1.The chinese version of the patient rated wrist evaluation (PRWE): cross-culture adaptation and reliability test

Objective To evaluate the test-retest reliability and internal consistency of a Chinese version ofthe PRWE in a group of patients with distal radius fracture and to compare it with the original PRWE. MethodsThe original PRWE was translated into Chinese. Test-retest reliability was obtained by repeating the PRWE to thesame subjects in one week interval and presented by intra-class correlation coefficient (ICC), internal consistencywas measured by Cronbachs coefficient alpha. Results The Chinese PRWE has good test-retest reliability and in-ternal consistency which are equivalent to the results from the English PRWE. The ICC for the Chinese PRWE rangedfrom 0. 88 to 0. 92. and Cronbach Alpha for the Chinese PRWE ranged from 0. 87 to 0. 97. Conclusion The Chi-nese version of PRWE is equivalent to the original PRWE with acceptable level of reliability and internal consistency.It is a good functional assessment tool for patients with distal radius fractures.
2.Prediction of acute kidney injury complicated by sepsis with neutronphil gelatinase-associated lipocalin as an early marker

Objective To investigate the accuracy of NGAL in prediction of acute kidney injury (AKI) complicated by sepsis. Methods Blood and urine samples were collected at different time points in 74 sepsis patients, and 17 AKI patients were got from them.Cysteine Cys C levels were detected with Latex enhanced turbidimetric immunoassay (LETIA). Urine NGAL and Scr levels were detected with solid-phase sandwich enzyme-linked immunosorbent assay and creatinine enzymatic assay respectively. At the same time, 57 non-AKI patients were set as controls. Dynamic changes of urine NGAL and Scr were observed in sepsis patients in test and control group. ROC curve was used to evaluate the performance of AKI diagnosis by urinary NGAL.Results The Scr base line was (59. 38 ±16. 72) μmol/L.The median time of diagnose of AKI was 24 (12, 48) h past-sepsis diagnosis, while the Scr was(100. 35± 28. 26) μmol/L. Of the group, 17 cases of sepsis were diagnosed with AKI, accounting for 23% (17/74) prevalence. In sepsis group, base line value[(0. 61 ± 0. 15) mg/L] of serum Cys C was not upregulated at 2, 4 and 6 h, which was (0. 63 ± 0. 14) mg/L, (0. 68 ± 0. 16) mg/L and (0.65±0. 14) mg/L respectively. As to 8 h [(0. 85 ± 0. 22) mg/L], a slight upregulation was found with no significant difference (t = 1.63, P > 0. 05).A trend of gradual increase of serum Cys C levels was found in sepsis AKI group at 12, 18, 24, 36, 48 and 72 h. A significant difference was found compared with base line (t = 2. 81, 2.98, 3.05, 3.11, 3. 38,3. 17,P <0.01). NGAL level[(96.21 ±45.32) μg/L] was significantly higher than base line value [(4. 98 ± 1.65) μg/L]. Subsequently, in each time point from the 2 h to 48 h, a trend of gradual increase in urinary NGAL levels was showed. The differences were significant compared with the baseline values (t =2. 74,2. 83,2. 91,3.04,3.15,3.22,3. 31,3.45,3.57 ,P <0. 01). Urinary NGAL levels of sepsis AKI group were significantly higher than those in non-AKI group at different time points with significant difference (t =2. 69,2. 73,2. 84,2. 96,3.02,3. 13,3.29,3.43,3.54,3.22,P <0. 01). ROC curve was selected to analyze diagnostic performance of urinary NGAL levels in AKI patients 2 h post-sepsis. Area under ROC curve was obtained as 0. 935, 95% confidence interval was 0. 683-0. 971. When cut-off value of urinary NGAL for patients 2 h post-sepsis was 50 μg/L, the sensitivity, specificity and accuracy were 94. 4%, 87. 5% and 96. 8% in AKI diagnosis respectively. Conclusions The occurrence of AKi can be accurately predicted by urinary NGAL levels 2 h post-sepsis , and its AKI diagnosis time is earlier than that of Scr. Urine NGAL can be used as early marker of AKI complicated by sepsis.
3.Application of antigen-antibody joint detection in increasing detection rate of HCV infection in patients with hemodialysis

