1.Aplastic anemia and clonal evolution:germline and somatic genetics

Clonal progression to myelodysplastic syndromes (MDS) or acute myeloid leukemia (AML) remains a dreaded complication for a subset of patients with bone marrow failure (BMF). Recognizing risk factors for the development of MDS or AML would inform individualized treatment decisions and identify patients who may benefit from early or upfront hematopoietic stem cell transplantation. Now that next-generation DNA sequencing is available in the clinical laboratory, research has focused on the implications of germline and somatic mutations for diagnosing and monitoring patients with BMF. Most germline genetic BMF disorders are characterized by a high propensity to develop MDS or AML. Recent studies of somatic variants in marrow failure revealed a high frequency of clonal hematopoiesis with the acquisition of mutations in genes associated with MDS or AML. The evaluation and implications of germline and somatic mutations for the development of clonal disorders in patients with BMF and challenges of clinical genetic testing will be summarized in this paper based on the reports from the 58th American Society of Hematology (ASH) Annual Meeting.
2.EXPERIMENTAL STUDY ON GUNSHOT CONCUSSION OF PERIPHERAL NERVE

Concussion injuries of peripheral nerves are frequently encountered in gunshot wounds of extremities. With the aid of high speed X-ray imaging technic, the sciatic nerves of dogs were visualized continuously during the formation of the temporary cavitation when a high-velocity of bullet was shot into the thigh.It was concluded that the concussional injury of the nerve was the result of a combination of stretching, compression, and concussion. There was a exponential correlation among cavity volume, absorption of energy and nerve displacement. The pathological changes included extensive microvascular injury and partial breakage of nerve fibres. Extensiveness and lack of uniformity in pathological changes may have some clinical implications.
3.Application of lymphocyte parameters in screening lymphocyte proliferation and reactive hyperplasia disease

Objective To investigate the application of lymphocyte parameters in screening the lymphocyte proliferation and re-active hyperplasia disease.Methods 128 cases as the healthy control,100 cases of lymphoma,35 cases of multiple myeloma(MM), 34 cases of lymphocytes reactive hyperplasia(RL)and 5 cases of chronic lymphocytic leukemia(CLL)were selected and performed the peripheral blood cells analysis by the Sysmex XE-2100 hematology analyzer.The relevant lymphocyte parameter values were re-corded and the comparative analysis in the difference between the disease groups and the control group was performed.The role of each lymphocyte parameter in screening the lymphocyte proliferation and reactive hyperplasia diseases was evaluated by the receiver operating characteristic curve(ROC curve).Results The high fluorescence lymphocytes(HFL),in the control group was 0.008 ± 0.008,in the lymphoma group was 0.016±0.058,in the MM group was 0.019 ±0.063,in the RL group was 0.040 ±0.070,and CLL group was 0.388±0.158.Compared with control group,the difference of RL and CLL group was statistically significant(P <0.05).The percentage of lymphocytes(LY%,:the control group:34.4%±5.9%,the lymphoma group:26.6%±13.1%,the MM group:31.0%±13.1%,the RL group:29.2%±15.4% and the CLL group:44.5%±38.5%.The difference between the lympho-ma group and the control group was statistically significant(P <0.001).The lymphocytes structural parameters X(LY-X),the con-trol group:833.5 ± 22.7,the lymphoma group:867.9 ± 28.5,the MM group:867.9 ± 26.6,the RL group:859.2 ± 27.8 and the CLL group:894.0 ± 65.4.Except for the CLL group,the differences between other groups and control group were statistically significant(P <0.001).The lymphocytes structural parameters Y(LY-Y),the control group:659.6 ± 23.0,the lymphoma group:669.4 ± 43.5,the MM group:665.9 ± 37.1,the RL group:665.9 ± 40.0 and the CLL group:778.4 ± 152.1.Compared with the control group,the difference of the lymphoma group was statistically significant(P <0.05).AUC of the ROC curve in the pa-tients with lymphocyte proliferation and reactive hyperplasia disease by LY-X screening was 0.819,with the sensitivity of 80.5%and the specificity of 60.1% when cutoff value was 842.5.Conclusion The lymphocyte parameters,especially LY-X can reveal the
morphological changes of lymphocytes sensitively and contribute to screening the patients with lymphocyte proliferation and reactive hyperplasia disease.
4.Relationship between pregnancy-associated plasma protein A and myocardial perfusion in patients with acute coronary syndrome

