Objective To investigate the analgesic efficacy of intravenous administration of lidocaine in patients following laparoscopic cholecystectomy. Methods Twenty-five ASA Ⅰ or Ⅱ patients, aged 30-55 yr,weighing 50-70 kg, scheduled for laparoscopic cholecystectomy, were enrolled in this study. Lidocaine 1.5 mg/kg was injected intravenously immediately before anesthesia. Anesthesia was induced with intravenous fentanyl 2 μg/kg, propofol 2 mg/kg and rocuronium 1 mg/kg and maintained with inhalation of isoflurane (end-tidal concentration 1.5%-2.0%) and intermittent iv boluses of rocuronium after tracheal intubation. Lidocaine was infused abdominal intravenously at a rate of 1.5mg·kg-1·h-1 form the end of ofperation to 24 h after operation. The degree of abdominal pain and shoulder pain was assessed with VAS score at 1, 6, 12 and 24 h after operation. The effective analgesia was defined as VAS score of ≤ 3. The adverse reactions were recorded. Results The effective analgesia rate was 100% within 24 h after operation. No adverse reactions occurred. Conclusion Intravenous administration of lidocaine exerts satisfactory analgesia in patients after laparoscopic cholecystectomy.

ObjectiveTo investigate the correlation between the chromosomal abnormalities and prognosis of the myelodysplastic syndrome(MDS)patients, and analyze the effects of treatment. Methods Karyotype analysis of 122 patients according to the international human cytogenetics(ISCN) criteria.Treatment of RA and RAS were mainly dependent on agents to induce differentiation of hematopoietic cells and drugs based.RAEB,RAEB-t,CMML treatment were dependent on low-dose chemotherapy and low-dose combination chemotherapy regimens. The treatments of 64 MDS patients with abnormal karyotype were analyzed and compared with control group, and 58 normal karyotype MDS patients were hospitalized in the same period.ResultsAfter treatments,17 cases gained complete remission among 64 patients with abnormal karyotype MDS patients. The CR rate was 26.6 %.While in control group,30 gained CR in 58 MDS patients with normal karyotype. The CR rate was 51.7 %. Comparing with the CR patients of normal karyotype, the number of patients with abnormal karyotype of CR was significantly lower (x 2 =8.1 3,P < 0.05).Conclusion Karyotype analysis shows important significance in the diagnosis and prognosis of MDS.Karyotype transformation demonstrates differently in the risk of leukemia progress.

Objective To explore the effect of expression of miRNA-21 on bone marrow mesenchymal stem cells (BMSCs). Methods In this study, flow cytometry was used to identify the surface-associated antigens of BMSCs. The 10 μmol/L 5-azacytidine was used to induce BMSCs to differentiate to cardiomyocyte-like cells. Immunofluorescence was used to detect the expression of troponin I (cTnI). The samples were assigned to 3 groups: a blank group, a miRNA-21 mimic group, and a negative control (NC) group. The proliferation of BMSCs was detected by methyl thiazolylte-trazolium (MTT), the apoptosis of BMSCs was analyzed by flow cytometry. Western-blotting was used to identify the expression of cTnI and myod in the BMSCs. Results The proliferation of BMSCs was increased, because of the over expression of miRNA-21. But the apoptotic rate of the BMSCs was slower in the miRNA-21 group, on account of the expression of miRNA-21 was higher than that in the NC group and the CK group. The expression of cTnI in the miRNA-21 group was higher than that in the NC group or the CK group. Conclusion The results suggest that the up-regulation of miRNA-21 enhances proliferation of BMSCs, reduces the apoptosis of BMSCs. miRNA-21 promotes the differentiation of BMSCs, which may pave the way for the treatment directed toward restoring miRNA-21 function for myocardial ischemia.

Objective To analyze the safety and follow-up results of on-pump coronary artery bypass grafting (CABG) for the treatment of multivessel diseases via left anterolateral minithoracotomy. Methods From January 2018 to March 2020, a total of 30 patients including 18 males and 12 females with an average age of 61.3±7.5 years having multivessel coronary heart diseases were treated in our hospital with on-pump CABG via left anterolateral minithoracotomy. Among them, 14 patients had three-vessel diseases and 16 patients had two-vessel diseases. Results There were 29 internal mammary artery-to-left anterior descending bypass grafts harvested in total while the rest were saphenous-vein bypass grafts. The average number of bypass vessels was 2.3±0.5. There was no perioperative death in the whole group, one patient underwent rethoracotomy due to hemorrhage, and one patient suffered acute renal insufficiency. The average time of postoperative tracheal intubation was 16.0±5.8 hours, and the postoperative ICU stay was 30.1±11.5 hours. Twenty five patients were followed up, including coronary CT angiography examinations at 6 months and 1 year after operation. Proximal anastomotic stenosis in one patient and distal anastomosis occlusion in one patient occurred. Conclusion On-pump CABG via left anterolateral minithoracotomy is safe for appropriately selected patients.

Based on ASP.NET framework, The Intelligent Estimated System for Rational Deployment of Medical Equipment (MERDIS) is designed and developed with SQL Server 2012 database and C# language. The system is used to realize the rational deployment suggestions and evaluation of medical equipment in hospitals. The system input the data of hospital medical equipment and clinical pathway into the database, and then feedback the deployment information to users which are calculated by big data information, so as to achieve the purpose of giving rational deployment of hospital medical equipment.
