1.Combined use of RT-PCR and gel electrophoresis to detect expression of transforming growth factor beta1 in mouse lung fibroblasts in vitro

BACKGROUND: As a combination of reverse transcription (RT) and polymerase chain reaction (PCR), RT-PCR has been used to detect gene expression levels in cells and tissues, RNA virus contents in cells and specific gene cloned cDNA sequences.OBJECTIVE: To detect the inhibitory effcet of Stealth siRNAs on the expression of transforming growth factor β1 (TGF-β1).METHODS: There were blank control, empty vector transfection, stealth-48, stealth-166, and stealth-594 groups. Three stealth siRNAs aimed at different sequences in TGF-β1 mRNA were made, and were then transfected into BALB/c mouse lung fibroblasts in vitro. The expressions of TGF-β1 and connective tissue growth factor were detected by RT-PCR.RESULTS AND CONCLUSION: In different time periods, the TGF-β1 expression was differentially depressed by three stealth siRNAs, especially stealth-166. The inhibitory effects varied with time, which could be detective at 48 hours,reached the peak at 72 hours and then began to attenuate at 96 hours. Our findings show that the inhibitory effect of stealth siRNAs on the TGF-β1 expression in mouse lung fibroblasts can be detected by RT-PCR.
2.The strategy for the confliction between teaching and learning in clinical teaching in terms of neurosurgical intensivie care

There are two critical factors that can reflect the quality of teaching in clinical teaching of neurosurgery.The first is the effectiveness of the teachers' teaching methods,the other is the intensity of the students' learning will.With both factors adapted to each other,the teaching quality will be greatly improved.However,in neurosurgical intensive care(NIC) practices,due to the particularity of the NIC,these two factors will lead to a confliction,and result in a negative effect of clinical teaching.This article is based on practical experiences of the author in clinical teaching.In view of this contradiction,brief analysis should be made both for clinical teachers and medical students,and a corresponding solution should be proposed in order to raise the standards of clinical teaching.
3.Development of the state-owned enterprise worker's stressor scale

Objective To develop the state-owned enterprise worker's stressor scale and assess its reliability and validity.Methods Combined qualitative and quantitive methods,the state-owned enterprise worker's stressor scale was developed based deep interviews.Data were collected from 6400 state-owned enterprise workers in China.The initial sample was split randomly into two samples (n 1 =2835 ; n 2 =2834).The first data were used to set for exploratory factor analysis (EFA) and the second data were used for confirmatory factor analysis (CFA).3 subscales (anxiety,depression and somatization) of symptom checklist 90(SCL-90) were used for testing criterion validity.Results The state-owned enterprise worker's stressor scale got 4 factors and 15 items,including job stressor,relationship stressor,development stressor and family stressor.The Cronbach α coefficients of the full scale and its four factors were 0.66 ~ 0.86.The results of CFA showed RMSEA =0.066,NFI =0.97,NNFI =0.97,CFI =0.97,IFI =0.97,GFI =0.95.In addition,stressor and its four factors had significant positive correlations with anxiety,depression and somatization (r =0.29-0.43,P < 0.01).Conclusion The stateowned enterprise worker's stressor scale meets psychometric criteria and is an effective tool.
4.Clinical analysis of severe complications after percutaneous dilational tracheostomy in patients of Neuro-surgery Department

Objective To analyze the clinical characteristics and treatment Methods of severe com?plications after percutaneous tracheotomy( PDT) in neurosurgery patients,and to provide reference for the treat?ment and rescue of these patients. Methods A retrospective analysis of 578 cases after PDT was performed in Neurosurgery Intensive Care Unit ( NSICU ) of Xuanwu Hospital of Capital Medical University from 2005 to 2015. The types of complications,treatment method and prognosis were analyzed. Follow?up was 3 months to 1 year. Results Eighteen cases with severe complications,including bleeding,wound infection,asphyxia caused by tracheal mucosa suffocation,airway stenosis caused by granulation tissue,subcutaneous emphysema and neu?momediastinum,etc. After treatment and active rescue,most patients recovered,no patients die directly associated with surgery. Conclusion PDT plays an important role in the treatment of patients in NSICU,but it is still pos?sible to have serious complications,so physicians must understand and be prepared for complications,and pay more attention to prevent poor prognosis.
5.Research on Optical Parameter along Puncture Path in Spinal Surgery Navigation Based on Near Infrared Spectroscopy.

Accurate placement of pedicle screws is a key factor of spinal surgery. Investigation of a new real-time intra-operative monitoring method is an important area of clinical application research which makes a contribution to planting pedicle screw accurately. Porcine spines were chosen as experimental objects. The changes of reduced scattering coefficient (μ'(s)) along normal puncture path, medial perforation path and lateral perforation path were measured and studied. A conclusion is drawn that there are two distinct peaks throughout the puncture process, appearing at the junction of cancellous bone and cortical bone, at the beginning and at the end, respectively. The reduced scattering coefficient is proved to be a good monitoring factor which can identify whether the screw is about to reach the critical position of the spine puncture. Moreover, the variation provides an important reference for spinal surgical navigation process.