Objective To investigate the relationship between the levels of circulating pregnancy-associated plasma protein A(PAPP-A,a novel marker of atherosclerotic plaque activity) and myocardial perfusion before or after percutaneous coronary intervention(PCI) in patients with non-ST elevation acute coronary syndrome(NSTEACS).Methods Forty-four patients with high-risk NSTEACS were examined for serum PAPP-A levels,cardiac troponin T and the brachial artery flow-mediated vasodilation(FMD) before PCI.The angiographic data was analyzed before and after PCI in terms of TIMI myocardial perfusion grade(TMPG) in the area of culprit vessels.Results Before PCI,twenty patients with TMPG 1-2 had higher PAPP-A levels(30.41?21.33 mIu/L vs.23.55?18.61 mIu/L,P=0.036) and lower FMD(2.95%?1.71% vs.3.54%?1.58%,P=0.027) companed with the patients with TMPG 2-3.After PCI,twelve patients with TMPG 1-2 had higher PAPP-A levels(35.64?20.58 mIu/L vs.21.87?19.30 mIu/L,P=0.009) and lower FMD(2.26%?1.83% vs.3.95%?1.43%,P=0.018) than those with TMPG 2-3.Conclusion The PAPP-A level and the degree of endothelial dysfunction play roles in myocardial perfusion in patients with high-risk NSTEACS.
5.Optimal combination of baicalin, icariin and Astragalus saponin I from a Chinese herbal compound Biminne.

Objective: To study the optimal combined ratio of baicalin, icariin and Astragalus saponin I from a Chinese herbal compound Biminne. Methods: Firstly, a mouse model of allergic rhinitis was established by intraperitoneal injection of ovalbumin (OVA) and aluminum hydroxide gel suspension, and the effective dose range of baicalin, icariin and Astragalus saponin I was detected by 2,3-bis(2-methoxy-4-nitro-5-sulfophenyl)-2H-tetrazolium-5-carboxanilide inner salt method. Secondly, 10 groups of combinations of baicalin, icariin and Astragalus saponin I assembled by U*(10)(10(8)) form were employed to determine the optimal combination by means of analyzing of the inhibitory effect on the splenocyte proliferation. Finally, the effects of each effective ingredient and the optimal combination were compared by observing the splenocyte proliferation, the contents of interleukin-4 (IL-4), IL-5, interferon-gamma (IFN-gamma) in supernatant of the splenocyte cultures and the ratio of IL-4 to IFN-gamma in order to verify the result. Results: Baicalin or icariin at concentrations ranging from 2 to 10 mumol/L, and Astragalus saponin I from 1 to 10 mumol/L effectively suppressed the splenocyte proliferation. When the proportion of baicalin, icariin and Astragalus saponin I was 1:2.14:2.65, the inhibitory effect was most remarkable. Further research confirmed the rationality of the optimal combination. Conclusion: An optimal combination of the major effective ingredients from Chinese herbal compound Biminne most effectively suppresses the proliferation of splenocytes from sensitized mice and regulates the cytokine secreting.
6.Risk Factors Study for Lung Cancer Patients with Chemotherapy-induced Severe Neutropenia

Objective:To discuss the risk factors in lung cancer patients with chemotherapy-induced severe neutropenia to provide reference for clinical drug use. Methods:A retrospective analysis was performed for the patients with lung cancer,and the risk factors of severe neutropenia were statistically analyzed and found out. Results:The results of single factor experiments showed that the incidence of severe neutropenia was related with radiotherapy history,cycles of chemotherapy and the use time of granulocyte colony factor. Based on a binary logistic regression analysis,the history of radiotherapy and the use of granulocyte colony factor were the significant risk factors of severe neutropenia in the lung cancer patients. Conclusion:For the patients with radiotherapy history,it is better to choose chemotherapy drugs with lower toxicity,decrease drug dosage or preventively use granulocyte colony factor. The rational use of rhG-CSF can alleviate chemotherapy-induced severe neutropenia.
7.Urinary metabolic profiling of rats with articular cartilage damage induced by T-2 toxin

