1.Application of Oral Hypoglycemic Agents in Our Hospital During 2004~2006

OBJECTIVE:To probe into the status quo and the developmental trend of oral hypoglycemic agents used in our hospital.METHODS:Using the defined daily dose as the study unit to make statistic analysis of oral hypoglycemic agents used in our hospital during 2004~2006 in respect of DDDs,consumption sum and average daily cost etc.RESULTS:Both the categories and consumption quantity of oral hypoglycemic agents increased year by year while the proportion of consumption sum was descending.Over the 3 years,dominating the first 5 places on the list of DDDs were glipizide,metformin,gliquidone,gliclazide and glibenclamide,respectively.CONCLUSION:The clinical use of oral hypoglycemic agents was basically reasonable in our hospital.
2.Isolation and Identification of a Quail-origin H9N2 Subtype of The Influenza Virus and Its Biologic Characterization.

A quail-origin subtype of the influenza virus was isolated from a human-infecting H7N9 subtype of the avian influenza virus found in a live poultry market and was given the name A/Quail/Hangzhou/1/ 2013 (H9N2). We analyzed the whole genome of this virus and its biologic characteristics. Sequence analyses suggested that the: HA and NS genes belonged to a CK/BJ/1/94-like lineage; NA, NP, PA and PB1 genes belonged to a SH/F/98-like lineage; M and PB2 genes belonged to a G1-like lineage. Analyses of key amino acids showed that the cleavage site in HA protein was PSRSSR ↓ GL, and that the HA protein had a human receptor-binding site with Leu226. Deletion of amino acids 69 - 73 was detected in the stalk of NA protein, the M2 protein had an Asn31 mutation, and the NS1 protein had two mutations at Ser42, Ala149. The intravenous pathogenicity of this virus was 0.36. A study in chickens suggested that all inoculated birds shed the virus from the trachea and cloaca on the third day post-infection (p. i. ) until 11 days. All chickens that had direct contact shed the virus on the second day p. i. until 8 days. Results of virus reisolation suggested that lung and tracheal tissues could shed the virus in 5 days, whereas the other organs could shed the virus in 3 days. These results suggest that this virus strain is H9N2 subtype LPAIV, whose lineage is prevalent in mainland China. This research provides evidence on how to monitor and prevent the H9N2 subtype of the avian influenza virus.

3.Study on the Preparation Process of Xiangsha Liu jun Granule

OBJECTIVE: To optimize the preparation process of Xiangsha Liujun Granule. METHODS: By the orthogonal design. taking the rates of recovery of volatile oil, Ginsenoside Re and qualified granules as the indexes, we optimized the oil extracting conditions, the decocting conditions and the conditions of granulation by spayer. RESULTS: The optimum oil ex- tracting conditions for four kinds of herbs(Pericarpium Citri Reticulatae etc.) were that water was 30 times of herbs in weight, soaking time was 3 hours and extracting time 15 hours. The optimum decocting conditions for four kinds of herbs(Fuling etc.) were that water was 15 times of herbs in weight, soaking time was 3 hours and decocting time 4 hours, herbs were decocted twice. The optimum conditions of granulation by sprayer were that the fliud extract was 2 times of Dextrin in weight, the rel- ative density of the fliud extract was 1. 30(measured at 25℃), the voltage of sprayer was 120 volts, the period of shaking bag was 65 times(shaking bag back and forth twice in each period). CONCLUSION: The results of the experiment make it clear that the preparation process of Xiangsha Liu jun Granule is stable and feasible.
4.Influence of Acupuncture on Power of Brain Electrical Activity Mapping and Hippocampal Amino Acid Transmitters in Acute Epilepsy Rats

