Objective To discuss the correlation between C-peptide level and autonomic neuropathy in diabetic foot in patients with type 2 diabetes mellitus (T2DM). Methods The levels of fasting blood glucose (FBG), 2 h postprandial blood glucose (2 h-PG) and fasting C-peptide (FC-P) were detected in 124 T2DM patients. The Neuropad was used to test functions of sudomotor nerves in foot. Patients were divided into two groups:diabetic autonomic neuropathy (DAN) group and non-autonomic neuropathy group (NDAN) according to the results of functions of sudomotor nerves in foot. Values of gender, age, body mass index (BMI), blood pressure, FBG, 2 h-PG and FC-P were compared between the two groups. The Pearson correlation analysis was carried out between the indexes mentioned above and the time for completing color change of Neuro-pad. Results There were no significant differences in gender, age, FBG and 2 h-PG between two groups (P>0.05). The FC-P level was (1.07 ± 0.75)nmol/L in DAN group, which was significantly lower than that in NDAN group (1.43 ± 1.00) nmol/L (P > 0.05). The time for completing color change of Neuropad was negatively correlated with C-peptide. Conclu-sion The autonomic nerve functions in diabetic foot is closely related with C-peptide. It is inferred that C-peptide may have protective effect on autonomic nerves in diabetic foot.

Objective To improve the skills and level of TURP and peri-operative period managements to the high risky senile patients with large volume benign prostatic hyperplasia.Methods The clinical data of 318 high risky and senile patients whose ages above 80ys,ASA score ＞ 2 and prostate volume ＞ 60g were analyzed retrospectively.They underwent the treatment of TURP.ResultsTotal 318 patients underwent TURP were safe.The operating time ranged from 40 to 85 minutes,averaged 58.2minutes;the volume of blood transfusion ranged from 200ml to 600ml;No serious complications happened during and after the operation.With follow up of 1 ～ 12 months,the International Prostate Symptom Scores(I-PSS) decreased 14.7 averagely,Quality of Life(QOL) decreased 3.3 averagely,Maximal flow rate( Qmax ) increased 6.4ml/s averagely,and Post-voided Residual(PVR) decreased 85.3ml averagely.Conclusion The actions including sufficient preparations and evaluations pre-operatively,the maintenance to the stability of circulatory system during the operation,guaranteeing the demands of blood exchange in the vital organs such as heart,lungs and brain,are the key points to the success.

Objective To isolate and identify stem-like cells (CSCs) from human ACHN renal cell carcinoma cell line.Methods ACHN cancer stem cells CSCs were enriched by serum-free culture condition.Methyl thiazolyl tetrazolium (MTT) method was performed to draw the growth curve.Flow cytometry was performed to evaluate the expressions of two CSCs markers,CD133 and CD44.Real-time polymerase chain reaction (RT-PCR) was performed to evaluate the expression of stem-related genes (Oct3/4,Bmi,Nanog,and 3-catenin).The tumorigenicity in vivo was also examined.Results ACHN CSCs were formed by sphere-forming culturing.The data demonstrated that there were stronger capacities of proliferation in ACHN tumor spheres than that of ACHN cells.And the expressions of CD133 and CD44 were also significandy enhanced in tumor spheres.The data showed that four genes (Oct3/4,Bmi,Nanog,and β-catenin)were upregulated in sphere-forming cells.Sphere-forming cells had a higher tumorigenic potential in vivo than incubated cells.Conclusions Human ACHN renal cell carcinoma cell line sphere-forming cells can be enriched by serum-free culture condition and exhibits several characteristic cancer stem cells properties.

OBJECTIVETo evaluate the clinical efficacy of intra-articular injection with 2% chitosan as a treatment for irreducible anterior disc displacement of the TMJ.

METHODSTotal 30 patients with irreducible anterior disc displacement were divided into two groups according to quasi-randomizaion. The patients in test group received intra-articular injection with 1.0 ml of 2% chitosan into upper cavities of the suffered joints, in control group with 12.5 mg of prednisolone. The patients were followed up at the 1st day and 14th day after injection and the maximal mouth opening was measured.

