1.A systemic review of postoperative complications following arthroplasty performed under spinal versus general anesthesia

To compare the effectiveness and safety of spinal versus general anesthesia for arthroplasty with respect to deep vein thrombosis(DVT),pulmonary embolism (PE) and epidural hematoma.Randomized controlled trials involving spinal and general anesthesia for arthroplasty were searched.Quality assessment were analyzed according to the Cochrane Collaboration's rule.A total of 12 trials involving 1023 participants met the inclusion criteria,Meta-analyses showed that spinal anesthesia may reduce the incidence of DVT or PE after arthroplagty without anticoagulant.There is no difference in the incidence of DVT and PE when anticoagulant is used.Whether anticoagulant was used or not,no epidural hematoma was observed after using the two anesthesia methods.

Objective To study and evaluate the feasibility and clinical effects of the treatment of premature ejaculation with modified Masters-Johnson sensate focus exercise.Methods 100 male outpatients with premature ejaculation were divided randomly into two equal groups randomly.The experiment group was treated with the modified Masters-Johnson sensate focus exercise and oral sertraline 50 mg/day for 8 weeks.The control group was given oral sertraline 50 mg/day for 8 weeks.The scores were evaluated with CIPE-5 and compared between the groups before and after the treatment and after the flown-up for 6 moths.Results After treatment,the score of CIPE-5 were better in experiment group than that in control group (P<0.01).The effective rate 92% (46/50) in experiment group was significantly higher than that in control group 68% (34/50) (P<0.01).The relapse cases (6 cases) were significantly lower than that in the control group (22 cases) (P<0.01).Conclusions The method of Masters-Johnson sensate focus exercise for treatment of premature ejaculation has a good curative effect,and low recurrence rate,which is worth the clinical promotion.

Aim To study the effect of Rutoside (Ru) on pancreatic acinar cell apoptosis in rats with acute pancreatitis(AP). Methods The AP model in rats was induced by retrograde injection of 3% sodium taurocholate into biliopancreatic duct. Ru (15, 30 and 60 mg?kg~ -1 ?h~ -1 )was administered by intravenous infusion for 6 hours immediately after the induction of AP.The histopathological changes of pancreas were observed under light microscope and electronic microscope.A terminal deoxynucleotidyl transferase-mediated dUTP-biotion nick end labeling(TUNEL) method was used to detect apotosis of pancreatic acinar cell and apoptosis immunohistochemical method. Results Ru (15, 30 and 60 mg?kg~ -1 ?h~ -1 ) improved the histopathological changes of pancreas significantly.The apotosis index of pancreatic acinar cells was significantly higher than that the AP model group. The gray value of Fas was lower than that of the AP model group. Immuno-staining revealed that moderate to strong FasL immunoreactivity was present in the cytoplasm of the acini of all groups.But in AP model group,the gray value of FasL was lower than that in Ru-treated groups.AI was positively correlated with the gray value of Fas,and was negatively correlated with the gray value of FasL and the severity of pancreas damages. Conclusion The protective effect of Ru on AP may be concerned with the induction of apoptosis in injured pancreatic acinar cells.And the Fas/FasL system may contribute to the process.
6.Dexmedetomidine on cognitive function of elderly patients who underwent hip orthopedic surgery

Objective To observe the effect of a variety dosage of dexmedetomidine on circulatory and cognitive func?tions in elderly patients who underwent hip orthopedic surgery. Methods A total of 120 patients who underwent hip ortho?pedic surgery, with age over 65 years old were recruited from 06/2013 to 09/2014 and were administered intravenously dex?medetomidine after epidural anesthesia. Based on the dosage of dexmedetomidine, patients were randomly divided into 3 groups with 40 cases in each group:Group A were given 0.2μg·h-1·kg-1 dexmedetomidine;Group B were given dexmedeto?midine 0.4μg · h-1 · kg-1, and group C were given 0.6μg · h-1 · kg-1. The heart rate (HR),mean arterial pressure (MAP), and pulse oxygen saturation (SpO2) were recorded at 6 time points, including the time entering operating room (T0), 10 min (T1), 20 min (T2) after drug administration, the time of drug withdrawal (T3), 10 min after drug withdrawal (T4) and the terminal time of surgery (T5) respectively. Cognitive function was evaluated by mini-mental state examination (MMSE) at the time points of 24 hours before surgery (t1), 12 hours (t2), 24 hours (t3), and 72 hours (t4) after surgery. Results All 3 groups of pa?tients show no significant difference of MAP, HR and SpO2 at T0 (P>0. 05).A significant decrease in MAP and HR were ob?served in Group C at T2 and T3 time points compared to those in Group A or Group B (P<0.05). There was no significant dif?ference in MMSE at time point T1 among all three groups (P > 0.05). However, compared to that in group A or group C, MMSE increased significantly at time point T2, T3 in group B (P<0.05). In addition, compared to group A or group C, the pa?tients in group B exhibited lower incidence of postoperative cognitive function disorder (P < 0.05). Conclusion Continu?ous intravenous infusion of Dexmedetomidine can be used in elderly patient who underwent hip orthopedic surgery at the dose of 0.4μg·h-1·kg-1 safely with little interference to circulatory and cognitive function in perioperative period.
7.The effect on the strees response of laryngeal mask airway as a transition method after double endotrache-al anesthesia

