2.Changes of plasma levels of catestatin and brain natriuretic peptide in patients with chronic heart fail-ure and their correlation

Objective:To explore changes of plasma levels of catestatin (CST)and brain natriuretic peptide (BNP)in patients with chronic heart failure (CHF)and their correlation.Methods:A total of 60 CHF patients treated in our department of cardiology from Jun 2014 to Dec 2014 were enrolled as CHF group,another 60 healthy subjects un- dergoing physical examination were regarded as normal control group.Enzyme linked immunosorbent assay was used to measure plasma levels of CST and BNP in two groups,then the results were compared.Linear correlation analysis was used to analyze the correlation between plasma levels of CST and BNP.Results:Compared with normal control group,there were significant rise in plasma levels of CST [(0.40±0.15)ng/L vs.(0.88±0.34)ng/L]and BNP [(30.84±12.56)ng/L vs.(678.21±120.35)ng/L]in CHF group,P<0.01 both.Linear correlation analysis indi- cated that plasma CST level was significant positively correlated with BNP level (r=0.38,P<0.05).Conclusion:Catestatin level is significant positively correlated with the most sensitive cardiac function index—brain natriuretic peptide level,suggesting that it could be used as a new biochemical marker diagnosing or excluding heart failure.
3.Related factors and clinical implication of vitamin metabolism in trauma and sepsis patients

Objective: To determine the related factors of vitamin metabolism after trauma and sepsis.Methods: The control group consisted of 20 healthy volunteers and 20 trauma or sepsis patients were selected into trauma-sepsis group.Body weight,body temperature,hct,wbc account and Apache Ⅱ scores were registered and plasma vitamins were measured,and the relationships among them were assessed with linear regression. Results: In these patients,plasma vitamin A,vitamin E and vitamin C were significantly lower than those in healthy volunteers,and plasma vltamin A,E and C were correlated with Apache Ⅱ score obviously. Conclusion: There were vltamin A,E and C deficiencies in trauma and sepsis patients,and the vitamin deficiencies were correlated with the severity of illness.

Vitamin E is an important nutrient to protect the integrity of body structure, metabolized mainly in liver. Recommended dietary allowance of vitamin E is 10 mg/d. As an important antioxidant, vitamin E can inhibit aggregation of blood platelet, prevent DNA damage and enhance immune function. As present, vitamin E is widely used for treating arteriosclerosis, preventing a few kinds of cancer and supporting treatment of trauma and infection.
5.Reaction between antineoplastic nitidine chloride and sodium hydroxide solution

Objective: To challenge the previously proposed anticancer mechanism of nitidine chloride, which suggest there exist anti tumor C(6)=N(5) double bond structure. Methods: An antitumor alkaloid, nitidine chloride, was treated in aqueous sodium hydroxide solution without the presence of certain kinds of amino acids and oxygen. Results and Conclusion: It had been revealed that virtually equal amount of 5, 6 dihydronitidine and oxynitidine was produced under such circumstances. The results suggest the anticancer mechanism of nitidine chloride need further investigation. [
6.Effects of vitamins A, E, and C with total parenteral nutrition (TPN) on antioxidation in peritoneal sepsis

Objective To study the effects of vitamins A, E, and C with TPN on antioxidation in peritoneal sepsis. Methods The rat model of intraperitoneal infection was established by cecal ligation and puncture (CLP). Health male SD rats were divided into control group (CLP+TPN), treatment group (CLP + TPN + Vitamin A, E, C), and delayed-treatment group ( CLP + TPN + delayed use of Vitamin A, E, C). Levels of the vitamins, MDA, and GSH in plasma and liver were measured. Results Vitamins A, E, C and GSH in the control group were lower than those in the normal control, and MDA was higher. Vitamins A, E, C and GSH in plasma and the liver in the treatment group and delayed-treatment group were higher than those in the control group, but delayed-treatment group was lower than treatment group. MDA in the treatment group and delayed-treatment group was lower than that in control group, but delayed-treatment group was higher than treatment group. Conclusion Vitamins A, E, C with TPN may effectively ameliorate the oxidative injuries in peritoneal sepsis.
7.Ultrasound guided foam sclerosis for the treatment of lower extremity incompetent perforating veins

