Objective To observe the factors that affect cryosurvival of frozen-thawed embryos .Methods A retrospective study was conducted on 98 patients undergoing FET with two embryos of which one was complete cryosurvival and the other not .1∶1 matched samples logistic regression analysis was employed to observe the influence of cytoplasm with granulation ,vacuolar cyto-plasm ,embryo fragments ,blastomere number and equality of size on the cryosurvival of frozen-thawed embryos .Results Presence of vacuolar cytoplasm(OR=13 .413) or embryo fragments(OR= 1 .101) significantly increased blasstomere damage ,but the in-creased blastomere number(OR=0 .569) decreased it(P<0 .05) .Conclusion Embryos with vacuolar cytoplasm ,or embryo frag-ments and blastomere number are very vital factors that affect the blastomere damage after cryopreservation .

OBJECTIVE:To investigate the relationship between the perioperative application of antibiotics in caesarean opera-tion and neonatal hyperbilirubinemia,and to provide reference for reducing the incidence of neonatal hyperbilirubinemia. METH-ODS:264 cases of cesarean section women were randomly divided into control group and observation group,with 132 cases in each group. Control group was given antibiotics after operation,and observation group was given prophylactic application of antibi-otics before operation,and equivalent dose of intravenous injection after operation. Postoperative maximum and average tempera-ture,white blood cell(WBC),neutrophils(NEUT),complications and economic index were compared between 2 groups after op-eration. RESULTS:There was no statistically significant difference in postoperative maximum and average temperature,WBC count,NEUT proportion,wound infection and fever rate between 2 groups (P>0.05). The costs of medication,drug dosage per capita,hospital stay and the incidence of neonatal hyperbilirubinemia in observation group were significantly lower than in control group,with statistical significance (P<0.05). CONCLUSIONS:The perioperative standard use of antibiotics in caesarean opera-tion could effectively reduce the incidence of postoperative neonatal hyperbilirubinemia and hospitalization cost,and cut down treat-ment time. It also has a high economic value and worthy of promotion.

Objective To explore the effects of cognitive impairment on the self-management ability and self-management behavior among the patients of end stage renal disease (ESRD) with maintenance hemodialysis (MHD).Methods 134 MHD patients were divided into the non cognitive impairment group(62 subjects) and cognitive impairment group(72 subjects) according to Montreal cognitive assessment (MoCA)scores,to asses and analyze the level of hemodialysis knowledge,hemodialysis adherence,self-management ability and self-management behavior between the two groups.Results The rates of hemodialysis skipping behavior and the rates of hemodialysis shorting behavior in cognitive impairment group (respectively,18.06％,22.22％) were higher than that in non cognitive impairment group(respectively,6.45％,8.06％) with significant difference (x2 =4.0495,5.0526,all P＜0.05).The cognitive impairment group had significantly less total score (respectively,16.62 ± 1.99,51.32± 9.92,56.99±8.3) in the level of hemodialysis knowledge,the self-management ability and the self-management behavior compared with the non cognitive impairment group(respectively,18.48±2.03,63.69±10.16,78.54± 10.28) (t=5.3448,7.1172,13.4140,all P＜0.05).Conclusion Cognitive impairment can produce an effect on the level of hemodialysis knowledge and hemodialysis adherence.Cognitive impairment can also reduce the function of self-management ability and self-management behavior and make badly further effect on curative effect of hemodialysis.

Objective To observe a recombinant mutant of HBV core protein for dominant negative gene therapy against HBV encapsidation in vitro. Methods C gene and S gene of HBV were acquired through PCR and subcloned into pGEM T to construct pGEM T C and pGEM T S respectively. After digestion and ligation of these two plasmids, pGEM T CS was constructed. The cloned gene was inserted into pcDNA3.1 + to construct pcDNA3.1 + CS, which was identified by DNA sequencing. The recombinant plasmids were transformed into HepG2 cells, and screened with G418. The resistant HepG2 cell clones were chosen to test the expression of core surface protein by RT PCR, and the expressing HepG2 clones were cultured with 10% HBV DNA positive human serum for 72 hours. The intracellular HBV particles were extracted and the DNA was subjected to dot hybridization. Results The analysis showed that the HepG2 cells expressing mutant C protein had capabilities to resist HBV invasion in varied degrees. The mutant C protein had a dominant negative role in the encapsidation of HBV compared with the naive part of core protein. Conclusions The production of recombinant mutant core protein has a potential value for gene therapy against HBV infection.

