1.A research: therapeutic application of pneumatic ballistic lithotripsy under percutaneous nephroscope ultrasound and holmium laser lithotripsy for complex renal calculus

Objective: To explore the effect of therapeutic application of pneumatic ballistic lithotripsy under percutaneous nephroscope ultrasound and holmium laser lithotripsy for complex kidney stone. Methods: 124 patients with complex renal calculus were selected and divided into observation group (62 cases) and control group (62 cases) as randomly number table. The patients of observation group were cured by pneumatic ballistic lithotripsy under percutaneous nephroscope ultrasound while patients of control group were cured by holmium laser lithotripsy. A series of correlative indicators, such as some correlative indexes in during and post operation, complication situation of post operation and kidney function, were compared. Results:The differences about bleeding volume during operation and postoperative hospital stay between the two groups were significant (t=12.475, t=7.659, P<0.05). For clearance rate of calculus and incidence of complication, the differences between the two groups were significant (x2=6.048, x2=7.159, P<0.05). And the differences of separation index of renal pelvis collective system, creatinine and urea nitrogen post treatment between the two groups also were significant (t=8.529, t=7.058, t=10.596, P<0.05). Conclusion: Comparing with the traditional holmium laser lithotripsy, the clinical better effect of pneumatic ballistic lithotripsy under percutaneous nephroscope ultrasound for complex renal calculi is significant. Its series of advantages, such as shorter operation time, faster recovery, safer and reliable for patients and lower incidence of postoperative complication and so on, can effectively relieve the renal function for patients.
2.A new coumarin glycoside from Bombyx Batryticatus

Object To study and identify the chemical constituents of Bombyx Batryticatus. Methods The constituents were isolated from the above materials by column chromatography using silica gel and Sephadex LH-20, purified by crystallization, and identified by spectroscopic methods. Results Eight compounds were isolated and identified. They are 6-methoxy-7-O-?-D-(4′-methoxy) glucopyranosyl coumarin (Ⅰ), ergost-6, 22-dien-3?, 5?, 8?-triol (Ⅱ), palmitic acid (Ⅲ), meso-erythritol (Ⅳ), D-mannitol (Ⅴ), uracil (Ⅵ), ?-sitosterol (Ⅶ), and daucosterol (Ⅷ). Conclusion Coumpound Ⅰ is a new coumarin glycoside.
4.Clinical predictive value of serum hsCRP for risk of acute cerebral infarction

Objective:To explore predictive value of serum high sensitive C reactive protein (hsCRP) for clinical risk of acute cerebral infarction (ACI).Methods: A total of 122 ACI patients diagnosed in our hospital were regarded as cerebral infarction (CI) group, another 122 patients without ACI who received physical examination in our hospital during the same period were selected as non-CI group.Intima-medium thickness (IMT), plaque location and hsCRP level were compared between two groups, and the relationship among different IMT, plaque types and hsCRP level was analyzed.Results: Compared with non-CI group, there were significant rise in percentages of IMT thickening (1.6% vs.19.7%), carotid atherosclerotic plaques (15.6% vs.69.7%) and unstable plaques (8.2% vs.60.7%) in CI group, P=0.001 all;among CI patients, compared with normal IMT patients, there was significant rise in hsCRP level [(4.7±1.6) mg/L vs.(8.5±2.5) mg/L vs.(12.6±3.9) mg/L] in IMT thickening and IMT plaque formation patients, and hsCRP level of plaque formation patients was significantly higher than that of IMT thickening patients, P=0.001 all;hsCRP level of unstable plaque patients was significantly higher than that of stable plaque patients [(13.7±2.7) mg/L vs.(9.1±2.1) mg/L, P=0.001].Conclusion: Compared with normal IMT patients, there was significant rise in hsCRP level in IMT thickening and IMT plaque formation patients, and hsCRP level of unstable plaque patients was significantly higher than that of stable plaque patients in acute cerebral infarction and non-CI patients, so hsCRP can be used as risk predictor for carotid atherosclerosis and acute cerebral infarction
5.MR Imaging and Proton Spectroscopic Study of Stroke-like Episodes in MELAS

Objective To study the characteristics of MR imaging and proton MR spectrscopy(~1H MRS)of stroke-like lesions in MELAS.Methods Clinical,MR imaging and proton spectroscopic findings of stroke-like lesions in 7 patients with confirmed MELAS were analyzed retrospectively.Results A total of 12 MR investigations had been performed in 7 patients.Stroke-like lesions showed by MR imaging included superacute in 12,acute in 12,subacute in 10 and chronic stage in 6.Early stroke-like lesions were demonstrated as focal edematous foci mainly involved cortex/subcotical areas of occipital,temporal and parietal lobes.At MR diffusion imaging,stroke-like lesions in the superacute(<3 days)stage were showed as well-circumscribed lesions with high signal intensities for cytotoxic edema.During the acute(4~7 days),sub-acute(2~4 weeks)and chronic(>4 weeks)stages,the lesions gradually expanded,and became blur,and presented with vasogenic edema mainly.Proton spectroscopy showed a prominently elevated lactate,varied decrease of NAA concentration and other brain motabolites in the stroke-like lesions early after onset,and depicted gradual decrease of lactate level and partial recovery of NAA concentration subsequently.Conclusion Stroke-like lesions in MELAS mainly involve the cerebral cortex and subcortical areas,in which cytotoxic edema appears early but for a short period.In ~1H MRS,the lesions are characterized by a double lactate peak with decrease of NAA concentration.
6.CYCLOARTANE TRITERPENES AND GLYCOSIDES FROM CIMICIFUGA ACERINA

