1.Observation on Peripheral Nerve Block in Patients with Coronary Heart Disease after Introcoronary Stent(ICS) undergoing Lower-extremity Surgery

[Purpose]To study the changes of hemodynamic response and the states of myocardial ischemia with peripheral nerve block in patients with coronary heart disease after introcoronary stent(ICS)undergoing lower-extremity surgery in order to guide the clinical application.[Methods]Peripheral nerve block was performed on 32 patients with coronary heart disease after ICS undergoing lower-extremity surgery,the SP,DP and HR were measured before anesthesia,at the beginning of incision,30 min after the incision of surgery and after completion of operation;the states of myocardial ischemia reflected by DCG were monitored before 24h of surgery,2~5h after the incision and 48h after completion of operation.[Results]The SP,DP and HR were decreased more significantly at the beginning of incision,30 min after the incision of surgery and after completion of operation than before anesthesia(P0.05).The frequency of occurences of myocardial ischemia during and after the surgery was increased significantly(P

3.Research and Development of Malaria Vaccine in China

It remains an urgent need to develop effective malaria vaccine for global control of malaria.Application of high technologies such as biotechnology has facilitated the process of vaccine development for malaria.In the past 30 years,a large number of vaccine candidate antigens for malaria have been identified and some of them are currently in clinical trials.Major progress in malaria vaccine development has also been made in China.The PfCP-2.9 blood stage vaccine for malaria has entered clinical studies and some other vaccine candidates including combination malaria vaccine are currently in pre-clinical studies.The availability of various national research programs and international funding has stimulated laboratory and pre-clinical studies of malaria vaccine candidates.It remains a long-term goal to develop a safe and effective malaria vaccine to control and even eliminate the disease in the world,and many issues including malaria immunology and various types of technologies need to be addressed.However,efforts need to be continued toward the goal.
4.Theory of selectivity of RP-LC C18 column and its application

In recent years, more and more researchers focus on the selectivity of RP-HPLC columns. Among these columns, C18 column is the most commonly used the column. Yet even the same filler can not give us the same separation effect with the columns of different brands and different types, therefore, it is important to choose the suitable HPLC column for carrying out an experiment. After study in recent years, some column classification, selection methods (e.g.: column parameter method, F value method) have been achieved and contributed to the finding of suitable columns and reducing the blindness of column selection. This paper summarizes the advantages of these methods and their limitations in the application process, and gives some advices and expectations to pharmaceutical analysis works based on the works of other researchers.
5.Effects of continuous wound infusion of ropivacaine for postoperative analgesia on local inflammation

Objective To investigate the effects of continuous wound infusion of ropivacaine for postoperative analgesia on local inflammation.Methods Forty ASA Ⅰ or Ⅱ patients,aged 45-64 yr,weighing 45-80 kg,undergoing simple nephrectomy,were randomized into either continusous wound infusion of local anesthetic group ( group CWI ) or control group ( group C),with 20 cases each.Group CWI were treated with continuous wound infusion of 0.25% ropivacaine for postoperative analgesia,while group C were infused with the same volume of normal saline.All patients received patient-controlled analgesia with intravenous morphine.At 6,12,24 and 48 h after operation,the accumulated consumption of morphine was documented,and the concentration of prostaglandinE2 (PGE2) of drainage was determined using ELISA.Results Compared with group C,the accumulated consumption of morphine was decreased,and the PGE2 concentration was decreased in group CWI at each time points.Conclusion The mechanism of postoperative analgesic effect of continuous wound infusion of ropivacaine is related to the suppression of local inflammation in wound sites.
6.Tripterygium glycosides combined with Xiangdan injection in HSPN efficacy and influence of the clotting mechanism

