1.Analysis of the influence factors of blastomere damage of frozen thawed embryos

Objective To observe the factors that affect cryosurvival of frozen-thawed embryos .Methods A retrospective study was conducted on 98 patients undergoing FET with two embryos of which one was complete cryosurvival and the other not .1∶1 matched samples logistic regression analysis was employed to observe the influence of cytoplasm with granulation ,vacuolar cyto-plasm ,embryo fragments ,blastomere number and equality of size on the cryosurvival of frozen-thawed embryos .Results Presence of vacuolar cytoplasm(OR=13 .413) or embryo fragments(OR= 1 .101) significantly increased blasstomere damage ,but the in-creased blastomere number(OR=0 .569) decreased it(P<0 .05) .Conclusion Embryos with vacuolar cytoplasm ,or embryo frag-ments and blastomere number are very vital factors that affect the blastomere damage after cryopreservation .
2.Study on the Relationship between the Perioperative Application of Antibiotics in Caesarean Operation and Neonatal Hyperbilirubinemia

OBJECTIVE:To investigate the relationship between the perioperative application of antibiotics in caesarean opera-tion and neonatal hyperbilirubinemia,and to provide reference for reducing the incidence of neonatal hyperbilirubinemia. METH-ODS:264 cases of cesarean section women were randomly divided into control group and observation group,with 132 cases in each group. Control group was given antibiotics after operation,and observation group was given prophylactic application of antibi-otics before operation,and equivalent dose of intravenous injection after operation. Postoperative maximum and average tempera-ture,white blood cell(WBC),neutrophils(NEUT),complications and economic index were compared between 2 groups after op-eration. RESULTS:There was no statistically significant difference in postoperative maximum and average temperature,WBC count,NEUT proportion,wound infection and fever rate between 2 groups (P>0.05). The costs of medication,drug dosage per capita,hospital stay and the incidence of neonatal hyperbilirubinemia in observation group were significantly lower than in control group,with statistical significance (P<0.05). CONCLUSIONS:The perioperative standard use of antibiotics in caesarean opera-tion could effectively reduce the incidence of postoperative neonatal hyperbilirubinemia and hospitalization cost,and cut down treat-ment time. It also has a high economic value and worthy of promotion.

4.The effects of cognitive impairment on the self-management ability and self-management behavior in patients of end stage renal disease with maintenance hemodialysis

Objective To explore the effects of cognitive impairment on the self-management ability and self-management behavior among the patients of end stage renal disease (ESRD) with maintenance hemodialysis (MHD).Methods 134 MHD patients were divided into the non cognitive impairment group(62 subjects) and cognitive impairment group(72 subjects) according to Montreal cognitive assessment (MoCA)scores,to asses and analyze the level of hemodialysis knowledge,hemodialysis adherence,self-management ability and self-management behavior between the two groups.Results The rates of hemodialysis skipping behavior and the rates of hemodialysis shorting behavior in cognitive impairment group (respectively,18.06%,22.22%) were higher than that in non cognitive impairment group(respectively,6.45%,8.06%) with significant difference (x2 =4.0495,5.0526,all P<0.05).The cognitive impairment group had significantly less total score (respectively,16.62 ± 1.99,51.32± 9.92,56.99±8.3) in the level of hemodialysis knowledge,the self-management ability and the self-management behavior compared with the non cognitive impairment group(respectively,18.48±2.03,63.69±10.16,78.54± 10.28) (t=5.3448,7.1172,13.4140,all P<0.05).Conclusion Cognitive impairment can produce an effect on the level of hemodialysis knowledge and hemodialysis adherence.Cognitive impairment can also reduce the function of self-management ability and self-management behavior and make badly further effect on curative effect of hemodialysis.
5.Effect of viral core protein mutant on HBV encapsidation

Objective To observe a recombinant mutant of HBV core protein for dominant negative gene therapy against HBV encapsidation in vitro. Methods C gene and S gene of HBV were acquired through PCR and subcloned into pGEM T to construct pGEM T C and pGEM T S respectively. After digestion and ligation of these two plasmids, pGEM T CS was constructed. The cloned gene was inserted into pcDNA3.1 + to construct pcDNA3.1 + CS, which was identified by DNA sequencing. The recombinant plasmids were transformed into HepG2 cells, and screened with G418. The resistant HepG2 cell clones were chosen to test the expression of core surface protein by RT PCR, and the expressing HepG2 clones were cultured with 10% HBV DNA positive human serum for 72 hours. The intracellular HBV particles were extracted and the DNA was subjected to dot hybridization. Results The analysis showed that the HepG2 cells expressing mutant C protein had capabilities to resist HBV invasion in varied degrees. The mutant C protein had a dominant negative role in the encapsidation of HBV compared with the naive part of core protein. Conclusions The production of recombinant mutant core protein has a potential value for gene therapy against HBV infection.
6.The diagnosis and treatment of upper extremity edema after radical mastectomy (A report of 40 cases)

Ojective To study the diagnosis and treatment of upper extremity edema after radical mastectomy. Methods Diagnosis depended on B type ultrasoundgraphy, CT, lymphography, venography, biopsy and axilla exploration. Lymphaticovenous anastomosis (LVA) with microsurgery technology was used for the treatment of simple lymphoedema. Results In 40 cases of lymphoedma after radical mastectomy, 7 patients were diagnosed as metastasis of cancer. 3 patients were diagnosed obstrcution of lymphatic and vein. 30 patients with simple lymphedema were treated with LVA. Short term efficacy was observed in all patients.The efficacy at 3~12 years was 83.3%. Conclusion The diagnosis of upper limb edema after radical mastectomy is essential. Metastasis and obstruction of vein are the causes. Biopsy conducted under high frequency B type ultrasoundgraphy may be a good method for the diagnsosis of matastasis, but a few patients were diagnosed only by axilla exploration. LVA may be given first priority to the theraphy of lymphedema after radical mastectomy.

Objective To evaluate mid-term clinical effects of laparoscopic intraperitoneal onlay mesh herniorrhaphy (IPOM) for adult inguinal hernia. Methods A retrospective analysis was made on 546 cases of inguinal hernia treated by IPOM (603 sides) between June 2002 to December 2004. We closed the hernia sac by the lift-insert method following with the implantation of the Prolene mesh with the help of a stapler. Results The operation was successfully completed in all the cases, except for 2 cases of conversions to open surgery because of difficulties in covering the defects. There were no intraoperative complications, and 5 cases of postoperative seroma which subsided after physical therapy. The operative time was 25~40 min (mean, 31 min), and the hospital stay was 36~72 h (mean, 46 h). Follow-up observations for 18~48 months (mean, 28 months) found 6 cases of recurrence, with the recurrence time at 30~45 months (mean,38.2 months). The racurrence rate was 0.995%. Conclusions Laparoscopic intraperitoneal onlay mesh herniorrhaphy a minimally invasive method with reliable outcomes, low recurrence rate, and rapid recovery.
