2.Diagnosis and treatment of delayed infection after hip arthroplasty

Objective To investigate the methods of diagnosis and treatment for delayed infection after hip arthroplasty. Methods From March 1998 to April 2004, 11 patients with delayed infection after hip arthroplasty were analyzed retrospectively. There were 4 males and 7 females with average of 66 years, ranging from 49 to 81 years. The interval between infection and primary hip arthroplasty varied from 9 to 96 months(mean 25.5 months). Pain is the most common symptom in all patients with an infected hip arthroplasty. Abnormal ESR was observed in 9 cases. The increase of CRP was found in 8 patients, and fistula occurred in 5 patients. Radiography findings, such as loosening, osteolysis, and endosteal scalloping, are present on 10 patients. One-stage exchange arthroplasty was performed in 8 patients, among whom 7 prostheses with antibiotic-impregnated cement and one with plain cement for fixation were chosen. Two-stage exchange arthroplasty was performed in 3 patients. After thorough irrigation and debridement of the infected joint and the removal of the femoral and acetabular components, a temporary spacer with irrigation device and antibiotic-loaded beads or a bulb were inserted between the first and second stage. The interval was a minimum of 4 weeks. During the delayed reconstruction, the final prostheses were fixed with antibiotic-loaded cement after the infection was eliminated. Results All 11 patients were followed up for a period of 8 to 72 months. Fracture of femoral shaft happened during the debridement in one patient. None of recurrent infection was found during the average 29.3 months follow-up. Conclusion No test is 100 percent specific; therefore, the diagnosis of infection relies on the patients' history, and the physical examination findings, and the lab test combined with results of imaging study, etiology as well as pathology. The key to success is that the treatment must be tailored to the patients' status.
3.Influence of percutaneous transhepatic gastric variceal embolization joint partial splenic embolization on hemodynamic of portal vein

Objective To explore the effect of percutaneous transhepatic gastric variceal embolization joint partial splenic embolization on hemodynamic of portal vein.Methods 54 cases who were clinically diagnosed as cirrhosis with portal hypertension,do the treatment percutaneous transhepatic gastric variceal embolization combined with partial splenic embolization.The preoperative and postoperative free portal vein pressure(FPP) were measured by portal vein intubation,and the preoperative and postoperative portal vein(PV),spleen vein(SV) in hemodynamic changes were compared between color dopplar ultrasound and CT scan scanning.Results Free portal vein pressure were significantly increased after percutaneous transhepatic gastric variceal embolization [(37.05 ± 4.27) cm H2O vs (42.60 ± 5.04)cm H2 O,P < 0.01],and reduced through continuing to partial splenic embolization.In the end there were no statistically significant differences (P > 0.05).After 3 months portal venous blood flow and inner diameter and blood flow velocity only is a little change,but spleen vein inner diameter,blood flow velocity and blood flow were decreased significantly(P < 0.01).Conclusion Only percutaneous coronary arteriovenous embolization can increase portal vein pressure,but which joint partial splenic embolization does not affect the thange of portal venous blood flow dynamics.

5.Stability Study of Tramadol,Fentanyl and Ondansetron Combined with 0.9% Sodium Chloride Injection in Analgesia Pump

OBJECTIVE:To study the stability of Tramadol hydrochloride,Fentanyl citrate and Ondansetron hydrochloride in-jection in 0.9% Sodium chloride injection. METHODS:At room temperature [(25±1)℃],three injections were poured into infu-sion bag of disposable analgesia pump,and diluted with 0.9% Sodium chloride injection. The changes in precipitation,turbidity, color and pH value of the mixture were observed. The relative percentage of three injections within 72 h were determined by HPLC method. RESULTS:There was no significant change in the pH value and appearance of the mixture. Relative percentage of tramad-ol hydrochloride,fentanyl citrate and ondansetron hydrochloride were all higher than 97% within 72 h. CONCLUSIONS:The mix-ture of Tramadol hydrochloride,Fentanyl citrate and Ondansetron hydrochloride injection in 0.9% Sodium chloride injection keep stable within 72 h under room temperature.
6.Research progress of the relationship between HBV and NHL

[Objective] China is one of high hepatitis B virus (HBV)-epidemic areas. The role of HBV and nonHodgkin lymphoma (NHL) has also recently been investigated. There is significantly higher prevalence rate of HBV infection among patients with NHL compared with other solid tumor and general population. HBV reactivation during chemotherapy is, however, a real concern. Several previous studies have reported increased risks of liver-related morbidity and mortality. The use of prophylactic anti-HBV drugs can effectively reduce the incidence and severity of hepatitis among HBV-infected patients who received chemotherapy for NHL.
7.Bibliometric Analysis of the Clinical Literature of Acupoint Thread-embedding for Simple Obesity

