1.Hospital Infection Rate of Our Hospital in 2005:Investigation and Analysis

OBJECTIVE To give an overview of our hospital′s actual infection situation and to draw attention to infection control,thus improving the weaknesses. METHODS With a method of transversal section,choosing the day of Jun 14 2005,we investigated all this day′s hospitalized patients. RESULTS The result was that the infection rate was 6.6%,mostly contributed by the lower respiratory infection. CONCLUSIONS The incidence of nosocomial infection is connected with following factors such as hospitalized time,age,surrounding contamination,and the long time usage of antibiotics.
2.Effect of three endophytic fungi on growth of Dendrobium candidum and Anoectochilus roxburghii

4.Neonatal respiratory virus infection

Infection is one of the most serious diseases during the neonatal period and the leading cause of neonatal death.Early virus detection is particularly important in the vulnerable infants hospitalized in modern, family centered NICUs.These units bear increasing risks of virus transmission as they provide kangaroo care, 24 hour visiting policies, and are not only open to parents and siblings, but do also encourage their involvement.Due to the atypical clinical features, susceptibility to mixed viral-bacterial infection, lower positive rate of detection, non-specific treatment options, and causing short-term as well as long-term complications or even death if the infection is severe.Thus, it is very important to recognize respiratory virus infection early to prevent and control its spread in neonatal wards.This article aimed to elaborate the susceptibility factors, common pathogens, detection methods and the management of prevention, and control of neonatal respiratory virus infection.
5.Clinicolpathologic study of 21 cases lymphoblastic lymphoma

ObjectiveTo investigate the clinicopathologic features of lymphoblastic lymphoma (LBL).Methods21 cases of LBL were examined by light microscopy and immunohistochemistry,and analyzed with clinical features.Results21 cases of LBL patients ranging in age from 4 to 86 years old,including 17 cases originated from the lymph nodes, the other 4 were from the nasopharynx, breast, pleura and mediastinum respectively. Histopathologically: lymphoid cells were median sized, showing a diffuse infiltration. The neoplastic cells had fine chromatin, higer nuclei mitoses. The tumors were positive for TdT and CD99.ConculsionPatients especially who have enlarged lymph nodes in adolescents favor the diagnosis of LBL.The tumor cells show a diffuse infiltration,usually with median size,fine chromatin.The positivity for TdT in immunostaining can confirm the diagnosis of LBL.
6.Detection of mutation in mitochondrial 12S and 16S ribosomal RNA genes in aminoglycoside-induced deafness

Objective To identify the possible mutations other than A1555G in mitochondrial 12S and 16S rRNA genes that responsible for aminoglycoside-induced deafness, and to provide the basis for genetic diagnosis for this disease. Methods A total of twenty-seven blood samples were obtained from five families with aminoglycoside-induced deafness for screening the A1555G mutation and other possible mutations by PCR- Alw26 I digestion and sequence analysis. Results All samples examined in four families (A, C, D and E) carried the same homoplasmic A1555G mutation, but no A1555G mutation was found in family B. Sequencing of the DNA samples from this family displayed a rare insertion of "AA" at nucleotide 2227 in 16S rRNA gene. Conclusions Our findings suggested that the 1555 G mutation was not the only genetic defect associated with aminoglycoside-induced deafness since we did not find the A1555G mutation in one family, in which the typical maternal inheritance pattern of the aminoglycoside-induced deafness was seen. It is not enough to identify prospectively patients who are likely to be hypersensitive to aminoglycoside ototoxicity by screening A1555G mutation only. Other possible mutations in mitochondrial DNA that associated with aminoglycoside -induced deafness should be tested also.
7.A Paired Case-Control Study on Risk Factors of Allergic Rhinitis in Adults

