1.Application of carboplatin-activiated carbon particle delivery system in the treatment of oral carcinoma and its cervical metastasis

?Objective: To estimate the effect of carboplatin activiated carbon particle delivery system (CP CH) in the treatment of oral carcinoma and its cervical lymph node metastasis. Methods: CP CH was prepared by absorption of carboplatin with activated carbon particles and injected into the submucosa around the tumors 72 h before operation in 6 patients with tongue or buccal squamous cell carcinoma (SCC) and metastasis in cervical lymph nodes,then extensive tumorectomy with neck dissection was conducted. The specimens of lymph nodes,primary carcinoma and blood were analyzed by microscope and plasma emitting spectrometry. Results:60 out of 70 (83.33%) lymph nodes obtained from the CP CH treated cases were found to be stained black by carboplatin.The carboplatin concentrations in lymph nodes and blood were (23.36 ?2.58) ?g/g and (1.76?0.32) ?g/ml respectively.Necrosis and degeneration were observed in SCC tissue.Conclusion:CH CP may be effective in the treatment of oral SCC with cervical lymph metastasis by providing high concertration and long lasting carboplatin in the lessions.

3.Analysis on the knowledge of non-communicable disease among general practitioners in the community

Objective To better promote the health education and health promotion among patients with chronic disease, general practitioners (GPs) in the community should master the knowledge of non-communicable diseases (NCDs). This paper aimed to describe the current situation of GPs' knowledge about prevention and treatment of NCDs. Methods Self-designed questionnaires were issued to 1 077 GPs in community health service centers, and all GPs finished the questionnaire without assistance. The content of questionnaire includes the general information of GPs and six questions focusing on NCDs. Results Overall, 1 076 GPs completed the questionnaire, and the response rate was 99.9% (1 076/1 077). With regard to the six questions about NCDs, 90.4% (973/1 076) of GPs were well aware of the diagnosis of hypertension. Apart from that, the knowledge of diabetes mellitus (60.9%, 655/1 076), factors related to NCDs (70.2%, 755/1 076), the concepts of mental illness (52.0%, 560/1 076) and the clinical significance of tumor markers (87.9%, 946/1 076) among GPs were adequate, while more than half of the GPs did not know a lot about home oxygen therapy. Conclusion The mastering level of knowledge about prevention and treatment of NCDs among GPs varies that they need further improvement.
4.Correlation between empowerment and illness perception among patients with type 2 diabetes mellitus

Objective To explore the correlation of empowerment with illness perception among inpatients with type 2 diabetes mellitus. Method The convenience sampling method was used to investigate the status of empowerment and illness perception among 102 patients with type 2 diabetes mellitus from October 2014 to July 2015, followed by analyzing the association between the two variables. Results The total score of empowerment was (4.00 ± 0.65), and the total score of illness perception was (38.00 ± 2 . 33 ) . The empowerment was positively correlated with illness perception ( P < 0 . 001 ) . The empowerment was positively correlated with the timeline, personal control, treatment control, illness concern and illness comprehensibility, respectively (all P<0.001). Conclusions The empowerment of patients with type 2 diabetes mellitus is at a high level, and positively correlated with illness perception, more pronounced chronic nature, the better personal control and treatment control, a higher degree of concern about the disease and better personal understanding of the illness contributed to a higher degree of empowerment. Medical staff should evaluate the illness perception of the patients with type 2 diabetes mellitus regularly in the daily work so as to improve the illness perception , improve their empowerment and promote the patients′quality of life eventually.
5.Quality standard for Yinxie Tablets

AIM: To establish quality standard for Yinxie Tablets(Fructus Gardeniae, Radix Astragali, Radix Angelicae Sinensis, etc.). METHODS: Fructus Gardeniae, Radix Astragali and Radix Angelicae Sinensis were identified by TLC, and the content of geniposide was determined by HPLC. RESULTS: Fructus Gardeniae, Radix Astragali and Radix Angelicae Sinensis could be identified by TLC. The calibration curve of geniposide was linear (r=0.999 5). The precision was perfect (RSD=0.98%). The average recovery was 101.81% with RSD of ~0.54% . CONCLUSION: This method is simple, quick and specific, suitable for the quality control.

