1.Effects of narrow-band ultraviolet B on the levels of interleukin-17 and -22 in patients with psoriasis

Objective To explore the therapeutic mechanism of narrow-band ultraviolet B (NB-UVB)in psoriasis. Methods Forty-two patients with psoriasis vulgaris and 20 healthy controls were enrolled into this study. All the patients received 20 sessions of NB-UVB radiation. Psoriasis area severity index (PASI)was used to evaluate the severity of psoriasis. Blood samples were collected from all the patients before and 15 cured patients after the treatment as well as from 20 healthy controls, and skin samples from 10 patients before and after the treatment as well as from 10 healthy controls. Enzyme-linked immunosorbent assay (ELISA)was performed to determine the serum levels of IL-17 and IL-22, and real-time fluorescence-based quantitative PCR to measure the mRNA expressions of IL-17 and IL-22 in skin specimens. Statistical analysis was carried out by using the two-sample t-test, paired t-test and Pearson correlation analysis. Results After 20 sessions of NB-UVB radiation, 15 out of the 42 patients were cured with a significant decrease in PASI. Compared with the healthy controls, the 15 cured patients showed a significant elevation in the levels of IL-17and IL-22 proteins(IL-17: 34.26 ± 10.05 ng/L vs. 16.34 ± 4.73 ng/L, t = 7.016, P < 0.01; IL-22: 13.72 ± 4.45 ng/L vs. 5.03 ± 1.84 ng/L, t = 8.282, P < 0.01)and mRNAs (IL-17: 13.43 ± 2.12 vs. 5.26 ± 0.87, t = 6.312, P < 0.01; IL-22:16.53 ± 2.65 vs. 7.72 ± 2.13, t = 6.823, P < 0.01)before the treatment. The PASI score was positively correlated with the levels of IL-17 and IL-22 proteins in sera (r = 0.76, 0.70, respectively, both P < 0.05)and their mRNAs in skin lesions (r = 0.65, 0.68, respectively, both P < 0.05)in these patients. The serum levels and mRNA expressions of IL-17 and IL-22 all significantly reduced in the cured patients after the treatment compared with those before the treatment(all P < 0.05). Conclusion NB-UVB may treat psoriasis by downregulating the levels of IL-17 and IL-22 in peripheral blood and skin lesions in patients with psoriasis.
2.Effects of Yiqi Huoxue Fang on necrosis and apoptosis of myocardial cell in rats with myocardial infarction

Objective To explore the protective effects of Yiqi Huoxue Fang(Qi-boosting Blood-activa-ting Formula,YQHXF)on rat myocardium with myocardial infarction and its possible mechanism by ob-serving the effects of YQHXF on necrosis and apoptosis of myocardial cell necrosis.Methods Male SD rats were randomly divided into the sham-surgery group,model group,YQHXF group,and perindopril group.Animal model of myocardial infarction was built by ligating coronary artery on the left anterior de-scending branch.Subsequently,YQHXF group received intragastric administration of Yiqi Huoxue Fang (compound medicine,21 g/kg)once per day,and perindopril group was given perindopril (0.4 mg/kg)once a day;Sham-surgery group and model group received sterile distilled water without drug inter-vention.The blood in abdominal aorta was drawn,and the tissue on the peripheral area of the myocardial infarction was taken at two fixed time points (on the 7 th and 28 th day)after administration.Ultrasonic ech-ocardiography was used to detect changes on cardiac morphology and function in rats:left ventricular e-jection fraction (LVEF),left ventricular short axis ratio (LVFS),left ventricular end systolic diameter (LVIDs),left ventricular end-diastolic diameter (LVIDd);serum superoxide dismutase (SOD),gluta-thione peroxidase (gsh-px),creatine kinase (CK),lactate dehydrogenase (LDH),aspertate amin-otransferase (AST),and troponin (cTnT)were measured with ELISA method.Protein imprinting meth-od (Western Blot)was used to measure cytochrome C (CytC)and protein expression of cysteine aspartic acid and protease-3 (caspase-3)in the border zone of infarcted area.Results On the 7 th day,com-pared with the model group,LVEF and LVFS in YQHXF group and perindopril group decreased without statistical significance(P >0.05).On the 28 th day,compared with the model group,LVEF and LVFS in YQHXF group and perindopril group increased significantly(P >0.05 ).LVIDs and LVIDd decreased markedly(P <0.05);LVIDd in perindopril group decreased sharply (P <0.05),and decrease presen-ted on LVIDs without statistical significace (P >0.05).On the 7 th and 28 th day,compared with the mod-el group,CK-MB,LDH,AST,cTnT in YQHXF group and perindopril group decreased significantly (P<0.05),and SOD,GSH-PX increased significantly.On the 7 th day,protein expression of cytC,caspase-3 decreased significantly in YQHXF group and perindopril group(P <0.05).Conclusion Administra-tion of YQHXF at early stage can effectively improve myocardial infarction on cardiac structure and func-tion in rats with myocardial infarction,improve oxidation resistance of myocardial cell,and inhibit myo-cardial apoptosis and necrosis.
3.Influence of Yiqi Huoxue Fang on Akt/mTOR signaling pathway in rats with myocardial ischemia of myocardial

