3.Radiographic appearance of esophageal intramural pseudodiverticulosis

Objective Our purpose was to improve the konwlege and diagnosis level of esophageal intramural pseudodiverticulosis(EIPD).Methods All 3 cases were underwent single and double contrast esophagograms.Analyze the findings on esophagography in 3 cases.Results Multiple small tiny cyst like outpouchings with a narrow neck extending outward esophageal wall were found in all of the 3 cases, the length range of outpouchings diameter was 1-4 mm,and the length range of neck was 1-2 mm,the pseudodiverticula had a diffuse distribution in 2 cases,and segmental distribution on the middle and the lower esophagus in the other case.Some necks of pseudodiverticula incline to stomach with 30?-45? in 2 cases;and intramural tracking was found in 1 case,the length range of tracking was 5-10 mm;esophageal stenosis in esophagogastic junction and reflux esophagitis was found only in 1 case;1 case underwent endoscopy and ostiums of pseudodiverticula were found;biopsy showed submucosal chronic inflammation surrounding the neck of pseudodiverticula,squamous metaplasia of the epithelium in both the neck and outpouchings.Conclusion EIPD is a rare benign disorder characterized by dilation of the submucosal glands.The value of esophagography is to distinguish sublimes ulcer caused by esophagitis and esophageal fenestrate from EIPD.The characteristic radiographic appearance is numerous intramural esophageal contrast-filled cystiform outpouchings,and some necks incline in the direction of stomach.When the typical appearance found on barium studies,the diagnosis of EIPD can be made.
4.Influence of previous cerebral Infarction on postoperative recovery in patients undergoing cornary artery bypass grafting

Objective To study the postoperative recovery in patients undergoing coronary artery bypass grafting (CABG)with a history of cerebral infarction.Methods From January 2007 to December 2007,112 patients undergoing elective off-pump coronary artery bypass grafting(OPCABG)with a history of cerebral infarction were studied retrospectively.Each of the 112 patients was matched with 1 control patient who had no cerebral infarction history and were undergoing an elective OPCABG either immediately before or immediately after the study patient,in most cases by the same surgeons.Preoperative characteristics,anesthesia time,awakening time,extubation time,ICU stay,hospital stay,reintubation,hospital mortality,postoperative confusion and stroke were compared in the two groups.Results There were no significant differences between the study and control Patients in the most preoperative characteristics,only the incidence of hypertension and diabetes mellitus were higher than those in control group,P<0.05(78 of 112,69.6% versus 61 of 112,54.5%;52 of 112,46.4% versus 26 of 112,23.2%).The study group took longer to awaken,extubate and stay in ICU and hospital than the control group,P<0.01[(6.4±4.0)h versus(4.6±2.7)h,(19.6±9.7)h versus(16.8±5.5)h,(27.3±16.8)h versus(22.1±10.6)h,(23.4±10.0)d versus(19.5±5.6)]d.The incidence of confusion,stroke and reintubation after operation among the study group was higher than those in control group,P<0.05(17 of 112,15.2% versus 4 of 112,6.3%;8 of 112,7.1% versus 1 of 112,0.9%;8 of 112,7.1% versus 1 of 112,0.9%),RR and 95% CI were 4.83(1.57~14.86)、8.54(1.05~69.45)and 8.54(1.05~69.45).There were no significant differences between the study and control patients in the anesthesia time and hospital mortality.Stroke after the operations was found in 8 of 112 study patients and included cerebral infarction in 6 and transient ischemic attack(TIA)in 2.The 8 patients included new stroke in 7 and worsening of old stroke in 1 without new abnomalities on head computed tomography.1 of 8 patients had early stroke(≤24 hours after operation)and others had late stroke(>24 hours).Conclusion The patients with previous cerebral infarction undergoing off-pump coronary artery bypass grafting took longer to awaken,extubate,stay in ICU and hospital.These patients were more likely to have reintubation,confusion and stroke after operation than those patients with no cerebral infarction history.
5.Adjustment principle and optimizing methods of CR image processing parameters

Objective To study the adjustment principle and optimizing methods of the image processing parameters of Computed Radiography(CR), including the image density, contrast degree, tissue equalization, spatial frequency, LUT curve,etc. Methods Konica REGIUS MODEL170 CR was applied to adjust various image processing parameters on its CS-1 console. Results Images of all parts of body could be optimized and improved. Conclusion High-quality medical images are available only if the adjustment methods of image processing parameters are completely comprehended and reasonably applied.
6.Epidemiological survey of attention-deficit hyperactivity disorder in pupils of urban districts in Zhenjiang

