1.Status investigation of disease medical management behavior in patients after coronary artery bypass grafting

Objective To investigate the disease medical management behavior patients who discharged from hospital after coronary artery bypass graft(CABG).Methods During June 2013 to March 2014,a total of 110 patients after CABG in Changhai Hospital Affiliated to Second Military Medical University were recruited to fill questionnaire,and the results were analysed.Results The average score of 110 patients of disease medical management behavior was 40.57±5.13,the total score index was 54.09%.In the four dimensions of disease medical management,treatment adherence was the worst which scored 8.19±1.28,and scoring index was 54.60%.Disease knowledge management was the best implementation which scored 13.83±2.05,and scoring index was 52.00%.Gender,education,medical treatment charge of payment and complication were the influence factors of disease medical management behavior.Conclusions Medical staff should adopt different self-management intervention strategy,according to different patients and self management content.
2.Study on the correlation between ultrasound imaging and pathology in acute lung injury of rats

Objective To evaluate the correlation between ultrasonography and histopathology in acute lung injury of rats.Methods Twenty male Wistar rats were randomly divided into control group (4 rats) and experimental group (16 rats),in the experimental group acute lung injury models were step-by-step induced by oleic acid (OA) and sequentially by OA and lipopolysaccharide (LPS).After the end of each step of models,ultrasound examination was applied,and the rats were sacrificed to take the lung specimens to be observed.The ultrasonography and pathological results were analyzed,and the numbers of alveolar were counted under microscope (200 times).Results With the aggravation of acute lung injury,the sonographic findings showed unclear or disappeared pleural line,close-set of B-lines,and pulmonary consolidation with air bronchogram.For control group,the ultrasonographic score was 0.25 ± 0.50,lung injury pathological score was 0.50 ± 0.58,and the numbers of alveolar were 25 ± 3.For OA group,the ultrasonographic score was 2.86-± 1.35,lung injury pathological score was 6.28-± 0.76,and the numbers of alveolar were 10-± 2.For OA + LPS group,the ultrasonographic score was 5.83-± 2.32,lung injury pathological score was 9.83 ± 0.98,and the numbers of alveolar were 6-± 2.All 3 groups were significant different on ultrasonographic score,pathological score and alveolar counts (P < 0.01).Conclusions The ultrasonography image of acute lung injury of rats were positively correlated with its histopathological alteration.The ultrasonography can effectively evaluate the degree of acute lung injury of rats.
3.Application of multiple support vector machine recursive feature elimination model in cancer feature gene selection

Objective To analyze the cancergene expression profile data using multi-support vector machine recursive feature elimination algorithm (MSVM-RFE) and calculate the genetic ranking score to obtain the optimal feature gene subset. Methods Gene expression profiles of bladder cancer, breast cancer, colon cancer and lung cancer were downloaded from GEO (Gene Expression Omnibus) database.The differentially expressed genes were obtained by differential expression analysis. The differential gene expressions were sequenced by MSVM-RFE algorithm and the average test errors of each gene subset were calculated. Then the optimal gene subsetsof four kinds of cancer were obtained according to the minimum average test errors. Based on the datasets of four kinds of cancer characteristic genes before and after screening, linear SVM classifiers were constructed and the classification efficiencies of the optimal feature gene subsets were verified. Results Using the optimal feature gene subsetobtained by MSVM-RFE algorithm, the classification accuracy was improved from (96.77±1.28)%to (99.85±0.46)%for the bladder cancer data, improved from (83.77±4.93)%to (88.30±3.85)%for the breast cancer data, and improved from (72.69±2.41)%to (90.21±3.31)%for the lung cancer data.Besides, theoptimal feature gene subsetkept the classification accuracy of colon cancer classifierat a high level (>99.5%). Conclusions The feature gene extraction based on MSVM-RFE algorithm can improve the classification efficiency of cancer.
4.The application of multi-modal MRI in venous thrombolysis therapy for hyperacute cerebral arterial thrombosis

