OBJECTIVE:To observe the efficacy and safety of amitriptyline combined with carbamazepine in the treatment of postherpetic neuralgia(PHN). METHODS:80 patients with PHN were randomly divided into observation group and control group. Control group was orally given Carbamazepine tablets 100 mg,bid, then increased to 100 mg,tid after 3 d. Observation group was additionally given Amitriptyline tablets 12.5 mg every night before bed,d1~3;then increased to 25 mg after 3 d every night be-fore bed,d4~5;and 12.5 mg every morning from 6th d+25 mg every night before bed,and the dose could be adjusted based on the pain and adverse reactions of 2 groups. The treatment course for 2 groups was 4 weeks. Visual analogue scale score(VAS),sleep-ing time, pain disappearing time and clinical efficacy in 2 groups were compared, and the incidence of adverse reactions were re-corded. RESULTS:The total effective rate in observation group was higher than control group,VAS was lower than control group, sleeping time was longer than control group and pain disappearing time was shorter than control group, the differences were statisti-cally significant(P<0.05). There was significant difference in the incidence of adverse reactions between 2 groups(P>0.05). CON-CLUSIONS:Amitriptyline combined with carbamazepine has obvious efficacy in the treatment of PHN,with good safety.

Objective To investigate the association between tumor necrosis factor ? (TNF ?) gene polymorphism and ankylosing spondylitis (AS).Methods Genomic DNA from 98 Chinese AS patients and 70 ethnically matched controls were typed for TNF(308) polymorphism by allele specific polymerase chain reaction (AS PCR).Results The TNF genotypes in AS patients were respectively TNF1 homozygote 37%,TNF2 homozygote 10% and TNF1 and TNF2 heterozygote 53%.While TNF genotypes in controls group were respectively TNF1 homozygote 67%,TNF2 homozygote 3% and TNF1 and TNF2 heterozygote 30%.Significant difference was found in the distribution of TNF 308 genotype between both groups ( ? 2=15 73, P

Objective To explore the correlation between the joint detection of cholinesterase(CHE),alpha-L-fucosidase(AFU) and alpha-fetoprotein(AFP)and primary hepatic cancer(PHC).Methods 56 cases of patients with PHC(PHC group),52 cases of patients with liver cirrhosis(LC group)and 60 cases of healthy individuals(control group)were enrolled in this study, and serum levels of CHE,AFU and AFP were detected and statistically analysed. Results Serum levels of CHE in patients with PHC were negatively correlated with levels of AFU and AFP(correlation coefficient was -0.889 and -0.797 respectively,P <0.05),the ser-um levels of CHE in patients with LC were also negatively correlated with levels of AFU and AFP (correlation coefficient was-0.598 and -0.653 respectively,P <0.05).The positive rates of joint detection of CHE,AFU and AFP in PHC group and LC group were higher than that in the control group, had statistically significant differences(P <0.05 ).Conclusion CHE,AFU and AFP are sensitive indicators of liver lesions, and the joint detection could provide reference value for diagnosis and monitoring pro-gression of PHC.

Objective To apply continuous subcutaneous pulse infusion of gonadotropin-releasing hormone (GnRH) to treat patients with central secondary amenorrhea, and to observe the induction of spontaneous ovulation and natural fertilization. Methods Using micro-pulse infusion pumps with each 90-minute infusion of LHRH 10 μg daily, the serum LH,FSH,estrogen, and progesterone levels ; and change in endometrium and ovarian size before and after treatment were monitored. The end point is natural pregnancy. Results The levels of serum LH,FSH,and estrogen were obviously increased after 4 weeks of treatment. The natural menstrual cycle was rebuilt after 8 weeks of treatment. The patient was pregnant and the treatment was stopped by 16 weeks. The experience of pregnancy was favourable.40 weeks later, the patient delivered a healthy female infant via caesarean section. Conclusions It was the first time in China that the technology of micro GnRH pump was applied in patients with secondary central amenorrhea. The result demonstrates that this technology can perfectly simulate the physiology of hypothalamic GnRH secretion. At present, more patients are included in this research to confirm the effectiveness.

Objective To raise the awareness of adrenal Castleman′s disease by analyzing the clinical features and management of a patient with adrenal Castleman′s disease. Methods A case of adrenal Castleman′s disease of our hospital was retrospectively analyzed, including clinical feature, laboratory findings, pathology, treatment, and follow-up. All the data and pertinent literatures were reviewed and analyzed. Results An incidentaloma measuring 4. 8 cm × 6. 3 cm in the right adrenal gland was observed in a 30-year-old men in a ultrasonography examination performed due to a medical check-up. Laboratory analysis showed that the lesion was not hyperfunctioning. The patient subsequently underwent an exploratory laparotomy. Pathological examination revealed retroperitoneally localized Castleman′s disease of the hyaline vascular type. Conclusion Adrenal Castleman′s disease is a rare cause of lymph node hypertrophy, and it is necessary to keep in mind the possibility of its occurrence and take it into consideration in the differential diagnosis of any solitary, heterogeneous, and hypervascular retroperitoneal mass. The proper cooperation between the clinician and pathologist allows early diagnosis and suitable therapy.

Objective To investigate the expression of Vacuolar-H +-ATPase (V-ATPase) and P-glyeoprotein (P-gp) protein in colon carcinoma tissues, the correlation between the expression of V-ATPase and P-gp and their clinicopathological significance. Methods In samples from 80 cases of colon cancer,20 cases of colon adenoma and 10 cases of normal colonic mucosa tissues, the expression of V-ATPase and P-gp protein were detected by immunohistochemical method, their relationship was analyzed, the clinicopathological features and prognosis were evaluated. Results In colon cancer,V-ATPase and P-gp protein expression was 72% and 80%,higher than that in colon adenomas (40%,35%),and in normal colon mucosa (20%,20%),the difference was statistically significant (P < 0.05).There was a positive correlation between the expression of V-ATPase and that of P-gp (r =0.567,P <0.01).V-ATPase and P-gp protein expression in colon cancer was associated with TNM stage, lymph node metastasis and tumor differentiation (P < 0.05).Patients with high V-ATPase expression had lower 5-year survival rate than those with low V-ATPase expression (P =0.023),and 5-year recurrence rate was higher than those with low expression (P =0.024).Conclusions The expression of V-ATPase is up-regulated in colon cancer, there is a positive correlation with colon cancer progress and metastasis, and high V-ATPase protein expression predicts poor prognosis.

Chinese Journal of Endocrinology and Metabolism 2016;(1):71-74
[Summary] Graves'ophthalmopathy ( GO) is the most common extrathyroidal manifestation of Graves disease ( GD) . It is an organ-specific autoimmune disorder. The estimated incidence of GO in the general population is 16 women and 3 men per 100,000 population every year. Approximately 3%-5% of patients with GO are exposed to the potential of losing their sight and requiring optimized medical intervention. For the clinicians it is helpful to assess the degree of activity and severity of GO;both are important in deciding whether a patient requires intervention and which type of intervention is indicated. These interventions include management of hyperthyroidism, measures to relieve local symptoms, immunosuppressive therapies for active moderate and severe GO, and finally rehabilitative surgery for inactive patients.
