Objective To assess the diagnostic value of Wells score and D-Dimer detection in acute pulmonary embolism. Methods Forty-two suspected cases of pulmonary embolism were collected from March 2012 to September 2014 Then Wells score and D-Dimer detection were applied to confirm the disease. Results Of the total, 26 patients were diagnosed by computed tomographic pulmonary arteriography (CTPA). There were 7, 24 and 11 suspected cases which had low, medium and high Wells score respectively. Among them, 1, 15, and 10 cases were diagnosed by CTPA. The diagnosis rates were 14.2% (1/7), 62.5% (15/24) and 90.9%(10/11) respectively. The D-Dimer level in pulmonary embolism group was significantly higher than that in non-embolism group(P < 0.01). The D-Dimer level of cases with medium or high Wells score was higher than those with low Wells score (P < 0.05). Conclusion Wells score combined with D-Dimer is convenient, accurate and safe for predicting acute pulmonary embolism.

The contracted nose is a unique entity that follows rhinoplasty in the Asian patient, which is mainly related to infection, graft implant, multiple invasive nasal repair operations, patients’ own inflammatory reaction and basic conditions, and is a complication that seriously affects the nasal aesthetics and functional characteristics of patients. Comprehensive understanding of the causes, mechanisms and repair method of contracture nose is helpful for its prevention and treatment. In view of the causes and mechanisms of capsular contracture, this paper reviews the research progress in improving the aesthetic and functional characteristics of patients from the three levels of nasal mucosa, cartilage scaffold and skin, as well as the adjuvant treatment of skin contracture release.

BACKGROUND:Physiotherapy is very important for the recovery after anterior cruciate ligament reconstruction. In recent years, many doctors are optimizing the physical rehabilitation program after anterior cruciate ligament reconstruction. However, there is still a lack of efficient rehabilitation training after anterior cruciate ligament reconstruction. OBJECTIVE:To investigate the effect of vibration therapy combined with suspension training on movement and knee joint function after anterior cruciate ligament reconstruction. METHODS:A total of 80 patients undergoing first unilateral anterior cruciate ligament reconstruction at the Affiliated Sport Hospital,Shanghai University of Sport were randomly divided into vibration therapy group(n=40)and vibration therapy+suspension training group(n=40).In the vibration therapy group, vibration therapy(10 minutes each, once a day,6 times per week)was performed at the 13th week after anterior cruciate ligament reconstruction.Patients in the vibration therapy+suspension training group were treated with vibration therapy(10 minutes each, once a day,6 times per week)and suspension training(twice a week)at the 13th week after anterior cruciate ligament reconstruction.Training in each group was performed for 8 weeks.Knee joint function was evaluated by knee joint Lysholm score before and 8 weeks after training. The symmetry index was evaluated by the isokinetic muscle strength evaluation training system. The balance test system was used to evaluate the average trace error difference of the bilateral multi-axes. RESULTS AND CONCLUSION:Compared with those before training, the knee Lysholm score and the knee extension and flexion symmetry indexes increased(P<0.05),and the average trace error difference decreased after training(P<0.05).Compared with the vibration therapy group, the knee Lysholm score in the vibration therapy+suspension training group increased(P<0.05),the knee extension and knee flexion symmetry index increased(P<0.05),and the average trace error difference decreased(P<0.05).To conclude, compared with vibration therapy training alone, vibration therapy combined with suspension training can significantly improve knee joint function, increase muscle strength and symmetry, and improve balance stability in patients undergoing anterior cruciate ligament reconstruction.

Objective:To investigate the clinical efficacy of fixed-bearing unicompartmental knee arthroplasty (UKA) in the treatment of anteromedial knee osteoarthritis or spontaneous osteonecrosis of the femoral condyle. Methods:Between June 2017 and June 2018, 62 consecutive patients were treated at Department of Joint Surgery, Fuzhou Second Hospital Affiliated to Xiamen University for anteromedial knee osteoarthritis or spontaneous osteonecrosis of the femoral condyle using a fixed-bearing UKA system. They were 27 men and 35 women, aged from 58 to 85 years (average, 69.8 years). There were 58 cases of anteromedial knee osteoarthritis and 4 cases of spontaneous osteonecrosis of the femoral condyle. The courses of disease ranged from 2 to 10 years (average, 5.1 years). Medial unicompartmental knee arthroplasty was performed for all the patients on the fixation platform SLED. Recorded were the hip-knee-ankle angle (HKA) of the patients 2 days after operation, Oxford knee score (OKS) and American Knee Society score (KSS) at the final follow-up and post-operative complications. Results:The 62 patients were followed up for 9 to 21 months (average, 15.6 months). Tibial plateau fracture occurred in one case who eventually obtained bony union after open reduction and internal fixation. Another case had a wound healing problem which responded to drainage and debridement with prosthesis preservation. Another case presented prepatellar pain which was improved after physical therapy. In the 62 patients, the HKA 2 days after operation (172.27°±1.61°) was significantly improved compared to the preoperative value (177.79°±1.32°), the OKS at the final follow-up (19.8±2.7) significantly lower than the preoperative value (50.2±3.1), and the KSS at the final follow-up (89.7±2.6) significantly higher than the preoperative value (49.6±5.2) (all P<0.05). Follow-ups revealed no cases of joint infection or prosthetic loosening. Conclusion:Fixed-bearing unicompartmental knee arthroplasty can lead to fine short-term outcomes for anteromedial knee osteoarthritis or spontaneous osteonecrosis of the femoral condyle.