RESULTSThe maximal mouth opening at 14th day was (36.73 +/- 4.69) mm in test group, with 11.73 mm increase from baseline; and (28.53 +/- 5.81) mm in control group, with 3.86 mm increase, respectively. The increase of maximal mouth opening in test group was significantly higher than that of control group (P < 0.05).

CONCLUSIONChitosan is an effective biomaterial in curing irreducible anterior disc displacement of TMJs.

Objective To observe the damage of liver cells and to investigate the distribution and expression of glu-cose-regulated protein 78 (Glucose regulated protein78, GRP78/Bip) in liver tissue under the positive acceleration (+Gz) exposure.Methods Totally 24 wistar rats were randomly assigned to four groups:blank control,+6Gz,+9Gz and +12Gz.Each rat was clamped to the centrifuge arm , prone position, with the head of the rat facing the axis of the centrifuge for +Gz orientation.The onset rate was +0.5 Gz/s, which was used trapezoidal acceleration curve effect and controlled by computer .Blank control group rats were placed on the arm of centrifuge and under-went a process similar to that described above , but they were not exposed to acceleration .+6Gz group,+9Gz group and +12Gz group were subjected at peak time 3 min in animal centrifuge , acceleration rate 0.5 G/s, five times with interval 30 min between times.In addition, liver tissue of rats were respectively observed by H .E.staining.Mean while, plasma aspartate aminotransferase ( AST) and alanine aminotransferase ( ALT) were tested determine the damageofliverfunction.Results +GzaccelerationstressinjuryincreasedserumASTandALTlevel.Compared with the stress control, +9Gz group and +12Gz group significantly increased in plasma ALT and AST as compared with control group ( P<0.05 ) .+12 Gz stress induced the highest level in these groups .The level of ALT in+2 Gz group was higher than that in +6 Gz group ( P<0.05 ) .HE staining showed derangement of liver cells , irregular shape, the cell gap is not clear, vacuolar changes in +Gz groups, and with the increase of G value.Compared with the control group, the expression of GRP78/Bip was focused in the cytoplasm;the expression of GRP78 in the experimental group is higher than that in the control group ( P<0.05 ) .+12 Gz group was significantly higher than+6Gz group and the control group (P<0.05).The expression of GRP78/Bip in liver tissue increased with the in-creasing of G value levels;the expression level of GRP78/Bip in +12Gz and +9Gz groups were higher than that in +6Gz and control group (P<0.05).Conclusions There is positively related expression of GRP78/Bip, which was associated with exposure of increasing G values .

Objective To investigate the correlation between serum adiponectin level and severity of coronary artery lesion. Methods The serum concentrations of adiponectin in 112 patients whom had received coronary arteriongraphy (CAG) were measured. Among them,58 cases had acute coronary artery syndromes(ACS)(ACS group), 25 cases had stable angina cordis(stable angina cordis group) and 29 cases had non-coronary disease (control group ). The severity of coronary artery lesion was evaluated by the number of stenosis vessel and Gensini score. All the patients were divided into single vessel lesion group (24 cases),double-vessel lesion group (26 cases ), triple-vessel lesion group (33 cases) by extent of disease; and 0- 1 score group (29 cases),2-20 scores group (28 cases),21-40 scores group (30 cases), ＞40 scores group (25 cases) by Gensini score. Results The serum concentrations of adiponectin in ACS group were lower than those in control group and stable angina cordis group[(6.43 ± 4.22 ) mg/L vs. ( 12.68 ± 6.47 ), (9.94 ± 8.48 ) mg/L]( P ＜ 0.01 or ＜ 0.05 ). The serum concentrations of adiponectin in control group were higher than those in single vessel lesion group, double-vessel lesion group and triple-vessel lesion group[( 12.68 ± 6.47 ) mg/L vs. (8.80 ±6.23), (8.04 ±5.93), (6.43 ±5.12) mg/L](P ＜0.01). The serum concentrations of adiponectin in single vessel lesion group were higher than those in triple-vessel lesion group (P＜ 0.01 ). The serum concentrations of adiponectin in 0-1 score group[.( 12.68 ±6.47) mg/L]and 2-20 scores group [(8.74 ± 6.68) mg/L]were respectively higher than those in 21-40 scores group[(7.64 ± 5.32) mg/L]and ＞ 40 scores group[(6.32 ± 5.46 ) mg/L](P ＜ 0.01 ). The serum concentrations of adiponectin in 21-40 scores group were higher than those in ＞ 40 scores group(P＜ 0.05 ). The serum concentrations of adiponectin were negatively correlated with the log of Gensini score (r = -0.584,P ＜ 0.01 ). Conclusion The decrease in serum adiponectin might reflect the increase in severity of coronary artery lesion.