Objective To investigate the myocardial expressions of protein kinase B (Akt)/ mammalian target of rapamycin (mTOR) signaling pathway and effective mechanism of Yiqi Huoxue Fang depending on model of myocardial ischemia.Methods The model of acute myocardial infarction (AMI) was established through ligation of left coronary anterior descending branch in SD rats, and randomly divided into model group, perindopril group, Qishen Yiqi Gutta Pills group, Yiqi Huoxue Fang group and sham-operation group, and all groups were given orally corresponding medications respectively 2 d after operation.The observation time points were set on the 7th d and 28th d.The changes of heart function and structure were detected by using ultrasonic testing, changes of myocardial pathomorphology were observed after HE staining, and expressions of Akt, mTOR, p-Akt and p-mTOR in infarction marginal zone were detected by using Western blot test, real-time fluorescence quantitative polymerase chain reaction (RT-PCR) and immunohistochemistry technique in all groups.Results The level of left ventricular ejection fraction (LVEF) and left ventricular fraction shortening (LVFS) decreased significantly in model group compared with sham-operation group on the 7th d and 28th d (P<0.01).The level of left ventricular end-systolic volume (LVESV) increased significantly on the 7th d (P<0.01), and levels of LVESV and left ventricular end-diastolic volume (LVEDV) increased significantly (P<0.01) on the 28th d in model group.The levels of LVEF and LVFS increased and levels of LVEDV and LVESV decreased significantly in all medicated groups compared with model group (P<0.01).The thinner ventricular wall, less cardiomyocytes, disorder myocardial cells arrangement and inflammatory cell infiltration were observed in infarction marginal zone in model group, and disorder myocardial cells arrangement and less cardiomyocytes were observed in all medicated groups.The ratio of p-Akt/Akt and p-mTOR/mTOR increased (P<0.05), expressions of Akt m-RNA and mTOR m-RNA increased significantly (P<0.01), and levels of p-Akt and p-mTOR positive cell rates increased significantly (P<0.01) in model group.The ratio of p-Akt/Akt and p-mTOR/mTOR increased (P<0.05), expressions of Akt m-RNA and mTOR m-RNA increased (P<0.05 or P<0.01), and levels of p-Akt and p-mTOR positive cell rates increased significantly (P<0.05) in all medicated groups.Conclusion Yiqi Huoxue Fang can improve heart function and protect myocardial cells through activating cardiac self-protective mechanism-Akt/mTOR signaling pathway in rats with myocardial infarction.
4.Optimization practice of youth research project management in children′s hospital: Based on the situation and demand of National Natural Science Youth Foundation in Beijing Children′s Hospital

