1.The changement of serum CRP and the effect of fluvastatin on CRP in the patients with heart failure of CHD

Objective; To observe the changement of serum c - reactive protein (CRP) and the effect of fluvastatin on CRP in the patients with heart failure of coronary heart diease (CHD). Methods ; 78 patients with CHFwere randomly divided into treated group and contol group , which were given usual treatment according to the guide of CHF treatment. Meanwhile, fluvastatin was added to the treated group . Two groups need to detected the TC , CRP and LEVF before and after treatment . Results : After 8 weeks , compared to the control group , the TC , CRP of the treated group decreased in varying degredd , but the LEVF increased obviously . Conclusion ; CRP can forecast dangerous of heart disease . It can be used as a index to prognose the patient with CHF . Fluvastatin can decrease the CRP and improve the prognosis for patient with CHF .
2.Reliability and validity tests of the Stroke Aphasic Depression Questionnaire Hospital Version in patients with poststroke aphasia

Objective To establish a standardized stroke aphasic depression questionnnaire hospital version (SADQ-H) in Chinese and to evaluate its reliability and validity in the application in patients with poststroke aphasia. Methods After acquiring the author's letter of attorney, we revised the English SADQ-H into Chinese version; the iupatients with poststroke aphasia who met the inclusion criteria were used as test group, and the patients without poststroke aphasia were used as control group; after evaluating the Aphasia Battery of Chinese (ABC), the test group was redivided into an auditory comprehension disorder group and a non-auditory comprehension disorder group; and all patients were assessed with SADQ-H by the trained health care workers.The patients in the non-auditory comprehension disorder group were evaluated by the Hamilton depression rating scale (HAMD) arid the Beck Depression Inventory (BDI). SADQ-H was retested in some patients 1 week later. Results The reliability test showed that the retested reliability was 0. 840, and internal consistency reliability Cronbach's α coefficient was 0.78; the validity test showed that the criterion validities were 0.590 (HAMD as criterion) and 0.495 (BDI as criterion). The accumulated variance contribution rate from factors of construct validity was 77. 123%. The discriminant validity was 2.078. Conclusions The SADQ-H in Chinese has favorable reliability and validity, and it can be used as an effective tool in the assessment of depression in patients with poststroke aphasia in China.
3.Meta-analysis of effect of maternal passive smoking during pregnancy on fetal birth-weight

Objective To analyze effects of maternal passive smoking at different exposure level in different period of pregnancy on fetal birth weight and to reveal the influence of different measurement methods on the effects of maternal passive smoking.Methods Meta analysis was applied.NCBI,OVIDC-MEDLINE,CNKI,VIP and CBM were searched,all searched studies were retrieved,and their references were checked for other relevant publications,with the language limited to Chinese and English.The search was finished by April 2008.Pooled mean differences with 95% confidence interval were estimated using data extracted from papers.Results A total of 38 papers were searched,and 19 of them were prospective studies and 19 were retrospective studies,respectively.Maternal passive smoking was associated with a reduction of 67.62 g(95% CI:-90.15~-45.09)in mean birth-weight,and the adjusted reduction was 44.92 g(95% CI:-67.07~-22.77).Based on biochemical markers measurement,the pooled effect size was-73.87 g(95% CI:-113.41~-34.34),based on self-report measurement,the pooled effect size was-62.93 g(95% CI:-84.49~-41.37).The lowest and highest level exposure's effect sizes were-44.61 g(95% CI:-78.36~-10.87)and-116.37 g(95% CI:-180.74~-52.01),respectively.Maternal passive smoking in early pregnancy can reduce the birth-weight by 2.70 g(95% CI:-37.74~32.33),however,it was not statistically significant.Conclusions Maternal passive smoking during pregnancy could reduce infant birth-weight.The middle or late period of pregnancy might be the sensitive period for passive smoking's effect.There is no clear threshold value for passive smoking causing low birth weight.

