OBJECTIVE To investigate the effect of plasma trough concentration of venetoclax and its influencing factors in patients with acute myeloid leukemia (AML). METHODS After 5 days of venetoclax administration, venous blood samples were collected from AML patients before the next dose. Plasma trough concentrations of venetoclax were determined using high-performance liquid chromatography-tandem mass spectrometry. Spearman correlation was used to assess the correlations between venetoclax plasma trough concentration and various parameters (including patients’ general information, venetoclax-related indicators, liver function indicators, kidney function indicators, and blood routine indicators). Multiple linear regression analysis was performed to identify independent factors influencing plasma trough concentration of venetoclax. Using efficacy as dependent variable [complete remission (CR)+partial remission (PR) vs. no remission (NR)], univariate and multivariate binary Logistic regression analyses were conducted to identify factors affecting efficacy. The receiver operating characteristic (ROC) curve was used to evaluate the predictive value of venetoclax plasma trough concentration for clinical efficacy (assessed as CR). RESULTS A total of 172 venetoclax plasma trough concentration measurements from 101 patients were included in this study. The median plasma trough concentration of venetoclax was 2.38 (1.18, 3.85) μg/mL; the median sampling time for plasma trough concentration of venetoclax was 10 (7, 15) d; the duration of venetoclax use was (34±12) d. Multiple linear regression analysis showed that alkaline phosphatase ( B =14.65, 95%CI: 5.35-23.95, P =0.002), total bilirubin ( B =-101.71, 95%CI: -197.16 to -6.25, P =0.037), and white blood cell count ( B =-106.84, 95%CI: -187.61 to -26.07, P =0.010) were independent factors influencing plasma trough concentration of venetoclax. Due to patient attrition during treatment, 114 venetoclax plasma trough concentration measurements from 69 patients were included for efficacy evaluation. The results showed that 46 patients (66.7%) achieved CR, 11 patients (15.9%) achieved PR, and 12 patients (17.4%) were NR. Multivariate binary Logistic regression analysis showed that age, hemoglobin, venetoclax plasma trough concentration, hematocrit, and mean corpuscular hemoglobin were independent factors affecting patient efficacy ( P <0.05). ROC curve analysis showed that the cut-off value of plasma trough concentration of venetoclax for predicting patient efficacy (assessed as CR) was 1.68 μg/mL (AUC=0.66, 95%CI: 0.54-0.78, P =0.014). CONCLUSIONS There is considerable inter-individual variability in plasma trough concentration of venetoclax among AML patients. Plasma trough concentration of venetoclax is significantly correlated with alkaline phosphatase, total bilirubin, and white blood cell count. Plasma trough concentration of venetoclax is an independent factor affecting patient’s efficacy, and when the cut-off value for predicting CR is above 1.68 μg/mL, better effects may be achieved.

Objective:To determine if preoperative 68Ga-fibroblast activation protein inhibitor (FAPI)-04 PET/MR could contribute to predicting pathological complete response (pCR) in patients with gastrointestinal cancer undergoing neoadjuvant immunotherapy. Methods:In this retrospective study, 35 patients (23 males, 12 females, age (59.1±7.9) years) with gastrointestinal cancer who underwent 68Ga-FAPI-04 PET/MR after receiving neoadjuvant immunotherapy between February 2021 and January 2024 were enrolled. Clinical data, PET imaging parameters including SUV, peak of SUV normalized by lean body mass (SUL peak), FAPI-positive tumor volume (FTV), and total FAPI-positive lesion burden (TLF), and pathological data were collected and analyzed. Patients were divided into pCR group and non-pCR group, and the independent-sample t test or Mann-Whitney U test was performed to compare those parameters between the 2 groups. ROC curve analysis (Delong test) was performed to evaluate the diagnostic efficiency of each parameter to predict pCR. Results:The overall pCR rate of the neoadjuvant therapy was 40.0%(14/35). In the visual evaluation, 68Ga-FAPI-04 PET was limited in predicting pCR, showing false positivity in 12 patients and false negative in 1 patent. While SUV max( t=2.50, P=0.018), SUL peak( t=3.11, P=0.004), FTV( U=3.00, P=0.030) and TLF( U=2.96, P=0.042) in non-pCR group were all higher than those in pCR group. The predictive efficiency of FTV <1.925cm 3 for pCR was better than the efficiency of PET visual evaluation ( Z=3.61, P<0.001), with the prediction accuracy of 82.86%(29/35). Conclusions:68Ga-FAPI-04 PET/MR may provide an effective clinical tool for guiding further treatment of patients with gastrointestinal cancer undergoing neoadjuvant immunotherapy. The quantitative features derived from 68Ga-FAPI-04 PET appear promising in predicting pCR, which are expected to provide a reference for avoiding surgery.

Objective:To construct a lentiviral interference plasmid targeting collapse response regulatory protein 1(CRMP1)gene,to establish a human neuroblastoma cell line(SH-SY5Y)with stable CRMP1 knockdown, and to investigate its impact on expression of NLRP3 inflammasome protein.Methods:Double-stranded shRNA was designed and synthesized targeting h-CRMP1 mRNA sequence, and cloned into PLKO.1 vector.Recombinant shCRMP1 plasmids were constructed correctly, which was transfected into HEK-293T cells for lentiviral packaging.Obtained lentivirus supernatant was concentrated and then infected into SH-SY5Y cells. The interference effect of shCRMP1 plasmid and protein expressions of NLRP3 inflammasome components in SH-SY5Y cells were detected by Western blot. Results:DNA sequencing results showed that insertion sequences of recombinant interference plasmids pLKO.1-shCRMP1 were consistent with designed sequences, which confirmed successful construction of shCRMP1 lentivirus interfering plasmids and transfected into HEK-293T cells for lentivirus packaging, and protein level of CRMP1 in HEK-293T cells were decreased.SH-SY5Y cells were infected with lentivirus concentrate obtained from packaging and screened with puromycin.Western blot results showed that shCRMP1 recombinant lentiviral plasmids could significantly down-regulate CRMP1 protein expression in SH-SY5Y cells. It was also found that in SH-SY5Y cell line with stable CRMP1 knockdown, inhibition of CRMP1 expression could effectively inhibit NLRP3 inflammasome activation under MPP+induction.Conclusion:pLKO.1-shCRMP1 lentiviral interfering plas-mids have been successfully constructed, and interference with CRMP1 can inhibit activation of NLRP3 inflammasome in MPP+-in-duced SH-SY5Y cells. This study provides guidance for further research on mechanism of CRMP1 in neurodegenerative diseases such as Parkinson's disease.
