS) were used to access the months of age, behavior characteristics, and the severity of autism before and after application. Results The language ability and behavior control ability improved, as well as the self-consciousness and eye contact, and the bad behaviors reduced. The months of age rose in PEP-3, and the scores of ABC and CARS de-creased. Conclusion IPad-based software Let'S Talk could be applied in the rehabilitation of children with autism.
10.Estimation of the Power Spectrum of Heart Rate Variability Using Improved Welch Method to Analyze the Degree of Fatigue.

Heart rate variability (HRV) is an important point to judge a person's state in modern medicine. This paper is aimed to research a person's fatigue level connected with vagal nerve based on the HRV using the improved Welch method. The process of this method is that it firstly uses a time window function on the signal to be processed, then sets the length of time according to the requirement, and finally makes frequency domain analysis. Compared with classical periodogram method, the variance and consistency of the present method have been improved. We can set time span freely using this method (at present, the time of international standard to measure HRV is 5 minutes). This paper analyses the HRV's characteristics of fatigue crowd based on the database provided by Physio-Net. We therefore draw the conclusion that the accuracy of Welch analyzing HRV combining with appropriate window function has been improved enormously, and when the person changes to fatigue, the vagal activity is diminished and sympathetic activity is raised.
