BACKGROUND:Class II malocclusion is commonly observed in clinics, with the main manifestation of skeletal malocclusion, and mandibular retrusion is the main reason. Class II malocclusion should be treated early to correct skeletal malformation and improve facial appearance. OBJECTIVE:To compare the biomechanics change in early treatment of Class II division 1 malocclusion with Twin-Block appliance and straight wire appliance combined with face-bow and Class II drawing. METHODS: Thirty patients with malocclusion who were at peak velocity on the growth curve were randomly divided into two groups, with 15 cases in each group. Patients in each group were treated with Twin-Block appliance and straight wire appliance combined face-bow and Class II drawing, respectively. The cephalometric records of al patients were examined before and after the treatments. The acquired data were processed by statistical analysis. RESULTS AND CONCLUSION:After treatment, al molars and cuspid teeth achieved or reached Class I, and ANB angle decreased to normal range. The bone effect of Twin-Block appliance was more significant than straight wire appliance. Using Twin-Block appliance, the length of mandible body and lower face height increased significantly. Experimental findings indicate that, Twin-Block appliance has significant therapeutic effects in treatment of early skeletal Class II division 1 malocclusion patients, who are at peak velocity on the growth curve. With the biomechanical effects, this treatment corrects asymmetrical jaw relation, reconstructs the tissue, and obtains satisfactory lateral facial profile of soft tissues. The therapeutic effect is more obvious for patients with obvious mandibular retrusion, flared upper incisors and upright lower incisors.

OBJECTIVE:To study the effects of volatile oil of Pogostemon cablin on the expression of tight junction proteins ZO-1 and Occludin in colonic mucosal epithelial cells of rats with post-infectious irritable bowel syndrome(PI-IBS). METHODS:Rats were randomly divided into normal control group(distilled water),model group(distilled water),Trimebutine maleate tablet group(TMT,0.1 g/kg)and volatile oil of P. cablin low-dose,medium-dose and high-dose groups [2,3,4 g(crude drug)/kg] with 8 rats in each group. Except for normal control group,those groups were given colon perfusion of acetic acid to induce PI-IBS mod-el. After modeling,they were given relevant medicine intragastrically once a day for consecutive 5 days. The expression of ZO-1 and Occludin [by IOD] and their distribution were detected by immunohistochemistry. RESULTS:In normal control group,ZO-1 and Occludin evenly distributed on the top of enterocyte,manifesting as alveolate and punctiform;in model group,ZO-1 and Oc-cludin scattered on the top of enterocyte,showing uneven dyeing or fade;the distribution of ZO-1 and Occludin in volatile oil of P. cablin groups was similar to normal control group,and its dyeing was lighter than that of normal control group. IOD value of ZO-1 and Occludin in model group were lower than in normal control group(P<0.01);those of volatile oil of P. cablin high-dose group and the IOD value of ZO-1 of P. cablin medium-dose group and TMT group were higher than those of model group (P<0.05 or P<0.01). CONCLUSIONS:The volatile oil of P. cablin can up-regulate the expression of tight junction proteins ZO-1 and Occlu-din in colonic mucosal epithelial cells of rats with PI-IBS.

Objective To investigante the effect on the count of intestinal mast cell in rats of botulinum toxin-A.Methods 40 Wistar rats of 8 weeks age were divided into four groups randomly and evenly.4 units,2 units,1 unit BTA was injected in every rat peritoneally in group A,B,C.D was control group.The samples of small intestine,ileocecal junction and distal colon was stained with toluidine blue improved staining method for mast cell at 4 weeks after injection.Results The counts of mast cell in all the samples in group A and B[small intestine (5.16?2.74) and (4.50?3.19);ileocecal junction (3.83?1.78) and (3.33?2.50);distal colon (7.00?2.53) and (7.50?3.29),respectively] were smaller than those of group D[(11.16?4.48),(8.50?3.04),(13.66?4.83),P

