1.Predictive value of ventricular wall motion score and left ventricular ejection fraction on prognosis of heart fail in patients with acute myocardial infaction

Objective To explore the prognostic value of ventricular wall motion score (WMS) and left ventricular ejection fraction (LVEF) in patients with acute myocardial infarction (AMI) combined with heart failure (HF) in the recent 12 months Methods We selected hospitalized AMI patients in our department from Jan. 2014 to Nov. 2014. Cardiac ultrasound was performed to detect WMS and LVEF within 48 hours and during the 6?month follow?up period and the occurrence of HF was recorded. Results Totally 127 AMI patients were recruited, including 20 cases combined with HF. The WMS was higher in HF group than those in non?HF group (23.55 ± 3.73 vs 20.11 ± 3.13, P<0.01), while the LVEF in HF group was lower than those in non?HF group (48.77 ± 8.08 vs 56.99 ± 5.17, P<0.01). Multiple logistic regression analysis revealed that WMS and LVEF were both independent predictors of HF in the recent 12 months (P < 0.05). The prediction effect of WMS ROC curve area was 0.81 (P < 0.01) , and LVEF 0.76 (P < 0.05). WMS and LVEF combined assessment area under ROC curve was 0.82 (P < 0.01). Conclusion WMS and LVEF are independent predictors of HF in patients with AMI in the recent 12 months and the combined application of WMS and LVEF can significantly improve the prediction effect.
2.Expression of soluble intercellular adhesion molecule-1 and inflammatory factor in elderly patients with colorectal carcinoma

Objective To investigate the clinical significance of soluble intercellular adhesion molecule‐1 and inflammatory factors (CRP ,IL‐17) in elderly patients with colorectal carcinoma .Methods The expression of sICAM‐1 ,CRP and IL‐17 in 76 ca‐ses of elderly patients and 32 cases of youg patients with colorectal cancer were detected by enzyme linked immunosorbent assay be‐fore and after surgery ,and to analyze its clinical significance correlated with pathological parameters .Meanwhile ,60 cases of healthy were controls .Results The serum levels of IL‐17 and sICAM‐1 were higher in patients with different ages of colorectal cancer than those of the normal control group (P<0 .05) ,and the concentrations of the two group after operation were significantly lower than those of the normal control group (P<0 .05) .The CRP levels of the young group and old group were similar to that of the normal control group (P>0 .05) .The level of CRP before operation in the young group was higher than that in the normal control group (P<0 .05) .The levels of serum IL‐17 and sICAM‐1 were significantly different between the young and the old group(P<0 .05) , while the CRP level was similar in the two groups (P>0 .05) .The serum levels of sICAM‐1 and IL‐17 in colorectal cancer patients were associated with the degree of differentiation ,depth of invasion ,lymph node metastasis and TNM staging (P<0 .05) .The level of CRP was significantly correlated with the depth of invasion ,lymph node metastasis and TNM staging (P<0 .05) .Conclusion The serum levels of sICAM‐1 ,CRP and IL‐17 reflect the invasion and metastasis of colorectal cancer in a certain extent ,which play important roles in predicting the development and prognosis of colorectal cancer .
3.Preoperative prediction of risk groups for medulloblastoma in children with MRI features

