1.Comparative observation on operative and non-operative effects in treatment of cervical spinal cord injury without fracture dislocation

Objective To compare the difference of function regain level of spinal cord after operative or non-operative treatments of cervical spinal cord injury without fracture dislocation. Methods Twenty-four cases with cervical spinal cord injury without fracture dislocation were retrospectively analyzed and divided into operative treatment group ( n =13) and non-operative treatment group ( n =11). JOA (Japanese Orthopaedic Association) scores at injury and during follow-up were recorded to make a comparison between operative treatment group and non-operative treatment group before and after treatment. Results JOA scores three months after treatment were 1.64?0.58 in the non-operative group and 3.29 ?0.90 in the operative treatment group. JOA scores 12 months after treatment were 2.00? 0.73 in the non-operative group and 4.93?0.96 in the operative treatment group. The clinical significance of a myeloperoxidase (MPO) gene polymorphism and inducible nitric oxide synthase (iNOS) expression in cirrhotic patients with hepatopulmonary syndrome (HPS) was explored. Enrolled subjects were divided into three groups according to their disease/health conditions: the HPS group (cirrhotic patients with HPS; n=63), the non-HPS group (cirrhotic patients without HPS; n=182), and the control group (healthy subjects without liver disease; n=35). The distribution of the MPO -463 G/A genotype and its relationship with iNOS expression in a typical cell block from ascitic fluid were detected by immunohistochemistry and polymerase chain reaction-restricted fragment length polymorphism analysis (PCR-RFLP). In the HPS group, the partial pressure of oxygen in blood and ascitic fluid was significantly decreased (8.95+/-1.58 kPa and 6.81+/-0.95 kPa, respectively; both P<0.01), while the partial pressure of carbon dioxide significantly increased (4.62+/-0.20 kPa and 5.92+/-0.45 kPa, respectively; P<0.01). MPO and iNOS levels were significantly increased in the HPS group as compared with the non-HPS group. These increases were even more remarkable in ascitic fluid (41.36+/-11.62 and 13.23+/-4.81 mug/L; 10.27+/- 3.20 and 4.95+/-1.12 mug/L) than in blood (16.66+/-5.24 and 4.87+/-1.73 mug/L; 5.79+/-2.31 and 2.35+/-0.84 mug/L). The distribution of the MPO genotypes GG, GA, and AA were 76.2%, 22.2% and 1.6% in the HPS group, and 57.7%, 37.9% and 4.4% in the non-HPS group (P<0.05). The expression of iNOS was significantly higher in patients with the G alleles (G/G and G/A) (61.54%, 48/78) than in patients with A alleles (G/A and A/A) (38.46%, 30/78) (P<0.01). It was suggested that the expression levels of iNOS and MPO were correlated with HPS-induced hypoxemia. The MPO-463 G/A mutation might be a protective factor that prevents the development of HPS. The MPO might be involved in the regulation of iNOS expression. In humans, MPO pathways, the iNOS/NO system, and their interaction might have an impact on the occurrence and development of HPS.
3.Comparative analysis on hemiarthroplasty and dynamic hip screws in treatment of osteoporotic femoral intertrochanteric fractures

Objective To discuss the differences between hemiarthroplaty and dynamic hip screws (DHS) by comparing their effect in treatment of osteoporotic femoral intertrochanteric fractures. Methods A retrospective study was done on 86 patients with femoral intertrochanteric fractures treated by hemiarthroplasty and dynamic hip screws respectively. After a follow-up for six months, the operation duration time, loss of ambulatory grades and prosthesis loosing were compared between two groups. Re-suits Operation was lasted for hmgcr time in DHS group, with significant difference between two groups. Loosening rate varied with different degree of osteoporosis in high-, moderate- and low-risk groups but not in hemiarthroplaty group. Internal fixators penetrating cortical bone occurred in DHS group, with inci-dence rate of 51.2%. On the contrary, no evidence proved loosening of prosthesis in hemiarthroplaty group. Conclusion For osteoporosis patients with intertrochanteric fracture, the hemiarthroplaty is a reasonable alternative to DHS device, for it can help obtain earlier and better functional recovery and less postoperative complication.
4.The clinical application of chromoendoscopy with acetic acid and indigo carmine dye in the diagnosis of early gastric neoplasia and precancerous lesion