Objective To investigate the related factors of the retinal hemorrhage in high-risk infants (HRI).Methods Eight hundred and sixty HRI with histories of high-risk pregnancy and/or neonatal asphyxia after 1-5 days of birth were enrolled in this study.In 860 cases of HRI,498 infants were vaginal delivery and 362 infants were delivered through cesarean sections.Among 498 vaginal delivered infants,407infants were eutocia and 91 infants were with forceps delivery; 298 infants were born following normal labor,102 infants experienced prolonged labor,and 98 infants were urgent birth.The retinal hemorrhages were observed and conditions were graded into three degrees of Ⅰ,Ⅱ,and Ⅲ.Conditions of neonatal asphyxia were evaluated based on criteria of Apgar score.The incidence of retinal hemorrhage in the different types of deliveries and labor processes were compared,and the relationship between degree of retinal hemorrhage and grade of neonatal asphyxia were analyzed.Results In 860 cases of HRI,retinal hemorrhages were found in 202 infants (23.5％).Within these 202 infants,75 infants (37.1％) were Ⅰdegree retinal hemorrhage,75 infants (37.1％) were Ⅱ degree retinal hemorrhage,and 52 infants (25.8％)were Ⅲ degree retinal hemorrhage.In these 202 infants of retinal hemorrhage,172 infants (85.1 ％) had histories of asphyxia; 119 infants (69.2％) were graded as mild asphyxia-risk,and 53 infants (30.8％)were graded as severe asphyxia-risk.There was a statistical difference of the degree of the retinal hemorrhage between the mild and severe asphyxia-risk infants (x2 =34.61,P＜0.01).The incidence of retinal hemorrhage after vaginal delivery was higher than cesarean section delivery with significant statistical difference (x2 =30.73,P＜ 0.01).The incidence of retinal hemorrhage after forceps delivery was significantly higher than eutocia with statistical difference (x2 =62.78,P＜0.01).Both prolonged and urgent childbirth had statistically significant higher incidences of retinal hemorrhage compared to normal labor in the process of vaginal delivery (x2＝45.86,71.51; P＜0.01).Asphyxia,types of delivery,prolonged and urgent labors were risk factors of retinal hemorrhage for HRI (r=7.46,4.87,15.03,6.47;P＜ 0.01).Conclusions The incidence of retinal hemorrhage in high-risk infant was 23.5％.And,asphyxia,types of delivery,prolonged and urgent labors may play roles of risk factor in retinal hemorrhage of HRI.

OBJECTIVETo explore the clinical efficacy and the effect mechanism of hooking therapy and safflower injection in the treatment of lumbar disc herniation (LDH).

METHODSSixty patients of LDH were randomized into an observation group and a control group at ratio of 1 to 1, 30 cases in each one. In the control group, safflower injection was applied to three points of lumbar region, once a day. Seven treatments made one session, and totally 2 sessions were required. In the observation group, under local anesthesia, the big hook needle was used to stimulate the three points of lumbar region first, followed by the safflower injection, once every 6 to 7 days. If the efficacy was not satisfactory enough, the second treatment was given. The visual analogue scale (VAS) and the modified Japanese Orthopaedic Association (M-JOA) scores were observed before and after treatment in the patients of the two groups and the clinical efficacy was evaluated. The levels of peripheral interleukin-6 (IL-6) and nitric oxide (NO) were determined.

RESULTSVAS and M-JOA scores were all improved apparently after treatment as compared with those before treatment in the patients of the two groups (both P < 0.01). The improvements in the observation group were much more obvious than those in the control group (both P < 0.01). In the observation group, the curative and remarkably effective rate was 76.7% (23/30) and the total effective rate was 96.7% (29/ 30), better than 43.3% (13/30) and 70.0% (21/30) in the control group separately (both P < 0.05). After treatment, the levels of IL-6 and NO were all reduced as compared with those before treatment in the two groups; (both P < 0.01). The above indices were changed more obviously in the observation group as compared with those in the control group (both P < 0.01).

CONCLUSIONThe combined therapy of hooking therapy and safflower injection apparently relieves pain and clinical symptoms of LDH. The effect mechanism is relevant with reducing the levels of IL-6 and NO in the peripheral blood.

Objective:To determine the effect of standardized patients in medical international students' graduation practice assessment.Methods:The students were divided into two groups ofstandardized patient assessment and traditional assessment,the theoretical and professional performance was compared and analyzed after the assessments.All the students were investigated using questionnaire.Result:The students assessed using standardized patients reported higher skill scores than those with traditional assessments,and the difference was statistically significant.However,no difference was observed in theoretical scores.Gorcltusion:The introduction of standardized patient to medical international students' graduation practice assessment alleviates the status that emphasizes theory but ignores practice due to the lack of clinical resources,eliminates the obstacles in the communication between students and patients,and is conducive to the cultivation of humanistic quality and adaptability of the students.It is worthy popularizing in international students' graduation practice assessment.