Objective:To analyze the MRI features of medulloblastoma (MB) in children, and screen out the key signs that can predict the risk of MB before surgery.Methods:Clinical and radiological data of 62 children with MB confirmed by pathology in Shenzhen Children′s Hospital from December 2012 to December 2021 were retrospectively analyzed. According to the diagnosis and treatment guidelines for children with MB (2021 edition), the patients were divided into standard risk group (43 cases) and high risk group (19 cases). MRI features of MB were observed and recorded, including tumor site, location of tumor center, tumor morphology, signal intensity of T 1WI, T 2WI and diffusion weighted imaging (DWI), enhancement pattern, cystic lesion size, location and number, peritumoral edema and hydrocephalus, and the maximum diameter of tumor was measured. The χ 2 test or Fisher exact probability method was used to compare the differences in age, gender and MRI signs between the two groups. The t test of two independent samples was used to compare the differences in the maximum diameter of tumors between the two groups. The indicators with statistically significant differences were included in binary logistic regression analysis to obtain independent influencing factors associated with the risk groups. The receiver operation characteristic curve was used to evaluate the diagnostic efficacy. Results:There were significant differences in age ( P=0.026), enhancement pattern ( P=0.018), cystic lesion size ( P=0.005), location ( P=0.011) and number ( P=0.003) between standard risk group and high risk group. There were no significant differences in gender, tumor site, location of tumor center, tumor morphology, signal intensity of T 1WI, T 2WI and DWI, peritumoral edema, hydrocephalus and maximum diameter of tumor between the two groups ( P>0.05). Binary logistic regression results showed the age (OR=0.207, 95%CI 0.040-0.983, P=0.042) and the number of cystic lesions (OR=0.215, 95%CI 0.073-0.630, P=0.005) were the protective factors for MB in high risk group, the enhancement pattern Ⅲ (OR=5.226, 95%CI 1.516-52.920, P=0.048) was the dangerous factor for MB in high risk group. The area under the curve of the combined diagnosis of high risk MB was 0.845 (95%CI 0.741-0.949). Conclusions:The age and MRI signs the pattern of tumor enhancement Ⅲ and the number of cystic lesion can be used to predict the risk grouping of MB preoperatively. When the child is younger and MB enhancement pattern is mainly peripheral enhancement without obvious cystic change, it may indicate high risk MB.
4.Effect of "Internet plus" exercise prescription intervention on upper limb function and quality of life of breast cancer patients at home after surgery

Objective:To explore the effect of "Internet plus" exercise prescription intervention on upper limb dysfunction and quality of life of breast cancer patients at home after surgery, so as to provide reference for health management of breast cancer patients after surgery.Methods:Adopting a prospective randomized controlled trial research method. From November 2021 to January 2023, 124 breast cancer patients in the breast and thyroid surgery department of Xiang′an Hospital Affiliated to Xiamen University were selected for the study. According to the random number table method, they were randomly divided into an intervention group (62 cases) and a control group (62 cases). The control group patients were given routine training, and the intervention group patients received routine training in the first four weeks after operation, and "Internet plus" exercise prescription intervention in the fifth week after operation. The upper limb dysfunction, quality of life before and after the intervention and motor compliance after the intervention between the two groups were compared.Results:A total of 117 patients were ultimately included, and they were all female, with 58 patients in the intervention group aged (51.01 ± 9.77) years old and 59 patients in the control group aged (51.47 ± 9.85) years old. There was no statistically significant difference in upper limb dysfunction and quality of life between the two groups of patients before intervention ( P>0.05). After the intervention, the degree of upper limb dysfunction in the intervention group was (63.55 ± 7.02) points, which were lower than that in the control group (67.13 ± 7.25) points, and the difference was statistically significant ( t = 2.71, P<0.01). After the intervention, the total score of quality of life and the scores of physiological status, social/family status, emotional status, functional status and additional attention of breast cancer patients in the intervention group were (115.27 ± 17.35), (22.65 ± 4.53), (22.79 ± 4.36), (20.96 ± 3.95), (19.56 ± 4.22), (29.31 ± 5.24) points, which were higher than those in the control group (104.28 ± 17.04), (20.57 ± 4.48), (20.85 ± 4.23), (18.75 ± 4.04), (17.18 ± 4.06), (26.93 ± 5.21) points, the differences were statistically significant ( t values were 2.44-3.46, all P<0.05). In terms of exercise compliance of breast cancer patients in the intervention group, the aerobic exercise completion rate was 91.38% (53/58), muscle strength training completion rate was 77.59% (45/58), stretching exercise completion rate was 86.21% (50/58), exercise frequency was (3.96 ± 1.13) times/week, exercise duration was (29.51 ± 7.64) min/time, which was superior to 77.97% (46/59), 57.63% (34/59), 69.49% (41/59), (3.38 ± 0.94) times/week, (23.96 ± 7.33) min/time in the control group, the differences were statistically significant ( χ2 = 4.04, 5.31, 4.73, t = 3.02, 4.01, all P<0.05). Conclusions:"Internet plus" exercise prescription intervention has the characteristics of convenience, intuition and strong operability, which is conducive to improving the upper limb dysfunction, quality of life and exercise compliance of breast cancer patients at home after surgery. It is recommended to be popularized and applied clinically.
5.Application of point-of-care ultrasound in monitoring gastric residual volume in neurosurgical critical patients with enteral nutrition support

