2.Investigation and Analysis on the Components of Excellent Physician's Professionalism

Objective:To explore the components of medical professionalism among excellent physicians , and discuss the approaches and ways of cultivating professionalism of physicians .Methods: We designed a question-naire, on the basis of literature retrieval , to investigate 50 excellent clinical doctors that came from 8 class 3A hos-pitals in Shanghai .Then we interviewed 20 famous doctors of them , who had good reputation and prestige .Re-sults:The components of excellent physician's professionalism include moral quality character , achievement behav-ior character and interpersonal communication character .And each character consists of a collection of different competencies .Conclusions:There is a stable positive correlativity relationship between the degree of excellence and physician's professionalism .The cultivation of medical professionalism should be based on ability , practice and humanistic quality , and through various ways and measures .
3.Three Points on Laparoscope Operation to Gallbladder Neck Incarceration Stone(with A Clinical Report of 81 Cases)

[Objective] To discuss the reasonable treatment and skil of gal bladder calculus.[Method] Select 81 cases of such patients, take their clinical data as examples for analysis foundation;and choose relative documents for evidence of theoretic analysis.[Result] 77 cases were successively operated of laparo-scope, 3 transferred for laparotomy owing to difficult anatomy of Colot triangle, 1 made probe of choledoch because of choledocholithiasis;compared with documents results, the incision length, bleeding, operaiotn time, in-hospital time and complications of laparoscope operation, transferred laparotomy in the study were similar to relative documentary reports, the differences had no statistical meaning;the intranperative bleeding, operation period, in-hopital time, complications were better than documents, the differences had statistical meaning. [Conclusion] The correct pre-operation evaluation of disease condition, operation chance and methods selection, 3 operation points were the key of successful operaiton.
4.Identification of Actinobacillus actinomycetemcomitans serotypes by polymerase chain reaction

Objective:To investigate a method to identify Actinobacillus actinomycetemcomitans serotypes by polymerase chain reaction. Methods:18 Actinobacillus actinomycetemcomitans strains representing serotypes a to f were used in the test. 6 pairs of specific oligonucleotide primers for gene clusters involved in the biosynthesis of serotype specific polysaccharide antigens were designed. The primers were developed and evaluated in a genetic method of identifying serotypes of Actinobacillus actinomycetemcomitans strains by using a PCR assay. Results: Each pair of primers can specifically identify one serotype of Actinobacillus actinomycetemcomitans strains. No cross reaction was observed in all 18 strains. The PCR product sizes were as follows: 428 bp (serotype a), 298 bp (serotype b), 559 bp (serotype c), 690 bp (serotype d), 211 bp (serotype e) and 232 bp (serotype f). Conclusion:PCR method may be useful to identify Actinobacillus actinomycetemcomitans serotypes rapidly and directly.

6.Construction of Curriculum System of Medical Humanity Based on the Quality Features of Famous Doctors

Objective:To discuss the structure of medical humanistic curriculum system based on the research of quality features of excellent doctors .Methods:The goal , content and form of humanistic curriculum were based the factor of famous doctors'competencies which was build on the basis of literature review and experts'interview. Results:Ability and attitude or values accounted for high proportion of overall quality features .Humanities con-tained in most factors of excellent doctors'quality features;guiding significance of the structure of medical humanis-tic curriculum system is provided with excellent doctors'quality features .Conclusion:We should focus on the inte-gration of humanistic knowledge and practice , push humanities course into every stage of the medical education , further enrich category and quantity of medical humanities course and teaching form of medical humanities course , perfect the examination mode of medical humanities course in order to cultivate more excellent doctors .
7.Endovascular therapy for dysfunctional native arteriovenous fistulas

Objective To assess the feasibility and effectiveness of percutaneous transluminal angioplasty (PTA)for the salvage of immature arteriovenous fistula (AVF) and to identify the incidence of arterial and venous puncture site spasm.Methods The medical records and radiological data of 88 patients with 112 interventional procedures for immature AVFs were retrospectively reviewed.Results The stenosis lesions were (2.0 ± 1.4) cm long.Technical success rate and clinical success rate were 80.4% (78/97) and 92.8% (90/97) for PTA via brachial artery,85.7% (6/7) and 100% (7/7) for PTA via vein,25% (2/8) and 50% (4/8) for PTA via both brachial artery and vein,respectively.Spasm of pure arterial PTA occurred in 2 patients (2.1%) and was mild and moderate.Spasm of pure venous PTA occurred in 2 patients (28.6%) and was both moderate.Spasm of combined arterial and venous PTA occurred in 3 patients (37.5%) and from being severe to completely occluded.By comparison,there were statistical differences of technical and clinical success rate (P =0.000,0.019 ; P =0.000,0.029),fistulas spasm rate was statistically significant different (P =0.000).Conclusions Endovascular therapy was effective in restoring the dysfunctional native AVFs,it was safer and more effective and with less sideeffects especially in selecting coronary balloon to treat patients without large phlebangioma and round fistulas.
8.Effect of lycopene on H9c2 cell apoptosis induced by angiotensin Ⅱ

