1.Effect of abdominal breathing combined with application of acupoint massage on relieving laparoscopic abdominal distention

Objective To investigate the effect of abdominal breathing combined with acupoint massage to relieve abdominal distention after laparoscopic surgery.Methods One hundred and eighty patients with abdominal distention after laparoscopic surgery were randomly divided into control group and observation group with 90 patients in each group. The control group was given conventional perioperative nursing and the observation group combined with acupoint massage including Zusanli, Shangjuxu,Xiajuxu,and Neiguanxue based on the use of abdominal breathing to relieve postoperative abdominal distension. Result The abdominal distension of the observation group was significantly lower than that in the control group,the recovery time of intestinal peristalsis and anus exhaust time were significantly shorter than those of the control group after 3 d(all P<0.01).Conclusion Abdominal breathing combined with acupoint massage can relieve abdominal distension of patients after laparoscopic surgery and promote intestinal function recovery.
2.Clinical observation of unfractioned heparin for preventive and therapeutic effects on severe sepsis

Objective To evaluate preventive and therapeutic effects of low dose of unfractioned heparin on severe sepsis. Methods 52 Patients with severe sepsis hospitalized in Intensive Care Units were randomly divided into control group and heparin group. On the basis of conventional therapy, patients with sepsis were given low dose of unfractioned heparin or isovolume physiologic saline by continuous intravenous drip infusion over 5 days. Acute Physiologic and Chronic Healthy Evaluation (APACHE-Ⅱ) score, blood platelet count(BPC), prothrombin time(PT), activated partial thromboplastin time(aPTT), thrombin time(TT), plasma fibrinogen(FDP) and D-dimer levels were assessed on admission, 3rd, 5th day after intervention. Incidence of disseminated intravascular coagulation(DIC) and 28-day mortality between heparin group and control group were compared. Results There were no significant difference in APACHE-Ⅱ, BPC, PT, aPTT, TT, and FDP levels, incidence of DIC, and 28-day mortality between heparin group and control group. The crystal structural data of TACE, MMP-1, MMP-2, MMP-3 and MMP-9 were obtained from PDB database, and then their catalytic domains' properties including conformation, molecular surface hydrophobicity and electrostatic potential were analyzed and compared by using Insight II molecular modeling software. It was found that the conformation and molecular surface hydrophobicity of catalytic domains of TACE and MMPs were not obviously different, but the molecular surface electrostatic potential of catalytic domain of TACE and MMPs had obvious differences. The findings are helpful in the Rational Drug Design of TACE selective inhibitor.
4.Molecular changes of PI_3K-AKT signal pathway in K562 cells treated with simvastatin in vitro and in vivo

Objective To analyze the apoptosis mechanisms of K562 cells in a PI3K-AKT-dependent manner.Methods K562 cells were cultured in vitro for experiments below:the proliferation assay of K562 cells detected by MTT,the analysis of apoptosis rate and cell cycle of K562 cells measured by flow cytometry(FCM),the construction of chronic myeloid leukemia(CML) animal model through the subcutaneous inoculation of K562 cells to 12 BALB/c-nu/nu nude mice,the early apoptosis changes of K562 cells detected by TdT-mediated dUTP nick end labeling(TUNEL) and the in vitro and in vivo changes of N-ras,PI3K,AKT1,IKK-?,NF-?B1 at transcription level detected by RT-PCR.Results Simvastatin inhibited the proliferation of K562 cells and induced their G0/G1 arrest and significant apoptosis.N-ras and most genes of PI3K-AKT signal pathway were expressed differentially in vitro.K562 cells on nude mice could be induced to apoptosis by simvastatin and the apoptotic index increased with the dose accumulation of simvastatin(P=0.000).The differential mRNA expression changes of N-Ras and most genes of PI3K-AKT signal pathway in K562 cells were observed after treatment of simvastatin at different doses(P=0.000 or P=0.003).However,mRNA expres-sion of the genes in PI3K-AKT pathway in vitro differed to that in vivo.Conclusion The differential expression at transcription levels of N-ras and the most genes in PI3K-AKT pathway that are involved in anti-apoptosis in K562 cells,to some degree,indicates that simvastatin can induce the apoptosis of K562 cells in a PI3K-AKT pathway-dependent fashion in vitro.The animal experiments confirm that there are different mechanisms of simvastatin in inducing the apoptosis of K562 cells in vitro and in vivo.
5.Factors affecting the motivation for weight self-management among pregnant women