Objective:The study takes the Youth Research Project of National Natural Science Foundation of China (NSFC) as an example, analyzes the development and management optimization strategy of such research project in a children′s hospital, to provide reference for the training of pediatric young talents.Methods:Personnel interview and questionnaire survey were adopted to analyze the common characteristics of project application and approval, trend of change, demand and bottleneck challenges of the NSFC Youth Research Project from 2016 to 2022.Results:The total number of approved project were increasing while the rate of the bids fluctuated, and the distribution of department, gender and age are not balanced, full-time scientific research personnel, male, young age and other factors have the advantages of being approved. Five influential factors, including scientific research accomplishment, supporting conditions, research foundation, methods & skills, and scientific research atmosphere and environment, were identified as necessary components of getting funded. The survey also founded that two thirds of the needs run through the whole application process that from the topic selection, nurtured seed-funding, team-building and proposal development.Conclusions:This study puts forward a management plan for youth scientific research projects in children′s hospital from three aspects that including hospital, department and individual. The management department should strengthen the transformation from " full application" to " effective application" , at the same time, more attention should be paid to the selection, training and resource allocation to further optimize the training strategy of youth talents.
5.Analysis of satisfaction and influencing factors of scientific research incentive mechanisms for young medical staff in a children′s hospital in Beijing

Objective:This study aims to explore the satisfaction and influencing factors of scientific research incentive mechanisms for young medical staff in a children′s hospital in Beijing, and to provide references for the management department to develop relevant incentive strategies.Methods:A satisfaction questionnaire survey of scientific research incentive mechanisms was carried out during April and May, 2022. Collected data were analyzed by rank sum test and ordered logistic regression.Results:The level of overall satisfaction of 339 young medical staffs was 81.42%. The satisfaction rate of incentive factors was 80.50% and hygiene factors was 77.51%.According to the findings, there were significant differences in educational backgrounds, professional titles and job types, hygiene factors, and incentive factors ( P<0.05). The contributing factors of the job satisfaction were professional title, hygiene factors and incentive factors. Conclusions:The overall satisfaction with the incentive work of the young research staff surveyed was relatively at high-level. It is necessary to improve tailed hygiene factors to meet the characteristics young medical staff, such as setting up full-time scientific research working hours, exploring the establishment of diversified of scientific research sharing mechanisms to increase the utilization of the scientific research platform. It is important to enhance incentive factors to establish a long-term incentive mechanism for young personnel, such as setting up interdisciplinary research projects, exploring the establishment of a recommendation mechanism for outstanding young talents, in order to provide them with more opportunities for further intensive training.
6.Research of prognostic immunophenotypes in 163 patients of diffuse large B-cell lymphoma

