1.Reliability and Validity of Morisky Questionnaire in Measurement of the Compliance with Hypertensive Medications

Objective To evaluate the reliability and validity of Morisky questionnaire in measurement of the compliance with hypertensive medications. Methods Seventy-four eligible subjects were selected and asked to fill a Morisky questionnaire about their adherence in taking antihypertensive drugs during the past 1 month; nineteen of them received two surveys at 3~7 day intervals. The internal consistency (Cronbach ? coefficients),test-retest reliability (interclass correlation coefficients),construct validity and convergent validity were calculated. Results The Cronbach ? coefficient of the questionnaire was 0.749 and the interclass correlation coefficients between the items and the total scores of the questionnaires ranged from 0.71 to 1.00. Factor analysis yielded one factor with an accumulated contribution rate of 57.3%,and the loading values of all items on this factor were above 0.6. 2.Two treatment methods for obstructive sleep apnea hypopnea syndrome

OBJECTIVE To investigate the two treatment methods for obstructive sleep apnea hypopnea syndrome(OSAHS)patients.METHODS 1.Four patients with habitual snoring and two patients with mild OSAHS accepted pillar palatal implant.2. Eleven patients with habitual snoring and fifteen patients with mild to moderate OSAHS accepted temperature- controlled bipolar radiofrequency(RF)ablation in both tonsils.RESULTS The postoperative examination was performed after 6 months.Pillar palatal implant: snoring VAS was reduced from 6.9?0.68 preoperatively to 3.8?0.54 postoperatively in 4 patients with habitual snoring,snoring VAS was down from 9.2?1.21 to 3.1?0.52 in 2 patients with mild to moderate OSAHS.Seven of 11 cases with habitual snoring were cured by RF,and 2 cases were effective,the other 2 cases were failed. Ten of fifteen cases with mild to moderate OSAHS were cured,3 cases were effective,and 2 cases were failed. The effective rate was 84.6%(22/26).CONCLUSION 1.The pillar palatal implant system is a safe method for patients with simple snoring and mild OSAHS in a short time observation.2.Temperature-controlled bipolar radiofrequency ablation is effective for mild to moderate OSAHS duo to hypertrophy of the tonsils.3.According to the clinical characteristics of the patients to select suitable method is the key to get a satisfactory curative effect.
3.Latissimus dorsi musculocutaneous flap for repairing severe soft tissue defect of the elbow

Objective To explore the effect of latissimus dorsi musculocutaneous flap for repairing severe soft tissue defect of the elbow.Methods Seventeen patients with severe soft tissue defect of the elbow were repaired by latissimus dorsi musculocutaneous flap.Ten patients were performed transfer of skin flap for functional reconstruction,9 patients reconstructed elbow flexion,and 1 patient reconstructed elbow extension.Seven patients' tissue defect were repaired by cover type musculocutaneous flap,and the areas of these flaps ranged from 6 cm × 10 cm-12 cm × 27 cm.Results All the musculocutaneous flaps kept alive.After an average postoperative follow-up for 12-40(22.0 ± 9.6) months,the color and luster of the flaps were satisfying.There were no scar contracture of the flap and no bulkiness of the elbow.Ten patients performed transfer of skin flap for functional reconstruction,and the range of motion was 100°-140° of flexion and-20°-0° of extension.Muscle power was M3-M5.Hand function was near normal.Conclusion Latissimus dorsi musculocutaneous flap is an ideal surgical method to repair the severe soft tissue defect of the elbow.