Objective To investigate the effect of lycopene (Lyc) on H9c2 cell apoptosis induced by angiotensin Ⅱ (Ang Ⅱ).Methods Using Ang Ⅱ (10 μmol/L) to stimulate H9c2 cells,we observed the protective effect of Lyc on H9c2 cells apoptosis.The H9c2 cells viability induced by different consideration of Lyc or Ang Ⅱ or both was detected by CCK8 assay.The expression levels of Bax and Bcl-2 in H9c2 cells were determined by real-time quantitative reverse transcription polymerase chain reaction (RT-PCR).Western blot was conducted to detect the protein expressions of Bax,Caspase 3,Caspase 9 and Bcl-2 in H9c2 cells.The apoptotic ratio of H9c2 cells was observed by TUNEL assay.Results Compared with control group,Ang Ⅱ could decrease the viability of H9c2 cells to (92.87±4.37)%.The result of RT-PCR showed that Ang Ⅱ decreased the expression level of Bcl-2,and Bax level was increased under the stimulation of Ang Ⅱ (P<0.05),while the expression level of Bcl-2 was increased and Bax level was decreased under the co-stimulation of Ang Ⅱ and Lyc in a concentration dependent manner,which indicated that Lyc ameliorated the apoptosis of H9c2 cells.The result of western blot showed that the protein expressions of Bax,Caspase 3 and Caspase 9 were increased,but Bcl-2 was decreased after the stimulation of Ang Ⅱ (P<0.05).While these phenomenon reversed apparently under the co stimulation of Ang Ⅱ and Lyc.A large number of apoptotic cells were observed under the stimulation of Ang Ⅱ through TUNEL assay.But the number of apoptotic cells reduced significantly under the co-stimulation of Lyc and Ang Ⅱ (P <0.05).Conclusions Lyc ameliorates the H9c2 cell apoptosis induced by Ang Ⅱ,which indicates that Lyc may have an important role in the treatment of various cardiovascular diseases.
9.Dynamic determination of serum osteopontin level in Pre-and postoperative patients with non-small cell lung cancer

Objective To explore the change of serum osteopontin levels in preoperative and postoperative patients with non-small cell lung cancer (NSCLC) and its clinic significance.Methods The levels of serum osteopontin were detected by ELISA.Results In 88 NSCLC patients,serum osteopontin levels were significantly higher than in benign pulmonary disease and healthy control group(P<0.05.P<0.01.respectively) There was no difference between benign pulmonary disease and healthy control group(P>0.05).Serum osteopontin levels were related to clinical stage and lymph node metastasis,and the levels of serum osteopontin were lower in postoperative patients than in preoperative cases(P<0.05).For NSCLC.the detection of osteopontin had a sensitivity of 84.1%,a specificity of 92.5%,and an accuracy of 88.1%. Conclusion Increased serum levels of osteopontin may be useful as a helpful clinical biomarker to diagnose earlier,predict prognosis and differentiate from benign pulmonary disease.
10.Effects of anti-viral therapy in combination with transcatheter arteria chemoembolization on recurrence of HBVrelated hepatocellular carcinoma after operation

Objective To investigate the preventive effects of anti-viral therapy in combination with transcatheter arteria chemoembolization (TACE) on recurrence of HBV-related hepatocellular carcinoma (HCC) after operation. Methods According to selection standards, 49 patients treated in this hospital from January 2003 to June 2009 were enrolled in this study and divided into a control group and combined group.The cumulative tumor-free survival rate, cumulative survival rate, the 1-, 2- and 3-year recurrence rates and cumulative recurrence rates were determined in all the patients and compared between the 2 groups. Results The cumulative tumor-free survival rate and cumulative survival rate were significantly higher in the combined group than in the control group (P=0. 019, 0. 008). The 1-, 2-, and 3-recurrence rates were 24% vs. 38.9%, 28% vs. 33.3%, and 28% vs. 22.2% in the combined group and control group, respectively. The 1-, 2-, and 3-year cumulative recurrence rates were 24% vs. 38. 9%, 52% vs. 72. 2%, and 80% vs.94. 4% in the combined group and control group, respectively. Conclusion Anti-viral therapy in combination with TACE can exert significant preventive effects on recurrence of HBV-related HCC after operation.
