1.Approach to the patient with Langerhans cell histiocytosis manifested as central diabetes insipidus and cervical lesions

[Summary] A 19-year-old male patient with central diabetes insipidus complained of polydipsia and polyuria and was found to be accompanied by cervical vertebra lesions.The diagnosis of Langerhans cell histiocytosis was confirmed by laboratory and pathological results.Therefore,it should be alert to consider the possibility of Langerhans cell histiocytosis in a patient with central diabetes insipidus coexisting with the signs of multisystem lesions such as bone disease.
2.Effects of caveolin-1 scaffolding domain peptide on LPS-induced acute lung injury in mice

AIM: To investigate the effects of caveolin-1 (Cav-1) scaffolding domain peptide, cavtratin, on lipopolysaccharide (LPS)-induced mouse acute lung injury and heme oxygenase-1 (HO-1) activity.METHODS: Adult male BALB/c mice were randomly divided into 6 groups (n=8 to 10): control, Antennapedia internalization sequence (AP), LPS, LPS+hemin, LPS+ hemin+cavtratin and LPS+hemin+cavtratin+zinc protoporphyrin IX (ZnPP) groups.After LPS administration for 24 h, the lung pathological changes, the wet/dry weight (W/D) ratio of lung tissues, total cell number in bronchoalveolar lavage fluid and serum lactate dehydrogenase activity were measured.The co-localization of HO-1 and Cav-1 was displayed by immunofluorescence, and the HO-1 activity were detected.The mRNA expression of pro-inflammatory cytokines IL-1β, IL-6, TNF-α, MCP-1 and iNOS was detected by real-time PCR.RESULTS: The mice in LPS+hemin+cavtratin group had the decreased interaction between HO-1 and Cav-1, and the increased HO-1 activity compare with LPS group (P<0.05).Compared with LPS group, the pulmonary damage was attenuated in LPS+hemin+cavtratin group, and the injury indexes, including W/D ratio, total cell number in bronchoalveolar lavage fluid and lactate dehydrogenase activity in the serum, and the mRNA expression of inflammatory cytokines all decreased (P<0.05).HO-1 activity inhibitor ZnPP abolished the above protective effect of cavtratin on the lung tissues with LPS-induced acute lung injury.CONCLUSION: Cavtratin has beneficial effects on the lung with LPS-induced acute injury by restoring the HO-1 activity.
3.Protective effects of methylene blue on ischemia/reperfusion-induced mitochondrial injury in isolated rat hearts

AIM:To study the effects of methylene blue (MB) on myocardial ischemia/reperfusion (I/R)-induced mitochondrial injury in isolated rat hearts.METHODS:Spragure-Dawley (SD) rats were divided into 3 groups randomly (n=6): control group, I/R model group and MB treatment group (IR+MB group).The isolated rat hearts were prepared and set up to Langendorff perfusion.The rats in I/R+MB group received MB (2 mg/kg) by intraperitoneal injection 2 h before operation.The hearts in control group were perfused with K-H solution for 110 min consecutively.The hearts in I/R group and I/R+MB group were in equilibrium for 20 min, following by 45 min of global ischemia, and then reperfused for 60 min.The heart rate (HR), left ventricular developed pressure (LVDP), left ventricular pressure maximum change rate (±dp/dtmax) and left ventricular end-diastolic pressure (LVEDP) were recorded.The perfusate was collected to determine the activity of creatine kinase-MB (CK-MB) and lactate dehydrogenase (LDH).The contents of reactive oxygen species (ROS), malondialdehyde (MDA) and adenosine triphosphate (ATP), and the activity of superoxide dismutase (SOD) in the myocardial tissues were all determined.Histopathological examination of left ventricle was performed.The mitochondria from the heart tissues was isolated and the mitochondrial swelling and mitochondrial membrane potential (MMP) were analyzed.RESULTS:Compared with control group, the hearts in I/R group showed poorer function, higher CK-MB and LDH levels in the perfusate, increased ROS and MDA contents, higher SOD activity and less ATP content in the heart tissues (P<0.05).Furthermore, the mitochondrial swelling level increased and MMP reduced in I/R group (P<0.05).Compared to I/R group, MB improved heart function and reduced the release of CK-MB and LDH (P<0.05).MB also decreased ROS and MDA contents, and increased the activity of SOD and the content of ATP (P<0.05).In addition, MB alleviated mitochondrial swelling and restored the reduced MMP (P<0.05).CONCLUSION: MB protects the isolated rat hearts from I/R-induced injury by attenuating the damage of mitochondria.
4.Role of hippocampal mTOR signaling pathways in cognitive dysfunction after splenectomy in aged rats

Objective To evaluate the role of hippocampal mammalian target of rapamycin (mTOR) signaling pathways in the cognitive dysfunction after splenectomy in aged rats.Methods Sixtythree male Sprague-Dawley rats,aged 18 months,weighing 400-540 g,were randomly divided into 3 groups (n=21 each) using a random number table:control group (group C),operation group (group O) and mTOR inhibitor rapamycin group (group M).Morris water maze test was performed to evaluate cognitive function before operation and at 1,3 and 7 days after operation.After the end of Morris water maze test carried out at 1,3 and 7 days after operation,7 rats selected randomly in each group were sacrificed,and the brains were removed for detection of the expression of mTOR and phosphorylated tau protein at Ser-396 site (pS396 tau) in hippocampal tissues by using Western blot.Results Compared with group C,the escape latency was significantly prolonged,the ratio of time spending in the target quadrant was decreased,and the expression of pS396 tau was up-regulated at 1 and 3 days after operation,and the expression of mTOR was up-regulated at each time points after operation in group O,and the escape latency was significantly prolonged,the ratio of time spending in the target quadrant was decreased,and the expression of mTOR and pS396 tau was up-regulated at 1 and 3 days after operation in group M.Compared with group O,the escape latency was significantly shorten,the ratio of time spending in the target quadrant was increased,and the expression of pS96 tau was down-regulated at 1 and 3 days after operation,and the expression of mTOR was down-regulated at each time point after operation in group M.Conclusion The hippocampal mTOR signaling pathways are involved in the development of cognitive dysfunction after splenectomy in aged rats.
5.A CCCG-HB-2016 regimen in the treatment of hepatoblastoma in children

