1.Analysis on urinary system lesions of IgG4-related disease

Objective To explore the clinical features of renal and urinary lesions in IgG4-related disease (IgG4-RD).Methods Clinical manifestation,laboratory profiles,iconography images,pathologic findings,treatment and prognosis of 6 IgG4-RD patients with renal and urinary system involvement from Peking Union Medical College Hospital during Aug 2010 to Dec 2011 were analyzed retrospectively.Results Six patients had renal and/or urinary lesions among IgG4-RD cases diagnosed in our hospital,including 4 males and 2 women,with median age of 59 years (36 to 72 years) and median disease course of 10.5 months.All the patients presented multiple organ involvement simultaneously.Urinary system lesion varied,including renal dysfunction,abdominal pain and edema.Hyperglobulinemia,elevated serum IgG (median 23.3 g/L) and IgG4 (median 4227.0 mg/L),tubular proteinuria were found in all the 6 patients,and elevated Scr (median 237 μmol/L) in 5 cases.Kidney CT image often showed renal swelling,hydronephrosis,multiple low density focus with attenuation and kidney atrophy.Renal pathology revealed interstitial inflammatory cells infiltration comprising predominantly plasma cells and lymphocytes,with a high prevalence of IgG4-positive cells often admixed with fibrosis,which fit the features of tubulointerstitial nephritis.Patients with IgG4-RD nephropathy presented good response to glucocorticoids.After therapy,the symptoms were improved and serum IgG,IgG4 and Scr decreased.Conclusions Renal and urinary lesions of IgG4-RD are heterogeneous in clinical manifestation,and are often complicated with various organ lesions.The feature of renal histopathology is tubulointerstitial nephritis infiltrated by plasma cells and lymphocytes with positive IgG4.Glucocorticoids treatment is effective for this disease.
2.Survey on satisfaction rate of vaccination service and awareness rate of vaccination knowledge among parents in Chengdu

Objective We conduct this research in order to understand the satisfaction rate of vaccination outpatient service and awareness rate of vaccination knowledge among parents ,and to provide scientific basis for strengthening immune programming . Methods We used unified and structured questionnaire suggested by Regulations for Management of Vaccination Units in Sichuan Province .A total number of 116 vaccination outpatient sections were included in our research and in each outpatient section ,30 par-ents were investigated .Results In children′s parents ,the overall satisfaction rate of vaccination outpatient service was 98 .07% , with the highest satisfaction rate(96 .41% ) in technical skills of vaccination staffs ,and lowest satisfaction rate(94 .34% ) in the en-vironment of vaccination outpatient section .The overall awareness rate of vaccination knowledge was 88 .02% ,with the highest awareness rate(100 .00% ) in the question “If it is necessary to take vaccines after child′s birth” ,and lowest awareness rate (67 .93% ) in the question“Which are the three kinds of national immunization vaccines” .Conclusion In Chengdu ,the satisfaction rate of vaccination service is quite high among parents and most of the parents understand the necessity of taking vaccines ,but the awareness rates of questions related to vaccines are low .Therefore ,it is necessary to continue pushing standardized construction of vaccination outpatient service and strengthen health education about immune programming .
3.Effect of Endogeneous Gangliosides on pp125 Focal Adhesion Kinase Expression during Adhesion of SK-N-SH Neuroblastoma Cells to Collagen

Objective To explore effect of endogeneous gangliosides(Gls) and integrin ?2?1 on protein phosphotyrosine expression of pp125 focal adhesion kinase (pp125FAK) after adhesion of SK-N-SH neuroblastoma cells to collagen(Col).Methods SK-N-SH cell line with high expression of integrin ?2?1 was cultured in presence of D-threo-1-phenyl-2-decanolamino-3-morphinoline-1-propanol(D-PDMP).Effect of endogeneous Gls,anti-?2 and anti-?1 monoclonal antibody on protein phosphotyrosine expression of pp125FAK during adhesion of SK-N-SH cells to Col were determined by immunoprecipitate and Western blotting.Results After 6 days,endogenous Gls in cells were almost depleted.Gls-depletion,anti?2 and anti-?1 monoclonal antibody were able to decrease pp125FAK expression of SK-N-SH cells adherent to Col respectively.GD2,the major component of neuroblastoma cell Gls could reco-ver pp125FAK expression to a certain degree.Conclusions Endogenous tumor Gls regulate protein phosphotyrosine expression of pp125FAK during adhesion of neuroblastoma cells to Col.It is suggested that tumor Gls may increase signal transduction of tumor cell integrin ?2?1 by increasing tyrosine phosphorylation of pp125FAK.
4. Preoperative serum CA19-9 level is an independent prognostic factor of patients receiving radical surgery for hilar cholangiocarcinoma

Objective To investigate the appropriate cut-off point of CA19-9 level for predicting the prognosis of patients with hilar cholangiocarcinoma (HC) after radical surgery, and to discuss other potential prognostic factors that may affect their prognosis. Methods Totally 168 patients who had undergone radical surgery for HC in Eastern Hepatobiliary Surgery Hospital were selected for this study. The relationship of preoperation serum CA19-9 level with the clinicopathological characteristics of HC patientswas analyzed, and ROC curvewas used to determine the appropriate cut-off point of CA19-9 level. The potential factors influencing the prognosis were analyzed by univariate model and the independent factors were analyzed by multivariate methods. Results Preoperative serum CA19-9 levil was related to pathologic stage, lymph node metastasis, and tumor size in HC patients. The strongest univariate predictor among the categorized preoperative CA19-9 measures was CA19-9 levil less than 150 U/mL (P = 0. 000). Inunivariate analysis, the age, tumor size, differentiation degree, Bismuth-Corlette classification, portal vein invasion, hepatic artery invasion, liver invasion, preoperative biliary drainage, lymph node metastasis, and resection margin were identified as significant prognostic factorsof HC. Multivariable analysis showed that the differentiation degree, lymph node metastasis, hepatic artery invasion, resection margin and preoperative serum CA19-9 levil were the independent prognostic factors of HC, with the OR values and 95 %confidence intervil (Cl) being 3. 359 (1. 440-7. 837), 2. 973 (1. 927-4. 587), 2. 096 (1. 271-3. 455), 2. 238 (1. 356-3. 694), and 2. 954 (1. 890-4. 618), respectively. Conclusion Our results demonstrate that preoperative serum CA19-9 level is an independent prognostic factor for HC patients, and the most discriminative cut-off point of CA19-9 level for prognosis is 150 U/mL. The differentiation degree, lymph node metastasis, hepatic artery invasion, and resection marginare also the independent prognostic factors of HC.
5.Experimental study of astrocyte reaction to injury in vitro