Objective To discuss the curative effect and clinical significance of sclerosing foam agent in the treatment of chronic venous diseases (CVD) with incompetent perforating veins.Methods 65 cases of C4,C5,C6 CVD (a total of 84 limbs) underwent foam sclerotherapy closure for incompetent perforating vein.Patients were followed-up for 3 months,ultrasonography was used to observe the closure of perforating veins and the curative effect.Results 7 patients (9 limbs) were lost to follow-up,all with grade C4.In the 66 (C4,C5) limbs,pigmentation significantly subsided in 45,partial regression in 9,no change in 2.In the 9 limbs with grade C6,complete healing of ulcer in 5,partially healed in 3.Ultrasound observed satisfactory perforating vein closure.Conclusion Foam sclerotherapy closure for incompetent perforating vein has satisfactory effect,with good short-term curative result.
8.Screening and identification of aptamers against the recombinant transpeptidase domain of PBP 2a

Objective To screen and identify the aptamers of the recombinant transpeptidase domain of PBP2a(penicillin binding protein 2a ,PBP2a) .Methods By using the recombinant transpeptidase domain of PBP2a as the screening target ,oligonucle-otides which were capable of specifically binding to the protein were screened by a random oligonucleotide library through the stem -atic evolution of ligand by exponential enrichment (SELEX )technique .The ssDNA was cloned and sequenced ,and the secondary structure of aptamer clones was predicted with mfold program .Results After 11 cycles of the selection ,the aptamers which were capable of binding to PBP2a with high affinity have been selected .40 clones from the 8 and 10 cycles were selected randomly and se-quenced .The aptamers obtained had no obvious homology according to their sequences by the sequence alignments ,and the 40 aptamers were classified to three groups according to their secondary structures .The aptamer 13 was found to be specific for the target protein with the highest affinity .Conclusion Aptamers for the recombinant transpeptidase domain of PBP2a with high affili-ty and specificity were successfully screened by SELEX ,which lays a foundation for exploring new ways of diagnosis and treatment of MRSA infection .
9.Pathogen profile of bloodstream infections in low birth weight preterm infants:a report of 95 cases

Objective To study the etiology and antibiotic resistance of bloodstream infections in low birth weight preterm infants .Methods A total of 95 cases of bloodstream infections in low birth weight preterm infants were treated in our hospital from January 2011 to April 2014 .The clinical data of these patients were analyzed retrospectively .Results A total of 96 pathogens were isolated ,including 57 strains of gram‐negative bacilli ,38 strains of gram‐positive cocci ,and 1 strains of Trichosporon asahii .The most frequently isolated pathogens were Klebsiella pneumoniae (40 strains)and coagulase‐negative Staphylococcus(31 strains).All gram‐negative bacilli were sensitive to carbapenems such as imipenem and panipenem . Streptococcus isolates were sensitive to most antibiotics .Most Staphylococcus isolates were methicillin‐resistant ,which were highly resistant to common antibiotics but all sensitive to linezolid , vancomycin and teicoplanin . Conclusions The most important pathogens responsible for bloodstream infections in low birth weight preterm infants in our hospital are K lebsiella pneumoniae and coagulase‐negative Staphylococcus . Early identification of responsible pathogen and rational antimicrobial therapy are critical for good prognosis of bloodstream infections in low birth weight preterm infants .
10.The diagnostic value of scoring systems in critical acute pancreatitis: a single center retrospective study

Objective To investigate the early diagnostic value of traditional scoring systems in critical acute pancreatitis.Methods From Jan 2007 to Ju12013,consecutive 184 patients with AP who were admitted to the surgical intensive care unit of the Institute of General Surgery,Nanjing Generai Hospital of Najing Militery Region within 72 h from the onset of the disease were studied.Patients were assigned to four groups,including mild,moderate,severe and critical AP according to the determinant based classification and the receiver operating characteristics(ROC) curve was used to predict the presence of critical AP,and Z test and logistic regression was applied to determine the diagnostic value of traditional scoring systems.Results The area under ROC (AUC) of Sofa,BISAP and CTSI score for early diagnosis of critical AP was 0.896,0.877 and 0.862;the best cut off value was 4.5,9.5 and 2.5.Moreover,the AUC of APACHE Ⅱ and Ranson score was 0.807 and 0.707,and the best cut off value was 8.5 and 3.5,respectively.Z test showed early diagnosis rate of Sofa,BISAP,CTSI score was significantly higher than that in Ranson score,and the difference between the two groups was statistically significant (P < 0.05);though they were higher than APACHE Ⅱ score,the difference between the two groups was not statistically significant (P > O.05).Logistic regression showed that high APACHE Ⅱ,Sofa,BISAP and CTSI score were independent risk factors for the development of critical AP (P < 0.05).Conclusions Sofa,CTSI and BISAP score may be clinically important for early diagnosis of critical AP.