Objective:To evaluate the clinical efficacy of multi-disciplinary single center's CCCG-HB-2016 regimen in the treatment of hepatoblastoma (HB) in children.Methods:Clinical data of 36 HB patients treated with CCCG-HB-2016 program from Aug 2016 to March 2020 were analyzed.Results:These 36 patients included 20 boys and 16 girls. The serum AFP was all higher than 2 792 ng/ml,there was a correlation between AFP and tumor risk stratification ( H=14.973, P<0.05). Twenty eight cases (77.78%) were epithelial type and 8 cases (22.22%) were mixed epithelial mesenchymal type.All children were treated by tumor resection combined with chemotherapy, and there was a correlation between tumor risk stratification and surgical resection of liver lobe ( H=8.847, P<0.05). The probability of bone marrow suppression in the low-risk group was 58.33% (35/60),that in the intermediate-risk group was 73.49% (61/83) and in the high-risk group was 80.23% (69/86).All 36 cases were followed up to March 31, 2020,with an average follow-up of 21.9 months and the median survival was 22.5 months.The overall survival rate (OS) and event-free survival rate (EFS) were 97.2% and 83.3% respectively. Conclusions:The multidisciplinary CCCG-HB-2016 regimen was with a high success rate and along with a high incidence of bone marrow suppression.
6.Effects of multilevel nutritional support based on Nutrition Risk Screening on nutritional status and prognosis of patients undergoing craniocerebral tumor surgery

Objective:To study effects of multilevel nutritional support based on Nutrition Risk Screening (NRS 2002) on nutritional status and prognosis of patients with craniocerebral tumor surgery.Methods:A total of 40 patients undergoing craniocerebral tumor surgery who were admitted to The Third People's Hospital of Hubei Province from November 2018 to October 2019 were included as the control group. A total of 40 patients undergoing craniocerebral tumor surgery who were admitted to the Third People's Hospital of Hubei Province from November 2019 to November 2020 were included as the observation group. The control group was given routine nutritional support, and the observation group was given multi-level nutritional support based on NRS 2002. The nutritional status, serum nutritional indexes, prognosis and complications were compared between the two groups before the intervention and one month after the intervention.Results:After 1 month of intervention, the thickness of triceps skinfold and muscle circumference of unaffected upper arm in the two groups were greater than those before intervention, and those of the observation group were greater than the control group, and the differences were statistically significant ( P<0.05) . The levels of prealbumin, albumin and hemoglobin in the two groups were higher than those before the intervention, and the observation group were higher than the control group, and the differences were statistically significant ( P<0.05) . The Glasgow score and ability of daily living score of the two groups were higher than those before the intervention, and the observation group were higher than the control group, and the differences were statistically significant ( P<0.05) . After 1 month of intervention, there were no statistically significant differences in body mass index, constipation, diarrhea, pulmonary infection and malnutrition between the two groups ( P>0.05) . Conclusions:Multilevel nutritional support based on NRS 2002 can improve the nutritional status and prognosis of patients undergoing craniocerebral tumor surgery and the application effect is good.
7.Effects of sensory integration therapy on cognitive function and self-care ability in patients with mild cognitive impairment after stroke

Objective:To investigate the changes of cognitive function and self-care ability in patients with mild cognitive impairment after stroke after sensory integration therapy.Methods:One hundred patients with mild cognitive impairment after stroke were divided into control group and intervention group according to the random number table method. The control group was given the routine health education path and rehabilitation training, and the intervention group was given the cognitive rehabilitation through sensory integration therapy on the basis of the control group. The cognitive function and self-care ability of the patients were evaluated by the Montreal cognitive assessment(MoCA) and daily self-care ability scale(ADL) before and after intervention.Results:After intervention, the total MoCA score, visual space, nominalization, language, abstraction and memory scores of the intervention group were 26.02±1.15, 3.50±0.76, 2.98±0.14, 2.90±0.30, 2.84±0.37 and 3.18±0.69, respectively, while those of the control group were 23.32±1.90, 2.86±1.20, 2.78±0.42, 2.82±0.39, 1.58±0.54 and 2.82±0.94, respectively. The difference between the two groups was statistically significant ( t value was -18.296--2.064, all P<0.05). The scores of attention and directional force in the intervention group were 5.44±0.67 and 5.18±0.85, respectively, while those in the control group were 5.32±0.84 and 5.14±0.78, respectively. There was no statistically significant difference between the two groups ( t value was -1.288, -0.704, P>0.05).After intervention, the total ADL score, stool, urine, toilet, eating, dressing and bathing scores of the intervention group were 65.00±5.98, 5.90±2.19, 6.40±2.27, 7.40±2.72, 7.80±2.70, 7.50±2.53 and 5.50±1.52, respectively, The control group was 53.60±10.20, 3.50±3.23, 5.00±3.50, 5.50±3.23, 5.40±3.76, 5.90±3.45, and 4.40±2.79, respectively. The difference between the two groups was statistically significant ( t value was -7.573~-2.615, all P<0.05). Conclusions:Sensory integration therapy can improve the cognitive function and self-care ability of patients with mild cognitive impairment after stroke.