Objective To invetigate the application value of antigen‐antibody joint detection in increasing the detection rate of hepatitis C infection in the patients with hemodialysis .Methods The clinical data of 100 cases of hemodialysis were collected .Hep‐atitis C virus(HCV) core antigen ,HCV antibody and HCV‐RNA were detected by adopting ELISA ,Chemiluminescence and RT‐PCR respectively .The positive expression situation of each detection item in the same specimen was performed statistics .The detec‐tion rates of RNA positive specimen were compared among the single antigen detection ,single antibody detection and joint detection schemes ;at the same time the antigen level of antigen positive specimens ant its RNA copy number were performed the correlation analysis .The antibody levels in the positive antibody specimens were compared between the RNA positive group and the RNA neg‐ative group .Results The single antigen or single antibody detection could cause the missed detection of HCV infection in hemodial‐ysis patients .For the specimens of positive RNA ,the joint detection had the highest detection rate (P<0 .05) ,the antigen level in the antigen positive specimens was positively correlated with the RNA copy number in the corresponding specimens (r=0 .85 ,P<0 .05) ,while the antibody level in the antibody positive specimens had no statistical difference between the RNA positive group and the RNA negative group(P>0 .05) .Conclusion The joint detection of HCV antigen antibody can effectively increase the detection rate of hepatitis C infection in hemodialysis patients .At the same time the HCV core antigen detection also can be used as one of monitoring indicators for the HCV re -infection in the patients with antibody positive .
4.Double Endobutton Technique for Repair of TypeⅢ Acromioclavicular Joint Dislocations

[Objective]To introduce a new surgical method for treatment of Tossy typeⅢacromioclavicular joint dislocation.[Methods]To reconstruct coracoclaviacular ligament with double ENDOBUTTON technology.[Results]The six patients undergoing the operation were followed up for an average time of 0.5 year,the result was excellent and good(Great A).All patients recovered from abnormal function and no complication happened.[Conclusion]Double ENDOBUTTON technology is an effective method for treating typeⅢ acromioclavicular joint dislocation;function recovery of acromioclavicular joint is fast without complication.
5.Central and peripheral hemodynamic changes in fetuses with absent end-diastolic velocity in umbilical artery and the correlation with perinatal outcomes

Objective To investigate the relationship between absent end-diastolic velocity of fetal umbilical artery (AEDV) and perinatal outcome. Methods The color Doppler ultrasound result of maximum flow velocity waveforms at the cardiac level (aorta, pulmonary artery) and the peripheral level (fetal umbilical artery, middle cerebral artery) in 27 cases with Umbilical artery AEDV were analyzed. Fetal heart rate monitoring was performed and perinatal outcome was followed up. Results All of the 27 cases presented with remarkable pulmonary artery contraction. The result of velocity time integral (VTI)?fetal heart rate (FHR) and right cardiac output declined significantly. Among 22 cases with continuous monitoring, 10 cases showed reduced PI of middle cerebral artery (MCA) without changes of left cardiac output; 12 cases had biphasic changes of the MCA PI which consisted of decrease followed by increase and showed significant reduction of VTI?FHR of aorta and left car-(diac) output. The interval between the first detection of AEDV and the presentation of abnormal FHR patterns was 2 to 7 days. Conclusions Umbilical artery AEDV usually precedes the presentation of abnormal FHR under fetal hypoxia. Delivery before decompensation may improve the perinatal morbidity and mortality.
6.Simultaneous Extraction of Atrazine and Its Toxic Metabolites Based on Functionalized Polyacrylonitrile Nanofiber Mat

A novel solid-phase extraction (SPE) adsorbent for simultaneous extraction of atrazine (ATZ) and its metabolites, deisopropylatrazine (DIA) and deethylatrazine (DEA) from environmental water samples was prepared. Polyacrylonitrile nanofibers (PAN NFs) mat was prepared via electrospinning, and was further functionalized to obtain polypyrrole modified polyacrylonitrile nanofibers (PPy-PAN NFs) mat, hydrazine modified polyacrylonitrile nanofibers (NH2-PAN NFs) mat and carboxyl modified polyacrylonitrile (COOH-PAN NFs) mat. The results showed that the adsorption capacity of COOH-PAN NFs mat was better than other three NFs mats in both static (2.0 mg/g) and dynamic (0.19 mg/g) experiments. Meanwhile, the runoff ratios of COOH-PAN NFs mat were the lowest (less than 30.0%) in the adsorption of three analytes, especially for high polar analytes, which showed that the hydrogen bond between carboxyl groups and analytes was the main interactive force. A combination of mat-based SPE and high performance liquid chromatography-diode array detection was further established for determination of 3 analytes in environmental water samples. The recoveries were 81.4%-120.3% and the limits of detection were 0.12 ng/mL for DIA, 0.09 ng/mL for DEA and ATZ, respectively.
7.MicroRNA expression in murine skin at different stages of hair cycle: a preliminary study

Objective To screen differentially expressed microRNAs at different stages of hair cycle in a murine model.Methods This study included 30 inbred female C57BL/6 mice (age,6-8 weeks; body weight,15-18 grams).Hair growth cycle was induced in the back skin of C57BL/6 mice by application of wax/rosin followed by depilation under anesthesia witl 1% chloral hydrate.Three mice were sacrificed by cervical dislocation on day 0,8 and 20 after the induction,and skin tissue was achieved from the same depilated areas parallel to the spine.Total RNAs were extracted from the murine skin and subjected to microarray analysis of microRNA expression.Results Compared with telogen skin,the murine anagen skin showed a higher expression of miR-690,obselote-49 and miR-1308,but a lower expression of miR-291a-5p and miR-212.The expressions of miR-690,obselote-49 and miR-31 were significantly up-regulated,while those of miR-127-3p and miR-212 were downregulated in the catagen skin in comparison with the telogen skin.Conclusion Seven microRNAs are identified in this study to be differentially expressed in murine skin between different stages of hair cycle,which may provide a direction for future research.
8.PATHOLOGICAL CHANGES OF HEART IN SCHISTOSOMIASIS RABBITS