Objective To investigate the significance of chromoendoscopy with acetic acid and indigo carmine dye in the diagnosis of early gastric neoplasia and precancerous lesion.Methods After conventional electronic endoscopic diagnosis,From 2011 fanuay to 300 patients of suspicious lesions were randomly divided into endoscopic dye group and control group,each was 150 cases.Patients of endoscopic dye group were directly performed biopsy after acetic acid and indigo carmine dye,while in control group were performed biopsy after only indigo carmine dye.Results In the endoscopic dye group,16 early neoplasia (10.7%),15 severe atypical hyperplasia (10.0%),79 moderate atypical hyperplasia or Intestinal metaplasia (52.7%) and 40 chronic gastritis were found.In the control group,5 early neoplasia (3.3%),10 severe atypical hyperplasia (6.7%),42 moderate atypical hyperplasia or Intestinal metaplasia(28.0%) and 93 chronic gastritis were found.Early gastric cancer and precancerous lesion detection rate of endoscopic dye was significantly higher than that of the control group,and the difference was statistically significant (P < 0.001).After samples were stained with acetic acid and indigo carmine dye,the image of the mucosa of early cancer and severe atypical hyperplasia were faded mucosa,and the image of moderate atypical hyperplasia and Intestinal metaplasia was dyed asymmetrical.Chronic gastritis and normal mucosa was showed dyed equality.Conclusion Chromoendoscopy with acetic acid and indigo carmine dye improve the detection rate of early gastric cancer and precancerous lesion,thereby of high application value.
5.Expression of TAP1 and TNF-α in HBV-related hepatocellular carcinoma and its clinical significance

Objective To explore the association of expression of transporter associated with antigen processing(TAP1),tumor necrosis factor-alpha(TNF-α)with occurrence and development of HBV-related hepatocellular carcinoma(HCC).Methods The expression of TAP1 and TNF-α in 35 liver carcinoma tissues,and 10 normal liver tissues were detected with immunohistochemical EnVision method,The expression and clinical significance of TAP1,TNF-α in HBV-related HCC were analyzed.Furthermore,the association of respective expression quantity with pathological typing of HCC was analyzed.Results The expression of TAP1,TNF-α was negative in hepatocytes of normal liver tissues.91.4% of HCC tissues expressed TAP1.There was significantly statistical difference in expression of TAP1 between HCC tissues and the normal hepatic tissues(P < 0.05);60% of HCC tissues expressed TNF-α.There was statistical difference in expression of TNF-α between HCC and the normal hepatic tissues(P < 0.05),and the positive expression rate of TNF-α in well-differenciated HCC tissues was significantly higher than that in poorly-differenciated ones(P < 0.05).Conclusion TAP1-related MHC-Ⅰ restricted antigen processing pathway was possibly normal in HBV-related HCC.The expression quantity of TNF-α was associated with the occurrence and degree of pathological differentiation of HBV-related HCC.
6.Advanced sensate proper digital artery escalating flaps for repair of fingertip skin defect

Objective To investigate the feasibility of advanced sensate proper digital artery escalating flaps in reconstruction of fingertip skin defect.Methods Between January 2012 and February 2015,15 cases (17 fingers) of fingertip skin defect accompanied with exposed tendon or phalange were treated,including 9 males and 6 females,with mean 41 years of age (range,7-55 years).Injury resulted from hit by heavy objects in 5 cases,knife cutting in 4,machine twist injury in 4 and mass resection in 2.Fingertip defects were covered with the flaps carrying proper digital artery and nerves in order to one-stage reconstruct finger pulp epicritic sensibility and avoid onychogryphosis.Joint function was assessed with the standard set up by Chinese Medical Association Hand Surgery Branch.Results All flaps survived.One case had ankylosis of the middle interphalangeal joint caused by poor weight-bearing exercise and was improved after exercise.Two cases with large defect in the distal finger pulp had slight onychogryphosis after operation.Period of follow-up was 6-14 months (mean,8 months).The injured fingers showed good appearance and sense,with two-point discrimination of the flap ranging from 6-8 mm.There was no linear scar or tenderness in the finger tip.Hypertrophic scar was not obvious in the flap donor area.Function assessment was excellent in 16 fingers and good in 1 finger.Conclusion The flaps result in good blood supply,good sense,no onychogryphosis and no linear scar and are easy to be operated,indicating an ideal method to reconstruct fingertip defect.
7.THE TOPOGRAPHICAL PROJECTION FROM THE VENTRAL LATERAL GENICULATE NUCLEUS TO THE SUPERIOR COLLICULUS AND PRETECTUM IN THE CAT

The present investigation was performed on six adult cats. The morphology and distribution of the labeled cells in the ventral lateral geniculate nucleus (LGNv) were observed. Following the injection of horseradish peroxidase (HRP) into the deeper layers of the superior colliculus the labeled cells were found to concentrate ipsilaterally in the ventral part of the rostromedial division of the LGNv. The HRP-positive cells were composed of large-sized, smaller, round and oval ceils. In the cases of injection of HRP into the rostral pretectum, the HRP-positive cells were seen bilaterally in the caudolateral division of LGNv mainly in the contralateral part, while the labeled cells were found ipsilaterally in the ventral part of caudolateral division after injection of HRP into the caudal pretectum. These cells were smaller than that large ones of superior colliculus injection. The results indicate that the projection of the LGNv to the superior colliculus and pretectum is organized topographically.
8.Treatment of the advanced stage avascular necrosis of femoral head combined with severe femoral anteversion with total hip arthroplasty

