1.Effect of gadolinium chloride hexahydrate on rat hepatocellular mitochondria du ring liver cold ischemia storage

Objective To study the effect of gadolinium chl oride hexahydrate (GDCL3) on the structure and lipid peroxide (LPO) of rat hep atocellular mitochondria during liver cold ischemia storage. Methods Forty Wistar rats were randomized into 8 gr oups ( 5 in each group). Group 1~4 served as control. The other 4 groups were p retreated with GDCL3 (7mg/kg). The ultrastructures of hepatocellular mitocho ndria were observed by electron microscope and LPO of hepatocellular mitochondri a were investigated. Results LPO o f hepatocellular mitochondria in group 1h, 2h, 3h pretreated with GDCL3 for 1、 2、 3 h were 1.0±0.3、1.7±0.2、2.0±1.0, LPO was suppressed sign ificantly compared with control group (2.2±1.0、2.8±1.0、3.5±1.0、 P<0.05). With time prclonging of liver storage, the damage of hepatocellular mitochondria ultrastructure became more severe and LPO of hepatocellular mitoch ondria increased significantly. Pretreated with GDCL3, the damage of hepatocel lular mitochondria ultrastructures were lessened and the elevation of LPO was s uppressed significantly (1 hour、 2 hours、3 hours,P<0.05). Conclusion Mitochondria damage is one of the sensitive characters of liver cold ischemia injury, GDCL3 inhibits rat hepatoc ellular injury during liver cold ischemia storage.
2.Full reconstruction of Ⅳ to Ⅵ-degree finger defect

ObjectiveTo introduce the clinical application of full reconstruction for 72 cases of Ⅳ to Ⅵ-degree finger defect.MethodsFrom December 1998 to December 2010, sixty-three cases (85 fingets) of Ⅳ-degree finger defect, thirty-three cases (49 fingers) of Ⅴ-degree finger defect and 17 cases (23fingers) with Ⅵ-degree finger defect were applied full reconstruction. The procedures of full reconstruction of Ⅳ to Ⅴ-degree finger defect were as follows:Harvest part of nail, skin which includes some skin harvested from dorsal and palmar metatarsal to ensure the length of the reconstructed finger,and dorsal part of distal phalanx from hallux to form a composite flap, which constitute the contour of a finger, and harvesting interphaalangeal joint from the second toes to reconstruct the proximal interphalangeal (PIP) joint. Bone transplantation from the iliac bone to the distal (for Ⅳ-degree and light Ⅴ-degree defect) or both proximal and distal (for severe Ⅴ-degree defect) stump of the reconstructed PIP joint was needed to get to an appropriate length.On the basis of the treatment of Ⅴ-degree defect, reconstruction of Ⅵ-degree finger defect was to harvest one more joint: the metatarsophalangeal joint of the second toe, and connect it with the proximal iliac bone rod.ResultsAbout half of the cases were conserved of 4 toes, and the donor wound of halluxes were covered with the composite flaps (composed of nail, skin) harvested from the second toes which had been sacrificed.The other cases were conserved of all the 5 toes,and the donor wound of halluxes were covered by free flap transplantation.The second toes were reconstituted by bone transplantation from the iliac bones.All of the 157 fingers survived completely, and 75 fingers underwent second-stage plastic surgeries. Sixty-seven fingers underwent second-stage tenolysis surgeries.Follow-ups 7 months to 11 years after surgery, and all the reconstructed fingers had realistic configurations, and the two-point discrimination of the finger pulps ranges from 5 mm to 12 mm. Dorsal extension of the PIP joints were -10°~10°, flexion of the PIP joints range from 55° to 85°,and the average was 76°. ConclusionThe full reconstruction is an ideal alteration for Ⅳ to Ⅵ-degree finger defect reconstruction for the realistic configuration and ideal function of the reconstructed fingers.The one disadvantage of the full reconstruction is that the surgery is much more complex.
3.Expressions and clinical significances of cyclooxygenase-2,p53 in osteosarcoma