Objective To explore the feasibility and guiding value of point-of-care ultrasound (PoCUS) in evaluating gastric residual volume (GRV) in the course of implementing enteral nutrition (EN) in patients with neurological critical illness. Methods Ninety patients with critical neurological diseases necessary for EN were admitted to the Department of Intensive Care Unit (ICU) of Taizhou Hospital of Integrated Traditional Chinese and Western Medicine from March 1, 2017 to September 30, 2018 were selected, and they were randomly divided into a GRV extraction group (control group) and a PoCUS GRV group (study group), 45 patients in each group. Both groups were fed with Ruidai nutrient solution by nasogastric pump at a rate of 18 hours per day to persistently administer the target feeding volume for consecutive 7 days. The differences in tolerance of EN, the incidence of feeding interruption, the daily volume of EN fluid, the ratio of reaching total enteral nutrition (TEN) and the levels of serum albumin (Alb), prealbumin (PA) and prognosis were compared between the two groups. Results The incidences of reflux and wrong aspiration in the study group were significantly lower than those in the control group [8.9% (4/45) vs. 24.4% (11/45) and 4.4% (2/45) vs. 17.8% (8/45), both P < 0.05]. The EN liquid volume, ratio of reaching TEN, Alb and PA levels in the study group were significantly higher than those in the control group [EN liquid volume (mL/d): 944.6±277.1 vs. 783.7±230.5, the ratio of TEN: 86.7% (39/45) vs. 68.9% (31/45), Alb (g/L): 30.6±3.0 vs. 29.4±2.4, PA (g/L): 280.5±31.8 vs. 267.7±28.4, all P < 0.05]. The incidence of diarrhea [15.6% (7/45) vs. 13.3% (6/45)], the incidence of feeding interruption [8.9% (4/45) vs. 13.3% (6/45)], the length of stay in ICU (days: 10.4±6.2 vs. 8.3 ±5.4), the mortality [8.9% (4/45) vs. 13.3% (6/45)] had no significant differences between the two groups (all P > 0.05). Conclusion Evaluating GRV by PoCUS to adjust EN implementation plan can reduce the incidences of reflux and wrong aspiration and increase the EN intake.

7.In vitro inhibitory effects of realgar nanoparticles on MCF- 7 breast cancer stem cells

OBJECTIVE To study in vitro inhibitory effects of realgar nanoparticles on breast cancer stem cells. METHODS Human breast cancer MCF- 7 parent cells were selected as subjects and cultured by serum-free culture to obtain breast cancer stem cells. Using adriamycin (1 mg/L)as positive control ,same concentration of water-processed realgar as reference ,the effects of realgar nanoparticles on the proliferation of MCF- 7 parent cells and stem cells were detected by CCK- 8 method. The effects of realgar nanoparticles on the formation of mammosphere ,the ability of differentiation ,migration and invasion ,the proportion of CD44+/CD24- subgroup in breast cancer stem cells were detected by mammosphere formation and differentiation experiment , scratch experiment ,Transwell invasion experiment and flow cytometry. Western blot assay was used to detect the expression of proteins related to epithelial mesenchymal transformation pathway (E-cadherin and vimentin ) in breast cancer stem cells. RESULTS The survival rates of MCF- 7 parent cells and stem cells (except for breast cancer stem cells in both 1 mg/mL groups )in 1,5,10,40,60,80 mg/L groups of water-processed realgar and realgar nanoparticles were significantly lower than blank control group(P<0.01). The number of mammosphere (>20 stem cells )in 1,2.5,5,10 mg/L groups of water-processed realgar and realgar nanoparticles was significantly lower than blank control group (P<0.01);the volume of mammosphere decreased and the differentiated adherent cells decreased ;the healing rate of wound ,relative invasion rate (except for water-processed realgar 1 mg/L group)and the proportion of CD 44+/CD24- subgroup were significantly lower than blank control group (P<0.01). The expressions of E-cadherin in 2.5,10 mg/L groups of water-processed realgar and realgar nanoparticles was significantly higher than blank control group ,and the expressions of vimentin was significantly lower than those in blank control group (P<0.01). The above effects of realgar nanoparticles were generally better than those of water-processed realgar with the same mass concentration (P< 0.01). CONCLUSIONS Compared with water-processed realgar with the same mass concentration ,realgar nanoparti cles can significantly inhibit the proliferation of breast cancer stem cells, the formulation and differential ability of mammo- sphere,and reduce the proportion of CD 44+/CD24- subgroup. The effect may be associated with the inhibition of migration and invasion of breast cancer stem cells by inhibiting the expression of proteins related to epithelial mesenchymal transformation pathway.
