OBJECTIVE:To establish a method for simultaneous determination of matrine,linarin,berberine hydrochloride and osthole in Shenju lotion. METHODS:HPLC was performed on the column of Dionex C18 with mobile phase of acetonitrle-0.1% triethylamine(pH was adjusted to 7.2 with phosphoric)(gradient elution)at a flow rate of 1.0 ml/min,detection wavelength was 210 nm,column temperature was 30 ℃,and injection volume was 20 μl. RESULTS:The linear range was 1.400-8.400 μg for matrine (r=0.999 4),0.152-0.912 μg for linarin (r=0.999 6),0.248-1.488 μg for berberine hydrochloride (r=0.999 9) and 0.128-0.768 μg for osthole (r=0.999 4);limit of quantitation was 84.85,3.15,15.03,7.76 ng,limit of detection was 24.13, 0.89,4.27,2.20 ng;RSDs of precision,stability and reproducibility tests were lower than 2%;recoveries were 96.29%-99.06%(RSD=1.10%,n=9),96.49%-98.27%(RSD=0.69%,n=9),99.26%-101.13%(RSD=0.62%,n=9) and 96.27%-98.64%(RSD=0.89%,n=9). CONCLUSIONS:The method is simple,accurate and reproducible,and can be used for the simultaneous determination of matrine,linarin,berberine hydrochloride and osthole in Shenju lotion.

Objective To compare the postoperative effect of pars plana vitrectomy,phacoemulsification and intraoeular lens (IOL)implantation with silicon oil removed,phacoemulsification and IOL implantation.Design A retrospective case-control study.Par- ticipants 91 cases(104 eyes)of proliferative diabetic retinopathy with tractive retinal detachment or vitreous hemorrhage were per- formed cataract surgery with IOL implantation.Methods All patients were followed up average 6 months,in whom 52 eyes with pars plana vitrectomy,phacoemulsification and IOL implantation(combined group),and the other 52 eyes with silicon oil removed,pha- coemulsification and IOL implantation(sequential group).The differences of the postoperative visual acuity and inflammatory reactions of the anterior chamber between the two groups were analyzed.Main outcome measures Visual acuity and inflammatory reactions of the anterior chamber.Results There were 14/52 eyes with best corrected visual acuity more than 0.1 in combined group and 8/52 eyes in the sequential group after the surgeries(X~2=-6.87,P=0.07) .Corneal edema happened in 16/52 eyes in combined group and 6/52 eyes in sequential group 1 week after the surgeries(X~2=11.53,P=0.001).Keratic precipitate happened in 20/52 eyes in combined group and 9/52 eyes in sequential group after the surgeries(X~2=5.79,P=0.019) .Conclusions The incidence of postoperative anterior chamber in- flammation is higher in Phacoemulsification combined with pars plana vitrectomy group.

Objective To investigate the occurrence of p16 gene methylation in the population chronically exposed to arsenic in Inner Mongolia,and to study the molecular mechanisms of carcinogenesis associated with arsenic exposure.Methods The study group was composed of 40 cases of typical arseniasis selected from the epidemic area,and the two control groups consisting respectively of 40 non-arseniasis cases selected from the same epidemic area,and 40 healthy persons enrolled from non-epidemic area.Methylation of p16 gene in the blood specimens were analyzed for all the subjects with MS-PCR techniques,and statistical analysis was performed using chi square test.Results The positive rates of p16 hypermethylation in blood specimens were 65.0%,47.5% and 20.0% respectively in study group and two control groups,and the rate of hypermethylation increased with the increase in arsenic exposure with drinking water in epidemic areas.The positive rate of p16 hypermethylation showed significant differences(P

Objective To explain the material foundation of anti-inflammatory and analgesic effects of Jinxuan Zhike Xunxi San. Methods The foot swelling experiments in rat and xylene-induced ear edema experiments in mouse were adopted to observe the anti-inflammatory effects of the material extracted and purified from Jinxuan Zhike Xunxi San. The hot plate-induced pain experiment for studying the analgesic effect was established. Results The material of supercritical CO2 extract, saponins and flavonoids extract and minerals in Jinxuan Zhike Xunxi San had a inhibiting effect on egg-induced foot swelling in rat, saponins and flavonoids extract had a distinct inhibiting effect on xylene-induced ear edema in mouse. Every extract had obvious analgesic effects on hot plate-induced pain and significantly increased the threshold of pain. Conclusion The active ingredient groups of Jinxuan Zhike Xunxi San are the material of supercritical CO2 extract, saponins and flavonoids extract and minerals.