5.Comparison of effects of the different dosage MK-801 on recognition memory in rats

Objective To observe the behavioral effects of acute administration of dizocilpine (MK-801) in rats,and to determine an appropriate doses of the drug to mimicking memory deficits of schizophrenia.Methods 192 rats were randomly divided into model groups and control group,and given different doses of MK-801 or saline.Locomotor activity,pre-pulse inhibition (PPI) test and novel object recognition test were studied respectively.Results The spontaneous activity increased in a dose-dependent manner after the treatment of MK-801.The total distances of locomotor activity in 0.3 mg/kg group((127.04 ± 32.35) m) exhibited a significant difference compared with control group((35.34 ± 12.81) m,P < 0.05).MK-801 decreased PPI in dose-dependent manner(P < 0.01).MK-801 0.1 mg/kg((103.45 ± 68.04) %) and 0.3 mg/kg ((41.55 ± 62.93) %) groups showed significant differences compared to control group ((200.39 ± 30.97) %) in PPI.The discrimination index of 0.03 mg/kg group((15.78 ± 6.23) %) and 0.1 mg/kg group((22.42 ± 3.85) %) were lower than that in control group((39.42 ±3.86)%,P<0.05).Conclusion It is necessary for select right doses for model different endophenotypes of schizophrenia by MK-801.0.03 mg/kg of MK-801 is a relatively appropriate dosage to cause recognition memory damage.
6.Preparation and Quality Control of Compound Gentamycin Sulphate Nasal Drops

OBJECTIVE: To determine the gentamycin sulphate,ephedrine hydrochloride and dexamethasone sodium phosphate in nasal drops without need of separation.METHODS: Gentamycin was determined via the dihydrolutidine derivatives produced by Hantzsh reaction using UV-spectrophotometry.The detecting wavelengths was 330nm.A dual-wavelength spectrophotometry was used for determination of ephedrine hydrochloride and dexamethasone sodium phosphate.The detecting wavelengths were 256.5nm and 241.6nm and the reference wavelengths were 228.4nm and 266.4nm for ephedrine hydrochloride and dexamethasone sodium phosphate,respectively.RESULTS: The average recovery rates of gentamycin sulphate,ephedrine hydrochloride and dexamethasone sodium phosphate were 100.74% (CV=0.2% ,r=0.9 999,n=5),100.15% (CV=0.66% ,r=0.9 997,n=5)and 99.46% (CV=0.35,r=0.9 996,n=5)respectively.CONCLUSION: This method is simple,rapid,accurate and stable and suitable for rapid quality control of compound gentamycin sulphate nasal drops.
7.The characteristics of post-stroke depression in patients with aphasia

Objective To compare the characteristics of post-stroke depression in patients with and without aphasia.Methods Seventy patients on the first infarction with aphasia and 70 stroke patients without aphasia were recruited.The aphasia deficits in patients were evaluated by using the Aphasia Battery of Chinese ( Aphasia Battery of Chinese, ABC) .The Stroke Aphasic Depression Questionnaire Hospital Version ( Stroke Aphasic Depression Questionnaire Hospital Version, SADQ-H) was applied to analyze the depression in the two groups.Results The sore in SADQ-H of the aphasia group was significantly higher than the control group( (22.03 ±9.55 )vs ( 16.81 ± 10.47 ), P < 0.01 ) .Loss interest, anhedonia, social avoidance, irritability, depression, decreased attention were more serious in patients with aphasia after stroke than that without aphasia.Compared with control group ,the incidence of depression in aphasia group was higher(64.28% vs 50.00% ).The incidence in different types of aphasia: the complete aphasia ( 78.26% ), transcortical mixed aphasia ( 62.50% ), Broca ( 61.53% ),Werincke (62.50%).There was significantly correlation between the incidence of depression and the severity of aphasia.Conclusion The incidence of depression in stroke patients with aphasia is high,especially who have complete aphasia, transcortical mixed aphasia, Broca and Werincke, and it closely relate to the severity of aphasia.
8.Validation of Stroke Aphasic Depression Questionnaire Hospital Version: comparison with HAMD-17 and BDI-13