Objective To search for the metabolites that associated with articular cartilage damage in the urine of rat model with articular cartilage destruction induced by T-2 toxin.Methods Thirty healthy male Wistar rats aged 4-5 weeks were numbered by weight,randomly divided into two groups (n =15 per group),namely the control group and the model group.The rats in the control group were fed with standard rat diets,and those in the model group were given diets contaminated by T-2-toxin (300 μg/kg).Throughout the experiment,all animals were given free access to distilled water and diets.After continuous treatment for 3 months,all the rats were sacrificed.The changes of articular cartilage in rat knee joints were observed by histopathological method,the metabolic profile of rats' urine was determined by ultra performance liquid chromatography/quadrupole time of flight-mass spectrometry (UPLC/QTOF-MS) technique.Combined with multivariate statistical analysis,database searching was applied to explore and confirm the different metabolites associated with cartilage damage.Results Light microscope showed that rats' articular chondrocytes in the control group presented cells in neat rows and eumorphism,rats' articular chondrocytes in the model group presented extensive areas of chondrocyte degeneration,necrosis and loss.In rats' urine metabolic profiles,5 different metabolites associated with cartilage destruction were detected,such as 4-hydroxynonenal (HNE),trans-4,5-epoxy-2(E)-decenal (EDE),5-methyldeoxycytidine,and 5-L-glutamyl-glycine and prolyl-valine.Compared with the control group (mass spectrum peak area:65820 ± 5200,22080 ± 3538,4292 ± 3520,3277 ± 2025,1104 ± 990),all of them increased in the model group (mass speetrum peak area:90240 ± 18863,25610 ± 5071,9702 ± 6562,6029 ± 3905,4144 ± 5322,t =-3.903,-2.209,-2.814,-2.424,-2.174,all P < 0.05).Conclusions The articular cartilage destruction induced by T-2 toxin could cause the changes of related metabolites in the urine;the 5 kinds of changed metabolites in urine are related to articular cartilage destruction.
8.Association of Serum Total Cholesterol Level and Prognosis in Patients with Acute Left Heart Failure

[Objective] The present study was to evaluate the association of serum total cholesterol level and prognosis in patients with acute left heart failure and associated mechanisms.[Methods] Sixty-eight patients due to acute episode of left heart failure prospectively enrolled,and baseline data and biochemical parameters were collected.After discharge,patients were follow-up for 1 month and they were divided into two groups (with and without cardiovascular events).Differences between groups were evaluated and the association of serum total cholesterol level and cardiovascular events were analyzed by logistic regression analysis.[Results] The mean age was 57.3 ± 12.6 years old and 52 cases were male patients accounting for 76.5 %.Among these patients,46 had a diagnosis of coronary heart disease (67.6 %),10 rheumatic heart disease (14.7 %),12 dilated cardiomyopathy (17.7%),38hypertension (55.9%) and 24 diabetes mellitus (35.3%).After 1 month's follow up,39 patients (57.4%) had experienced cardiovascular events,36 cases were re-hospitalized,and 3 died from heart failure.Compared to those with cardiovascular events,event free individuals were younger and were less likely smokers (P < 0.05).In addition,event free group had lower serum levels of N-terminal pro-BNP and C-reactive protein (P < 0.05) while serum levels of total cholesterol and albumin were significantly higher (P < 0.05).There was no significant difference in medication between these two groups.After adjusted for age,gender,smoking,systolic blood pressure,serum albumin level,diabetes,hypertension and medications,increased total cholesterol level was independently associated with better prognosis with odds ratio of 0.91 (95 % confidence interval 0.80-0.96).Further adjusted for C-reactive protein,the association was attenuated to non-significance,with odds ratio of 0.97 (95 % confidence interval 0.87-1.09).[Conclusion] Adequate serum total cholesterol level was beneficial for improving short-term cardiovascular outcomes in patients with left heart failure and the potential mechanisms might be related to cholesterol effects on improving nutritional status and anti-inflammation.
9.The expression and function of galanin system in the hippocampus of anxiety-and depression-like behavior female mice