ObjectiveTo improve the treatment of urethral stricture and evaluate the applicative effects of wire-guided supporting catheterization in clinical urology. MethodsDuring January 2004 and December 2005, the patients with urethral stricture were dwelled with the catheters using the traditional guideless catheterization (group A). If the dwelling failed,it will be replaced by the wire-guided supporting catheterization using the improved catheters. During January 2006 and December 2009, the patients with urethral stricture were dwelled with the improved catheters using wire-guided supporting catheterization ( group B) straightly. Compared the first-time dwelling success incidence,the incidence of catheter associated urinary tract infection and side-effect events. Then after 1 year, compared the urethral stricture recrudescence and the course of treatment. ResultsThe success incidence of first-time catheterization in group A was 48.98％ (24/49) ,and in group B was 97.94％ (95/97) ,there was significant difference between 2 groups;The incidence of catheter associated urinary tract infection in group A was 12.24％, and 8. 25％ in group B, there was significant difference between 2 groups; As for the course of treatment, group A was 46. 2w,group B was 32.7w;The urethral stricture recrudescence in group A and B were 16.33％ and 9.28％ respectively,and there was significant difference between 2 groups. ConclusionThe wire-guided supporting catheterization, which minimize the injury and simplify the operation of internal urethrotomy,could makes the improved catheter easy to induct and replace, improve the success rate of first time-catheterization and prevent the false tunnel damage and new scar expansion. It could make benefit to reduce of incidence of catheter associated urinary tract infection and urethral stricture recrudescence,but also could shorten the course of treatment significantly.

ObjectiveTo investigate the correlation between serum uric acid level and severity of coronary artery disease.MethodsThe concentrations of serum uric acid in 112 patients whom had received coronary arteriongraphy(CAG) was measured,among of them,acute coronary artery syndrome (ACS) with 58 cases( ACS group ),stable angina cordis with 25 cases( stable angina cordis group),non-coronary disease with 29 cases(control group).The severity of coronary artery lesion was evaluated by the number of stenosis vessel and Gensini score.They were divided into control group (29 cases),single vessel lesion group (24 cases),double vessel lesion group(26 cases),triple vessel lesion group(33 cases ) according to the number of stenosis vessel and 0-1 score group(29 cases),2-20 scores group(28 cases),21-40 scores group(30 cases),＞40 scores group (25 cases) according to the Gensini score.ResultsThe concentrations of serum uric acid in ACS group were higher than those in control group and stable angina cordis group[ (369.61 ± 91.97 ) μ mol/Lvs. (298.33 ±92.46),(330.43 ±87.42)μmol/L] (P ＜0.05).The concentrations of serum uric acid in control group were lower than those in single vessel lesion group,double vessel lesion group and triple vessel lesion group [(298.33 ±92.46)μmol/L vs. (331.77 ±86.33),(368.24 ±95.21),(396.82 ±94.45) μ mol/L] (P ＜ 0.05).The concentrations of serum uric acid in single vessel lesion group were significantly lower than those in double vessel lesion group and triple vessel lesion group (P ＜ 0.05 ).The concentrations of serum uric acid in 0-1 score group [ (298.33 ± 92.46) μ mol/L] and 2-20 scores group [ (320.77 ± 86.33 ) μ mol/L ] were respectively lower than those in 21-40 scores group [ (366.61 ± 91.97 ) μ mol/L ] and ＞ 40 scores group [ (402.82 ± 91.97 ) μ mol/L](P ＜ 0.05 ).The concentrations of serum uric acid in ＞ 40 scores group was higher than that in 21-40 scores group (P ＜ 0.05 ).Serum uric acid concentrations was positively correlated with the log of Gensini score (r =0.348,P ＜ 0.05 ).ConclusionThe increase in serum uric acid might reflect increase in severity of coronary artery stenosis.

