1.Interventional Treatment of Hepatic Cavernous Hemangioma in 32 Cases

Hepatic cavernous hemangioma of 32 cases had been undergone embolization by transfemorel catheterization with lipiodol emulision and lipiodol-gelfoam particles.All of the patients showed Lipiodol accumulation and fibrosis in tumor sites and the feeding arteries were blocked,with diminution in size of the masses.The relationships between image features and tumor fibrosis were dis cussed.
2.Comparative analysis of ultrasonic evidences and serological findings of schistosomiasis liver fibrosis

Objective To explore the relationship between the liver fibrosis scales of B-ultrasound and the serological indicators of schistosomiasis liver fibrosis for guiding clinical diagnosis and treatment. Methods A total of 206 schistosomiasis patients including the acute, chronic and advanced were classified by ultrasonic evidences, and serological indicators of liver fibrosis were detected. The correlation among them was analyzed statistically. Results Ultrasonic diagnosis was 0 scale for acute group and control group, and Ⅱscale for chronic group, as well as Ⅲ scale for advanced group. Advanced group had the highest level of 4 serum indicators of liver fibrosis, and chronic group ranked the middle level, and acute group had the lowest level. The more advanced the ultrasonic scales of liver fibrosis, the higher level the 4 serum indicators. Conclusions There is a consistency relationship between ultrasonic classifications of liver fibrosis and serological indicators in schistosomiasis patients. It also conforms to clinical diagnosis. When ultrasound is applied in schistosomiasis, serological detection is essential and it could make up for the shortage of ultrasonic findings and give better guidance for clinical diagnosis and treatment.
3.Effect of labels management on of high-risk drugs

Objective To explore the effect of labels management on high-risk drugs. Methods Labels management group was established, to be responsible for use and storage of high-risk drugs. The management methods included taking analysis on adverse nursing events, designing and making labels in drug storage and use, and making a field survey on adverse nursing events before and after the management. Result After manipulation of the labels management, the number of adverse nursing events of drug use fell from 35 to 0. Conclusion Drug storage and use with special, standard labels is effective in preventing nursing adverse events in high risk drugs.
4.Endoscopic papillary balloon dilation in the treatment of common bile duct stones

Objective To explore the indications and efficacy of endoscopic papillary balloon dilation(EPBD) in the management of choledocholithiasis. Methods EPBD combined with a retrieval basket or balloon or both was performed to remove common bile duct stones in 42 cases from April 1999 to August 2001. The length and maximum diameter of the balloon were 5 0cm and 1 2cm respectively. The number of common bile duct stones must be less than 3, and the diameter must be less than or equal to 1 0cm. The balloon was inflated at pressure of (8~12)atm for 2min two times and the corresponding dilated diameter was 1 0cm~1 2cm. Results EPBD was performed successfully in all patients. The common bile duct stones were removed in 40 out of 42 patients, including endoscopic sphincterotomy(EST) in 4 patients, with the success rate being 95 2%(40/42). Stone removal failed in 2 patients. 3 cases had mild pancreatitis postoperatively. No complications such as bleeding or intestinal perforation occurred. Conclusions It is suitable to treat common bile duct stones by EPBD when the number of stones is less than 3 and the diameter is less than or equal to 1.0cm.
5.Application of ultrasound in diagnosis of thyroid nodular disease

Objective To explore the diagnostic value of ultrasound in thyroid nodular disease.Methods Eighty-seven patients with thyroid nodule in hospital from 2010-2011 were taken ultrasound examination,including 23 cases with benign nodules and 64 cases with malignant nodules.Univariate analysis and mutivariate Logistic regression analysis were taken to test the ultrasonic index (internal echo,boundary,size,shape,microcalcifications,and aspect ratio) for discovery of the best diagnostic ultrasonic index to identify benign and malignant nodules.Results The ultrasound in the patients with malignant thyroid nodules showed the hypoecho,unclear border,irregular shape,microcalcifications,low blood supply,the aspect ratio > 1.The ultrasound in the patients with benign thyroid nodules showed high or equal echo,clear border,regular shape,high blood supply,and the aspect ratio≤ 1.The ultrasonic indices between two groups had statistically significant difference (P < 0.05).The shape and microcalcifications were the best ultrasonic diagnostic index to indentify the benign and malignant nodules with an EXP(B) 20.12 and 19.14,respectively.Conclusions The ultrasound is helpful in the diagnosis of thyroid nodular disease and identification of benign and malignant thyroid nodules.The ultrasonic images are the basic data for the clinical diagnosis of thyroid nodules.
6.Effect of propofol preconditioning on endoplasmic reticulum stress induced by hypoxia-reoxygenation in HEPG2 cells