Objective To detect the expressions of Numb and α-catenin in the gastric cancer and matched normal gastric mucosa,and to analyze their relationship with clinicopathological factors of gastric cancer and explore their roles in gastric carcinogenesis and progression.Methods Immunohistochemical method was used to detect the expressions of Numb and α-catenin in 109 cases of tumor specimens and 97 cases of matched normal gastric mucosa.The correlations between Numb,α-catenin and clinicopathological factors were analyzed.Results The positive rates of Numb and α-catenin in gastric cancer were significantly lower than those in normal gastric mucosa (60.6％ ∶ 100.0％,x2 =48.361,P =0.000;76.1％ ∶ 100.0％,x2 =26.480,P=0.000).The expression of Numb protein was correlated with Lauren type (x2 =17.018,P =0.000) and tubular adenocarcinoma differentiation (x2 =17.586,P =0.000).The expression of α-catenin protein was correlated with Borrmann type (x2 =6.700,P =0.035),Lauren type (x2 =11.098,P =0.001),tubular adenocarcinoma differentiation (x2 =8.203,P =0.017) and lymph node metastasis (x2 =6.402,P =0.011).The expressions of Numb and α-catenin were statistically correlated in 109 cases of gastric cancer (rk =0.184,P =0.028).Conclusion Compared with normal gastric mucosa,both Numb and α-catenin expressions are down-regulated and the two expressions are correlated in gastric cancer.Numb and α-catenin may be involved in the regulation of histological differentiation of gastric cancer by certain passages,Numb protein may be adjusted by α-catenin pathway which is involved in Wnt-β-catenin pathway,and thus plays an important role in promoting cell proliferation,invasion and metastasis in human gastric cancer.

Heart rate variability (HRV) is an important point to judge a person's state in modern medicine. This paper is aimed to research a person's fatigue level connected with vagal nerve based on the HRV using the improved Welch method. The process of this method is that it firstly uses a time window function on the signal to be processed, then sets the length of time according to the requirement, and finally makes frequency domain analysis. Compared with classical periodogram method, the variance and consistency of the present method have been improved. We can set time span freely using this method (at present, the time of international standard to measure HRV is 5 minutes). This paper analyses the HRV's characteristics of fatigue crowd based on the database provided by Physio-Net. We therefore draw the conclusion that the accuracy of Welch analyzing HRV combining with appropriate window function has been improved enormously, and when the person changes to fatigue, the vagal activity is diminished and sympathetic activity is raised.

Objective To investigate the diagnosis and treatment value of digital subtraction angiography (DSA) and transcatheter arterial embolization (TAE) for post pancreatectomy hemorrhage (PPH),and influencing factors of severity of PPH.Methods The retrospective case-control study was conducted.The clinicopathological data of 20 patients with PPH who were admitted to the Zhongshan Hospital of Fudan University from August 2009 to November 2016 were collected.Patients with PPH in the early stage underwent reoperations for hemostasis;patients with PPH in the later stage received conservative treatment,and then DSA and TAE were considered when patients had the stable vital signs.Observation indicators:(1) DSA situations:overall times,positive rate and bleeding sites;(2) TAE situations:successful rate of hemostasis,operating time and postoperative complications;(3) follow-up situations;(4) influencing factors analysis of severity of PPH.Follow-up using outpatient examination and telephone interview was performed to detect occurrence of complications after discharging from hospital up to April 2017.Measurement data with skewed distribution were described as M (range).Count data were evaluated by the ratio and proportion.The univariate analysis was done using the Fisher exact probability.Results (1) DSA situations:all the 20 patients underwent DSA,with overall times of 27.The direct sign was 18 times extravasation of the contrast medium,with a positive rate of 66.7％ (18/27).Of 18 times positive DSA,clear bleeding sites were located in 5 times gastroduodenal artery (3 times with pseudoaneurysm of gastroduodenal artery stump),in 4 times common hepatic artery (3 times with pseudoaneurysm of common hepatic artery),in 3 times superior mesenteric artery,in 2 times splenic artery,in 1 time left gastric artery,in 1 time right gastric artery,in 1 time left hepatic artery (pseudoaneurysm of left hepatic artery) and in 1 time inferior mesenteric artery.(2) TAE situations:of patients with 18 times positive DSA,patients with 15 times positive DSA received TAE,with a successful rate of hemostasis of 13/15,and patients with 5 times positive DSA received successful hemostasis by reoperation.A median operating time of TAE for patients with 15 times positive DSA was 30 minutes.There was no occurrence of adverse reaction,including fever,abdominal pain,melena,elevated aminotransferase and liver abscess.One patient complicated with splenic abscess after transcatheter splenic arterial embolization underwent puncture drainage and then had a good recovery.Of patients with 9 times negative DSA,patients with 8 times negative DSA were cured by conservative treatment and patient with 1 time negative DSA received successful hemostasis by operation.All the 20 patients were cured and then discharged from hospital.(3) Follow-up situations:20 patients were followed up for 4-92 months,with a median time of 24 months.During the follow-up,20 patients recovered well,without long-term complications.(4) Influencing factors analysis of severity of PPH:the results of univariate analysis showed that gender,age,preoperative blood sugar,preoperative combined jaundice,preoperative albumin (Alb),preoperative prothrombin time (PT) extended,preserving pylorus,pancreatic duct stent placement,pancreatic operation time,volume of intraoperative blood loss,intraoperative blood transfusion,property of tumor,postoperative pancreatic fistula and time of PPH were not factors affecting the severity of PPH (P＞0.05).Conclusion DSA is minimal-invasive in the diagnosis for PPH,and TAE is safe and effective for patients with positive DSA.