[Objective]To observe the expressional variability and correlation of cox-2 and p53 protein between osteosarcoma and osteochondroma and explore their clinical significance.[Method]Sixty Specimens divided into 2 groups were taken from the department of pathology,Third Hospital of Hebei Medical University during 1998~2004.Among them,40(24 male and 16 female)aged 8~38(18.68?6.47)at time of therapy were from cases of osteosarcoma excision;20(13 male and 7 female)aged 9~27(17.95?4.77)from benign lesion of bone(osteochondroma)operation.Immunohistochemistry staining(Streptavidin-Peroxidasc,S-P method)was performed to detect the expressions of cox-2 protein and p53 protein in 40 cases of osteosarcoma.The criteria of positive expression was that brown granule was found in cytoplasm(cox-2)or in nucleus(p53).The percentage of cox-2 or p53 positive expression in five high power fields was cunted randomly in each case.Positive(-):positive oncocytes account 50%.Software SAS v6.12 was used in data analysis.The difference of cox-2 and p53 expressions between osteosarcoma and osteochondroma was analyzed with Chi-square Test.Spearman method was used,with the standard ?=0.05,to analyze the relationship between cox-2 and p53 expression among osteosarcoma.[Result]The positive rate of cox-2 and p53 expressions were 67.5% and 55.0% in osteosarcoma,there was noticeable distinction between osteosarcoma and osteochondroma,most of them was high.There was a positive correlation between them,rs=0.58,P
4.Early clinical protocols for traumatic bilateral femoral shaft fracture

Objective To investigate the mechanism,injury characteristics,and early interventions of bilateral femoral shaft fracture.Methods Twenty-five cases of bilateral femoral shaft fracture treated between December 2009 and June 2012 were enrolled in the study.Eighteen patients were males and 7 were females with mean age of 31.2 years (range,17-40 years).Nineteen cases were injured due to traffic accidents and 6 due to the hit by heavy objects.There were 47 sides with closed fracture and 3 sides with open fracture.Intramedullary nailing was performed at 32 sides and plate fixation was used at 18 sides.ISS,blood routine,some blood biochemical items,blood or plasma use,and hospitalization days were measured.Results ISS was (14.6 ± 5.2) points.Preoperative readiness time and hospital stay were (10.1 ± 3.7) days and (28.9 ± 8.4) days respectively.After hospitalization,measured values were (3.0 ± 0.4) × l012/L for red blood cells,(86.0 ± 13.5) g/L for hemogIobin,(47.0 ± 10.6) g/L for total protein,(31.4 ± 6.5) g/L for albumin,(124.1 ± 95.1) U/L for aspartate aminotransferase,(114.2 ± 107.1) U/L for alanine aminotransferase,and (1 848.7 ± 1 044.8) U/L for creatine kinase with significant differences from the reference ranges (P < 0.05 or 0.01).Amount of blood transfused for 23 cases was (1 444.4 t 726.5) ml and plasma transfused for 19 cases was (305.6 ± 98.3) ml before operation.Volume of intra-and post-operative blood transfusion was (2 005.7 ± 1 153.1) ml.Conclusions Stabilization of vital signs predominates the early treatment of bilateral femoral shaft fracture.After the condition is stable,intramedulary nailing or plating can be performed concurrently.
5.Application of autotransfusion hemodilution in iaryngectom

Objective:To study the application and clinical significance of autotransfusion hemodilution inlaryngectomy. Method:Autotransfusion hemodilution was adopted in 30 patients with Ⅲ or Ⅳ stage laryngealcarcinoma during operation as the experimental group, and the blood transfusion only was adopted in the patientswith excessively blood loss. The other 30 patients with the same stages of disease and the same kinds ofoperation were performed as the control group, without autotransfusion hemodilution and the blood transfusionwas adopted if necessary. Result: Blood transfusion was necessary to be performed only in 8 patients of theexperimental group, on the contrary,it was necessary in 19 of 30 patients of the control group (P<0.01).There were no complications during the operations , and the postoperative complications were no significance (P>0. 05) between two groups. Conclusion:The application of autotransfusion hemodilution may be of help to aplenty of patients with laryngeal carcinoma in advanced stage to keep off the blood transfusion duringlaryngectomy.
6.Treatment of 225 patiens with chronic infective wounds

Objective To explore the treatment of chronic infective wounds .Methods A retrospective analysis of clinical data in 225 patients were admitted from 2000 to 2010 .Results (1) They were mainly traumatic ulcers ,pressure ulcers ,postoperative ul-cers ,diabetes ulcers ,vascular ulcers in the group ,accounted for 80 .4% (181/225) .(2) Bacterial culture positive rate was 87 .1%(196/225) ,a total of 46 kinds with 342 pathogens were cultured ,gram-positive bacteria 40 .6% (139/342);gram-negative bacteria 57 .6% (197/342);Fungi 1 .8% (6/342) .The main pathogens were S .aureus(52) ,E .coli(43) ,P .aeruginosa(44) ,Klebsiella .SPP (27) ,which were highly resistant to penicillin ,erythromycin ,ampicillin ,gentamicin ,cotrimoxazole and the multidrug resistance rate was 37 .1% (127/342) .Chronic wounds and multidrug resistant bacteria showed rapidly increasing trend from 2007 .(3) 201 pa-tients with topical antibiotic treatment ,208 patients(49 patiens underwent re-operation)underwent operations to close wounds ;213 patiens were recovery ,12 patiens had to leave hospital because economic burden .Conclusion Chronic infective wounds were affect-ed by many factors .emphasizing on debriding ,reasonablechoice ,circulative ,alternate use of antibiotics and wound bed preparation , appling surgery to close wounds in early stage could effectively control wound infection and promote wound healing .
7.Analysis of Pesticides in Blood Specimen by GC/MS with Accelerated Solvent Ex-traction

