1.The application value of CT and MRI on children’s vein of Galen malformation

Objective To investigate the clinical application value of CT and MRI on the children’s vein of Galen malformation (VGAM).Methods CT and MRI images of 10 children cases with VGAM were analyzed retrospectively.Results CT and MRI find-ings:10 cases with dilatation of the vein of Galen,8 cases with straight sinus dilation,6 cases with dilation of confluence sinus,and 6 cases with other sinus dilatation.Blood supply vessels:basilar artery in 6 cases,posterior cerebral artery in 2 cases,anterior cerebral artery in 1 case and other vascular in 1 case.Complications:bleeding in 4 cases,thrombosis in 2 cases,and calcification in 2 cases. Conclusion CT and MRI can confirm the diagnosis of children’s VGAM,and abnormal blood-supply vessels.CT has advantages of observation of calcifications,and MRI has advantages of observation of hemorrhage and thrombosis.
3.Determination of Lead in Air With Hydride Generation Atomic Fluorescence Spectrophotometry

Objective To establish a method to determine lead in air. Methods The filter film used to collect the lead in air was digested with acid followed by adding mixed solution of potassium ferricyanide,oxalic acid and hydrochloric acid, then double ways atomic flurescence spectrophotometry was employed to determine lead. Results The optimal concentration of hydrochloric acid, potassium ferricyanide and oxalic acid were 0.12 mol/L, 8 g/L and 0.4 g/L respectively. The detection limit of lead was 1 ?g/L, the relative standard deviation was 2.5%. The recovery rates ranged from 90% to 95%, the linar range was 10-80 ?g/L. Conclusion This method was simple, rapid, accurate and sensitive. It was suitable for determining lead in air.
5.Determination of thiodiglycolic acid in urine with gas chromatography-mass spectrometer.

OBJECTIVETo establish a rapid determination method with gas chromatography-mass spectrometer (GC-MS) for thiodiglycolic acid (TDGA), a vinyl chloride (VCM) biomarker.
METHODSA high- sensitivity determination method was established using a moderate methyl esterification instead of methyl esterification of highly toxic diazo reaction.
RESULTSThe standard curve regression linear equation of the method was: y=8460.5x-4758.2, the linear coefficient was 0.999 7, the minimum quantity concentration was 2.0 µg/L, the range of precision value was 0.81%-2.38%, and the average recovery of standard addition was 99.0%-102.9%.
CONCLUSIONThis method reduces the risk of traditional methyl esterification, improves the determination sensitivity compared with the GC-FPD method, and meets the determination requirement of TDGA.

6.Influence of previous cerebral Infarction on postoperative recovery in patients undergoing cornary artery bypass grafting

Objective To study the postoperative recovery in patients undergoing coronary artery bypass grafting (CABG)with a history of cerebral infarction.Methods From January 2007 to December 2007,112 patients undergoing elective off-pump coronary artery bypass grafting(OPCABG)with a history of cerebral infarction were studied retrospectively.Each of the 112 patients was matched with 1 control patient who had no cerebral infarction history and were undergoing an elective OPCABG either immediately before or immediately after the study patient,in most cases by the same surgeons.Preoperative characteristics,anesthesia time,awakening time,extubation time,ICU stay,hospital stay,reintubation,hospital mortality,postoperative confusion and stroke were compared in the two groups.Results There were no significant differences between the study and control Patients in the most preoperative characteristics,only the incidence of hypertension and diabetes mellitus were higher than those in control group,P<0.05(78 of 112,69.6% versus 61 of 112,54.5%;52 of 112,46.4% versus 26 of 112,23.2%).The study group took longer to awaken,extubate and stay in ICU and hospital than the control group,P<0.01[(6.4±4.0)h versus(4.6±2.7)h,(19.6±9.7)h versus(16.8±5.5)h,(27.3±16.8)h versus(22.1±10.6)h,(23.4±10.0)d versus(19.5±5.6)]d.The incidence of confusion,stroke and reintubation after operation among the study group was higher than those in control group,P<0.05(17 of 112,15.2% versus 4 of 112,6.3%;8 of 112,7.1% versus 1 of 112,0.9%;8 of 112,7.1% versus 1 of 112,0.9%),RR and 95% CI were 4.83(1.57~14.86)、8.54(1.05~69.45)and 8.54(1.05~69.45).There were no significant differences between the study and control patients in the anesthesia time and hospital mortality.Stroke after the operations was found in 8 of 112 study patients and included cerebral infarction in 6 and transient ischemic attack(TIA)in 2.The 8 patients included new stroke in 7 and worsening of old stroke in 1 without new abnomalities on head computed tomography.1 of 8 patients had early stroke(≤24 hours after operation)and others had late stroke(>24 hours).Conclusion The patients with previous cerebral infarction undergoing off-pump coronary artery bypass grafting took longer to awaken,extubate,stay in ICU and hospital.These patients were more likely to have reintubation,confusion and stroke after operation than those patients with no cerebral infarction history.
7.The differential diagnosis value of detection of platelet specific autoantibody on idiopathic thrombocytopenic

