1.Research advances in postoperative adjuvant radiotherapy for stage pN0 esophageal cancer
Chinese Journal of Radiation Oncology 2017;26(1):111-115
Chest?regional recurrence and metastasis are the main modes of treatment failure after surgery for patients with esophageal cancer. As one of the primary approaches for local therapy, radiotherapy has a satisfactory efficacy in the reduction of postoperative chest?regional recurrence in patients with esophageal cancer. Right now, the value of postoperative adjuvant therapy for patients with positive lymph nodes has been widely recognized by clinicians. Postoperative adjuvant chemoradiotherapy has become a standard treatment mode. However, there is no consensus on whether adjuvant radiotherapy should be performed in patients with negative lymph nodes. To determine whether patients with stage pN0 esophageal cancer should receive postoperative adjuvant therapy, this paper reviews the current research advances in possible evidence for the necessity of postoperative adjuvant radiotherapy for patients with stage pN0 esophageal cancer, current related reports, and the effects of different surgical methods on treatment outcomes of postoperative adjuvant therapy.

Objective: Patient-reported outcome (PRO) method was used to evaluate clinical effects of acupuncture for cervical spondysosis. Methods: With RCT method used, 162 cases were adopted, and 150 cases finished the whole course (treatment group: 76 cases, control group: 74 cases). Treatment group cases were treated by real-acupuncture while control group cases treated by sham-acupuncture. Northwick Park Pain Questionnaire(NPQ)was used as main outcome index, while SF-36 as secondary index. Both groups’ patients were evaluated before and after treatment course, plus one month and 3 months after the end of the treatment course. Results:Treatment group cases got lower NPQ Scores and higher mental health dimensions scores than the control group cases at any evaluation time-point. At the point of one month after the end of the treatment course, the treatment group cases got better social function scores than the control group cases. At the point of three months after the end of the treatment course, the treatment group cases got better body pain and energy scores than the control group cases. Multiple regression analysis showed that three dimensions of SF-36 (social function scores body pain and energy scores) had close relationship with NPQ scores. Conclusion: Acupuncture had satisfactory long-term effect for cervical spondylosis.

ATP-sensitive potassium channels (KATP channels) play important roles in various tissues under physiological and pathophysiological conditions by coupling cell metabolic status to electrical activity. The KATP channe l is a tetrameric complex of inwardly rectified potassium (Kir) and ATP binding protein (ABP). The Kir subunits form the channel pore, whereas ABP is required for activation and regulation. Both Kir and ABP are divided into different subu nits and the various Kir and ABP subunits “mix and match” to form KATP c hannels with different pharmacological and nucleotide sensitivities. This revie w focuses on the molecular structure, physiological roles, pathophysiological an d pharmacological properties of KATP channels in different tissues.

Objective To investigate the effectivety and safety of carotid angioplasty and stenting (CAS) with cerebral protection fitler for elder and high risk patients with symptomatic carotid stenosis. Methods 25 patients with symptomatic carotid stenosis over 60 years old at high risk were treated with CAS. Results The procedures were technically successful in all patients. All the degrees of stenosis after CAS were less than 20%. Clinical symptoms disappeared. Heart rate and blood pressure were decreased differently in 24 cases.None of them experienced serious clinical complications. Conclusion CAS with cerebral protection fitler are safe and effective in treatment of elder and high risk of patients with symptomatic carotid stenosis.

The application of intracranial stent-assisted angioplasty has been in controversy. In recent years, the development novel stents, preoperative evaluation, and accumulation of periprocedural management experiences have significantly improved the success rate of this operation. The incidence of complications has decreased significantly. However, the efficacy and safety of intracranial stent-assisted angioplasty are still lack of support from prospective multicenter studies. This article reviews the recent advances in this field.

Objective To study CT diagnostic value of benign and malignant salivary gland lesions. Methods CT features in 36 cases with pathologically-proved salivary lesions were retrospectively analyzed. There were 19 cases with benign tumors,5 cases with chronic inflammation,1 case with tuberculosis and 11 cases with malignant tumors. Results Benign tumors were regular in 17 and well-defined in 16.All cases with chronic inflammation and 1 case with tuberculosis were regular but ill-defined.Benign lesions showed homogeneous or inhomogeneous density.Of all 11 malignant tumors,7 cases presented ill-defined margin with irregularity in shape, infiltration of adjacent subcutaneous fat tissue and disappearing of epiderm.Salivary glands enlarged and looked like "cast form" in malignant tumors. Most of the malignant tumors and benign tumors presented inhomogeneous density. Conclusion CT scan is of important value in the diagnosis of salivary diseases.

Journal of Chinese Physician 2000;0(12):-
Objective To investigate the effects of comprehensive intervention on population death rate. Methods To analyze population crude death rate, cause-specific death rate, and compare the death rate in intervention group with that of control. Results The average crude death rate in communities of Changsha was 617 79 per 100 000, with 557 27 per 100 000 of standarized death rate. Cerebrovascular disease, cardiocascular disease, tumour, disease of respiratory system injury and intoxication were from the first rank to the fifth in order the death causes. The death rate in intervention group was lower than that of control group significantly, and the death rate of cerebrovascular disease and cardiocascular disease in intervention group were lower than that of control significantly. Conclusions Cerebrovascular disease and cardiocascular disease are the major causes of death. Intervention on risks factors of cardio- and cerebrovascular diseases can lower the death rate of cardio- and cerebrovascular diseases and the population death rate.

Objective To study the correlation between serum homocysteine (Hcy) levels and chronic heart failure (CHF) with ultrasound cardiac function patients. Methods The subjects were 88 cases of CHF (CHF group) in our hospital between April 2014 to June 2015 between patients, selected 65 cases of healthy people as a control group over the same period in our hospital, using color Doppler ultrasound measurement of two groups patients of the left ventricular ejection fraction of patients groups (LVEF) and left ventricular end-diastolic diameter (LVEDd),and compared serum Hcy and NT-proBNP levels,LVEF and LVEDd in different NYHA classification patients, analysis of serum Hcy and NT-proBNP levels and LVEF,LVEDd between correlation of serum Hcy levels on cardiovascular incident. Results CHF group of the Hcy,NT-proBNP levels and LVEDd were significantly higher than control group (P < 0.01),LVEF was significantly lower than the control group (t =31.78,P =0.00);different NYHA functional class (Ⅱ ~ Ⅳ level) of Hcy,NT-proBNP,LVEDd and LVEF compared were statistically difference (P < 0.01);Pearson correlation analysis showed,CHF patients with LVEF serum Hcy levels were negatively correlated (r2 =0.974,P < 0.01),with LVEDd was positively correlated (r2 =0.896,P < 0.05),and higher serum Hcy levels, the higher the rate of cardiovascular happened in patients with CHF.Conclusion The serum Hcy and NT-proBNP levels in patients with CHF were significantly higher than healthy, and with the deterioration of heart function and increased while the LVEF was negatively correlated positively with LVEDd,cardiovascular events in high Hcy levels may also increase the incidence risk,so Hcy levels is expected as a new diagnostic marker CHF conditions change.
