Objective To evaluate the effects of rabeprazole-based triple therapy for associated gastric ulcer caused by the helicobacter pylori(Hp) infection. Methods 100 patients with associated gastric ulcer caused by the Hp infection were randomly divided into treatment group and control group. The treatment group was given rabeprazole-based triple therapy( rabeprazole 10mg bid, amoxicillin 1.0g bid and clarithromycin 500mg bid), and the control group was given omeprazole-based triple therapy( omeprazole 20mg bid, amoxicillin 1.0g bid and clarithromycin 500mg bid). The two groups were given 7 days treatment. Results The total effective rate of clinical symptoms of treatment group was 96. 0% ,the healing rate of the treatment group was 88.0%, the total effective rate of gastroscopy effect of treatment group was 94.0% ,the Hp clearance rate of treatment group was 92.0% and the Hp eradication rate of treatment group was 90. 0% ,which were higher than the control group,but the difference was not significant( P ＞ 0. 05 ). The two groups had no significant adverse reactions. Conclusion Rabeprazole-based triple therapy for associated gastric ulcer caused by the Hp infection was effective, safe and worth popularization and application.

Objective To explore the interventional effect of two different methods in the prevention of chemotherapy phlebitis caused by 5-fluorouracil.Methods Eighty cases of patients treated with cisplatin+5-fluorouracil (PF) chemotherapy in the First Affiliated Hospital of Guangxi Medical University from May 2015 to July 2016 were selected and divided into the observation group and control group according to admission time,40 cases in each group.The observation group was smeared with 2% anisodamine along the direction of vein puncture after peripheral venous puncture success,while the control group was smeared with mucopolysaccharide polysulphate emulsion paste.The occurrence of phlebitis and degree of local pain after intervention were compared between the two groups.The blood biochemical indexes and inflammatory factors levels before and after intervention were compared too.Results After the intervention,the distribution of phlebitis level and local pain degree had statistical difference between the two groups (Z=2.57,2.53;P=0.01);the observation group was superior to the control group in the aspects of phlebitis lesion degree and local pain alleviation;WBC,MPV and PDW in the observation group had statistical difference between before and after intervention;WBC,PLT and PCT in the control group had statistical difference between before and after intervention (P<0.05);PLT count was positively correlated with PCT in the two groups (r=0.952,P<0.05);the CRP level and WBC count after intervention had no statistical difference between the two groups(P>0.05).Conclusion 2% anisodamine can effectively prevent and alleviate the occurrence of 5-FU caused chemotherapy phlebitis and local pain.At the same time,PLT can provide a basis for preventing chemotherapy phlebitis.

Varicella zoster virus (VZV) is a widespread human herpesvirus which causes chickenpox (varicella). VZV infection can produce a broad spectrum of neurologic disorders including multiple cranial nerve palsies. Among the cranial nerves, trigeminal and facial nerves are the most commonly involved. However, multiple lower cranial nerve palsies caused by VZV infection is very rare. It is a diagnostic dilemma for VZV infection if there are no skin or mucosa lesions, which are characteristic signs of VZV infection. We report the case of a 54-year-old man with sudden right hearing loss, sore throat, odynophagia, hoarseness, dysphagia and vertigo. Pure tone audiometry revealed right sensorineural hearing loss and laryngoscopy proved that there were paralyses of the right upper pharyngeal constrictor muscle and vocal cord. These findings were consistent with acute unilateral vestibulocochlear, glossopharyngeal and vagus nerve palsies. There were no skin or mucosa lesions noted in our patient. The diagnosis was assisted by the existence of anti-VZV serum IgM even if the polymerase chain reaction result was negative. The symptoms of the patient improved after receiving anti-viral therapy. We emphasize the role of VZV infection in multiple cranial nerve palsies and the importance of serologic test.

Objective To investigate the methodology and feasibility of an anatomic survey of fetal limbs in the first trimester. Methods In this study, 533 normal fetus and 6 fetal corpse without limb abnormalities form June 2011 to August 2012 underwent an anatomic survey on the limbs in our consultation center. The limbs were scanned using a systematic continuous sequence approach:(1) Displaying upper arm and humerus, forearms and radius/ulna, hands in proximal to distal direction. (2) The thigh and femur, crus and tibia/fibula, feet was attempted from the sagittal section of each structure. (3) If the whole limb couldn′t be visualized in a section then the three segments will be demonstrated respectively. The position and move-ment also should be observed during the scan. Data were collected regarding gestation age, examining time and the structures. Results The limbs except toes of 533 normal fetus were clearly observed, which was completely unaffected by gestational age, with the mean time of scanning is 98.6±37.4 s (rang:36-189 s). The structures (including upper arms and humerus, forearms ,radius and ulna, hands, thigh and femur, crus and tibia, fibula, feet) of fetal limbs were clearly observed on the 6 fetal corpse, but the ossification center of fingers were steadily visualized until 13 weeks and toes after 13 weeks. Conclusions Scanning fetal limbs in the first trimester were feasible by a systematic continuous sequence approach in a short period of time.

The evaluation of visual quality about human eyes has always been an important issue in the field of eye science and visual optics,and it is mainly divided into subjective evaluation method and objective evaluation method,evaluation indicators including visual acuity,contrast sensitivity,wavefront aberration and retinal straylight,and so on.In addition,according to different principles and standards,visual system imaging quality evaluation methods can also be divided into visual refractive system and retinal nerve system evaluation,geometrical optics and physical optics evaluation,static and dynamic visual function evaluation.The selection of visual quality evaluation method should be considered from multiple dimensions on comprehensive consideration.in recent years,the improvement of quality of life promotes the development of visual quality assessment methods,and more attention is paid to the evaluation of continuous functional visual acuity.This article reviews the research progress of visual quality assessment methods.