BACKGROUNDTo study the diagnostic value of detection of p16 methylation of sputum exfoliated cells for peripheral lung cancer.

METHODSBefore operation, p16 methylation of the sputum exfoliated cells was detected in 20 normal persons and 50 patients with peripheral pulmonary nodi by p16 methylation-specific PCR (MSP) for judging the nature of the nodi. The results were compared with pathological reports after operation.

RESULTSThe p16 MSP positive rate of sputum exfoliated cells of peripheral lung cancer patients (27/44, 61.4%) was much higher than that of benign pulmonary nodi (1/6, 16.7%) and normal persons (3/20, 15.0%)(Chi-Square=4.281 and 11.869 respectively, both P < 0.05). No significant difference of p16 MSP positive rate of sputum exfoliated cells was found between benign pulmonary nodi and normal persons (Chi-Square= 0.136, P > 0.05). No significant difference of p16 MSP positive rate of sputum exfoliated cells was found between squamous cell carcinoma (13/18, 72.2%) and adenocarcinoma (8/19, 42.1%) (Chi-Square=3.416, P > 0.05 ). If the positive p16 MSP of sputum was chosen as criterion for peripheral lung cancer, its positive predictive value, negative predictive value, sensitivity and specificity were 96.4%, 22.7%, 61.4% and 83.0% respectively.

CONCLUSIONSThe detection of p16 methylation of sputum exfoliated cells is contributable to the diagnosis of lung cancer.

Objective To explore the trend of mortality and years of life lost due to Esophageal Cancer in residents in Tieling,so as to provide the basis data on preventing Esophageal cancer in Tieling.Methods The data of residents in Tieling dying of Esophageal cancer from 2007 to 2015 was collected and cleared up to calculate the evaluation indexes including the mortality rate,the average percentage change of mortality rate.GM(1,1) model was used to predict the future mortality.Results From 2007 to 2015,the Average Esophageal cancer Mortality Rate of in residents in Tieling was 5.26 per 100000 persons,and especially 1.95％ raised a year.The Mortality Rate would increase from 2016 to 2019.Conclusion Tieling Esophageal Cancer mortality rate is on the rise,especially for elder men more than 60.So that the proper prevention measures should be car ried and strengthened.

Objective To investigate the prognostic value of National Early Warning Scores (NEWS) combined with arterial lactate level in elderly patients with critical illness in emergency department.Methods A total of 413 elderly patients with critical illness randomly (random number) selected from resuscitation room in emergency department from September 2013 to July 2015 were enrolled in this study.NEWS system was employed to determine the patients' condition.Arterial lactate level was detected.The patients were followed for the 30-day death by telephone calls and patients were divided into survival group and death group.Multivariate logistic regression analysis and receiver operating characteristic curve (ROC) were used to evaluate the prognostic values of NEWS score,arterial lactate level,and NEWS score combined with arterial lactate level in patients without survival.Results There were significant differences in NEWS score and arterial lactate level between the death group and the survival group.The results of multiple factor regression showed that NEWS score,arterial lactate level,and NEWS score combined with arterial lactate level were all independent predictors of 30-day death (P ＜ 0.05,OR ＞ 1).The AUCs of the three evaluation modalities were 0.721,0.746 and 0.762,respectively.There was a significant difference in the area under the ROC curve between NEWS score and NEWS score combined with arterial lactate level.The optimal cut-off point of NEWS score was 7,and arterial lactate level was 2.7 mmol/L.Conclusions NEWS score,arterial lactate level,and NEWS score combined with arterial lactate level were independent predictors of 30-day death in elderly patients with critical illness in emergency department.They all had some predictive value,and NEWS score combined with arterial lactate level had higher capability to predict 30-day death in elderly patients with critical illness in emergency department than NEWS score alone.