Objective:
To investigate the motivation for weight self-management and analyze its influencing factors among pregnant women in a tertiary hospital in Hohhot City, Inner Mongolia Autonomous Region, so as to provide insights into body weight management during pregnancy.
Methods:
Pregnant women at ages of 18 years and older that underwent prenatal examinations in a tertiary hospital in Hohhot City from January to March, 2022 were sampled using a convenience sampling method. Subjects' age, height, weight, occupation, residence, monthly household income, history of gestation and childbirth and gestational period were collected, and the motivation for weight self-management was evaluated among pregnant women using a pregnancy weight management protein motivation scale. The factors affecting the motivation for weight self-management were identified among pregnant women using a multivariable linear regression model.
Results:
Totally 969 pregnant women were recruited, including 841 women at ages of <35 years (86.79%), 780 women with an educational level of diploma and above (80.50%), 794 women living in urban areas (81.94%), 729 primiparas (75.23%), 421 women in the third trimester of gestation (43.45%) and 758 women with pre-pregnancy body mass index (BMI) of <25 kg/m2 (78.22%). The mean score of motivation for weight self-management was 127.53±14.60 among the pregnant women. Multivariable linear regression analysis showed that an educational level of high school and below (β′=-0.201), unemployed/self-employed individuals (β′=-0.077), living in rural areas (β′=-0.059), monthly household income of <10 000 yuan (<5 000 yuan, β′=-0.238; 5 000 to 10 000 yuan, β′=-0.169), in the third trimester of gestation (β′=-0.135), pre-pregnancy BMI of 25 kg/m2 and higher (β′=-0.214) and reduced the motivation for weight self-management among pregnant women.
Conclusion
The motivation for weight self-management correlates with gestational period, pre-pregnancy BMI, residence, occupation, educational level and monthly household income among pregnant women.
6.The relationships among organization climate, psychological empowerment and job embeddedness

Objective To explore the relationships among organization climate,psychological empowerment and job embeddedness by using path analysis.Methods A total of 514 clinical nurses from 26 departments in 3 hospitals were recruited by convenience cluster sampling method and investigated with demography questionnaire,Nurses Organizational Climate Scale,Psychological Empowerment Scale and Job Embeddedness Scale.Results The mean scores of organization climate,psychological empowerment and job embeddedness was (3.01±0.46),(3.27±0.44),(3.08±0.39).The predictors of job embeddedness of nurses were resources support,work experience,human resources management,marital status,job title,management support,quality management,and self-egicacy,explained 48.9% of its variance; organization climate,psychological empowerment had direct positive influence on nurses' job embeddedness,explained 33.3% of its variance.Conclusions Improvement and maintenance of sound organizational climate,increase the sense of psychological empowerment of nurses,are effective ways and means to increase the degree of job embedding of nurses.
7.Development of glipizide push-pull osmotic pump controlled release tablets by using expert system and artificial neural network.

The purpose of this study is to develop glipizide push-pull osmotic pump (PPOP) tablets by using a formulation design expert system and an artificial neural network (ANN). Firstly, the expert system for the formulation design of osmotic pump of poor water-soluble drug was employed to design the formulation of glipizide PPOP, taking the dissolution test results of Glucotrol XL as the goal. Then glipizide PPOP was prepared according to the designed formulations and the in vitro dissolution was carried out. And in vivo evaluation was carried out between the samples which were similar to Glucotrol XL and the Glucotrol XL in Beagle dogs. The range of the factors of formulation and procedure, which could influence the drug release, was optimized using artificial neural network. Finally, the design space was found. It was found that the target formulation which was similar to Glucotrol XL in dissolution test could be obtained in a short period by using the expert system. The samples which were similar to Glucotrol XL were bio-equivalent to the Glucotrol XL in Beagle dogs. The design space of the key parameter coating weight gain was 9.5%-12.0%. It could be concluded that a well controlled product of glipizide PPOP was developed since the dissolution test standard of our product was more strict than that of Glucotrol XL.
8.Investigation of six-degree-of-freedom image registration between planning and cone beam computed tomography in esophageal cancer

