1.Attention resource allocation deficits in patients with nonfluent aphasia in Chinese character processing

Objective To determine whether attention allocation deficits contribute to nonfluent aphasia during orthography, phonology, and semantic processing with Chinese characters. Methods Sixteen patients and sixteen controls participated in single and dual tasks. In the single task participants were required to read Chinese characters silently and to make radical, semantic, and final sound judgments (for example, whether the charactersincluded the sound 'ang' ) simultaneously. During the dual task the only difference was that the participants wereasked to read the presented characters aloud and make the same three judgments. Reaction time and error rate were recorded and analysed. Results Reaction times in orthographic judgment were significantly longer for patients than for controls in both task setups. The error rate was also significantly higher. The patients' reaction times in makingphonological judgments in the dual task were significantly longer than in the single task, but there was no significantdifference in error rates between the patients and the controls. In making semantic judgments, neither reaction times nor error rates differed significantly with either the single or dual task protocol. Conclusion Attention allocation deficits play a key role in Chinese character orthographic and phonological processing for nonfluent aphasia sufferers.
2.Magnetic Resonance Imaging in the Evaluation of Leptomeningeal Metastasis of Gliomas

Objective To evaluate the characteristics of leptomeningeal metastasis of gliomas on magnetic resonance(MR) imaging and to provide evidence for clinical early diagnosis and treatment.Methods Twenty-one cases of intracranial gliomas with leptomeningeal metastases were scanned with conventional spin echo(SE) and fast spin echo(TSE),8 cases with fluid attenuated inversion recovery(FLAIR),and all of cases were operated with enhancement.Results ⑴ With MR plain scan,18 cases revealed vary range of distortion and illegibility structure of cerebral cistern and sulcus,and 6 cases revealed the asymmetrical enhancement of the wall of ventricle.Eleven meningeal neoplasms nodes were found in subarachnoid space in 7 cases,and some of the appearances could be seen in several cases.Eight cases of them were added with FLAIR,and the neoplasms nodes were showed more clearly and higher signal than TSE-T2W imaging.⑵ With MR enhancement,all cases with encephalic gliomas showed obvious enhancement with vary features of the enhancement of leptomeningeal metastasis,including tail sign(5/21),line sign(10/21),strip sign(3/21),node sign(7/21) and a few of cases included two signs.Conclusion It was very important for clinician to improve the cognition of MR imaging characteristics of leptomeningeal metastases of glioma,and to choose the proper MR imaging scan technique to increase the detection rate of early meningeal diseases.
3.The changes in proinflammatory cytokines in the circulation following acute pulmonary thromboembolism in dogs

Objective In the present study we examined the roles of three proinflammatory cytokines, i.e. tumor necrosis factor-?(TNF-?), interleukin 1?(IL-1?), and IL-8 in the early phase of APTE, and analyze the impact of these cytokines on the systemic circulation. Methods APTE was induced in 7 anesthetized dogs by injecting the thrombus of own blood into the right jugular vein. Seven animals receiving only warm sterile saline served as controls. 5.A sduty on changes of hepatic energy metabolism and their management in acute obstructive cholangitis in rats

:Mitochondrial respiratory rate,respiratory control rate(RCR),oxidative phospho-rylation efficiency(ADP/O),and hepatic ATP content were determined in rats under 5 conditions including acute obstructive cholangitis(AOC),bile duct ligation(BDL),verapamil pretreatment in AOC(VET),dexamethasone pretreatment in AOC(DET).and sham operation(SO).It was found that rnitochondrial respiratory rate especially its state 4 phase was significantly increased and RCR,ADP/O,and ATP were significantly decreased at 6th hour after AOC and at 24th after BDL.These changes occurred earlier and more dramatically in AOC than in BDL.Pretreatment with verapamil or dexamethasone in AOC could minimize the changes of RCR,ADP/O and ATP in varying intervals.The findings indicate that AOC and BDL can both result in severe functional impairment of the mitochondria of hepatocytes and verapamil and dexamethasone exert effective protection on the rnitochondrial functions of respiration and energy metablism.

Objective To evaluate the effects of different doses of dexmedetomidine administered intranasally on the median effective concentration (EC50) of sevoflurane inhibiting responses to laryngeal mask airway (LMA) insertion in the pediatric patients.Methods American Society of Anesthesiologists physical status Ⅰ or Ⅱ pediatric patients of both sexes, aged 1-3 yr, with body mass index of 20-25 kg/m2, scheduled for elective surgery under general anesthesia, were randomly divided into 3 groups: control group (group C) , dexmedetomidine 1 μg/kg group (group D1) , and dexmedetomidine 2 μg/kg group (group D2).In D1 and D2 groups, dexmedetomidine 1 and 2 μg/kg (in 1 ml of normal saline) were administered intranasally at 1 h before induction of anesthesia, respectively.Group C received the equal volume of normal saline administered intranasally.The children were separated from their parents at 45-60 min after intranasal administration and admitted to the operating room.The children inhaled 8% sevoflurane until they lost consciousness.After loss of consciousness, the end-tidal concentration of sevoflurane was determined using the modified Dixon's up-and-down method.The initial end-tidal concentration of sevoflurane was set at 2%.The concentration was increased/decreased by 0.2% in the next patient according to the response to LMA insertion.The patient's sedation status and with LMA acceptance were evaluated, and the patient's satisfaction with sedation and with LMA acceptance was recorded.Probit analysis was used to calculate the EC50 and 95% confidence interval of sevoflurane inhibiting responses to LMA insertion.Results The patient's satisfaction with sedation and with LMA acceptance was significantly higher in D1 and D2 groups than in group C, and in group D2 than in group D1 (P<0.05).The EC50 (95% confidence interval) of sevoflurane inhibiting responses to LMA insertion was 2.03% (1.91%-2.15%), 1.76% (1.65%-1.87%) and 1.63% (1.53%-1.73%) in C, D1 and D2 groups, respectively.The EC50 was significantly lower in D1 and D2 groups than in group C, and in group D2 than in group D1 (P<0.05).Conclusion Dexmedetomidine 1 and 2 μg/kg administered intranasally can decrease the EC50 of sevoflurane inhibiting responses to LMA insertion in the pediatric patients, and 2 μg/kg produces better efficacy.
8.The tracing application of nano-active carbon loaded with anticancer drug in the radical operation of gastric carcinoma