Objective To evaluate the effect of propofol preconditioning on endoplasmic reticulum stress induced by hypoxia-reoxygenation (H/R) in HEPG2 cells.Methods HEPG2 cells were randomly divided into 4 groups using a random number table:control group (group C),propofol group (group P),H/R group and H/R + propofol preconditioning group (group PP).In group C,the cells were cultured routinely for 42 h.In group H/R,after being cultured routinely for 6 h,the cells were exposed to 1% O2 + 5% CO2 + 94% N2 for 12 h followed by 12 h reoxygenation.In group PP,the cells were cultured for 6 h in the culture medium containing propofol 10 μmol/L (final concentration),and then H/R was induced.The cell viability was detected by MTT assay.The expression of immunoglobulin heavy chain-binding protein (BIP),C/EBP homologous protein (CHOP) and activated caspase-3 was determined by Western blot.The expression of BIP,CHOP and caspase-3 mRNA was determined by RT-PCR.Results Compared with group C,the cell viability was significantly decreased,and the expression of BIP,CHOP and activated caspase-3 protein and mRNA was up-regnlated in H/R and PP groups,and no significant changes were found in the parameters mentioned above in group P.Compared with group H/R,the cell viability was significantly increased,and the expression of BIP,CHOP and activated caspase-3 protein and mRNA was down-regulated in PP group.Conclusion Propofol preconditioning can promote the cell proliferation and attenuate H/R injury to HEPG2 cells through inhibiting endoplasmic reticulum stress.
7.Matrix metalloproteinase-9 level and its clinical significance in patients with progressive ischemic stroke

Objective To study the level changes of matrix metalloproteinase-9 (MMP-9)and its clinical significance in patients with progressive ischemic stroke(PIS).Methods 136 cases of acute ischemic stroke including 46 cases of PIS and 90 cases of non-PIS,were studied.56 healthy subjects were selected into control group.MMP-9 level was measured by ELISA at 2,7 and 14 day after admission.The neurologic dysfunction score was observed.Results The MMP-9 level was ( 249.43 ± 63.76)μg/L,( 271.50±72.08 )μg/L,and (183.20 ±66.69)μg/L in PIS group,while it was (158.81±49.18 )μg/L,( 188.67±57.96 )μg/L,and ( 93.86 ±22.16)μg/L in non PIS group and was (88.60±15.93 )μg/L in control group at 2,7 and 14 days of admission.The MMP-9 level of PIS group at 2,7 and 14 day and the level in non PIS group at 2 and 7 day were higher than that of control group(P<0.01 ).The MMP-9 level of PIS group was higher than that of different phrase of PIS group (P<0.01 ).Linear X2 showed that the increase of MMP-9 level would increase the risk of ischemic stroke progression( X2=38.96,P<0.01 ).Conclusion The remarkable increase of MMP-9 in patients with progressive ischemic stroke may be the indepent risk factor of the progression of ischemic stroke.
8.Reparation of human epithelial growth factor receptor 2-targeted polymer ultrasound contrast agents with liquid perfluorocarbons and in vitro experiment