Objective To observe effects of tripterygium glycosides combined with Xiangdan injection in henoch schonlein purpura nephritis (HSPN)and influence of the clotting mechanism,providing reference for clinical diagnosis and treatment.Methods 76 cases of children with allergic purpura nephritis were randomly divided into control group(n=38)and observation group(n=38)according to the random number table.Control group was given Xiangdan injection treatment.Observation group was given Xiangdan injection treatment combined with tripterygium glycoside.12 weeks was considered as a course of treatment. Results After treatment,the total efficiency rate in treatment group(94.74%)was significantly higher than control group(68.42%),which was statistically different(P <0.05).Before treatment,prothrombin time(PT),fibrinogen(FIB),activated partial thromboplastin time(APPT),platelet(PLT)levels were not significantly different between two groups;FIB and PLT levels in the control group were significantly lower than before treatment(P<0.05 ),PT and APPT had no statistically significant difference before and after treatment.After treatment in observation group,FIB and PLT levels were significantly lower than before treatment(P<0.05 ),PT level was significantly higher compared with before treatment(P<0.05 ),APTT level had no difference before and after treatment.After treatment,PT in treatment group was significantly higher than the control group(P<0.05 ),PLT in treatment group were significantly lower than the control group (P <0.05 ),FIB and APTT had no statistical difference.There was no adverse reaction in two groups after treatment.Conclusion Tripterygium glycosides combined with Xiangdan injection treatment for HSPN has significant effect,which affect the children clotting mechanism to improve the PT,FIB,PLT levels.It is safe and valuable in clinical research.
7.Changes of parathyroid hormone in primary aldosteronism and its effects

Primary aldosteronism (PA) is a common form of secondary endocrine hypertension,which is characterized by hypertension,hypokelamia,myathenia,elevated serum aldosterone concentration and suppressed plasma renin activity.Besides,accumulating research evidences showed that parathyroid hormone (PTH) level was elevated in patients with primary aldosteronism,accompanied by secondary hyperparathyroidism.This review systemically introduces the interaction between aldosterone and PTH in PA patients.

9.Preparing domain Ⅱ F2 fragment of Plasmodium falciparum 175000 erythrocyte binding antigen and its combined immunization

Objective:To synthesize the eba 175Ⅱf2 gene of Plasmodium falciparum and express it in Pichia pastoris in the secreting form. The recombinant protein and PfCP 2.9 protein were used for combined immunization to see if there is antigen competition. Methods: Asymmetric PCR based metho d was utilized to synthesize the 959bp eba 175Ⅱf2 gene. Plasmid containing the synthetic gene was introduced into Pichi a pa storis by electroporation for inducible expression. The recombinant EBA 175Ⅱ F2 p rotein was purified by ion exchange and gel filtration ch romatography. Results: The eba 175Ⅱf2 gene was successfully ex p ressed in Pichia pastoris in the secreting form. The antibody titers in mice immunized with the combined EBA 175ⅡF2 and PfCP 2.9 proteins w ere much higher tha n those with individual protein by ELISA. Conclusion: No antigen competition is found when using the combined EBA 175ⅡF2 and PfCP 2.9 immunization in mice, indicating the potential of combining the 2 antigens for vaccination.
10.Continuous infusion of paclitaxel combined with cisplatin in the treatment of advanced non-small-cell lung cancer

Background and purpose:The incidence and mortality of lung cancer are markedly increasing all over the world, new drug and new methods in the treatment of lung cancer has been continuously explored and reported. Our study was to evaluate the efficacy and toxicity of continuous infusion of paclitaxel combined with cisplatin in the treatment of advanced non-small-cell lung cancer. Methods:61 patients were randomized into two groups, in the study group (31cases), the patients were treated with continuously infused paclitaxel for 48 hour at day1-2 with total dose 150 mg/m 2 , and cisplatin at day3,4,5 by intravenous injection with total dose 75 mg/m 2 . The patients in the control group(30 cases) received paclitaxel by intravenous injection for at least 3 hours at day 1, 8 with 150 mg/m 2 , and the same dose and administration of cisplatin as study group. More than three cycles were deliveried with three weeks apart to each cycle.Results:The objective response rates(RR) in the study group and control group were 54.84%(17/31)vs 43.33%(13/30),respectively; time to progress (TTP) were 7.1 months in the study group vs 5.8 months in the control group; median survival time (MST) were 14.0 months vs 10.2 months; 1-year survival rates were 67.74%(21/31)vs 33.33%(10/30)(P0.05). The toxicities that occurred in the two group included bone marrow suppression,nausea/vomiting, alopecia, peripheral neurotoxicity,impaired liver function,impaired renal function and phlebitis. There was no significant difference between the two groups in terms of the incidence of toxicites.Conclusions:In the treantment of advanced NSCLC , compared to conventional administration in delivery of the drugs, continuous infusion of paclitaxel can increase response rate and significantly improve 1-year survival fraction without the increase of toxicities.