Objective To investigate the value of MRI in thmmbolytic thempy of hyperacute cerebral arterial thrombosis.Methods One hundred and sixteen patients with acute cerebral arterial thrombosis were recruited.plain CT and multi.modal MRI were performed in all patients.Thirty-three patients with hyperacute cerebral infarction were treated bv recombinant tissue plasminogen activator(rt-PA) and followed-up periodically using MRI.Results The 33 patients with thrombolysis selected by MRI demonstrated clinical improvement.90 d moclified Rankin scale scores(mRs)were less than 2 and life quality Barthal indexes(BI)were from 80 to 100.The complication included one asymptomatic parenchymal hematoma(PH1)one weeks after thrombolytic therapy and 4(12.2%)hemorrhagic infarction(HI)6 to 24 hours after thrombolytic therapy.Condusions MRI has significant clinical value for the screening and follow-up of intravenous thrombolytic therapy of hyperacute ischemic stroke.MRI-based thrombolysis is a safe and effective method for hyperacute ischemic stroke.
5.Summary of the evidence for the prevention of lower limb ischemic complications associated with venous-arterial extracorporeal membrane oxygenation patients

Objective:To retrieve, evaluate and integrate the relevant evidence of prevention of lower limb ischemic complications in venous-arterial extracorporeal membrane oxygenation (VA-ECMO) patients, and provide reference for the development of scientific and complete prevention and management of lower limb ischemic complications.Methods:According to the evidence-based methodology, clinical decisions, guidelines, expert consensus, evidence summary, systematic review, randomized controlled trials and experimental studies related to lower limb ischemia complications in VA-ECMO patients were searched from CNKI, Wanfang, PubMed, Cochrane Library and other domestic and foreign databases as well as relevant professional websites. The literature search period was from the establishment of the database to August 2023. Two researchers independently evaluated the literature quality, and then extracted and summarized the evidence according to the theme.Results:A total of 27 004 articles were obtained in the preliminary search, and 11 articles were finally included after screening, including 1 guide, 2 expert consensus, 2 systematic reviews and 6 original studies. Through literature reading, evidence extraction and classification, and expert group meetings, a total of 24 best evidences were concluded in four dimensions, including team training and management, VA-ECMO pre-computer evaluation, VA-ECMO catheter selection, and the monitoring and management of lower limb ischemia.Conclusions:This study summarized the best evidence for the prevention of lower limb ischemia complications associated with VA-ECMO patients, and can provide reference for healthcare providers in clinical practice. In order to ensure the safety of VA-ECMO treatment and reduce the incidence of related complications, healthcare professionals should carefully select and apply evidence according to the clinical context and patients′ wishes.
6.Management of patients with chronic kidney disease in the community

Huacao Community Health Service Center has established an integrated chronic kidney disease (CKD) management system since 2015,which greatly improved CKD management in the community.From September 2016 to August 2017,4084 residents were screened by the family doctor team,975 were suspected to have CKD (23.87%),among whom 202 (20.72%) were referred to district CKD treatment center,and 197 (97.52%) were confirmed as CKD.The detection rate of suspected CKD among patients with hypertension and diabetes mellitus was the highest,which was much higher than that among outpatients and those with routine health check up.It is suggested that the integrated management of CKD should be widely implemented to cope with the prevalence of CKD,particularly for those with hypertension and diabetes.
7.Knowledge-attitude-practice of chronic kidney disease among type 2 diabetic patients in a township of Shanghai suburb

A total of 510 patients with type 2 diabetics mellitus (T2DM)from Huacao Township of Shanghai suburb were enrolled in the study. The gender, age, duration for diabetes, HbA1c level, blood pressure level, obesity status and health education of chronic kidney disease (CKD) were recorded. The knowledge, attitude and practice (AKP) of the subjects regarding prevention and treatment of CKD were assessed by using a self-restraint questionnaire. The data were analyzed with logistic regression. The knowledge awareness rate, attitude positive rate and behavior execution rate were 44.1%(225/510), 49.0%(250/510) and 16.5%(84/510), respectively. Scores of knowledge,attitude,behavior and medication compliance of the subjects were 16.8 ± 4.8, 18.3 ± 3.6 and 13.1 ± 4.1, respectively. The attitudes were closely related to educational level, HbA1c level, systolic blood pressure, obesity, health education history, and awareness level of CKD;behaviors were closely related to education level, systolic blood pressure, obesity, health education history and CKD prevention and treatment(P<0.05).
8.Application of healthcare failure mode and effects analysis in risk management of extracorporeal membrane oxygenation transport adverse events