Objective To investigate the tracing value of drug-loaded nano-active carbon for lymphatic visualization and its potential application in the lymphadenectomy of gastric cancer. Methods Sixty-three patients with advanced gastric carcinoma were randomized into 2 groups:control group and tracing group. For the tracing group, 5-Fu-loaded nano-active carbon (NP-5-FU) suspension was intratumorally administered by preoperative gastroscope-guided or intra-operative injection. No tracing reagents was given in the control group. The numbers of removed lymph nodes and metastasis-involved lymph nodes as well as the lymph node staining were recorded and compared between the two groups. The adverse effects was monitored. Results The resected lymph nodes in the tracing group were much more than that of control group (43.73 ± 9. 25 vs. 25. 36 ± 4. 73, t = 3.82,P < 0. 05). For the N1, N2 and N3 lymph nodes, 27. 17 ± 5. 82,13.73 ± 4. 61 and 2. 83 ± 0. 15 per case of the tracing group were desected, while they were 14. 45 ± 2. 75,8. 27 ± 1.89, and 2. 64 ± 0. 11 per case in the control group,respectively (P < 0. 05 ). 26. 2% (211/842) of the lymph nodes in the tracing group showed black intensity,higher than that of unstain lymph nodes (4. 5% ,21/470,χ2 =95. 108,P <0. 05) and that in control group (16.9%, 142/837)( χ2 =7.485 ,P < 0.05, ). No local and systemic adverse reactions were observed for the local injection of nano-active carbon. No extension of operating time and increased complications occurred. Conclusion Local injection of nano-active carbon which was loaded with 5-FU around the tumor is a very useful measure to trace the lymph nodes and facilitate the lymphedetectomy and chemotherapy.
9.Risk factor analysis for 30 - day mortality in patients with malignant hilar obstruction after percutaneous transhepatic biliary stent deployment

Objective To analyze the risk factors of 30-day mortality in patients with malignant hilar obstruction (MHO) after percutaneous transhepatic biliary metal stent deployment. Methods One hundred and fifty-nine consecutive patients with MHO caused by cholangiocarcinoma or gallbladder carcinoma were enrolled in this study. Percutaneous transhepatic biliary stent (PTBS) implantation was carried out in all the patients. Independent predictors for 30-day mortality were evaluated by logistic regression analysis. Covariates that were incorporated into the multivariate analysis were the variables that reached statistical significance (P < 0.1) in univariate analysis. Two-tailed, P value of less than 0.05 was considered to be statistically significant. Results The 30-day mortality of patient with MHO after metal stent deployment was 9.4%. Univariate analysis indicated that the differences in WBC (OR = 1.224.95%CI [1.07 - 1.44], P < 0.01), INR (OR=78.75, 95%CI [5.02-1 235.70], P<0.01), PT(OR=1.55, 95%CI [1.18-2.04], P<0.01), BUN (OR=1.19, 95%CI [1.02- 1.38], P < 0.05), CRE(OR = 1.02, 95%CI [1.000 - 1.041], P < 0.1) and lymph nodes metastasis(OR = 0.334. 95%CI[0.105 - 1.131], P < 0.1) were statistically significantly between 30-day mortality group and non-30-day mortality group. Multivariate analysis showed that statistically significant differences in WBC (OR = 1.19, 95%CI[1.026 - 1.380], P < 0.05), INR(OR = 151.5, 95%CI [3.13 - 5 440.7], P < 0.05) and CRE (OR = 1.025, 95%CI [1.002 - 1.048], P < 0.05) also existed palliative treatment for patients with malignant hilar obstruction. Active preoperative measures to improve hepatic and renal functions as well as to control infection are necessary in order to reduce 30-day mortality.

In order to solve the difficucties of renaturation and immunogenicity of new bifunctional fusion protein GAD,inclusion bodies of GAD were modified by PEG-maleimide.Conformational changes of the modified GAD were compared by circular dichroism and tryptophan fluorescence spectroscopy.The biological activity was verified by oral glucose tolerance test and lipid scavenging.The results showed that PEG-maleimide completed the specific-point modification of GAD,and improved its refolding efficiency.The secondary and tertiary structures of mPEGylated GAD were consistent with that of GAD.PEG-GAD has significant hypoglycemic and lipid-lowering effects (P <0.001) with longer half life in vivo and lower immunogenicity (P <0.01).This study provides effective strategies for the development of strongly hydrophobic peptide drugs.