Objective This study was to investigate the effect of using laryngeal mask airway (LMA)as a transition method after double lumen endotracheal anesthesia on the stress response dur-ing the extubation period.Methods Ninety patient underwent general anesthesia with double lumen endotracheal intubation were randomly divided into 3 groups:group A,B and C (n=30).Patients in group A were extubated directly after surgery.The double lumen endotracheal tube were changed to single lumen endotracheal tube under deep anesthesia in group B patients,while the double lumen en-dotracheal tube were changed to LMA in group C.MAP,HR and rate pressure product (RPP)at the end of surgery (T0 ),1 minute before extubating the endotracheal tube (laryngeal mask)(T1 ),1 mi-nutes (T2 ),5 minutes after extubating (T3 )were recorded.The reaction of extubation,such as buc-king or restlessness,was also observed.Results Compared with T0 ,the MAP,HR,and RPP were significantly increased in groups A and B at T1-T3 (P <0.05 ).Compared with group A,the MAP, HR,RPP were significantly decreased in groups B and C at T1-T3 (P <0.05 ).The number of buc-king and restlessness in group C were lower than that in group A and group B (P < 0.05 ). Conclusion The use of laryngeal mask during the transition period in double lumen endotracheal anes-thesia to can reduce hemodynamic fluctuations and stress reaction during the extubation.
8.Epidemiological characteristics and clinical outcome of 62 cases diagnosed with novel coronavirus pneumonia in Qingdao area

Objective:To explore the epidemiological characteristics and clinical outcomes of patients diagnosed with novel coronavirus (COVID-19) pneumonia (NCP), and to provide clinical reference for subsequent epidemic prevention and control.Methods:The pathogenesis, clinical characteristics and clinical outcomes of 62 patients with imported novel coronavirus COVID-19 pneumonia in Qingdao area from January 21, 2020 to April 6, 2020 were analyzed.Results:There were 24 males and 38 females, aged from 5 to 91 (52.5±15.7)years. Among them, 31 cases (31/62, 50%) were family cluster disease, 11 cases (17.74%) were nosocomial cross infection, 14 cases (22.58%) came back from Wuhan to Qingdao and community contact infection, and 6 cases (9.68%) were imported from abroad. The longest time from fever to diagnosis was 26 days. Clinical classification: 40 cases were mild, 13 were general, 7 were severe, 2 were critically ill. 61 cases were discharged from hospital, and 1 was dead. The clinical cure rate was 98.39%. The average stay was (18.5±4.8)days. All patients were treated combined with Chinese medicine on the basis of symptomatic treatment.Conclusions:The clinical symptoms of imported NCP are relatively mild, and mild cases were more, the recovery rate was higher. The intervention strategy of traditional Chinese medicine is conducive to rehabilitation. The external input cases and symptomless infected patients are important for epidemic situation in the future.
9.User Security Authentication System for Campus Network

Objective To introduce and analyze the current user security authentication system in the campus network and point out the secure problems and hidden dangers of the correspondence entities in the campus network. Methods The authentication technologies and protocols in the TCP/IP network model were compared. It was proposed that the campus network security authentication solution should include access control in data link layer, connection control in network layer and unified authentication in application layer. Results The solution could improve the campus network authentication security rank. Conclusion The security and reliability of the network system are enhanced.
10.A study on the predictive factors of intestinal necrosis in acute mesenteric ischemia

Objective:To explore the predictive factors of intestinal necrosis in acute mesenteric ischemia.Methods:This retrospective study enrolled 81 patients diagnosed as acute mesenteric vascular occlusive diseases in Zhongshan Hospital, Fudan University between Nov 2012 to May 2017. Univariate analysis and multivariate logistic regression analysis were used to identify predictive factors for intestinal necrosis.Results:In univariate analysis, the predictive factors of intestinal necrosis were peritoneal irritation sign ( P<0.001), white blood cell count ( P<0.001), serum albumin ( P=0.028), blood creatinine ( P=0.025), serum lactic acid ( P=0.008), D-dimer ( P=0.037), intestinal pneumatosis ( P=0.017), decreased or disappeared enhanced bowel wall ( P<0.001) and bowel loop dilation>2.5 cm ( P=0.01) on CT scan. According to multivariate logistic regression analysis, white blood cells ( OR=3.60, 95% CI: 1.51-5.47, P=0.007), lactic acid ( OR=4.80, 95% CI: 1.36-9.89, P=0.032), reduced or disappeared enhanced bowel wall ( OR=10.57, 95% CI: 1.82-61.10, P=0.008) were independent predictive factors of intestinal necrosis in patients with acute mesenteric ischemia. Conclusions:The predicted risk factors for intestinal necrosis in mesenteric ischemic diseases are increased white blood cells, elevated serum lactate levels, and reduced or disappeared enhanced bowel wall on CT scan.