Objective To develop the accelerated solvent extraction (ASE ) for determ ining pesticides pre-sent in blood sam ples. Methods Pesticides were extracted by ASE with optimized param eters to study recovery rate affected by extraction tem perature, time and agent. GC/MS was used to perform quantita-tive analysis.Results The recovery rates of eight pesticides were 70.6%-92.4%. The coefficient of variation was less than 5.0%. Agood linear relationship was obtained at the concentration range of 0.5-5.0μg/m L . Conclusion The m ethod was fast and sim ple with high recovery rate and good repeatability. It can be applied to analyze pesticides present in the blood specimen.
8.The effects of diammine silver fluoride solution on demineralized dentine adhesion ability

Objective:To investigate the effect of silver diamine fluoride(SDF)on the bonding strength between dentine and glass ion-omer cement(GIC).Methods:1 2 extracted sound molars were prepared into dintine samples and distributed into sound dentine group and demineralized dentine group.According to the treatment methods,the samples in each group were respectively divided into 3 sub-groups:A(control group),B[coated with 38% Ag(NH3 )F2 ]and C(SDF treatment with additional lighting-curing)(n =20).Then a hand-mixed conventional glass ionomer cement Fuji IX was placed on the dentine surface.After 24 h,micro tensile bond strength test and scanning electron microscope (SEM)analysis were conducted.Results:The bonding strength of demineralized dentine was higher than that of sound dentine(P <0.01 ).SDF with additional lighting-curing treated dentine showed a higer bonding strength value than only SDF treated dentin(P <0.01 ).Conclusion:SDF may improve the bonding between dentine and GIC.
9.Application of adjacent flap in reparation of operative defect of malignant nasal cutaneous tumor resection

OBJECTIVE To introduce experiences of cosmetologic reparation after malignant nasal cutaneous tumor resections.METHODS Cutaneous flap was chosen to reconstruct 37 cases of defects after nasal malignant tumor resection according to their area,shape and site,in which basal cell carcinoma were 25 cases,squamous cell carcinoma were 12 cases,respectively.Nasal cutaneous defects rangeed from 0.8 cm?0.8 cm to 5.0 cm?4.0 cm.The flap came from diamond flap 9 cases,from nasolabial island flap 15 cases,from subcutaneous pedicel flap 13 cases,by which to supply directly pulling saturation of flap area. RESULTS All cutaneous flaps of 31 cases survived with primary healing;nasal appearances were good and no malignant tumor recurred after 1-18 months follow up.CONCLUSION Employ local cutaneous flap with color and texture matched to cosmetologic reconstruction of defects after nasal malignant tumor resections are capable of reach satisfied clinical effect.
10.Clinical significance of SKP2 and MRP-1/CD9 expression in glottic carcinoma and adjacent tissues

Objective:To study the expression of SKP2 and MRP-1/CD9 protein in glottic cancer and adjacent tissues,and to analyze its significance for a safe surgical margin.Method:Thirty-eight cases of glottic squamous cell carcinoma were studied for its cancer tissue, tissue 2 mm, 5 mm , and 10 mm away from cancer ,and 10 cases of vocal cord polyp were served as control. SKP2 and MRP-1/CD9 protein were examined by immunoh istochemical method.Result:The positive expression of SKP2 proteins decreased in sequence of polyp mucosa, those adjacent to carcinoma (10 mm, 5 mm, 2 mm ) and cancer tissue, and there was significant difference between them(P<0.05);On the contrary, the positive expression of the MRP-1/CD9 proteins increased in sequence of polypusmucosa, those adjacent to carcinoma (10 mm,5 mm, 2 mm) and cancer tissue,and there was significant difference between them (P<0.05).Conclusion:SKP2 and MRP-1/CD may act as the reference index for judging the biological speciality of LSCC. It is appropriate to regard 5 mm or above 5 mm away from tumors as a safe margin for surgical treatment of glottic carcinoma.