Objective To study the occurence, development and regulation of reactive gliosis with astrocyte (Ast) in vitro. Methods Ast was isolated and cultured in vitro and its model of reactive gliosis was established by scratching the cultured astrocytes. The reactivity and rules of Ast to injury was studied by morphological changes, RT-PCR, immunocytochemistry, in situ hybridization and imaging analysis. Results After scratching, the astrocytes showed typical features of reactive gliosis, with the hypertrophic cell body, thickened and lengtheded processes, and enhanced glial fibrillary acidic protein (GFAP) staining. In situ hybridization and RT-PCR analysis confirmed that the expression of GFAP mRNA was markedly increased. These changes occurred 1 d after scratching and reached the peak 5 to 7 d after injuring. Conclusion A model of reactive astrogliosis was successfully established in vitro which showed an active reaction to injury. The characteristics of reactive gliosis parallel that seen in vivo.
6.Experimental study of intracerebral transplantation of TH gene modified astrocytes for treating Parkinson's disease

Objective To explore the role of tyrosine hydroxy lase (TH) gene in the treatment of Parkinson's disease (PD) by using ex vivo gene transfer. Methods After the construction of TH gene in a retroviral vector, the astrocytes were cultured with the supernatant containing the recombinant DNA and then grafted into the cerebrum of PD rats. The reduction of the rat rotation beharior was evaluated. Results The rotati on of PD rats was markedly improved in the rats with ex vivo gene transfer. Conclusion TH has an obvious efficiency on the treatment of PD and the astrocytes can be used as effective gene transfer cells.

8.Apoptosis and oxidative injury of donor islets during isolation and purification

Objective To observe the changes of islet cell apoptosis and oxidation-antioxidation before the transplantation, and to explore the pathways of islet protection. Methods Fifteen human pancreases were perfused with the Hanks solution containing collagenase, then digested and isolated. During the procedure, islet cell apoptosis was detected by TUNEL, SOD and MDA in the pancreas were measured by colorimetric method, and the morphologic changes were observed by H-E staining and dithizone staining. Results In the procedure of human islet isolation, especially in the stage of digestion, the apoptosis of human islet cells occurred. In the stages of perfusion and digestion, the MDA contents reached the high levels (6. 18 ± 2. 38 and 9. 21 ± 2. 75 umol/mg protein respectively),and the structures of the islets and tissues around the islets were damaged. Conclusion In the stages of perfusion and digestion, apoptosis of islet cells can be caused by oxidation. It suggests that antioxidation is a pathway for protection of islets before transplantation.

The lack of effective in vitro infection model for hepatitis E virus (HEV) has greatly hindered the quantitative analysis of neutralizing titers of anti-HEV antibodies and human sera, thus impeding further studies of HEV-stimulated antibody responses and the immunological mechanisms. In order to improve this situation, the infection of HepG2 cells that are inefficient for HEV replication was continuously monitored until the viral load reached the limit of detection on day 13, the results of which confirmed the feasibility of using this cell line to establish the infection model. Then, neutralization assays of five anti-HEV murine monoclonal antibodies and serum samples collected from four HEV vaccine recipients (collected before and after vaccination) were performed by 96 multi-channel parallel infections, nucleic acid extraction, and qPCR. The results showed that the cell model can be applied for quantitative evaluation of the neutralizing capacity of different antibodies and antiserum samples from HEV vaccine recipients. In this study, we have successfully established a high-throughput in vitro HEV replication model, which will prove to be useful for the evaluation of HEV vaccines and studies of HEV epitopes.

10.Possibility and predictors of successful cigarette purchase attempts by 201 primary school students in Guangzhou, China.

OBJECTIVETo explore the access to tobacco and exam the predictors of successful tobacco purchase attempts among Chinese minors.
METHODSA simulative trial of purchasing cigarettes was participated by 201 sixth grade students to assess the prevalence of illegal cigarette sales to minors in Guangzhou. Methods of Chi-square and unconditional logistic regression were used to identify the significant predictors,with the result of tobacco purchase as the dependent variable and the characteristics of stores, retailers and minors as the independent variables.
RESULTSA total of 165 students succeeded in purchasing cigarettes but 36 failed, and the percentage of successful purchase attempts was 82. 1% . Data from univariate analysis indicated that 9 factors were significantly associated with students' success in purchasing cigarettes. They were age and height of the purchasers, types of stores, seller's gender and age, posting cigarette advertisements,showing warning signs of 'no cigarette selling to minors' ,asking buyer's age,and asking whom you buy the cigarettes for. The results of multivariable analysis showed that only three variables entering the final logistic regression: the age of students, the type of stores, and showing warning signs of 'no cigarette selling to minors'.
CONCLUSIONChinese minors have easy access to purchasing cigarettes, especially in groceries and small markets. Selling cigarettes by sellers to minors should be monitored and managed in the future.
