Journal of Chinese Physician 2021;23(5):797-800
MicroRNAs (miRNAs) are a class of non coding small RNAs. MiRNAs can regulate cell signal transduction and gene expression, affect cell proliferation, apoptosis and other cell physiological processes, and play a crucial role in maintaining normal biological metabolism and homeostasis of the intracellular environment. In addition, miRNAs are widely involved in the occurrence and progress of cancer. Studies have shown that miRNA27a, a member of the miRNAs family, can play the role of oncogene or tumor suppressor gene in different cancer disease models. This article will review the expression, mechanism and potential clinical application of miRNA27a in different malignant tumors.

Objective:To investigate the efficacy and safety of three-dimensional navigation template assisted osteotomy for fracture malunion of lateral condyle of humerus in children. Methods:Eighteen children with ma-lunion of fracture of lateral condyle of humerus were treated in Southern Medical University Third Affiliated Hospital from August 2012 to December 2019 and analyzed retrospectively.Among them, 12 cases were treated with routine ope-ration: according to CT reconstruction and empirical osteotomy during the operation, the location of deformity was analyzed.Six cases were treated with navigation template assisted osteotomy: according to the CT data of bilateral elbow joints, a three-dimensional bone model was established, and the coronal and sagittal deformities of the distal humerus were evaluated by comparing the images of the distal humerus of the affected side with the distal humerus of the healthy side, so as to determine the best plane of osteotomy and the target position of reduction. The preoperative simulation was carried out on the computer, and the osteotomy navigation template and reduction navigation template were designed. The navigation template was printed out with a 3D printer.During the operation, the osteotomy was corrected with the assistance of navigation template, and the broken end of osteotomy was fixed with hollow screw and fixed with 80 degree flexion plaster. The X-ray films of the two groups were reexamined regularly after operation. The amount of intraoperative blood loss, operation time, incidence of postoperative complications (necrosis of humeral head and trochlea, delayed ulnar nerve paralysis, and bone nonunion) and the recovery of flexion and the extension function of elbow joint were observed in the 2 groups. Results:The elbow joint activity of all children basically returned to normal, the orthopedic site of osteotomy achieved bony healing, and the orthopedic effects were satisfactory. All the 18 cases were followed up for 6-24 months (mean 15 months). According to the postoperative Dhillon score, the navigation template group was excellent in 1 case and better in 5 cases, while in the routine group, 11 cases were batter and 1 case was fair. There were no significant differences in Dhillon score between the 2 groups after operation ( P>0.05). There were no significant differences in the range of the motion of elbow joint between the 2 groups ( P>0.05). In the comparison of operation time and intraoperative blood loss, the navigation template group [(200.0±24.2) min, (85.0±10.9) mL]was better than the routine group[(232.0±20.1) min, (139.1±18.3) mL](all P<0.001). Conclusions:The application of three-dimensional humerus reconstruction and mirror image contrast to evaluate the distal humerus deformity of the affected side, and the design of osteotomy and reduction navigation template auxiliary operation can restore the normal anatomical structure of elbow joint to the greatest extent.Accurate correction, and the function of elbow joint recover well after operation, which optimizes the operation procedure, improves the safety and effectiveness of the operation.

BACKGROUND:At present, design of pedicled island skin flap with arterial arch at the superior border of abductor hal ucis muscle mainly depends on clinician’s abstract thinking, which makes it difficult for quantitative analysis and exchange learning, and makes it limited in clinical application. OBJECTIVE:To perform the three-dimensional reconstruction of pedicled island skin flap with arterial arch at the superior border of abductor hal ucis muscle, and to establish visible models of the flap. METHODS:One fresh adult cadaver specimen was col ected and perfused with the mixture of lead oxide-gelatine through anterior tibial artery and posterior tibial artery, and then received spiral CT scanning. The branch of anterior medial mal eolus artery, medial tarsal artery and the superficial branch of medial plantar artery as wel as its anastomotic branch were reconstructed with mimics10.0 software, in order to observe the constitute, track-way and distribution of arterial arch at the superior border of abductor hal ucis muscle. According to the feature of anatomical structures, reconstruction was conducted in skin and vessel to obtain three-dimensional arterial skin flap structure. RESULTS AND CONCLUSION:The layers of three-dimensional reconstruction were distinctively displayed, and the arterial arch at the superior border of abductor hal ucis muscle was displayed wel with strong physical sense and clear relationship between skin and arteries. The morphology of various anatomical structures could be observed clearly on the three-dimensional reconstruction image, especial y the formation of arterial arch at the superior border of abductor hal ucis muscle and its surface projection. Three-dimensional reconstruction of pedicled island skin flap with arterial arch at the superior border of abductor hal ucis muscle may provide great value for basic research, clinical experiments and surgical planning, and provide visual reference for the individual operation design and can be able to enhance the successful rate of surgery.

ObjectiveTo explore the molecular mechanism of neural tube defects (NTDs) caused by hyperglycemia and thiadiazole and the antagonistic effect of taurine MethodsThe pregnant mice were divided into hyperglycemia groups, thiadiazole group, taurine groups and control groups The mRNA and the protein of Pax3 or Cx43 gene were detected respectively by reverse transcription-polymerase chain reaction assay and immunohistochemical method ResultsAs compared with mice treated by thiadiazole-stomach-perfusing, NTDs were significantly increased from mice treated with glucose-injection when blood glucose levels were ≥ 13 4 mmol/L Elevated glucose and thiadiazole could cause changes in Pax3 and Cx43 expression Hyperglycemia had stronger developmental toxicity on mice embryos Expression of Pax3 (mRNA 0 97?0 20, protein 0 11?0 02) in hyperglycemia group was significantly decreased, while expression of Cx43 (mRNA 7 05?1 63, protein 0 94?0 05) was significantly increased, and the relationship of dose-effect was demonstrated In the thiadiazole group, the expression of Cx43 (mRNA 6 96?0 73, protein 0 92?0 12) was significantly stronger than control groups, but there were no significant differences in expression of Pax3 between thiadiazole and its control groups Both of their teratogenicity could be antagonized by taurine ConclusionsThis study suggests that congenital malformation associated with diabetic pregnancy is caused by disruption of regulatory genes,Pax3 and Cx43 expression in embryo in response to elevated glucose Thiadiazole can only disturb the regulation of Cx43 gene causing NTDs Taurine can correct the disruption caused by the two teratogens