Objective To explore six-degree-of-freedom (6-DF) registration methods between planning and cone beam computed tomography (CBCT) during image-guided radiation therapy (IGRT) in esophageal cancer.Methods Thirty pairs of CBCT images acquired before radiation and the corresponding planning computed tomography (CT) images of esophageal cancer were selected for further investigation.Registration markers for 6-DF image registration were determined and contoured in those images.The results of registration as well as time cost were compared among different registration methods of bone match, gray value match, manual match, and bone plus manual match.Results Contouring bone and spinal canal posterior to the target volume of esophageal carcinoma as registration marker could make 6-DF registration quick and precise.Compared with manual match, set-up errors of v rotation in bone plus manual match (-0.55° vs.-0.88°, t=2.55, P=0.020), of x-axis and v rotation in bone match (0.12 mm vs.-2.33 mm, t=5.75, P=0.000; -0.35° vs.-0.88°, t=3.00, P=0.007), and of x-axis and w rotation in gray value match (7.20 mm vs.-2.33 mm, t=3.10, P=0.006; -0.10° vs.-0.59°, t=2.81, P =0.011) were significantly different.Compared with manual match, the coincidence rate of bone plus manual match was the highest (85.55%), followed by bone match and gray value match (74.45% and 74.45%).The time cost of each registration method from longest to shortest was:6.00 -10.00 minutes for manual match, 1.00 - 5.00 minutes for bone plus manual match, 0.75 - 1.50 minutes for gray value match, and 0.50 - 0.83 minutes for bone match.Conclusions Registration marker is useful for image registration of CBCT and planning CT in patients with esophageal cancer.Bone plus manual match may be the best registration method considering both registration time and accuracy.
9.Effect and mechanism of liraglutide on proliferation and apoptosis of human pancreatic cancer cells

Objective To investigate the effects and mechanism of glucagon-like peptide-1 ( GLP-1 ) receptor agonist liraglutide on the proliferation and apoptosis of human pancreatic cancer cells. Methods The human pancreatic cancer cell line MIA PaCa-2 was incubated for 24 h with liraglutide at various concentrations (10-1 000 nmol/ L), or with 100 nmol/ L liraglutide for various durations (0-72 h). Cell proliferation was determined by Cell Counting Kit-8 (CCK-8) analysis. RT-PCR and Western blot were used to detect the mRNA and protein expression levels of related genes. Results GLP-1 receptor was expressed in the MIA PaCa-2 cells. Liraglutide suppressed cell proliferation, up-regulated the expression levels of pro-apoptotic protein Bax and down-regulated the expression levels of anti-apoptotic protein Bcl2 in human pancreatic cancer cells in a dose- and time-dependent manner. Meanwhile, liraglutide down-regulated the expression levels of insulin receptor (INSR) and insulin-like growth factor-1 receptor (IGF-1R), and the phosphorylation levels of their downstream signaling proteins Erk1 / 2 and Akt, in a dose- and time-dependent way. Conclusion Liraglutide inhibits proliferation and promotes apoptosis of human pancreatic cancer cells; the process may be mediated via suppressing the expression of INSR and IGF-1R and inhibiting activation of the downstream MEK/ Erk1 / 2 and PI3k/ Akt pathways.
10.Distribution of apocrine sweat glands in axillary region of patients with axillary osmidrosis

Objective To determine the distribution profile of apocrine sweat glands in axillary region of patients with axillary osmidrosis,and to compare their distribution at different sites.Methods Fifteen patients with axillary osmidrosis were enrolled in this study from September to December 2010.All the patients underwent surgical removal of apocrine sweat glands under direct vision.Full-thickness skin tissue measuring 2 mm in width was excised down to the axillary superficial fascia at the incisional surgical sites from five patients.Five points,which were at the center of axillary region (point 1),1 cm away from the center of axillary region (point 2),1 cm inside the edge of axillary hair-bearing area (point 3),the edge of axillary hair-bearing area (point 4),and 1 cm outside the edge of axillary hair-bearing area (point 5),were marked,and dark red,rough granular subcutaneous tissue was obtained at these points in 10 patients with axillary osmidrosis.Results The secretory portion of apocrine sweat glands was mainly distributed in the reticular dennis and superficial subcutaneous adipose tissue,but no apocrine sweat glands were obs erred in the epidermis,dermal papilla or axillary superficial fascia.The distribution profile of apocrine sweat glands was consistent with that of axillary hairs.There were numerous apocrine sweat glands in the center of axillary region,but only a small number at the edge of axillary hair-bearing area,and no apocrine sweat gland was observed at 1 cm outside the edge.The percentage of apocrine sweat gland area at point 1-5 was 74.1%,46.6%,25.3%,2.1%,and 0 respectively,with significant differences between point 1 and 2 (t--29.78,P< 0.01),point 2 and 3 (t--9.76,P< 0.01),point 3 and 4 (t =20.83,P< 0.01),but not between point 4 and 5 (t =1.96,P > 0.05).Conclusions During the surgical treatment of axillary osmidrosis,the removal of apocrine sweat glands should be extended to the reticular dennis and superficial subcutaneous adipose tissue in depth and the edge of axillary hair-bearing area in width,and there is no need to blindly increase the extent of removal.
