1.Effect of air pressure wave on the prevention of lower limb deep venous thrombosis in patients with cerebral stroke and the application value of color Doppler ultrasound

Objective To explore the effect of air pressure wave on the prevention of lower limb deep venous thrombosis in patients with cerebral stroke and the application value of color Doppler ultrasound (CDUS).Methods 72 stroke patients were randomly divided into experimental group (n =37) and control group (n =35) by random digits table.The control group received routine nursing,conventional and rehabilitation therapy.The experimental group was treated by the air pressure wave for 15 days on the basis of the control group.DVT formation of lower extremities and hemodynamic changes before and after treatment was observed by CDUS,and the results were compared with the X-ray phlebography examination results.Results CDUS displayed the incidence rate of DVT in the experimental group was significantly lower than that in the control group (2.7% vs 20.0%,x2 =5.449,P <0.05).The changes of blood flow velocity and the average reflux time of femoral,popliteal vein in the experimental group had significant differences between before and after treatment (t =8.503,4.254,8.587,2.062,all P < 0.05) ;and in the control group,only popliteal vein blood flow velocity had statistically significant difference (t =6.895,2.116,4.169,all P < 0.05).Compared with the X-ray angiography,8 cases of DVT diagnosed by CDUS were consistent,but 1 case of DVT in the control group was missed diagnosis.Conclusion Air pressure wave can effectively reduce the occurrence of DVT in patients with cerebral stroke.CDUS can accurately diagnose the DVT formation,and can help to understand the hemodynamic change,it has important clinical value for monitoring the preventive effect of DVT.
2.ADR Reports:Analysis of 1451 Cases in Our City in 2007

OBJECTIVE:To discuss the characteristics and regularities of adverse drug reactions(ADR)occurring in our hospital.METHODS:A total of 1 451 ADR cases collected by ADR monitoring center in 2007 were analyzed statistically in respect of patients' age,routes of administration,drug category,ADR-involved organs or systems and clinical manifestations etc.RESULTS:Of the 1 451 ADR cases,72.16% of the ADR-inducing drugs were administered by intravenous drip,and most were associated with anti-infective drugs,predominantly penicillins,cephalosporins and quinolones.The ADR manifested predominantly as lesions of skin and its appendants.CONCLUSION:Rational drug use and ADR monitoring should be strengthened to reduce or avoid the occurrence of ADR.
3.Effect of resveratrol on coxsackie virus B3m-induced mycocarditis in mice

Objective: To observe the intervening effect of resveratrol on coxsackie virus B3m-induced mycocarditis in Balb/c mice and explore its mechanism. Methods: An animal model of viral mycocarditis induced by coxsackie virus B3m (CVB3m) was used, taking ribavirin as control drug, to examine the changes of general condition, mortality, the weights of heart, liver and spleen, serum MDA and NO levels, and cardiac histology in Balb/c mice. Results: By comparison with ribavirin, it was found that in the mice model of viral mycocarditis induced by coxsackie virus B3m resveratrol significantly improved the changes of general condition, mortality, the weights of heart, liver and spleen, serum MDA and NO levels, and cardiac histology. Conclusion: It suggested that resveratrol might have some chemopreventive and chemotherapeutic effects in the treatment of viral mycocarditis.
4.Changes of B lymphocyte subsets in peripheral blood of patients with rheumatoid arthritis and their ;significance

Objective To investigate the changes of B lymphocyte subsets ( naive B cells, memory B cells and plasmablasts) in peripheral blood of patients with rheumatoid arthritis ( RA) and their correla-tions with the clinical manifestation and laboratory indexes.Methods Sixty-six patients with RA were di-vided into two groups including the group with active RA and the group with inactive RA according to the dis-ease activity score in 28 Joints (DAS28).A control group with healthy subjects was set up accordingly.The distributions of B lymphocyte subsets in peripheral blood samples were detected with flow cytometry analysis and their correlations with clinical manifestations and laboratory indicators were analyzed.Results ( 1 ) Compared with healthy subjevts, the mean fluorescence intensities ( MFIs) of CD19 and the percentages of memory B cells in peripheral blood of the patients with RA were significantly decreased, while the percenta-ges of naive B cells were increased (P<0.05).The percentages of plasmablasts in the patients with active RA were significantly increased as compared with those of healthy subjects and the patients with inactive RA (P<0.05).(2) The percentages of plasmablasts in peripheral blood of the patients with RA were positively correlated with the joint tenderness count, joint swelling count and joint swelling index (P<0.05).(3) A positive correlation was found between the MFIs of CD19 and the erythrocyte sedimentation rates ( ESRs ) among the patients with RA.The percentages of plasmablasts were positively correlated with C reaction pro-tein (CRP) and anti-cyclic citrullinated peptide (anti-CCP) antibody (P<0.05).(4) The percentages of plasmablasts were also positively correlated with the DAS28 among the patients with RA ( R2=0.343, P<0.01).Conclusion The distributions of B lymphocyte subsets varied among the patients in different stages of RA, which were related to the patient′s clinical symptoms and laboratory indexes.The study suggested that different subsets of the B lymphocytes might play an important role in the pathological process of RA.
5.Analysis on501Cases of Adverse Drug Reaction Reports in Lishui City of Zhejiang Province