Objective To explore the operative method and clinical effect of total hip arthroplasty with normal prosthesis in treatment of the advanced stage avascular necrosis of femoral head combined with severe femoral anteversion. Methods There were 15 patients ( 15 hips) including nine males and six females, at age range of 30-42 years (mean 37 years). The femoral anteversion was 40°-50° and Harris score of (59 ± 8) points. Total hip arthroplasty with normal prosthesis was performed to reduce the femoral anteversion for 20°-30° and increase the acetabulum anteversion for 10°-15°so as to recover a good involution relationship of the femoral head and the acetabulum and avoid anterior dislocation. The imaging examination and Harris scoring were performed regularly postoperatively. Results A follow-up for mean 2.9 years (2.5-3.7 years) in 15 patients showed that all patients obtained good range of joint motion and good stability of all the hip prostheses. Harris score was (88±6) points at 2 years post-opera-tively, which was significantly better than preoperation (P < 0.01 ). Conclusions Total hip arthro-plasty with normal prosthesis can obtain good involution of the femoral head and the acetabulum in patients with advanced stage avascular necrosis of femoral head combined with severe femoral anteversion by simul-taneously regulating implant angle of femoral prosthesis and acetabular cup, which helps avoid use of small or specially made femoral stem or subtrochanteric derotational osteotomy.
9.Soluble CD40 Ligand and fibrinogen in acute myocardial infarction

Objective To observe the changes of serum soluble CD40 ligand (sCD40L) and fibrinogen in acute myocardial infarction (AMI) patients and to investigate the clinical predictive value of increased serum sCD40L and fibrinogen. Methods Serum sCD40L level of 60 AMI patients was determined by enzyme-linked im-munosorbent assay (ELISA). Plasma level of fibrinogen was measured. The patients were followed up for 2 years af-ter discharge from the hospital and were observed for cardiovascular event. Results AMI patients had higher sCD40L and fibrinogen levels than those of controls [(15.36±7.32) μg/L vs. (5.79±2.78) μg/L, (4.60±1.37)g/L vs. (3.03±0.82) g/L,P<0.001] ,which were significantly higher in the patients experiencing cardio-vascular event than those without cardiovascular event [(18.14±6.34) μg/L vs. (14.38±6.67) μg/L and (4.97±1.33)g/L vs. (4.20±1.24} g/L] (P<0.05). The patients with sCD40L≥14.5 μg/L or fibrinogen≥ 4.4 g/L experienced increased risk of adverse cardiovascular events (P<0.05). In AMI patients, sCD40L level was significantly higher in patients with diabetes than in nondiabetics [(18.38±6.71) μg/L vs. (14.46±6.48) μg/L, P<0.05)]. Fibrinogen level was related to sCD40L (r=0.27, P<0.05) and LVEF(r=-0.319, P<0.05). Conclusion Increased sCD40L and fibrinogen levels,which maybe related to the pathogenesis of AMI,can be found in AMI patients and can indicate an independent increased risk of major adverse cardiovascular events. Diabetes is independently associated with elevated sCD40L level in AMI patients.
10.Study of efficacy and safety of Paclitaxel drug coated balloon in elderly patients with de novo coronary disease

Objective To evaluate the efficacy and safety of Paclitaxel drug coated balloon (DCB) (SeQuent Please) in an elderly patients with de novo coronary disease.Methods We performed a retrospective study of 158 consecutive patients (63 patients aged ≥65 yrs and 95 patients aged <65 yrs) received DCB therapy in Cath Lab of Beijing Hospital.Clinical characteristic was recorded and coronary angiography was analyzed with quantitative coronary angiography (QCA) software.Results In elderly group,more patients have hypertension (65.1% vs.56.8%),atrial fibrillation (7.9% vs.2.1%),previous percutaneous coronary intervention (PCI) history (44.4% vs.23.2%,P<0.01) and non ST-elevated myocardial infarction (NSTEMI) (14.3% vs.4.2%,P <0.05).In non-elderly group,more patients were male (71.6% vs.50.8%,P<0.05) and current smoker (52.3% vs.30.2%,P< 0.01).Old patients had more complicated lesions,especially calcified lesions (36.8% vs.14.0%,P<0.01).Despite of that,our study showed a higher success rate of PCI in elderly group.Both groups of patients showed significant acute gain in minimal lumen diameter (MLD) after DCB released.At 4 days post-operation,there was one case that underwent acute myocardial infarction requiring emergent target lesion revascularization (TLR) in non-elderly group.No major adverse cardiac event (MACE) was observed in the elderly group during hospitalization.Twenty-one patients underwent coronary angiographic followed up at average 9 months post PCI.The QCA analysis showed that MLD of lesions treated with DCB had mildly increased [(2.00±0.67) mm vs.(1.91 ± 0.47) mm,P>0.05],the late lumen loss (LLL) was (-0.09±0.50) mm.At 9 months follow-up,the MACE rate in the elderly group was 1.6% and 1.1% in non-elderly group,with TLR rates at 0.0% and 1.1% respectively (both P>0.05).No death was observed in either group.Conclusions The efficacy and safety of DCB on the elderly patients with de novo lesions is as good as non-elderly patients despite more complex anatomy and comorbidities.