Objective To establish a project team to assess and manage the risk of out-of-town transport of extracorporeal membrane oxygenation(ECMO)patients using the healthcare failure mode and effects analysis(HFMEA),analyze the effectiveness of its application,and develop targeted improvement measures and processes.Methods Patients with ECMO who were treated in the department of intensive care unit(ICU)of Affiliated Hangzhou First People's Hospital,Zhejiang University School of Medicine March 2020 to September 2021 were selected as the study subjects.By comparing the differences in the accuracy of ECMO transport risk assessment,the rate of fixation against catheter slippage,the incidence of adverse transport events between the two groups before and after HFMEA,and the application effect of this model in ECMO transport adverse event risk management was evaluated.Results A total of 48 ECMO patients were enrolled,and 42 times of out-of-home transfers were performed,including 22 before HFMEA and 20 after HFMEA.Compared with pre-HFMEA,the accuracy of ECMO transport risk assessment[95.00%(19/20)vs.54.55%(12/22),P<0.05]and the compliance rate of catheter slip fixation[100.00%(20/20)vs.68.18%(15/22),P<0.05]in patients after HFMEA were significantly increased,the incidence of adverse events in transport was significantly lower[5.00%(1/20)vs.40.91%(9/22),P<0.05].Conclusion The application of HFMEA in the risk management of ECMO transshipment adverse events is beneficial to standardize the prevention of ECMO transshipment accidents,effectively reduce the incidence of transshipment adverse events,and ensure patient safety.
9.Value of lymphocyte subsets in assessing the prognosis of adult hemophagocytic syndrome

Objective:To explore the prognostic value of lymphocyte subsets in adult hemophagocytic syndrome (HPS).Methods:A total of 172 adult HPS patients diagnosed in 8 medical centers from January 2013 to August 2020 were selected for the study, of whom 87 were male (50.6%, 87/172), and 85 were female (49.4%, 85/172), with 68 survivors and 104 deaths. The clinical data were summarized, and variables such as lymphocyte subsets, immunoglobulin characteristics and fibrinogen were retrospectively analyzed, and the correlation between the mentioned variables and patient prognosis was analyzed. The optimal cut-off values of continuous variables were calculated by MaxStat, and the prognostic factors of HPS patients were screened based on the Cox proportional hazard regression model.Results:The median age of HPS patients was 56 (42, 66) years old, and the 5-year cumulative survival rate was 37.4% (37.4/100). The median age, platelet and albumin were 48 (27, 63) years, 84×10 9/L and 32.3 g/L in the survival group, and 59 years, 45.5×10 9/L, and 27.3 g/L in the death group, respectively. The differences between the two groups was statistically significant ( Z=?3.368, P=0.001; Z=?3.156, P=0.002; Z=?3.431, P=0.001). Patients with differentiated cluster 8+(CD8+)<11.1%, CD3+<64.9%, CD4+>51%, and CD4/CD8 ratio>2.18 had poor prognosis (χ 2=7.498, P=0.023; χ 2=4.169, P=0.041; χ 2=4.316, P=0.038; χ 2=9.372, P=0.002). Multivariable analysis showed that CD4/CD8 ratio, age, fibrinogen and hemoglobin were independent prognostic factors in HPS patients ( HR=2.435, P=0.027; HR=5.790, P<0.001; HR=0.432, P=0.018; HR=0.427, P=0.018). Conclusion:Peripheral blood lymphocyte subsets can be used to evaluate the prognosis of patients with HPS; CD4/CD8 ratio, age, fibrinogen, and hemoglobin are independent prognostic factors in HPS patients.