Glomerulonephritis and rheumatic heart disease are usually inferred as immune complex disease caused by infections, but the pathogenesis of rheumatic heart disease is still not clear. In this experiment, we infected 38 rabbits with cercariae of S. japonicum and de-monstrated the histopathological changes of endocarditis, myocarditis and perivasculitis in the heart tissues of 33(86.8%) infected rabbits. Thrombosis was also found in heare vessels, but adult schistosomes or eggs were not found in the heart tissues. Direct and ndirect immune enzyme staining studies showed the presence of schistosome-specific antigenst and host antibodies in the heart tissues, suggesting that the histopathological changes in ithe infected rabbit hearts were resulted from the deposition of immune complexes and th secondary events pf host autoimmune reactions. In this paper, correlative clinical cases were reviewed and the possibility of heart disease in human schistosomiasis caused by immune mechanism was discussed. We hope that the above findings might broaden our understanding of the pathogenesis of rheumatic heart disease, myocardiopathy and cardio-vascular diseases (Plates 1-3).
9.Role of 3D printing positioning guide template in pedicle screw fixation of unstable atlas fractures

Objective To investigate the clinical outcomes of pedicle screw fixation assisted with the 3D printing positioning guide template for treatment of unstable atlas fractures.Methods A retrospective case series review was made on 10 patients with unstable atlas fractures undergone direct posterior C1 pedicle screw fixation assisted with the 3D printing drill guide template from September 2012 to May 2015.There were 7 males and 3 females,with a mean age of 52.6 years (range,23-75 years).All patients complained of neck pain,stiffness and decreased range of motion without neurologic deficit.Preoperative skull traction was used routinely.After the three-dimension reconstruction of cervical vertebrae,ideal trajectory for C1 pedicle screws was designed with a complementary basal template for posterior surface of atlas corresponding anatomical structure.Then the drill guide template was materialized in a rapid prototyping machine and used during operation.Start point and direction of the ideal and actual trajectories were measured after matching the position of the pre-and post-operative patients' cervical spine.Safety of pedicle screw fixation was assessed in the transverse and sagittal planes of CT scan.Operation time and blood loss were recorded.Visual Analogue Scale (VAS) of neck pain was recorded before operation and 3 months after operation.Clinical efficacy,fracture reduction,stability and surgical complications were reviewed at the follow-up.Results A total of 20 screws were inserted safely.No significant differences existed in deviation of entry point and direction between ideal and actual trajectories (P >0.05).Operation time was 60-90 min (mean,75 min) and intraoperative blood loss was 110-300 ml (mean,160 ml).No spinal cord or vertebral artery injury was noted during operation.All patients were followed up for 12-36 months (mean,20.5 months).VAS was improved from preoperative 7.3 (6.3-9.5) points to 1.4 (0.3-2.5) points 3 months after operation (P < 0.05).All patients had normal range of motion of the cervical spine 3 months after operation.Bony fusion was achieved 6 months after operation.At the follow-up,good cervical alignment was maintained with no instrument failure and C1.2 instability.Conclusion For treatment of unstable atlas fractures,direct posterior C1 pedicle screw fixation assisted with the 3D printing drill guide template can improve the precision of screw placement,reduce complications,and preserve the function of the occipital-atlantoaxial junction.
10.Mutation analysis of type Ⅱ hair keratin gene in a pedigree with monilethrix

Objective To investigate the mutation of type Ⅱ human hair basic keratin (hHb) gene in a family with monilethrix.Methods Scanning electron microscopy was used to observe the structure of hair shafts.With informed consent,blood samples were drawn from affected and unaffected membets in this family,as well as from 50 healthy controls.Genomic DNA was isolated from these samples.The exon 1 and exon 7 of hHb1,hHb3 and hHb6 were amplified by polymerase chain reaction (PCR).All the PCR products were sequenced directly using ABI3730 automated sequencer.DNA sequence alignment was carried out with BLAST software.Results A typical beaded appearance was observed in affected hairs by using scanning electron microscopy.There were obvious longitudinal ridges and sulcuses in hair node.and hair cuticles were irregularly shaped.Most cortex and medullary substance were absent in affected hairs of a patient.After sequence alignment,a G1289A point mutation in exon 7 of hHb6 gene,which led to a substitution of arginine for glutamide at codon 430,was detected in affected members of this family,but not in unaffected family members or 50 unrelated human controls.No mutation was observed in exon 1 or exon 7 of hHb1 and hHb3 gene or exon 1 of hHb6 gene.Conclusion The missense mutation of R430Q is a novel mutation.which may be associated with the pathogenesis of monilethrix in this pedigree.