Objective:To screen and analyze the prognostic protein biomarkers of DLBCL, and to explore their value in the prognostic evaluation.Methods:163 cases of confirmed DLBCLs from January 2011 to December 2016 were collected with their clinical, pathological and follow-up data, which were all from our hospital. The expression of protein markers were tested using immunohistochemical staining (IHC) . The immune phenotypes independent of the International Prognostic Index (IPI) that affect overall survival (OS) and progression-free survival (PFS) of DLBCL were explored by COX regression model, and the effect of their co-expression on the prognosis were also analyzed.Result:BCL6 negative (PFS: HR=1.652, 95% CI 1.030-2.649, P=0.037) , P53 positive (OS: HR=1.842, 95% CI 1.008-3.367, P=0.047) , and BCL2 strong positive expressions (S+) (OS: HR=2.102, 95% CI 1.249-3.537, P=0.005; PFS: HR=2.126, 95% CI 1.312-3.443, P=0.002) are adverse prognostic factors of DLBCL that are independent of IPI. BCL6 - (PFS: HR=2.042, 95% CI 1.021-4.081, P=0.043) , P53 + (OS: HR=3.069, 95% CI 1.244-7.569, P=0.015) and BCL2 S+ (OS: HR=2.433, 95% CI 1.165-5.082, P=0.018; PFS: HR=3.209, 95% CI 1.606-6.410, P=0.001) are adverse prognostic factors in the group of age≤60-year-old; in the group of IPI score 0-2, cases with BCL6 - (OS: HR=2.467, 95% CI 1.322-4.604, P=0.005; PFS: HR=2.248, 95% CI 1.275-3.965, P=0.005) and BCL2 S+ (PFS: HR=2.045, 95% CI 1.119-3.735, P=0.020) have worse prognosis. The co-expression of BCL6 - and BCL2 S+ has significant influence on prognosis of DLBCL ( P=0.005 and P<0.001) , in which BCL6 +/non-BCL2 S+ ( n=86) has the best prognosis[3-year-OS (71.6±4.9) %, 3-year-PFS (67.0±5.1) %], and BCL6 -/BCL2 S+ ( n=10) has the worst prognosis[3-year-OS (20.0±12.6) %, 3-year-PFS (10.0±9.5) %]; the co-expression of BCL6 - and P53 + has no significant influence on prognosis ( P=0.061 and P=0.089) , however, those cases with BCL6 +/P53 - ( n=98) often get better prognosis[3-year-OS (70.6±4.7) %, 3-year-PFS (64.6±4.9) %] than others; the co-expression of P53 + and BCL2 S+ has significant influence on prognosis of DLBCL ( P<0.001 and P<0.001) , and P53 +/BCL2 S+ ( n=5) has the worst prognosis (3-year-OS and 3-year-PFS are both 0) ; BCL2 S+ cases get shorter OS and PFS, regardless of the expression of BCL6 and P53. Conclusion:The expression and co-expression of BCL6 negative, P53 positive and BCL2 S+ have certain value in the prognostic evaluation of DLBCL, especially in the group of age≤60-year-old and IPI score 0-2.
7.Clinicopathological features of intravascular peripheral T-cell lymphoma

Objective:To summarize the clinical and pathological features of intravascular NK and T cell lymphoma for better understanding of such disease to reduce misdiagnosis and miss-diagnosis.Methods:Clinical and pathological features were analyzed retrospectively in one case of intravascular peripheral T-cell lymphoma, not otherwise specified (IVPTCL, NOS) , with literatures review.Results:The case presented in this study was a 66-year-old man. PET/CT scan showed multiple lymph nodes enlargement throughout the body. Normal lymph node structure could not be observed by tissue biopsy, while lymph follicles were partially disrupted. High-power light microscope revealed a large number of blood vessels with diffuse proliferation and dilation, where atypical lymphoid cell mass was restricted in the lumen and partially infiltrated the large blood vessel wall. These tumor cells were medium to large with moderate cytoplasm. The nucleus was irregular, single or multiple nucleoli could be seen, chromatin was condensed, some were empty and bright, and mitotic figures could be seen. Immunohistochemical staining showed that the neoplastic cells were positive for expression of CD3, CD43, CD8, GrB, TIA-1 and perforin. EBER in situ hybridization result was negative. Polymerase chain reaction test identified a clonal gene rearrangement of T-cell receptor γ. The patient was treated with CHOP in combination with chidamide, but died of infection and cardiopulmonary failure within 2 months. 56 cases of intravascular NK/T cell lymphoma with definite classification were collected from relevant literatures, including 47 cases with nasal type of extranodal NK/T cell lymphoma (27 were male and 20 were female) , 8 cases with anaplastic large cell lymphoma (3 males and 5 females) , and only one case with de nova IVPTCL, NOS in brain. We report the second case of IVPTCL,NOS, and notably originated from lymph node for the first time.Conclusions:Intravascular NK/T cell lymphoma is a highly aggressive disease with no effective treatment at present. Involvement of Lymph node has rarely been reported, and further studies on more cases are necessary.