Objective:To study the epidemiology of attention-deficit hyperactivity disorder(ADHD) in pupils of Zhenjiang urban districts. Methods: Questionnaire was designed and employed for parents of 3 698 pupils at schools in Zhenjiang urban districts. ADHD was diagnosed according to the international DSM-Ⅳ scale. Differences were determined by Qi-square test and multiple regression analysis. 7.EFFECTS OF VITAMIN C ON THE ELECTROPHYSIOLOGICAL PROPERTY OF MYOCARDIAL CELLS OF THE RATS FED WITH LOW SELENIUM AND HIGH CADMIUM FODDER

Objective: To observe the effects of vitamin C(VC)on the electrophysiological property of myocardial cells of the rats fed with low selenium and high cadmium fodder. Method: Eight groups of rats were fed separately with: normal (2 groups), high Cd, high Cd+high VC, low Se+high Cd, low Se+high Cd+high VC, high Se+high Cd and high Se+high Cd+high VC fodder for 14 w. With intracellular microelectrode technique, the electrophysiological changes of rats were observed. Results:Compared with the control, there were no changes in high Cd+high VC group and in high Se+high Cd+high VC group. However,in high Cd group, RP and APA decreased, APD50 and APD90 prolonged; in low Se+high Cd group ,RP and APA decreased, APD50 and APD90 prolonged significantly; in low Se+high Cd+high VC group and high Se+high Cd group, though RP still decreased, no change had been detected on APA and APD. Conclusion: Se deficiency and simultaneous Cd overabundance may change the electrophysiological properties of myocardial cells of the rats significantly. However, VC may protect myocardial cells against the injury effectively.

Objective:To elucidate the etiology of the impairment of myocardial cells in Keshan disease. Methods:With intracellular microelectrode technique, we observed the electrophysiological changes of the eight groups of rats fed separately:normal, low Se, low Se+high VC, low Cr, low Cr+high VC, low Se+low Cr and low Se+low Cr+high VC fodders. Results:Compared with the control, there was no change in low Cr group. However, in low Se group, RP and APA decreased, APD 50 and APD 90 prolonged. In low Se+low Cr group, APA decreased, APD 50 and APD 90 prolonged significantly. In low Se with high vitamin C groups, though RP still decreased, no change had been detected on APA and APD. Conclusion:It is suggested that Se and Cr deficiency may jointly change the electrophysiological properties of myocardial cells significantly. However, vitamin C may protect myocardial cells against the injury effectively.
10.Effects of triptonoterpene methyl ether on proliferation and apoptosis of human gastric adenocarcinoma AGS cells

Aim Toinvestigatetheeffectsoftriptonot-erpene methyl ether ( TME ) , a diterpene derived from the medicinal plant Triptergium wilfordii, on human gastric cancer AGS cell proliferation inhibition and ap-optosisinducedinvitro.Methods MTTassaywas used for screening tumor spectrum and detecting the vi-ability of AGS cells and normal human gastric epitheli-al cells GES-1 . Cell morphology was observed by light microscopy and AO / EB staining. Flow cytometry was used to detect cell apoptotic rate and cell cycle. JC-1 staining and fluorescence probe DCFH-DA were em-ployed to detect the changes of mitochondrial mem-brane potential and reactive oxygen species ( ROS ) . The effect of inhibiting AGS clonogenic survival was as-sayed by the method of plate clone formation. Western blot was used to analyse the expression of caspase-3 , caspase-8,Bcl-2andBax.Results MTTresults showed that TME exhibited significantly higher cytotox-icity to gastric cancer AGS cell line than to noncancer-ous cell line GES-1. IC50 for AGS of 48 h treatment was 23 . 85 μmol · L-1 . TME significantly inhibited colony formation and caused morphological changes in AGS cells. Annexin V-FITC / PI double staining showed the apoptotic rate increased. DCFH-DA stai-ning showed TME resulted in an increase in intracellu-lar ROS levels. Mitochondrial membrane potential de-creased after TME treatment. Western blot results showed that TME increased the proportion of Bax /Bcl-2 , with the activation of caspase-8 and caspase-3 . The broad-spectrum caspase inhibitor z-VAD-fmk pre-treatment reduced the expression of caspase-8 and caspase-3. TME enabled AGS cell cycle arrest in G0/G1phase.Conclusion TMEpossessespotenttumor selected toxicity and can induce apoptosis of AGS cells through cell cycle arrest, which is associated with Bcl-2 protein family.