OBJECTIVES:To fathom situation and the cause about adverse drug reactions(ADR)from each drug manu?facturing enterprise and drug handling enterprise in our city.METHODS:501cases of ADR reports in our city collected by ADR monitoring center in2004were classified and analyzed statistically.RESULTS:Of the501ADR cases,226cases were resulted from antibiotic drugs which accounted for45.1%,80cases were resulted from by traditional Chinese drugs prepara?tions which accounted for16.0%;ADR most commonly manifested itself in the injury of skin and appendants,which accounted for48.9%(245cases),then the damage of digestive system and circulation system,there were21(4.2%)comparative serious ADR cases among which;most cases were resulted from iv which accounted for65.9%(330cases)of the total.CONCLU?SION:Cultivation of professional quality and service quality of the professional staff should be strengthened and the om?nibearing ADR monitoring work should be deployed so as to lessen and avoid the occurrence of ADR.
6.Retrospective Study of Rivaroxaban in the Prevention of Deep Vein Thrombosis after Bones of Lower Ex-tremity Surgery

Objective: To retrospectively analyze the clinical efficacy and safety of rivaroxaban in the prevention of deep vein thrombosis ( DVT) after bones of lower extremity surgery. Methods:Totally 94 patients with bones of lower extremity surgery were col-lected in our hospital during June 2010 to June 2013. According to the taken medicine, the patients were divided into groupⅠ(rivarox-aban 10 mg,po,qd) and groupⅡ( low molecular weight heparin injection 4 000 IU, ih,qd) . The bleeding after the surgery, the data of blood routine and coagulation, the occurrence of DVT and the adverse reactions in the two groups were compared. Results:After the medicine treatment, the number of hemoglobin and platelet in the two groups was declined significantly (P<0. 05), while reached the normal value after the medicine withdrawal. No significant difference was found in the blood routine and coagulation routine in the two groups. One case of hemorrhea showed in group I, while three cases showed in groupⅡ. GroupⅠhad no DVT, while groupⅡ had 3 cases of DVT, and the difference was significant (P<0. 05). No severe adverse reactions appeared in the two groups. Conclusion:Rivaroxaban has good effect in the prevention of DVT with better efficacy and safety when compared with low molecular weight heparin.
7.Clinical implication of complement split product C4d in patients with antibody-mediated chronic rejection

Objective To explore the significance of peritubular capillary C4d deposition in histopathological changes, renal function and prognosis of the patients with antibody-mediated chronic rejection (AMCR). Methods Deposition of C4d in the kidney was examined by irnmunohistochemistry on routine paraffin-embedded sections using anti-C4d polyclonal antibody. Seventy-seven patients were divided into C4d+ group (n = 35) and C4d- group (n = 42). The relationship of C4d and renal function,histopathological changes and prognoses of allografts were analyzed. Results The number of patients with tubular atrophy and glomerular basement membrane proliferation in C4d+ group was significantly more than that in C4d group (P<0.05). Mean serum creatinine level was significantly higher in C4d+ group than in C4d- group 12 months after renal transplantation [(379.1 + 260.2)μmol/L vs (260.5 + 175.3) μmol/L, P<0.05]. According to Kaplan-Meier analysis, the one-year graft survival rate was lower in the C4d+ group (62.9% ) than in the C4d- group (83.3% ) (logrank P<0.05). Conclusion Patients with C4d deposition are associated with tubular atrophy and glomerular basement membrane proliferation. The serum creatinine level in C4d+ patients was significantly higher than in C4d- group at the 12th month after transplantation. More patients with C4d deposition lost their grafts during the study period.
8.Effects of grasp seed procyanidins on homocysteine-induced proliferation and migration in vascular smooth muscle cells