Objective To detect the changes and clinical significance of the expression of anti‐GPⅡb/Ⅲa and anti‐GPⅠb/ⅠX on anti‐secreting B cells in patient with thrombocytopenia .Methods Expression of anti‐GPⅡb/Ⅲa and anti‐GPⅠb/ⅠX specific autoantibodies in thrombocytopenia were tested with (CBA) .Results There were significantly more circulating anti‐GPⅠ b/ⅠX and anti‐GPⅡb/Ⅲa antibody‐producing B cells in primary ITP(P<0 .05) for all comparisons .For diagnosing primary ITP ,the an‐ti‐GPⅠb/ⅠX had 43% sensitivity and 89% specificity ,whereas the anti‐GPⅡ b/Ⅲ a had 86% sensitivity and 83% specificity . When two tests were combined ,the sensitivity improved to 90% without a reduction in specificity .Conclusion The assay for detec‐ting anti‐GPⅠb/ⅠX is useful for identifying patients with ITP ,but its utility for diagnosing ITP is inferior to the anti‐GPⅡb/Ⅲa assay .
8.Clinical significance of serum calcium-binding protein A9(S100A9)levels in patients with rheumatoid ar- thritis

Objective To investigate the serum level of calcium-binding protein A9(S100A9)in patients with rheumatoid arthritis and explore its potential clinical significance. Methods The serum level of S100A9 was measured by ELISA in 79 rheumatoid arthritis (RA) patients and 20 healthy controls (HC). The correlation of S100A9 level and relevant clinical parameters were analyzed. Results The serum level of S100A9 was significantly increase in RA patients than those in HC. The serum level of S100A9 was higher in high disease activity than that in moderate and low disease activity in the RA group. There was a positive correlation in serum S100A9 level and DAS28 score,Rheumatoid factor,tender joint count and swollen joint count. Conclusion The serum level of S100A9 increases in RA patients and correlates with RA activity.
9.Clinical Observation of Automatic Ventricular Capture Management Confirmed by Paced Depolarization Integral Evoked Response Detection

Objective: To observe the automatic ventricular capture management (VCM) conifrmed by paced depolarization integral (PDI) evoked response detection via the follow-up study in patients with Zephyr5826 pacemaker implantation. Methods: A total of 102 relevant patients were enrolled. In order to conduct PDI calculation, pacemakers were set by bipolar sensing and bipolar pacing at immediately after implantation. VCM functions were observed at 1 day, 7 days and 1 month, 3, 6, 12 months after implantation, the ventricular threshold by VCM test and manual test were compared. The symptoms of pectoralis major stimulus, diaphragm stimulus and palpitation were observed in all follow-up patients. Results: There was 1 patient died by MI at 1 month after Zephyr5826 pacemaker implantation, the rest 101 patients were followed-up for 12 months. VCM function was successfully turned-on at immediately after implantation in all patients, no pectoralis major stimulus and diaphragm stimulus occurred. VCM function was turned-off in 6/101 (5.9%) patients at 7 days after implantation due to intolerable palpitation caused by daily automatic VCM, instead they received manual test at follow-up visit. The coincidence rate of ventricular thresholds between VCM test and manual test were 100%. Ventricular pacing output voltage by VCM was (0.99 ± 0.48) V,n=608. Compared with regular pacing output voltage (2.5V, 0.4ms), VCM function may save 84% of energy consumption; compared to high pacing output voltage (3.5V, 0.4ms), VCM may save 92%. Loss of ventricular capture and poor sensation were not found by ECG and 24 h dynamic monitoring. Conclusion: Zephyr5826 pacemaker may conduct bipolar pacing and scanning with VCM function, it can be effectively and safely operated by low energy output. A few patients may not use VCM function due to intolerable palpitation.