Objective To prepare a novel extravascular human epithelial growth factor receptor 2 (HER2)-targeted polylactic-co-glycolic acid (PLGA)-COOH ultrasound contrast agent with liquid perfluorocarbons(PFH-PLGA-tra),and to observe it's general properties and it's effects on ultrasound imaging in vitro.Methods Polymeric nanocapsules of liquid perfluorocarbons were prepared by the single emulsion technique and conjugated with trastuzumab monoclonal antibody by EDC/NHS.The general characteristics were observed.The conjugation was demonstrated by immunofluorescence and the targeting performance of the agent was checked in human MCF-7 cells line in vitro.The encapsulation of liquid fluorocarbons was detected by heating in vitro.The effects on ultrasound imaging were observed in vitro.Results The mean diameter of agents was (261 ± 28)nm.The targeted nanocapsules were positive in immunofluorescence.In the targeted study,it was shown that a number of targeted nanocapsules conjugated with MCF-7 cells in vitro.Droplet-to-bubble transition experiment in vitro showed that there were no nano/microbubbles formed by heating in control groups at 80℃,while there were a lot of nano/microbubbles appeared in nanocapsules formulations at 80 ℃.The agents presented a good ultrasound imaging in vitro.Conclusions The HER2-targeted polymeric nanocapsules of liquid perfluorocarbons are successfully prepared,which may become a novel extravascular targeted ultrasound contrast agents.
9.Effect of propofol pretreatment on endoplasmic reticulum stress-mediated apoptosis during focal cerebral ischemia-reperfusion in rats

Objective To evaluate the effect of propofol on the endoplasmic reticulum stress-mediated apoptosis during focal cerebral ischemia-reperfusion (I/R) in rats.Methods Eighty adult male Sprague-Dawley rats,weighing 240-280 g,were randomly divided into 4 groups (n =20 each):shame operation group (group S) ; focal cerebral I/R group (group FCIR); propofol pretreatment group (group P); intralipid pretreatment group (group Ⅰ).Focal cerebral I/R was induced by 4 h middle cerebral artery occlusion followed by reperfusion.Propofol was infused at a rate of 12 mg· kg-1 · h-1 starting from 30 min before ischemia until 15 min of ischemia in group P,while intralipid was given instead of propofol in group I.Neurological deficit scores (NDSs) were measured at 6 h of reperfusion in 10 rats chosen from each group and the rats were then sacrificed.Their left brains were removed for determination of brain water content.The left 10 rats were sacrificed and their brains were immediately removed for determination of the expression of C/EBP homologous protein (CHOP),Bcl-2,and activated caspase-3 in the left hippocampi by Western blot.Results Compared with group S,NDSs and brain water content were significantly increased,the expression of CHOP and activated caspase-3 was up-regulated,and the expression of Bcl-2 was down-regulated in group FCIR,NDSs was increased in group P (P < 0.05).Compared with group FCIR,NDSs and brain water content were significantly decreased,the expression of CHOP and activated caspase-3 was down-regulated,and the expression of Bcl-2 was up-regulated in group P,and no significant change was found in each parameter in group Ⅰ (P > 0.05).Conclusion Propofol can reduce focal cerebral I/R injury through inhibition of the endoplasmic reticulum stress-mediated apoptosis in rats.
10.Trans-gastric peritoneoscopy with technique of natural-orifice transluminal endoscopic surgery for diagnosis of tuberculosis peritonitis: a report of 20 cases

Objective To investigate the diagnostic value of trans-gastric peritoneoscopy with technique of natural orifice transluminal endoscopic surgery(NOTES)for tuberculosis peritonitis.Methods Clinical data of 20 patients with tuberculosis peritonitis diagnosed by trans-gastric peritoneoscopy via NOTES were retrospectively analyzed.Results All diagnoses were confirmed by biopsy.The findings of peritoneoscopy were defined as miliary type with miliary nodes scattered in ascites and on peritoneum,adhesive type with thickening of peritoneum and adhesion between peritoneum and intestines,cheese-like type with parietal peritoneal ulcer and cheese-like substances,and mixed type with 2 or 3 of above mentioned types.Positive findings in other laboratory examinations were hemoglobin decrease in 10(50%)patients,blood sedimentation rate increase in 16(80%),C reactive protein increase in 13(65%),CA125 increase in 18(90%),and positive tuberculin test in 9(45%).Abnormal findings were detected by chest X-ray in 8(40%)patients,by abdominal ultrasonography examination in 2(10%),by abdominal CT in 7(35%),and by colonoscopy in 1(5%).No abnormal results were found in all patients in anti-tuberculosis antibody test,ascites bacteria culture and gastroscopy.Conclusion Trans-gastric peritoneoscopy via NOTES with biopsy is effective for diagnosis of tuberculosis peritonitis.