5.Application of voice acoustic analysis in microlaryngoscopic surgery

Objective:To evaluate the relationship between the effect of microlaryngoscopic surgery of vocal polyp and the change of all-parameters.Method:Acoustic parameters of 80 patients with vocal polyp were measured by Dr .Speech Sciente for Windows, and analyzed preoperatively and postoperatively.Result:The jitter, shimmer and NNE were different significantly between preoperative and postoperative,the NNE and shimmer were more sensitive than jitter.Conclusion:The NNE and shimmer of acoustic parameter are objective parameter to evaluate the effect of the microlaryngoscopic surgery of vocal polyp.
6.Study on inhibition of lymphangiogenesis in gastric cancer by NM-3

Objective To evaluate the inhibitory effect of 2-(8-hydroxy-6-methoxy-1-oxo-1-H-2-benzopyran-3-Y1) propionic acid (NM-3) on lymphangiogenesis in gastric cancer using orthotopic implantated tumor models of BALB/C nude mice. Methods A BALB/C nude mouse model of transplanted in situ human gastric cancer was established. Twenty-eight nude mice were divided into four groups with 7 each: control group, NM-3 treated group, carboplatin (10 mg/kg) treated group,and NM-3 combiantion group injected with normal saline, 5 mg/kg of NM-3, 10 mg/kg of carboplatin or 5 mg/kg of NM-3, + 10 mg/kg carboplatin, respectively, twice a week for 8 weeks. At the end of the 8th week, all mice were sacrificed for detection of lymphatic microvessel density (LMVD),lymphatic vessel endothelial hyaluranic acid receptor 1 (LYVE-1), podoplanin and Prox-1 byimmunohistochemistry with staining. Results In comparison with control group, the LYVE-1 level in other three groups was decreased with no significant difference (P> 0.05). The concentrations of podoplanin and Prox-1 in NM-3 group and combination group decreased significantly than those in control group and carboplatin group (P < 0.05). The number of LMVD in NM-3 group and combination group was 4.72±0.50 and 4.78± 0.38, respectively, which was significantly lower than that in control group (7.35±0.55)and carboplatin group (6.98i0.35, P<0.05). Conclusion The NM-3 can inhibit the growth of gastric cancer by interfering lymphangiogenesis of gastric cancer.
7.Effects of Xuefu Zhuyu Decoction on the Morphological Features of Vascular Endothelial Cells Injured by Serum of Blood-stasis-syndrome Rabbits

Objective To observe the protective effect of Xuefu Zhuyu Decoction (XZD) on vascular endothelial cells (VEC) injured by the serum of blood_stasis_syndrome ( BSS) rabbits. Methods The morphological features of VEC cultured with norma l rabbit serum (Group A), BSS rabbit serum(Group B), the serum of normal rabbit medicated with XZD(Group C) and the serum of BSS rabbit medicated with XZD(Group D) were observed respectively under light microscope and electron microscope. Results Under light microscope, VEC was shrinked, intercellular space widene d and cytoplasm contained dark granules in Group B. The change of intercellular space was not obvious in Group D. Under electron microscope, pinocytotic vesicle s increased, rough endoplasmic reticulum (RER) decreased and dilated, mitochondr ia became smaller and indistinct and only a small amount of microvilli appeared on cell membrane with top expanded and merged in VEC of Group B as compared with Group A and Group C. Less RER but no dilatation and fewer microvilli without to p merged were found in VEC of Group B. Conclusion The serum of BSS rabbits c an injury normal VEC cultured in vitro and XZD exerts a certain protective effe ct on VEC injured by the serum of BSS rabbits.
8.The improvement of living quality for the patient with carcinoma of esophagus: Reconstruction for the defect of pharynx and cervical esophagus with inferior antebrachial skin flap on the basis of retaining the laryngeal function

Objective To investigate the effect of reconstruction for inferior pharyngeal and cervical esophagus defect with inferior antebrachial skin flaps on the living quality of patients after surgery. Method Perform radical operation of the tumor, but retain larynx and trachea. Free antebrchial skin flaps were used to reconstruct the defect of inferior pharynx and cervical esophagus. Roll the skin flap to form a skin tube, and then carry out anastomosis with floor of mouth and cervical esophagus. Result The laryngeal function was retained after surgery. And permanent orifice of trachea was unnecessary. The effect was satisfying. The vocalization wasn't affected and food- intake through oral was normal in the 2 patients followed up. Conclusion It is a feasible way to reconstruct the inferior pharynx and cervical esophagus defect caused by radical operation of the tumor at pharynx and cervical esophagus. It can improve the living quality of patients effectively.
9.Percutaneous cannulated screw fixationversus open reduction and internal fixation for medial malleolus fracture:recovery of joint function