Objectives To reveal the epidemiological trends of leprosy and the achievements of leprosy control in Fujian Province. Methods In the light of methods of Basic Statistics in Handbook of Leprosy Control written by Ma Hai De, each statistic index was analyzed. Results① The incidence and detection rate decreased from 0.782/10 000 and 1.231/10 000 in the period of 1955～ 1959 to 0.036/10 000 in the period of 1990～ 1994 and to 0.033/10 000 in the period of 1995～ 1998 respectively;② prevalence dropped from the highest of 0.65‰ in 1959 to 0.007‰ in 1998;③ with the elapse of time, the average age at onset showed a shift to advance age group, and incidence of leprosy in children decreased dramatically;④ the ratio of MB to PB was higher in early period and in late period than that in middle period, and the male ratio of MB to PB was always higher than female ratio;⑤ the average disease duration reduced, and the disability rate of new cases decreased. Conclusion Through the comprehensive measures against leprosy, the endemic of this disease in Fujian Province has been evidently under control.

Objective To explore the effect of transabdominal preperitoneal prosthetic(TAPP) repair combined with intraperitoneal onlay mesh(IPOM) on laparoscopic repair of inguinal hernia.Methods From January to December 2006,laparoscopic hernioplasty was performed on 30 adult patients with inguinal hernia.The hernia area was divided into two parts by the subperitoneal vessel across the area.The medial part was repaired by TAPP using polypropylene patch,and the lateral part was repaired by IPOM with Proceed patch.During to operation,an incision was made on the peritoneum along the subperitoneal vessel,and then the peritoneum was separated medially to expose an operative area(7.5 cm?6.5 cm),which was an avascular area with loose connective tissues and few vital nerves.Results All the operations were accomplished under epidural anesthesia.The operative time was 30-40 min(mean 35 min) for unilateral hernia,and 50-60 min(mean 55 min) for bilateral ones.The amount of blood loss during the operation was 1-5 ml.The patients could walk on the second day after the operation.They were discharged from the hospital 3-4 days after the operation,and were back to work in 2 weeks.During a 2-12-month follow-up(mean,9 months),no adhesive intestinal obstruction and local or systemic complications was observed.Conclusions The combined technique of IPOM and TAPP is a minimal invasive method with confirmed efficacy.By using this easily mastered method,the operative time is reduced,and the patients can recover quickly.

Objective To observe the therapeutic effects of endovascular stenting on the patients with drop attacks caused by carotid artery or vertebra-basilar artery stenosis.Methods Two drop attacks patients,who were diagnosed with symptomatic carotid artery or vertebra-basilar artery stenosis by DSA,were performed endovascular stenting.Case one had four serious artery stenoses at basilar artery,left vertebra artery and subclavian artery.Case two had double internal carotid artery.Six stents were placed,and filter protection devices were used in case two.After operating,the flowing up was performed for nine months or two years.Results The clinical symptoms of two patients were disappeared after operation.Case one performed flowing up for 9 months after operation,but he died by infarction of brain stem for his irregular taking medicine for 3 months since 6 months after operation.During flowing up for 2 years,case two was no relapse and no restenosis by DSA peoformed at 1 year after operating.Conclusions Endovascular stenting is a safe and efficient method for treating the patients with drop attacks caused by carotid artery or vertebra-basilar artery stenosis.

Objective: To explore the effects of life events, automatic thoughts, social support and coping styles on the severity of depression in major depression at discharge. Methods: A cohort of 97 major depressed in-patients was assessed at discharge. The assessment included Self-Rating Depression Scale (SDS), Life Events Scale, Social Support Scale, Automatic Thoughts Questionnaire and Coping Styles Questionnaire. 100 normal controls were assessed, too.Results: Compared with the controls, the patients experienced more stressful life events in the last one year(t=2.6,P

Ojective To study the diagnosis and treatment of upper extremity edema after radical mastectomy. Methods Diagnosis depended on B type ultrasoundgraphy, CT, lymphography, venography, biopsy and axilla exploration. Lymphaticovenous anastomosis (LVA) with microsurgery technology was used for the treatment of simple lymphoedema. Results In 40 cases of lymphoedma after radical mastectomy, 7 patients were diagnosed as metastasis of cancer. 3 patients were diagnosed obstrcution of lymphatic and vein. 30 patients with simple lymphedema were treated with LVA. Short term efficacy was observed in all patients.The efficacy at 3～12 years was 83.3%. Conclusion The diagnosis of upper limb edema after radical mastectomy is essential. Metastasis and obstruction of vein are the causes. Biopsy conducted under high frequency B type ultrasoundgraphy may be a good method for the diagnsosis of matastasis, but a few patients were diagnosed only by axilla exploration. LVA may be given first priority to the theraphy of lymphedema after radical mastectomy.
