1.Analysis of the clinical diagnosis and therapy and misdiagnosis for congenital middle ear cholesteatoma

[ABSTRACT]OBJECTIVETo investigate the methods of diagnosing and treating congenital middle ear cholesteatoma
and causes of misdiagnosis.METHODSThis was a retrospective review of 9 cases of congenital middle ear cholesteatoma in this hospital.RESULTS9 patients all received operative treatment and definite diagnosis was made by histopathologic investigation. The ossicular reconstruction was performed to 6 patients. All of the cases were followed-up for 3 months-2.5 years. A-B gap was 20 dB. After 1 year, no residual or recurrence of cholesteatoma was found in CT scan. All patients had been misdiagnosed. Misdiagnosis rate was 100%.CONCLUSIONCongenital middle ear cholesteatoma is a clinically rare and diagnosis is usually delayed in clinical practice due to the silent nature of the disease in its early stage. Preoperative diagnosis may be based on Levenson diagnostic criteria and CT imaging. Early surgical treatment can obtain a good effect of hearing reconstruction. To avoid misdiagnosis and incorrect treatment, radiological evaluation and careful local examination are really important, which may also avoid serious complications.
2.Analysis of the dection results of atypical lymphocytes in peripheral blood of patients with influenza

Objective To analyze testing results and values of blood heterotypic lymphocyte in flu patients.Methods 100 cases of influenza patients and 100 healthy people were collected.The observation group for flu patients and control group of healthy people.Extraction of peripheral blood in patients with microscopic examination observing leukocyte,neutrophil,lymphocyte,intermediate cells,atypical lymphocytes under a microscope,and calculated the percentage in the patients in the two group cells to make analysis and comparison.Ratio in peripheral blood of patients and analysis of specific lymphocytes were also calculated between different ages.Results Peripheral blood lymphocytes in observation group was (11.1 ± 2.6) %,which was significantly higher than than of the control group [(1.5 ± 0.5) %].The difference was statistically significant (chi-square =16.9,P < 0.05) ; White blood cells,the ratio of neutrophils,lymphocytes in observation group were obvious differenent compared with the control group(t/chi square are:13.2,11.3 and 12.6,P < 0.05) ;Three ages of influenza patients peripheral blood lymphocytes of the opposite sex ratio has no difference(P > 0.05).Conclusion Detection of peripheral blood lymphocyte heterosexual has great significance to the diagnosis of influenza with simple,convenient and feasible characters.It is worth to be used widely in clinical practice.

Objective: To observe the effect of polysaccharide ingredients from six TCM complex prescriptions on releasing the cytokine(CK) level of mouse celiac macrophages (M?), to explore the possible mechanism and direct the extractionand separation of active components for above compounds. Methods: The total polysaccharide ingredients of six complex prescriptions were prepared: Siwu Decoction, Sijunzi Decoction, Liuweidihuang Decoction, Guizhi Decoction, Longdanxiegan Decoction and Yupingfeng Pulveres, then the mouse celiac M? were incubated together in 96 holes board. Objective To investigate the infiltration of dendritic cell (DC) in Vocal cord polyp, laryngeal leukopla-kia and glottic squamous cell carcinoma,and to observe laryngeal mucosal lesions of the state of the immune microenviron-ment, and to research significance of DC on the development of glottic squamous cell carcinoma. Methods The infiltration of S-100+and CD83+DC in 20 cases of Vocal cord polyp, 47 cases of laryngeal leukoplakia, 45 cases of glottic squamous cell carcinoma tumor and 20 cases of laryngeal normal mucosa were examined using immunohistochemistry. Results The num-ber of S-100+and CD83+DC were significantly higher in glottic squamous cell carcinoma, laryngeal leukoplakia and vocal cord polyp than that in laryngeal normal mucosa (P<0.05). The number of S-100+and CD83+DC were higher in laryngeal leukoplakia than that in glottic squamous cell carcinoma (P<0.05). The number of S-100+and CD83+DC in mild dysplasia and moderate dysplasia were less than that in severe atypical hyperplasia (P<0.01). In glottic squamous cell carcinoma, S-100+ and CD83+DC with poor-differentiated was significantly less than that with well-differentiated (P < 0.01). Conclu-sion Changes in the number of dendritic cell was found in vocal cord polyp, laryngeal leukoplakia and glottic squamous cell carcinoma, which indicated that there were an abnormal immune status. Changing of dendritic cell in laryngeal mucosa plays an important role in laryngeal cancer development.
10.Experimental study on treatment of epigastralgia with Shugan Pills

AIM: To study the action of Shugan Pills (SHGP) (Fructus Toosendan, Rhizoma corydalis, Radix Paloniae Albe, Radix Aucklandiae, Cortex Magnoliae Officinalis, Fructus Aurantii, etc.) on treatment of epigastralgia. METHODS: The analgesic effect was tested by the hot plate test and writhing method. The anti gastric ulcer action of SHGP was observed on the gastric ulcer induced by water immersion stress, reserpine and absolute ethyl alcohol. The effect on gastric secretion in rats was studied with pylorus ligation. The effect on gastrointestinal motility was observed by determination of gastric emptying and small intestinal propulsion ability in mouse. RESULTS: SHGP enhanced hot pain threhshold and decreased the number of twisting body in mouse. SHGP markedly inhibited gastric ulcer induced by water immersion stress, reserpine and absolute ethyl alcohol. SHGP significantly inhibited the secretion of gastric acid and pepsin and promoted the secretion of gastric mucus. SHGP markedly delayed gastric emptying in normal mouse and when gastric emptying and small intestinal propulsion was stimulated by neostigmine. SHGP faintly strengthed the effect of atropine on inhibition of gastrointestinal motility. CONCLUSION: Shugan Pills could have analgesic effect, anti gastric ulcer action, it inhibited the secrection of gastric juice and the activity of gastric smooth muscle. These effects might be the pharmacological mechanisms of SHGP on treatment of epigastralgia.