Objective To analyze the bibliometry of the literature of acupoint thread-embedding in treating simple obesity, and to provide references for clinic.Method By both computer and manual retrieval, the relevant literatures over the past 30 years were collected from the Chinese Journal Full-text Database (CJFD), Vip database, WanFang database. By using the SPSS 17.0 software for statistical analysis, the distribution (province or city), publication year, and journal of the literatures, supporting fund, sample size, research method, material of the thread, use frequency of the involved acupoints, and the meridians of the selected acupoints were summarized.Result The included eligible 175 articles were distributed over many provinces and cities; the publication amount had been increasing year by year; there was a variety of thread materials; the literature with supporting funds only occupied a small percentage, while the research with a small sample size occupied a comparatively large percentage, and the randomized controlled trials (RCTs) also had a small percentage.Conclusion As a common Chinese external therapy in treating simple obesity, acupoint thread-embedding has been paid more and more attention for its practical value and bright future. However, the relevant governmental department hasn’t paid enough attention to this therapy, and the low clinical research quality also makes it difficult to scientifically evaluate its clinical efficacy and safety. Therefore, more attention is required, RCTs of multicenter and large sample size are expected, and clinical research quality also needs improvement, to provide references for clinic.
8.Eenie, Meenie, Miney, Moe, who is responsible for the antibody-dependent enhancement of Aleutian mink disease parvovirus infection?.

Aleutian mink disease parvovirus (AMDV) causes a persistent infection associated with immune complex disease, hypergammaglobulinemia, and high levels of antiviral antibodies. Despite the presence of an antibody, the virus is not cleared in vivo. Pre-existing antibodies may enhance viral infections, by Fc-receptor-mediated antibody-dependent enhancement (ADE), but the mechanism that underlies ADE has not been fully defined. Three models have been proposed, including: (1) interactions between antibody and FcR, complement C3 fragment and CR, or between C1q and C1qR, which promotes viral attachment to cells; (2) suppression of IFN-gamma-mediated host-cell antiviral gene expression by the upregulation of negative regulators of pathogen pattern recognition; and (3) the promotion of early IL-10 secretion. In addition, the role of cytokine IL-6 in ADE mediated disease development is discussed, to facilitate a better understanding of the pathogenesis of AMDV infection, as well as give insights into rational vaccine design approaches.

9.Acupuncture and stress.

The relationship between acupuncture and stress is discussed from three aspects, including is it possible for acupuncture stimulation to be a stressor, whether acupuncture will start stress reaction, and whether acupuncture effects contain some stress factors. It is believed that correct acupuncture manipulation will not cause stress response, however, under some circumstances, such as inaccurate manipulation, improper treatment or patients who are very nervous but do not receive effective intervention, acupuncture is likely to cause stress response. Acupuncture-induced stress response is totally different from acupuncture anti-stress. The possible stress factors in acupuncture effect are explored, which can provide a new angle for the research on action mechanism of acupuncture. From the view of stress to review acupuncture treatment, there are three enlightenments: emphasizing on communication between doctors and patients, avoiding over-pursuit of deqi and focusing on analysis of the body constitution.

10.Analysis of CT features of pulmonary sarcoidosis

Objective To investigate the CT features of pulmonary sarcoidosis.Methods Ninety patients with histologically proved pulmonary sarcoidosis were retrospectively studied by using CT scans and clinical recording.Results The main CT findings of pulmonary sarcoidosis were nodules which were seen in 69 cases(76.7%),and the nodules mostly distributed around the bronchovascular bundle(n=37, 41.1%).Other abnormalities included consolidation(n=31,34.4%),ground-grass(n=39,43.3 %), thickening of bronchovascular bundle(n=30,33.3%),interlobular septal lines(n=58,64.4%), fibrosis(n=17,18.9%)including bronchial distortion(n=8,8.9%),linear shadow(n=5,5.6%), and honeycombing shadow(n=4,4.4%),air-trapping(n=3,5.3%),bronchial straitness(n=8, 8.9%),pleural thickening(n=42,46.7%),and hilar and mediastinal adenopathy(n=76,84.4%). Two or more abnormal findings co-existed in 83 cases.The pulmonary lesions co-existed with hilar and mediastinal adenopathy in 76 cases.The nodules(n=25),consolidation(n=9),ground-grass(n=11), thickening of bronehovascular bundle(n=10)were improved after therapy.Ten cases of the interlobular septal(10/22),0 of bronchial distortion(0/4),1 case of diffuse linear(1/3),and 0 case of honeycombing(0/2)were improved.Conclusion CT manifestations of pulmonary sarcoidosis are varied, but has some specific radiographic features.A correct diagnosis can be made.combined with hilar and mediastinal adenopathy.