【Objective】To investigate the influence of acupuncture on the power of brain electrical activity mapping(BEAM) and hippocampal amino acid neurotransmitters in acute epilepsy rats.【Methods】Forty SD rats were randomized into groups A(normal),B(model),C(treated with acupuncture) and D(treated with sodium valproate).Group C was pretreated with acupuncture on acupoints of Dazhui and Fenglong and with needling through Ganshu toward Danshu.Group D was pretreated with sodium valproate by gastric gavage.The treatment in groups C and D lasted 3 days.Except group A,the mice in other 3 groups received intraperitoneal injection of sodium penicillin to induce acute epilepsy.Within seventy to eighty-five minutes after modeling,different frequency-band power of BEAM in rat brain middle Pz region was detected,and rats hippocampal contents of excitatory amino acids such as glutamic acid(Glu) and aspartic acid(ASP) as well as inhibitory amino acids such as gamma-aminobutyric acid(GABA),glycine(Gly),alanine(Ala) and taurine(Tau) were examined.【Results】Power of frequency band ?,?,?_1,?_2,?_1 and ?_2 in group B increased(P
5.Mechanism of Isometric Exercise in Cardiac Rehabilitation

Isometric exercise(IE) is an essential component of daily living activities.Lack of IE may result in deconditioning of IE capacity.IE has not been used in cardiac rehabilitation because of poor understanding in cardiovascular response to IE and the Valsalva Maneuver associated with IE.Eingteen healthy young subjects aged 23.7 yrs on average was studied by administration of beta blocker and antagonist of opioids during IE.The result demonstrated that the regulation of cardiovasular response during IE is attributed to both sympathetic system and opioids activity.Twenty patients with coronary artery disease (CAD) were also studied during brief maximal IE,sustained maximal IE and dynamic exercise(DE).The patients included 13 cases with more than one vessel lesions and 7 post myocardial infarction and/or angina.Ten age matched apparent healthy subjects were examined as the control group.The result showed that IE without the Valsalva Maneuver in patients with CAD might increase coronary artery perfusion by enhancing diastolic pressure and prolonging diastolic period,which had no harmful effect on CAD.Hemodynamic responses and cardiac function were weaker during IE than DE.The intensity of the responses to IE was correlated to the central command but not to the absolute work load.It is concluded that significant release of endogenous opioids due to strong central command during IE,especially perceived maximal sustained IE,may help to attenuate activity of sympathetic system,thus reducing risk of exercise in patients with CAD.
6.Clinical Observation on Treatment of Anxiety with Combined Acupuncture and Medicine

Purpose To observe the curative effect of combined acupuncture and medicine on anxiety. Method An acupuncture plus medicine group and a medicine group were established for a comparison of their curative effects on anxiety. Results The total effective rate was 96% in the acupuncture plus medicine group and 64% in the medicine group, and there was a significant difference between the two groups (P<0.001). Conclusion The curative effect of combined acupuncture and medicine on anxiety is better than that of simple medicine.
7.Analysis of clinical characteristics of juvenile scleroderma

Objective To describe and analyze the clinical and laboratory findings in a group of children diagnosed with scleroderma at Beijing Children's Hospital in the last 10 years.Methods The clinical charts of children with scleroderma in the Rheumatology Department at Beijing Children's Hospital,between January 2002 and October 2013 were reviewed.All of them fulfilled the classification criteria for juvenile sclerodema,both systemic scleroderma (SSc) and localized scleroderma (LS) types.T test was used for comparison between the two groups.Results Forty-six patients were enrolled and were diagnosed as scleroderma.Seven patients(15%) suffered from SSc and 39 patients(85%) were LS.Mean age-at-onset of LS was (5±4) years old.The male to female ratio was 1.2:1.Mean age-at-onset of SSc was (9±4) years old.All patients were female.The lesions found in LS were linear scleroderma (54%),mixed morphea (36%),generalized morphea (8%),and panclerotic morphea (3%).Twenty-six patients had internal organs involved.Three patients with nerve system involvement was found in en coup de sabre (ECDS).Systemic involvement included lung and gastrointestinal tract primarily.The heart,nerve system,kidney,eye involvement was also found.One girl had SSc combined with renal crisis.Antinuclear antibodies were positive in 77% of LS patients and 100% of SSc patients.Rheumatic factor was positive in 6 patients (15%),5 patients had joint involvement.Tests for anti-Scl-70 antibodies were positive in 5 (71%) patients with SSc.The most common drugs used were methotrexate and prednisone.Conclusion In this study,LS is common in children.SSc is more severe than LS.Multi-center and large sample study is needed to know the characteristics of juvenile scleroderma in China.
8.In vitro susceptibility to 6 antimicrobial agents and genotyping of clinical isolates of Chlamydia trachomatis