6.Combined use of posterior unilateral open-door laminoplasty and leverage fixation for treatment of multi-segmental cervical spondylotic myelopathy

Objective To evaluate the clinical efficiency of posterior unilateral open-door laminoplasty and leverage titanium plate internal fixation in the treatment of cervical spondylotic myelopathy ( CSM ) with multi-segmental spinal stenosis.Methods Between Mar 2011 and May 2015, 25 patients with multi-segmental CSM with multi-segmental spinal stenosis were treated by posterior unilateral open-door laminoplasty and leverage fixation .There were 16 males and 9 females, whose mean age was 60.6 ±9.9 years during the surgery.The change of clinical symptoms and signs was recorded during follow-up,and they all received X-ray and MRI.In all the patients, the preoperative and postoperative neurological function, the cervical curvature,cervical vertebra tube volume and axial symptoms were measured , recorded and analyzed. There was statistically significant difference (P<0.05) in the mean Japanese Orthopaedic Association (JOA) score, and Visual Analogue Scale ( VAS) .Results All the 25 patients were followed up for more than 6 months ( 6-24 months ) .No symptoms of C5 nerve root were found in our series .According to the JOA score and VSA score ,the neurological functions of each patient were significantly improved .The preoperative JOA score was 10.16 ±1.35 and the improvement rate 61.24%. There was statistically significant difference between the preoperative VSA score and the postoperative one (6.68 ±1.12 vs 2.32 ±0.84) ( P<0.05).The preoperative and postoperative meansurement of the spinal vertebrai canal diameter was (9.22 ±2.01) and (15.64 ±2.08) mm, respectively,so there was statistically significant difference (P <0.05), indicating that the cavical spinal canal was increased after operation .Conclusion Leverage titanium plate internal fixation can effectively help maintain the expanded vertebral canal after unilateral open -door laminoplasty ,reduce the incidence of postoperative axial symptoms , and maintain the cervical physical curvature .
7.Surgical and interventional treatment for superior mesenteric artery thrombosis

Objective To observe the operation process implementation and the therapeutic effect of interventional therapy for the superior mesenteric artery thrombosis.Methods 21 cases with superior mesenteric arterial thrombosis who had the diagnosis and clinical treatment in our hospital from January 2005 to December 2013 were retrospectively analyzed.Results Among 21 cases,19 cases had obvious risk factors,and their early symptoms and signs were not consistent.The artery angiography showed that 17 cases had superior mesenteric artery thrombosis,2 cases had ileum arterial thrombosis,2 cases had inferior mesenteric arterial thrombosis.All surgeries were performed successfully.The length of hospital stay was 10 14 days with an average of 11 days.After interventional treatment,the symptom of abdominal pain had obvious remission.The follow-up period was 12 to 36 months and no recurrence was found.Conclusions The diagnosis of mesenteric arterial thrombosis is difficult.Mesenteric arterial thrombosis needs the early diagnosis and timely interventional treatment.

After the antidepressant patents were retrieved from Innography, their distribution, R& D level, hot spots and core patents were analyzed , which showed that the largest number of antidepressant patents was produced in USA, the medicinal preparations containing organic active ingredients were the hot spots, the technological and economic strengths of Pfizer, Novo and Sanofi-Aventis were relatively tremendous.The law-suited patents, reex-amined patents and patent strength were mined.
9.Prediction of early bacterial infection after liver transplantation by donor complement factor component 7 polymorphism

Objective To investigate the association between donor complement factor component 7 (C7) rs6876739 gene polymorphisms and risk of early bacterial infection following orthotopic liver transplantation (OLT).Methods A total of 113 patients who had undergone OLT in Shanghai General Hospital between July 2007 and January 2011 were included.A single nucleotide polymorphism (SNP),donor C7 rs6876739 was genotyped and analyzed together with clinical data.Results We demortstrated that donor C7 rs6876739 CC genotype had higher risk of early bacterial infection than TT genotype following OLT (55.6% vs.26.5%,P =0.021).The multivariate logistic regression analysis revealed that gender (P =0.018),biliary complications (P =0.018),ICU stay after LT (P<0.001) and donor C7 rs6876739 genotype (P =0.001) were identified as independent factors of early bacterial infection.Conclusion Donor C7 rs6876739 genotype polymorphism is associated with early bacterial infection following OLT and may be a new marker of risk for the development of potentially serious bacterial infection after liver transplantation.
10.Effects and Safety of Metformin Combined with Vildagliptin on the Glycemic Control for Patients with Newly Diagnosed Type 2 Diabetes Mellitus

Objective:To study the effects and safety of metformin combined with vildagliptin on the glycemic control for patients with newly diagnosed type 2 diabetes mellitus.Methods:60 patients with type 2 diabetes mellitus who were treated from February 2015 to April 2016 were selected and divided into the control group and the observation group according to different treatment methods.The control group was treated with routine treatment.The observation group was treated with vildagliptin based on the control group.The blood glucose,glycosylated hemoglobin,two-hour postprandial blood glucose and serum as well as urinal amylase were measured before and after treatment,and the clinical curative effect of the two groups and the levels of interleukin-6,tumor necrosis factor and C-reactive protein were compared.Results:After treatment,the total effective rate of observation group was 90%,which was significantly higher than that of the control group(66.7%,P<0.05).After treatment,the serum interleukin-6,tumor necrosis factor,C-reactive protein and fasting blood glucose,glycosylated hemoglobin and postprandial blood glucose levels were significantly lower than those of the control group[(7.63± 1.12)dvs(8.68± 1.30)d;(7.23± 0.95)d vs(7.89± 1.20)d;(11.14± 1.56)d vs(12.12± 1.89)d];[(12.12± 1.89)d vs(ll.20± 1.34)d;(6.89± 0.96)d vs(8.23± 1.10)d;(1.65± 0.23)d vs(3.65± 0.48)d] (P<0.05).After treatment,the INS level of observation group was significantly lower than that of the control group (P<0.05) and the GLP-1 level was significantly higher than that of the control group (P<0.05).Conclusion:Metformin combined with vildagliptin could effectively control the blood glucose of patients with newly diagnosed type 2 diabetes and enhance the safety.