AIM To investigate the chemical constituents from the rhizomes of Cimicifuga acerina (Sieb. et Zucc.) Tanaka. METHODS Column chromatgraphy (including silica gel and ODS) together with HPLC was used to separate the chemical constituents whose structures were determined by FAB-MS, NMR (1D and 2D) and hydrolysis methods. RESULTS Five cycloartane triterpenoids were isolated and identified as: (22R)-22-hydroxycimigenol (I), (22R)-22-hydroxy-24-O-acetylhydroshengmanol 3-O-β-D-xylopyranoside (II), dahurinol (III), 24-epi-24-O-acetyl-7,8-didehydroshengmanol 3-O-β-D-xylopyranoside (IV), 25-O-acetyl-7,8-didehydrocimigenol 3-O-β-D-xylopyranoside (V). CONCLUSION Compound I is a new natural product, compound II is a new compound and compounds IV and V were isolated from this plant for the first time.
7.HTLV-1 bZIP Factor (HBZ): Roles in HTLV-1 Oncogenesis.

Human T-cell leukemia virus type 1 (HTLV-1) is a retrovirus demonstrated to be associated with human disease. Infection by the HTLV-1 can cause T-cell leukemia (ATL) in adults. HTLV-1 bZIP factor (HBZ) is a viral protein encoded by the minus strand of the HTLV-1 provirus. Among the regulatory and accessory genes of HTLV-1, HBZ is the only gene that remains intact and which is expressed consistently in all patients with ATL. Moreover, HBZ has a critical role in the leukemogenesis of ATL. Here, we review the function of HBZ in the oncogenesis of HTLV-1 and its molecular mechanism of action.

8.Utilization of combined flow cytometry and clonal TCR gene rearrangements in the diagnosis of T-cell lymphoma

Objective To study the usefulness of combined flow cytometry (FCM) and polymerasechain reaction examination for clonal TCR gene rearrangements in the diagnosis of T-cell lymphoma (T-NHL). Methods Histopathologic features, immunohistochemistry, flow cytometric immunophenotyping, cytomorphologic evaluation and TCR gene rearrangements of 32 T-NHL were reviewed retrospectively. The control cases were 18 reactive lesions and 1 histiocytic necrotizing lymphaderitis. Results Out of 32 T-NHL,23 were diagnosed as T-NHL by FCM / TCR gene rearrangements. Of 19 control group, 17 were diagnosed as reactive lesions by FCM / TCR gene rearrangements. The sensitivity, specificity and accuracy were 71.9%, 89.5% and 78.4%, respectively. Conclusions FCM / TCR gene rearrangement is a very important technique in diagnosing T-NHL. Thus, patients with fine needle aspiration cytology can be saved from having an invasive surgery.
9.Chemical constituents from the leaves of Callicarpa nudiflora

From the n-butanol extract of the leaves of Callicarpa nudiflora,thirteen compounds were isolated by repeated column chromatography with silica gel,C_(18) and Sephadex LH-20.Their structures were identified by spectroscopic (~1H,~(13)C NMR and so on) and chemical methods as luteolin (1),luteolin-7-O-β-D-glucopyran coside (2),apigenin (3),5,7,4'-trihydroxy-3'-methoxyflavone (4),luteolin-3'-O-β-D-glucopyranoside (5),api genin-7-O-β-D-glucopyranoside (6),luteolin-7-O-(6-trans-caffeoyl)-β-D-glucopyranoside (7),luteolin-7-O-(6-trans-feruloyl)-β-D-glucopyranoside (8),arjunglucoside I(9),luteolin-7-O-(6-p-coumaryl)-β-D-glucopyranoside (10),forsythoside B (11),isomartynoside (12),deacylisomartynoside B (13).Among them,compound 11 was isolated from this plant for the first time and compounds 4,7-10,12-13 were isolated from this genus for the first time.
10.CT features of giant lymph node hyperplasia in abdomen

Objective To describe the CT features of giant lymph node hyperplasia in abdomen.Methods A retrospective study of CT features of giant lymph node hyperplasia in abdomen confirmed by surgery or biopsy pathology was performed.CT findings of all lesions were assessed by two radiologists including size,location,pattern of enhancement and density characteristics.Results 1 2 patients with giant lymph node hyperplasia in abdomen were confirmed including 7 women and 5 men with age range from 11 to 75 years (median age 3 9 years).All patients underwent pre-and post-contrast enhanced CT.CT showed a single mass in 9 patients lo-cated at retroperitoneum in 5,porta hepatic in 2,and mesentery in 2.Multiple masses were located at chest,abdomen and neck in 3.The lesions ranged in size from 1.2 to 11.9 cm in maximum diameter with an average size of 3.7cm.CT showed all lesions with well-defined margin and regular shape.The lesions less than 5 cm in diameter usually showed homogeneous enhancement,and how-ever those more than 5 cm showed heterogeneous enhancement.The calcification was seen in two patients.Conclusion CT features of giant lymph node hyperplasia in abdomen are characteristic.