Objective To preliminarily investigate the cutoff value of Stroke Aphasic Depression Questionnaire Hospital Version (SADQ-H) in the diagnosis of post-stroke depression. Methods Using receiver operating characteristic (ROC) curves, the total scores of 17-item Hamilton Depression Rating Scale (HAMD-17) greater than 7 points and Beck Depression Inventory (BDI-13) greater than 4 points were taken as the cutoff values for the assessment of depression. SADQ-H for the assessment of the depression classification standard in patients with post-stroke aphasia was investigated preliminarily by HAMD-17 (8-17 were mild depression; 18-24 were moderate depression, and>24 was severe depression). Results When HAMD-17 was used as a standard, ROC area under curve (AUC) was 0. 909 (95% confidence interval [CI] 0.814-1.005). When 19.50 was used as a cutoff value, the sensitivity, specificity, positive predictive value, negative predictive value, and κ coefficient of SADQ-H were 82. 6%,91.7%, 87. 0%, 83.3%, and 0. 77, respectively. When BDI-13 was used as a standard, ROC AUC was 0. 916 (95% CI 0. 824-1. 008). When choosing 18.50 as a cutoff value of diagnosis,the sensitivity, specificity, positive predictive value, negative predictive value, and κ coefficient of SADQ-H were 80. 0%, 90. 0%, 84. 0%, 90. 0%, and 0. 68, respectively. The classification standards of SADQ-H for the assessment of depression in patients with post-stroke aphasia were 19, 22 and 26 points. Conclusions When the cutoff value of SADQ-H was 19 points, it had higher sensitivity, specificity, positive predictive value and negative predictive value for the assessment of depression; 19, 22 and 26 points could be used as the classification diagnostic standard of SADQ-H in the assessment of depression in patients with post-stroke aphasia.
9.Treatment of recurrent sinonasal melanoma

OBJECTIVE To study the causes of recurrence and treatment methods in patients with recurrent sinonasal melanoma. METHODS From 1993 to 2003, a retrospective analysis of consecutive 11 cases with recurrent sinonasal melanoma was conducted. RESULTS Four out of the 11 cases with recurrent sinonasal melanoma were caused by misdiagnosis, while the other 7 cases recurred after operation with pathological diagnosis. Among the 7 cases, 5 were found recurrences after operation alone, and the other 2 after the operation and radiation therapy. The recurrence lesions located in nasal and paranasal sinus in 7 cases, in orbit, skull base, soft and hard palate in 1 case, in parotid gland and skin in 1 case. In face, orbit and extended skull base in 1 case. Cervical lymph node recurrence developed in 1 patient. The time of first relapse was 10 months after therapy. The 1-, 2-and 3-year survival rates after reoperation were 72.7 %, 18.2 % and 9.1 % respectively. CONCLUSION The reasonable treatment and decrease the misdignosis are the main measures to improve the cure rate of recurrent sinonasal melanoma.
10.Comparison between ultrasound-guided cervical transverse process block and interscalene brachial plexus block in patients undergoing proximal humeral surgeries

Objective To compare effectiveness,performance,onset time and complications between ultrasound-guided cervical transverse process block and interscalene brachial plexus block in patients undergoing proximal humeral surgeries.Methods Sixty patients,27 males and 33 females, aged 18-70 years,scheduled for proximal humeral surgeries were randomly divided into two groups. They were given either cervical transverse process block (group T,n =30)or ultrasound-guided in-terscalene brachial plexus block (group I,n =30).All patients received a total of 8 ml of 0.5% ropiv-acaine.The performance, anesthetic onset time, the side effects and block success rate were evaluated.Results Block procedure was quicker in group T than in group I [(8.73 ±3.1 7)min vs. (14.40±8.21)min,P <0.01].The severity of diaphragmatic paralysis in group T was significantly lower than in group I (P <0.01).The ultrasound-guided cervical transverse process block was more effective than the interscalene brachial plexus block in patients undergoing proximal humeral surgeries (100% vs.80%,P <0.05).Conclusion The ultrasound-guided cervical transverse process block has a higher success rate and fewer incidence of diaphragmatic paralysis than the interscalene brachial plexus block in patients undergoing proximal humeral surgeries.