Objective To assess the vitro susceptibility to 6 antimicrobial agents and genotypes of clinical isolates of Chlamydia trachomatis (Ct) from Guangzhou region. Methods Ct was isolated from clinical specimens by using McCoy cell culture and subjected to propagation. The minimum inhibitory concentration (MIC) and minimal bactericidal concentration (MBC) of 6 antimicrobial agents (clarithromycin, roxithromycin, azithromycin, doxycycline, tetracycline, ofloxacin) against Ct isolates were determined in McCoy cell culture. Nested PCR was performed to amplify the outer membrane protein 1 (omp1) VS1-2 gene followed by sequencing. Results Seventy-six Ct strains were isolated from 346 urogenital specimens, and 40 strains met the require ments for susceptibility testing after serial propagation. The MIC50/MIC90 of clarithromycin, azithromycin, roxi thromycin, doxycycline, tetracycline and ofloxacin were as follows: 0.008/0.032, 0.080/0.160, 0.125/0.500, 0.032/0.064, 0.250/0.500 and 0.500/1.000 mg/L. Seven genotypes were observed. The most prevalent geno types in decreasing order were E (14, 35%), J (10, 25%)and F (6, 15%). The MIC50 was consistent for azithromycin among the 7 genotypes, but varied by 1 - 4 folds for doxycycline, ofloxacin and roxithromycin. Conclusions Clarithromycin, doxycycline and azithromycin exhibit an excellent activity against Ct, and the activity of azithromycin is consistent among the 7 genotypes of Ct.
9.Drug Resistance of Alcaligenes xylosoxidans

OBJECTIVE To investigate the distribution and the antimicrobial resistance of Alcaligenes xylosoxidans and to provide the guidance of rational use of antibiotics.METHODS A.xylosoxidans was isolated and identified through VITEK-AMS all-automatic microbiology analysis system and its G~-bacilli identification cards(GNI) and drug sensitivity cards(GNS-KI/121) were also analyzed.Drug-resistance was tested by Kirby-Bauer disk(sensitivity) method((Bio-Merieux) and Oxoid products).RESULTS The drug-resistance rates to the 1st to 4th((except) CAZ) generation cephalosporins were more than 77%.The drug-resistance rate to the aminoglycosides was more than 76%,and to(ampicillin,) amoxicillin/CA,ampicillin/sulbactam,cefoxitin and cefuroxime were all 100%;but to(aztreonam) was 92.86%;66.67% and 44.44% strains were resistant to ciprofloxacin and(levofloxacin).(CONCLUSIONS) The detection rates of the multidrug resistant A.xylosoxidans have the tendency to increase yearly.The antibiotics should be used reasonably according to the results of drug sensitivity test in clinic.
10.Investigation and intervening therapy to depression of malignant hematologic diseases

Objective To evaluate the mental status of malignant hematologic patients, explore the morbidity of depression in malignant hematologic patients, and investigate valid interventional treatment on them. Methods 134 malignant hematologic patients were evaluated by SDS and HAMD, and 49 patientswere selected who was diagnosed depressive disorder then randomly divided into 2 groups. One was experimental group and the other control group. The patients of experimental group were treated with antidepressant drug and mental intervention during common therapy, while the patients of control group only took common therapy. The change of immunological function after treatment was detected. Results The morbidity of depression in malignant hematologic patients was 37 %. The scores of SDS and HAMD were significandy decreased and the depressive symptoms were notablely improved in experimental group and there were significant differences after treatment and before treatment (P <0.01), but there was no significant difference in control group (P >0.1). The NPY plasma levels significantly increased after treatment in experimental group(P <0.01), but there was no significant difference in control group(P >0.05); the CD+4/CD+4 values of patients in the experimental group were significantly increased after treatments. Statistical analysis revealed significant differences in the experimental group patients between pre-treat and after-treat (P <0,05),but no obvious difference in the patients of control group (P>0.5). Conclusion Mental intervention and antidepressive treatment can improve all of the depression, immunological function and quality of life of malignant hematologic patients.