AIM:To investigate the effects of grasp seed procyanidins(GSP) on homocysteine-induced proliferation and migration in vascular smooth muscle cells(VSMC) and related molecular mechanisms.METHODS: Cell count and -TdR assay were used for detecting cell proliferation and DNA synthesis,ELISA assay was used for detecting inflammatory response,DCFH-DA assay for examining the levels of reactive oxygen species(ROS),Western blotting for detecting protein expression.RESULTS: Homocysteine(0.1-1 mmol/L) increased VSMC proliferation and migration,and the levels of ROS were in a dose-dependent manner.The results of Western blotting showed that homocysteine significantly increased the expression of MCP-1,IL-6 and TNF-?.However,Compared with control group,in GSP(5-20 g/L) group,the increased VSMC proliferation,migration and the production of ROS and the expression of MCP-1,IL-6 and TNF-? mediated by homocysteine were markedly suppressed.EMSA showed that in GSP treatment group,the NF-?B activation was also almost completely inhibited.CONCLUSION: GSP inhibits homocysteine-induced VSMC proliferation,migration and inflammatory response through interfering with ROS dependent on NF-?B signal pathway.
9.Efficacy of the program of rapamycin combined with CNI in chronic allograft nephropathy

ObjectiveTo investigate the efficacy of rapamycin combined with CsA/Tacrolimus (Tac) in chronic allograft nephropathy (CAN).MethodsFifty-three cases of CAN accepted the quadruple immunosuppressive drug program,which contained rapamycin combined with CsA/Tac and MMF and prednisone,and CsA/Tac and MMF were reduced to the original amount of 25% to 50%.After treatment for 12 months,more relevant indicators,including serum creatinine,glomerular filtration rate,serum cholesterol,triglycerides,urinary protein,GPT and bilirubin and other changes were observed.ResultsIn the patients receiving quadruple regimen of rapamycin during 12 months,the blood Ccr was decreased from (161.51 ± 106.48)μmol/L before treatment to (126.51 ± 56.2)μmol/L after treatment for 6 months (P<0.05) and to (123.43 ± 54.18)μmol/L after for 12 months (P<0.01).The GFR was increased from (0.754 ± 0.302) ml/s before treatment to (0.952 ± 0.347)ml/s after treatment for 6 months (P<0.05) and to (1.007 ± 0.394) ml/s after treatment for 12 months (P<0.01).Cholesterol and triglycerides in patients had no significant change before and after treatment.The positive rate of proteinuria after treatment showed an increasing trend from 9.4% before treatment to 26.4% after treatment for 12 months.ConclusionThe quadruple program of rapamycin combined with CsA/FK506 and MMF can significantly improve Ccr and GFR in patients with CAN,but it can increase the incidence of proteinuria in patients:
10.Analysis of Clinical,Endoscopic and Pathological Characteristics of Inflammatory Bowel Disease

Background:Inflammatory bowel disease (IBD)is a group of chronic gastrointestinal inflammatory disease and its diagnosis depends on analysis of clinical,endoscopic and pathological characteristics. Aims:To analyze the clinical, endoscopic and pathological characteristics of IBD. Methods:Clinical,endoscopic and pathological characteristics of 99 patients with IBD [including 61 ulcerative colitis (UC)and 38 Crohn's disease (CD)]from January 2010 to April 2017 at Yijishan Hospital were retrospectively analyzed. Results:Compared with CD patients,age in UC patients was significantly increased,incidences of diarrhea,bloody stool were significantly increased while incidence of abdominal mass was significantly decreased (P < 0. 05). Extensive type UC and ileocolonic type CD were common. Proportion of patients received 5-aminosalicylic acid (5-ASA)was significantly higher in UC than in CD (90. 2% vs. 71. 1%,P = 0. 014). Two UC patients achieved remission shown by endoscopy,and endoscopic remission achieved in 3 CD patients. The main pathological features of UC were cryptic branching (57. 1%),cryptic twisting (76. 2%),cryptic atrophy (54. 8%)and cryptic irregularity (28. 6%). Ulcer was seen in 78. 9% of CD patients with active chronic inflammation (mainly lymphocyte and plasmocyte infiltration),and the detection rate of non-caseating granuloma was 15. 8% . Conclusions:UC patients are characterized with older age,diarrhea,bloody stool and higher administration rate of 5-ASA,and the endoscopic features were mucosal bleeding and ulcers. Pathological characteristics of UC were mainly cryptic architectural change and inflammatory infiltration,while those of CD were mainly segmental lesions and chronic inflammation with lymphocyte and plasmocyte infiltration and some with non-caseating granuloma.