8.Extended pancreaticoduodenectomy for carcinoma of head of pancreas

Objective To study the effect of extended Whipple′s resection for carcinoma of head of pancreas.Methods Seventy-five patients who underwent surgery for carcinoma of head of pancreas were divided into control group(traditional Whipple′s operation,n=38) and study group(extended Whipple′s operation,n=37).The survival,complication,mortality and recurrence rates were compared between the 2 groups.Results In control group and study group,the complication rate was 60.53%and 29.73%;the mortality rate was 5.26% and 2.70%;the recurrence rate was 78.95% and 35.14%;and 3-year survival rate was 15.79% and 35.14%,respectively.Conclusions Extended Whipple′s resection for carcinoma of head of pancreas can decrease postoperative recurrence rate and increase 3-year survival rate.
9.Systemic complications of functional endoscopic sinus surgery in patients with chronic rhinosinusitis.

OBJECTIVE:
To analyses the causes and prevention of systemic complications of functional endoscopic sinus surgery (FESS) in patients with chronic rhinosinusitis.
METHOD:
Three typical cases were reported including their medical history, preoperative diagnosis, medications during preoperational period, complications and treatment. The causes and preventive measures of systemic complications were analyzed.
RESULT:
Three patients were all suffered from chronic rhinosinusitis with nasal polyps (CRSwNP). After FESS, 1 case was complicated with coma and hyponatremia, 1 case with acute myocardial infarction, and 1 case with lower extremity deep venous thrombosis. The patient with coma and hyponatremia was soon waked after intravenous infusion of 10% sodium chloride. Two patients with acute myocardial infarction and lower extremity deep venous thrombosis were soon completely rehabilitated after emergency thrombolytic therapy and endovascular intervention. Three patients were completed recovered from their systemic complications without any severe sequela.
CONCLUSION
Systemic hemostatic drugs should be banned in patients with hypercoagulable state in perioperation period of FESS in order to avoid severe systemic complications. Timely vascular interventional treatment can prevent severe sequels.

10.Study of effect of silencing RACK1 expression by shRNA on enhancing radiosensitivity of oral squamous cell carcinoma cells

Objective:To evaluate the effect of down-regulation of RACK1 expression on growth and radiosensitivity of oral squamous cell carcinoma cells.Methods:The shRNA vector for RACK1 gene was constructed and transfected into HSC-3 cells by lipofectamine. The stably-transfected cell line was obtained by constructing G418. The expression levels of RACK1 mRNA and protein were detected by RT-PCR and Western blot. The cell proliferation was detected by CCK8 assay. Cell apoptosis was examined by flow cytometry. The invasive and metastatic capabilities of cancer cells were assessed by cell invasion assay in vitro.The effect of X-ray irradiation combined with down-regulation of RACK1 expression upon cell proliferation was assessed by clone formation assay. The xenograft tumor nude mouse model was established to observe the inhibitory effect of down-regulating RACK1 gene expression combined with X-ray irradiation on oral squamous cell carcinoma. Results:RT-PCR revealed that the expression level of RACK1 mRNA of transfected HSC-3 cells was significantly down-regulated ( P<0.05). Western blot showed that the expression level of RACK1 protein was significantly down-regulated ( P<0.05). CCK8 assay demonstrated that down-regulation of RACK1 expression could remarkably inhibit the growth of HSC-3 cells ( P<0.05). RACK1 gene shRNA interference combined with X-ray irradiation significantly enhanced the apoptosis rate of HSC-3 cells ( P<0.05). The number of invasion cells in vitro in the RACK1 silencing group was evidently decreased ( P<0.05). Clone formation assay showed that the survival fraction in the shRACK1 group was significantly lower than that in the control group. The sensitization enhancement ratio was 1.37(ratio of D 0 value). Xenograft tumor experiment in nude mice showed that tumor growth was significantly inhibited in the shRACK1 group, the tumor volume was significantly decreased and the tumor mass was significantly lower than those in the control group (all P<0.05). Conclusion:Down-regulating RACK1 expression can enhance the radiosensitivity of oral squamous cell carcinoma cells, providing novel thinking to improve the radiosensitivity of oral squamous cell carcinoma.