Objective To explore the correlation between maternal serum total bile acid (TBA) of pregnant women with intrahepatic cholestasis of pregnancy (ICP) and varying degrees of neonatal lung injury.Methods A total of 52 cases of pregnant women with ICP and their corresponding newborns were enrolled into the ICP group from March 2014 to December 2015 in the People's Hospital of Shapingba District.Other 52 cases of pregnant women received cesarean delivery whose gestational age and birth weight of newborns were close to the ICP group and their corresponding newborns were selected as the control group.The conditions of neonatal lung injury were recorded,and the correlations of maternal serum level of TBA and exposure time of high serum level of TBA to degrees of lung injury were analyzed as well.Results The incidence rate of neonatal lung injury in the ICP group (67.3 ％) was higher than that in the control group (17.3 ％),there was statistically significant difference (P＜0.05).The degree of lung injury was positively correlated with maternal serum level of TBA and exposure time of high serum level of TBA (r=0.687,P=0.000;r=0.523,P=0.001).Conclusion The probability of neonatal lung injury of corresponding pregnant women with ICP is significantly increased,and the extent of lung injury is positively correlated with concentration of maternal serum TBA and exposure time of high serum level of TBA.

This paper reports 20 patients with cryptococcal meningitis,which were misdiagnosed before anti-fungal therapy,and the majority of them was diagnosed as tuberculous meningitis or viral meningitis.These patients were treated either with amphotericin B or fluconazale alone,or in combina- tion with flucytosine respectively,of the 15 patients could be evaluated,10 were cured(66.7%),2 im- proved(13.3%),3 died(20%),1 relapsed(10%).The mortality rate among the under age group and the aged group was significantly higher than that of the adult group(71.4% vs 23%).It is the authors' experience that in the early stage intrathecal amphotericin B combines with intravenous fluoconazole and changes to oral fluconazole or itraconazole in the later stage may be a valuable approach for cryptococcal meningitis.

Objective To study the expression and clinical significance of Notch3 and Notch intracellular domain (NICD) in ovarian carcinoma and the effects of N-[N-(3 ,5-difluorophenyl) acetyl-L-alanyl]-S-phenyl glycine t-butyl ester (DAPT), a γ-secretase inhibitor on the proliferation and apoptosis in OVCAR3, A2780 ovarian carcinoma cell lines. Methods Western blot was used to detect the expression of NICD in the tissues from 58 ovarian carcinomas patients and 21 normal ovarie, who were admitted in Peking University First Hospital from July 2006 to June 2009. Immunohistochemistry was also used to detect the expression of Notch3 in these tissues. The relationship with clinical features of ovarian carcinoma was also analyzed. Proliferation of OVCAR3 and A2780 ovarian cancer cells was determined by methyl thiazolyl tetrazolium (MTT) assay, cell cycles and apoptosis and index of proliferation were detected by flow cytometry method. The expression of NICD in OVCAR3 and A2780 cells incubated with DAPT was detected by western blot. Results (1)The expression level of NICD in ovarian carcinomas was significantly higher than that in normal ovarian tissues (1.64 ±0. 19 vs. 0.98 ±0.20;P <0.05). The NICD expression was higher in ovarian cancers with low grade or advanced stage than those in high-middle grade or early stage,respectively (1.90 ± 0. 22 vs. 1.25 ± 0. 21,1.80 ± 0. 21 vs. 1.21 ± 0. 15; all P < 0. 05). The Notch3 protein was stained positively in cytoplasm, nuclear and cell membrane. The expression of Notch3 was higher in ovarian carcinomas than that in normal ovaries [78% (45/58) vs. 24% (5/21); P < 0. 01]. While,there were no stasistical difference in different pathological types, stages, differentiation of ovarian carcinoma. There was no difference between the patients with adjuvant chemotherapy or not. (2)After OVCAR3 and A2780 cells incubated with DAPT 24, 48, 72 hours, NICD expression was significantly lower than that in control group (P < 0. 05). The effects of DAPT inhibited the proliferation and prompted the apoptosis of OVCAR3 and A2780 cells were depended on the concentrations and times. Conclusions Notch3 and NICD may play a key role in the occurrence and progress of ovarian carcinoma. The mechanism of DAPT inhibited the proliferation and prompted the apoptosis of OVCAR3 and A2780 cells may be due to decreased the formation of NICD.

Objective To investigate the DNA typing, observe relationship between DNA fingerprinting patterns and serotypes of Cryptococcus neoformans, and find a suitable genotyping standard for Cryptococcus neoformans. Methods Three primers, including CN 1(GTG) 5, CN 2(GACA) 4 and CN 3(GATA) 4, were used to distinguish variations among strains of C.neoformans. Results The distinguishable fingerprinting bands for serotype of C.neoformans were yielded by primer CN 2. Using this primer, of 24 clinical and environmental isolates of serotype A of C.neoformans and 8 standard strains investigated by PCR, 20 strains produced complete identical fingerprinting patterns, the other 4 strains had different fingerprinting patterns. 2 strains of serotype B and C yielded indistinguishable fingerprinting patterns. Conclusions ①The majority of strains of serotype A had similar and stable fingerprinting patterns. ②Some strains of serotype B and C had an indistinguishable fingerprinting patterns. ③The same serotype strains from different sources may produce different DNA fingerprinting patterns. ④The DNA fingerprinting is a rapid, simple and feasible method for identifying Cryptococcus neoformans.
