China Pharmacy 2015;26(33):4639-4641
OBJECTIVE:To observe the efficacy and safety of amitriptyline combined with carbamazepine in the treatment of postherpetic neuralgia(PHN). METHODS:80 patients with PHN were randomly divided into observation group and control group. Control group was orally given Carbamazepine tablets 100 mg,bid, then increased to 100 mg,tid after 3 d. Observation group was additionally given Amitriptyline tablets 12.5 mg every night before bed,d1~3;then increased to 25 mg after 3 d every night be-fore bed,d4~5;and 12.5 mg every morning from 6th d+25 mg every night before bed,and the dose could be adjusted based on the pain and adverse reactions of 2 groups. The treatment course for 2 groups was 4 weeks. Visual analogue scale score(VAS),sleep-ing time, pain disappearing time and clinical efficacy in 2 groups were compared, and the incidence of adverse reactions were re-corded. RESULTS:The total effective rate in observation group was higher than control group,VAS was lower than control group, sleeping time was longer than control group and pain disappearing time was shorter than control group, the differences were statisti-cally significant(P<0.05). There was significant difference in the incidence of adverse reactions between 2 groups(P>0.05). CON-CLUSIONS:Amitriptyline combined with carbamazepine has obvious efficacy in the treatment of PHN,with good safety.

Objective To explore the correlation between the joint detection of cholinesterase(CHE),alpha-L-fucosidase(AFU) and alpha-fetoprotein(AFP)and primary hepatic cancer(PHC).Methods 56 cases of patients with PHC(PHC group),52 cases of patients with liver cirrhosis(LC group)and 60 cases of healthy individuals(control group)were enrolled in this study, and serum levels of CHE,AFU and AFP were detected and statistically analysed. Results Serum levels of CHE in patients with PHC were negatively correlated with levels of AFU and AFP(correlation coefficient was -0.889 and -0.797 respectively,P <0.05),the ser-um levels of CHE in patients with LC were also negatively correlated with levels of AFU and AFP (correlation coefficient was-0.598 and -0.653 respectively,P <0.05).The positive rates of joint detection of CHE,AFU and AFP in PHC group and LC group were higher than that in the control group, had statistically significant differences(P <0.05 ).Conclusion CHE,AFU and AFP are sensitive indicators of liver lesions, and the joint detection could provide reference value for diagnosis and monitoring pro-gression of PHC.

Objective To evaluate clinical value of denaturing high performance liquid chromatography (DHPLC) used in detecting transforming growth factor beta receptor 3 (TGFBR-3) exons 11 and 12 polymorphism in women with idiopathic premature ovarian failure (POF).Methods From Feb.2009 to Dec.2011,110 patients with idiopathic POF undergoing treatment at Shenzhen Maternal & Child Health Institute affiliated to Southern Medical University were enrolled as POF group in this study. In the mean time,110 women under 40 years old with normal hormonal level and menstrual cycles as control group. The exons 11 and 12 of TGFBR-3 gene polymorphism were screened by using DHPLC,and results of DNA sequencing was as golden standard. Some related indexes were calculated, such as sensitivity, specificity, false negative value, false positive value,Youden index, positive predictive value, and negative predictive value. At the same time,20% of the tested specimens were chosen randomly and detected by DHPLC again. The value of Kappa index were calculated by comparing the results between the first and second DHPLC analysis. Results The exon 11 of TGFBR-3 were not identified gene polymorphism and two nucleotide polymorphisms were identified in exon 12.For 2022 T/C polymorphism, the frequencies of CC with 0.9% (1/110),TC with 22.7% (25/110),TT with 76.4% (84/110),Cwith12.3% (27/220) and T with 87.7% (193/220) in POF group were significantly different from CC with 0,TC with 9.1% (10/110)and TT with 90.9% (100/110),C with 4.5% (10/220) and T with 95.5% (210/220) in control group (all P < 0.05).Allelic and genotypic frequencies of 2161-75 C/T were not differed significantly between the two groups (all P > 0.05).As DNA sequencing as golden standard,DHPLC showed that the sensitivity was 100%,specificity was 97.9%,Youden index was 97.9%,positive predictive value was 96.3%,negative predictive value was 100%,and Kappa index was 0.888 (P < 0.05).Conclusion DHPLC analysis is higher validity, reliability and practicability method in detecting TGFBR-3 polymorphism in idiopathic premature ovarian failure.

Objective To raise the awareness of adrenal Castleman′s disease by analyzing the clinical features and management of a patient with adrenal Castleman′s disease. Methods A case of adrenal Castleman′s disease of our hospital was retrospectively analyzed, including clinical feature, laboratory findings, pathology, treatment, and follow-up. All the data and pertinent literatures were reviewed and analyzed. Results An incidentaloma measuring 4. 8 cm × 6. 3 cm in the right adrenal gland was observed in a 30-year-old men in a ultrasonography examination performed due to a medical check-up. Laboratory analysis showed that the lesion was not hyperfunctioning. The patient subsequently underwent an exploratory laparotomy. Pathological examination revealed retroperitoneally localized Castleman′s disease of the hyaline vascular type. Conclusion Adrenal Castleman′s disease is a rare cause of lymph node hypertrophy, and it is necessary to keep in mind the possibility of its occurrence and take it into consideration in the differential diagnosis of any solitary, heterogeneous, and hypervascular retroperitoneal mass. The proper cooperation between the clinician and pathologist allows early diagnosis and suitable therapy.

Objective To investigate the expression of Vacuolar-H +-ATPase (V-ATPase) and P-glyeoprotein (P-gp) protein in colon carcinoma tissues, the correlation between the expression of V-ATPase and P-gp and their clinicopathological significance. Methods In samples from 80 cases of colon cancer,20 cases of colon adenoma and 10 cases of normal colonic mucosa tissues, the expression of V-ATPase and P-gp protein were detected by immunohistochemical method, their relationship was analyzed, the clinicopathological features and prognosis were evaluated. Results In colon cancer,V-ATPase and P-gp protein expression was 72% and 80%,higher than that in colon adenomas (40%,35%),and in normal colon mucosa (20%,20%),the difference was statistically significant (P < 0.05).There was a positive correlation between the expression of V-ATPase and that of P-gp (r =0.567,P <0.01).V-ATPase and P-gp protein expression in colon cancer was associated with TNM stage, lymph node metastasis and tumor differentiation (P < 0.05).Patients with high V-ATPase expression had lower 5-year survival rate than those with low V-ATPase expression (P =0.023),and 5-year recurrence rate was higher than those with low expression (P =0.024).Conclusions The expression of V-ATPase is up-regulated in colon cancer, there is a positive correlation with colon cancer progress and metastasis, and high V-ATPase protein expression predicts poor prognosis.