Objective To understand the risk factors of allergic rhinitis in adults. Methods Using 1:1 paired matching case-control study design, 100 pairs of adults allergic rhinitis patients and the relevant controls from the E.N.T. department of Renmin Hospital of Peking University were recruited. Cases and controls were interviewed face to face using a designed health questionnaire in which the general social demographic characters, disease history, smoking history, occupation, indoor environmental situation and family genetic history were included. The questionnaires were analyzed by signal and multiple regression model of SPSS software. Results Allergic rhinitis was associated with the pollen allergic history (OR=2.04,95%CI: 1.31-3.20), occupational exposure to dust was a risk factor of allergic rhinitis (OR=1.46, 95%CI:0.83-2.57), the mother allergic rhinitis history could increase the risk of their off-springs suffering from allergic rhinitis(OR=2.05, 95%CI:1.03-4.07) and keeping ventilation could significantly decrease the risk suffering from allergic rhinitis(OR=0.70, 95%CI:0.34-1.18). Conclusion The occupational dust exposure, pollen allergy and mother allergic rhinitis history are related to allergic rhinitis in adults. Keeping ventilation may be a protective factor of allergic rhinitis in adults.
8.A method to optimize the selection of tube current for consistent image noise and dose control in 64-slice spiral helical CT cardiac imaging

Objective To establish and clinically evaluate a method to adjust tube current(mA)individually for obtaining images with consistent noise and optimized radiation dose in 64-slice spiral helical CT coronary angiography.Methods (1)80 patients,as a control group,were underwent CT coronary angiography(CTCA)using 64-muhislice spiral helical CT with fixed mA(650 mA).Two radiologists first assessed image quality of the fixed mA group by scoring from 1 to 5.A minimal dose requirement for meeting clinical diagnosis with the desired noise level(standard deviation SD0)were decided.By analyzing the images of the control group,we also established correlation between the image noises of timing bolus(TB)scans and cardiac(CA)scans and set up a formula to predict the required mA value for obtaining the desired cardiac image noise for individual patient.(2)Subsequently,another group of 80 patients(study group)wero scanned with individually adjusted mA using the established formula and the practicability of this method was then evaluated.The independent sample t test was used to compare the values of the two groups.Results (1)The noise of TB and CTCA scans was linearly correlated with r2=0.994. (2)A cardiac image noise level of 28 HU was found to be adequate for clinical diagnostic purpose based on the image quality analysis.(3)The average noise for the acquired cardiac images was found to be 28.97 HU for the study group with individually adapted mA using the established formula.The standard deviation of the image noisein study group was 1.93,which was much smaller than 5.14 in the fixed mA group,but the average image scores for the study group(4.27±0.68)and fixed group mA(4.03 4-0.93)were not statistically different.(4)In the study group,63 out of 80 patients required tube currents below the standard 650 mA,yielding an average CTDIvol of58.32 mGy,which was about 32.1%dose reduction than that of fixed mA(85.94 mGy).The lowest CTDIvol was 19.27 mGy with 160 mA which was 22.4%of the average CTDIvol in control group.Conclusion Adjusting mA individually based on timing bolus scan noise analysis is an accurate and practical method to obtain consistent image quality for different cardiac pailents and to minimize the radiation dose for individuals compared to the fixed mA.
9.Restless Legs Syndrome:10 Cases Report

Objective To investigate the clinical manifestations,diagnosis and treatment of restless legs syndrome.Methods 10 cases with restless legs syndrome were analyzed retrospectively and the relative literature were reviewed.Results and Conclusion Patients with restless legs syndrome showed predominant paraesthesia in their lower limbs,and it appeared or became serious during resting and relieved or disappeared after active or passive movements.All patients complain of aggravated symptom in the night,which resulted in sleep disturbance.Restless legs syndrome is often misdiagnosed.The diagnosis of restless legs is based on its characteristic clinical presentation.Levodopa and dopamine receptor agonist were effective.
10.Clinical analysis of surgical treatment to 26 cases Orbital Blowout Fracture

Objective To estimate the effection of orbital blowout fracture when Medpor was implanted to restore it. Methods The plastic operations on 15 patients with orbital blowout fracture were reviewed and studied. Results On discharge from the hospital, function and outline form was improved greatly in all cases. Conclusion Early diagnosis, early treatment and early movement training after surgery were emphasized. Medpor is one of the preferred material on reparing orbital blowout fracture.