BACKGROUND:The ankle is one of the most important joints of human body. Medial maleolar fractures are very common, and there are lots of surgical methods to treat it. A traditional approach is open reduction and internal fixation. As views changed, percutaneous cannulated screw internal fixation become increasingly popular, but various clinical studies are stil needed to analyze the efficacy of these two methods. OBJECTIVE:To compare the recovery of joint function after medial maleolus fracture repaired by open reduction and internal fixation and percutaneous cannulated screw fixation. METHODS:A total of 63 cases of medial maleolus fracture, who were treated in the Department of Orthopedics, People’s Hospital of Xinyi City from March 2009 to March 2013, were enroled. According to repair plan, they were divided into two groups: open reduction and internal fixation group (n=29) and percutaneous cannulated screw fixation group (n=34). Ankle function was assessed in accordance with Kofoed ankle score standard on admission and at 3 months after repair. RESULTS AND CONCLUSION:The patients were folowed up for 3 to 12 months. Among 29 cases in the open reduction and internal fixation group, the wound was healed at grade A in 27 cases and at grade B in 2 cases; 29 cases were reset, and no poor reduction was found. In the percutaneous cannulated screw fixation group, 34 cases had healing at grade A, with the presence of good reduction. Al patients experienced bone union, and no infection appeared. In accordance with Kofoed score, at 3 months of folow-up, the satisfaction rate was 97% in the open reduction and internal fixation group, and 100% in the percutaneous cannulated screw fixation group. These data suggest that both open reduction and internal fixation and percutaneous cannulated screw fixation for medial maleolus fracture obtained positive effects, but percutaneous cannulated screw fixation showed smal surgical trauma, which could reduce the rate of infection and contributed to early functional recovery of ankle joint.
10.Prevalence of mental disorders in the male juvenile detention centers of Hunan and Sichuan

To investigate the prevalence of mental health problems in male violent offenders of Hunan and Sichuan Provinces; and to compare the types and severity of problems between the violent and nonviolent offenders.Methods:Ninety-one violent juvenile offenders and 64 nonviolent juvenile offenders in the juvenile detention centers of Hunan,and 81 violent juvenile offenders in the juvenile detention centers of Sichuan; 39 high school students from a middle school of Hunan Province and 49 from a middle school of Sichuan Province were investigated using the Investigation Screening Inventory for Child Mental Disorder and the Kiddy Schedule for Affective Diseases and Schizophrenia (K-SADS-PL),and classified according to the Diagnostic and Statistical Manual of Mental Disorders.Results:In the Hunan violent group,86.6% met the criterion of conduct disorder (CD),17.6% of substance abuse,9.9% of substance dependence,while in the Hunan nonviolent group,75.0% met the criterion of CD,and 11.7% of substance abuse; these were all significantly higher than those of the control group (P<0.05).In the Sichuan violent group,17.3% met the criterion of attention deficit/hyperactivity disorder (ADHD),18.5% of oppositional defiant disorder (ODD),69.1%of CD,and 22.2% of substance abuse; these were all significantly higher than those of the Sichuan control group.Moreover,63.7% of Hunan-violent,55.6% of Sichuan-violent,and 45.0% of Hunannonviolent offenders had a previous offence records.Conclusion:Mental and behavioral disorders among delinquent youth is becoming a serious problem,and there is an urgent need to develop and implement effective assessment and treatment approaches for juvenile offenders with the aim of reducing offence and recidivism in this population.