iRGD-modified sterically stabilized liposomes loaded doxorubicin (iRGD-SSL-DOX) were prepared and their cellular toxicity and anti-tumor efficacy were evaluated, comparing to doxorubixin loaded sterically stabilized liposomes (SSL-DOX) and RGD modified doxorubixin loaded sterically stabilized liposomes (RGD-SSL-DOX). The iRGD peptide, with both tumor targeting and cell penetrating functions, was conjugated to DSPE-PEG-NHS and DSPE-PEG-iRGD was obtained. DSPE-PEG-RGD was gained in the same way. iRGD-SSL-DOX, RGD-SSL-DOX and SSL-DOX were prepared by ammonium sulfate gradient method. The size and zeta potential of the liposomes were characterized by dynamic laser light scattering. The cellular toxicity study was done on B16 melanoma cell line and the anti-tumor efficacy study was carried on B16 cell line bearing C57BL/6 mice. The results showed that the particle sizes of liposomes were all around 90-100 nm. DOX entrapment efficiency was above 95%. The formulations were with good preparation reproducibility. iRGD-SSL-DOX showed no significant difference in B16 cellular toxicity with SSL-DOX and RGD-SSL-DOX, but the anti-tumor efficacy on B16 melanoma bearing C57BL/6 mice was significantly better than that of SSL-DOX, similar as that of RGD-SSL-DOX. Therefore, iRGD modified liposomes loaded DOX would be a promising drug delivery system for tumor therapy.

Objective To evaluate the safety and efficacy of microwave tissue coagulation assisted laparoscopic hepatectomy for the treatment of hepatic tumors.Methods 29 patients underwent laparoscopic hepatectomy using microwave liver tissue coagulation from December 2011 to June 2014 in the General Air Force Hospital.All patients underwent intraoperative ultrasound guided microwave coagulation assisted laparoscopic local hepatectomy.Intraoperative blood loss,operation time,postoperative hospital stay and complications were observed.Results All the operations were successfully performed without conversion to open surgery.There were 18 cases of hepatocellular carcinoma (HCC),2 cases of intrahepatic cholangiocarcinoma,2 cases of hepatic metastases from the rectal carcinoma,5 cases of hepatic hemangioma,1 case of hepatic focal nodular hyperplasia and 1 case of hepatic cystadenoma.Drainage tubes were placed in 3 patients and removed 5-7 d after the operation.The operation time was 150 (125-210) miu,the median amount of intraoperative bleeding was 120 (30-250) ml,the postoperative hospital stay was 7 (5-10) d.All patients recovered well after the operation and no complications such as intraabdominal bleeding and bile leakage were observed.The cutting edges of liver cancer of these 22 cases were negative.The postoperative follow-up time of these 22 cases were 3-36 months and no tumor recurrence and metastasis were observed.Conclusions Laparoscopic liver resection using intraoperative ultrasound guided microwave hepatic tissue coagulation can effectively control the bleeding,improve the safety of laparoscopic liver resection,which makes it a safe and effective minimally invasive technique.