Objective To explore the expression and function of galanin and its receptor in the female mice of anxiety-and depression-like behavior.Methods In situ hybridization was used to detect the expression of galanin,GalR1,GalR2 and GalR3 in the hippocampus of C57BL/6J female mice.A total of thirty female mice was divided into two groups:experiment group (n=15) and control group (n=15).The experiment group was subjected to the chronic unpredictable mild stress (CUMS) for four weeks,and the control group was not subjected to any treatment.Four weeks later,a series of tests were performed on those two groups,including the sucrose preference test,the novel object recognition test,the open field test and suspension tail test.After behavior tests,the hippocampus RNA was extracted from eight mice in each group to test the expression of galanin and its receptor through qPCR.The rest part of mice were used to mark C-Fos immunostaining in the dentate gyms (DG) of hippocampus.Results The result of in situ hybridization showed that galanin,GalR1 and GalR2 distributed in the Hillus of ventral hippocamous,GalR3 had no positive signal.In the sucrose preference test,the experiment group drunk less sucrose when compared with the control group (P<0.05).In the novel object recognition test,the experiment group contacted shorter time to the novel object when compared with the control group (P<0.05).In the open field test,the experiment group had shorter in session time when compared with the control group (P<0.05).In the suspension tail test,the experiment group had longer immobility time when compared with the control group (P<0.05).The qPCR result showed that the higher expression of galanin and GalR1 in the hippocampus of experiment group (P<0.05).More C-Fos positive cells in the DG of hippocampus of experiment mice were immunostained (P<0.05).Conclusions Galanin,GalR1 and GalR2 mainly distribute in the Hillus of the ventral hippocampus.Galanin may be involved in the anxiety-and depression-like behavior of C57BL/6J through GalR1.
10.Correlation between weight loss and setup errors in intensity-modulated radiotherapy for rectal cancer

Objective To investigate the change in body weight over time in rectal cancer patients receiving radiotherapy and the correlation between setup errors and weight loss,and to establish the image-guided radiotherapy regimens in different periods of treatment.Methods A total of 24 postoperative patients with rectal cancer admitted to our hospital in 2016 were selected.Before each fraction of radiotherapy,the body weight was recorded,and the patients underwent cone-beam computed tomography (CBCT) with different frequencies in every week.The planning CT was matched with CBCT to obtain setup errors.The paired t test was used for difference analysis;the Pearson method was used to analyze the correlation between setup errors and weight loss.Results Body weight was measured 456 times in the 24 patients,and these patients underwent CBCT scans and image registration 456 times.Two patients were excluded because of treatment discontinuance.In the first and second weeks,there was no significant change in body weight.In the third week,the mean weight loss was 1.53 kg.In the fourth week,the mean weight loss was 2.48 kg.In the fifth week,the mean weight loss was 3.24 kg.The setup errors obtained by CBCT image registration in the superior-inferior (SI),anterior-posterior (AP),and left-right (LR) directions were 0.19 cm,0.20 cm,and 0.18 cm,respectively,in the first week,0.18 cm,0.17 cm,and 0.15 cm,respectively,in the second week,0.20 cm,0.22 cm,and 0.21 cm,respectively,in the third week,0.19 cm,0.25 cm,0.24 cm,respectively,in the fourth week,and 0.34 cm,0.33 cm,and 0.31 cm,respectively,in the fifth week.The Pearson correlation analysis showed that weight loss increased the setup errors,with P values of 0.140,0.046,and 0.044 in the SI,AP,and LR directions,respectively.Conclusions For rectal cancer patients receiving radiotherapy,the body weight decreases significantly in the late period (especially in the fifth week),which influences the setup errors.Therefore,in the fourth and fifth weeks of radiotherapy for rectal cancer,the weight loss should be closely monitored,and the number of CBCT scans can be increased before the treatment fraction to ensure the accuracy and optimization of treatment.
