OBJECTIVE:To provide feasible basis for establishment of new model of pharmaceutical care in community.METHODS:Pharmaceutical care in time sequence of before or during visiting hospital,after medication were surveyed and analyzed,according to the pharmacy practice of the community medical service stations which belonged to our hospital.RESULTS & CONCLUSIONS:New model was established through pharmaceutical propaganda,medication counseling,consummate patients' information,monitoring of medication process,communication of pharmacists and patients,data analysis,etc.The established model of pharmaceutical care in community is feasible and of high performance.
2.Ultrasonic manifestation in basal cell adenoma of salivary gland

Objective To investigate the ultrasonographie characteristics of basal cell adenoma in salivary gland and to improve the coincidence of ultrasound diagnosis for the tumor. Methods The ultrasonographic characteristics for 31 cases of basal cell adenoma which were pathologically confirmed were analyzed and summarized combining with its pathological characteristics retrospectively. A differential diagnosis between basal cell adenoma and pleomorphic adenoma was also made. Results Basal cell adenomas of salivary gland showed low and equivalent internal echoes,sometimes with colliquation in their substantial component. They showed enhanced posterior echo,middle hardness, clear boundary and regular shape. Color Doppler flow signals were also found in the center or periphery of the tumors. Conclusions The ultrasonic manifestation of the basal cell adenoma has its own characteristics,which is complied with its pathological basis. The combination of ultrasonography and clinical information is conducive to differentiate basal cell adenoma and pleomorphic adenoma of salivary gland.
3.Toxic effects of participate matter components in vivo on the NOS/NO system in the endothelial cells from rat thoracic aorta

Objective To investigate the effects of different fine particulate matter (PM2.5) components (water-soluble, organic, water-soluble+organic) in vivo on the NOS/NO system in the endothelial cells from rat thoracic aortas. Methods Three PM2.5 components were used to inject by vein in rat tail. ELISA and immunohistochemical technique were used to examine the change of NOS/NO system in the blood-serum and the endothelial cells from rat thoracic aorta. Results Compared with control group, three PM2.5 components increased NO level of rat blood-serum [control, (38.00±5.40) μmol/L; water-soluble group, (44.66±16.59) μmol/L, t =3.58, P< 0.05; organic groups, (58.28±12.30) μmol/L, t =12.85, P <0.01; mixtural component group, (84.02+19.24) μmol/L, t =31.39, P<0.01]; increased iNOS level of rat blood-serum [control, (17.47±5.33) U/ml; water-soluble group, (21.87±4.63) U/ml, t =4.92, P <0.05; organic groups, (25.27±6.58) U/ml, t = 6.22, P<0.05; Mixtural component group, (32.79±5.86) U/ml, t =32.84, P <0.01]. Mixtural component decreased eNOS level of rat blood-serum [(12.58±4.59) pg/ml vs (17.47±2.97) pg/ml, t =5.91, P <0.05], no significant difference on eNOS level of blood-serum among other component groups (P >0.05). Three components decreased the expression of eNOS protein in the endothelial cells from rat thoracic aorta(control, 164.68±4.80; water-soluble group, 185.43±5.63, t =4.26, P <0.05; organic groups, 194.07±9.77, t =5.26, P <0.05; Mixtural component group, 209.24±15.43, t =43.19, P <0.01), increased the expression of iNOS protein (control, 160.17±5.79; water-soluble group, 134.78±8.22, t =5.21, P <0.05; organic groups, 134.78±8.22, t =5.98, P <0.05; Mixtural component group, 81.62 ±10.59, t =34.98, P <0.01) in the endothelial cells from rat thoracic aortas.Conclusion Three components of PM2.5 induced the dysfunction of endothelial cells and disorder of NOS/NO system in the endothelial cells.
4.Relationship between parathyroid hormone level and central arterial stiffness in patients with high risk of coronary artery disease and preserved renal function

Objective To study the relationship between parathyroid hormone(PTH)level and central arterial stiffness in patients with high risk of coronary artery disease(CAD) and preserved renal function.Methods Seventy-seven patients with at least one risk factor of coronary artery disease (CAD) were divided into increased arterial stiffness group(n=35) and control group(n=42),based on whether the noninvasive pulse wave analysis index of the central pulse pressure(CPP) was higher than 40mmHg or not.Coronary artery angiography was performed in all cases.Serum intact PTH,calcium,phosphor,and highly sensitive C reactive protein (hsCRP) was assessed by radioimmunoassay.Coronary angiography was also performed.Results The patients in increased arterial stiffness group were elder and had higher serum uric acid compared with those in the control group.Forty-four patients were confirmed to have CAD; CAD patients in the stiffness group (n=23) had significantly higher serum iPTH level compared with those in the control group(n=21) (P40 mmHg)after adjustment for age,BMI,hs-CRP,and male gender.Conclusion CAD patients with increased central arterial stiffness and preserved renal function have higher serum PTH level.The serum PTH level is positively correlated with central arterial stiffness,and is the independent risk factor for increased arterial stiffness.
5.Clinical Study of Bacillus Coagulans Tablets Combined with Minus Xiaoyaosan Powder, Nifuratel nystatin Soft Capsule in Treatment of VVC

Objective To observe the effect of Bacillus coagulans tablets Combined with minus Xiaoyao powder, Nifuratel nystatin soft capsule in treatment of VVC. Methods A total of 108 cases of VVC. patients were randomly divided into 2 groups:observation group and control group. In the observation group (n=54), patients were given vaginal placement of Nifuratel nystatin soft capsule, 1 capsule daily, for 6 days;oral administration of minus Xiaoyao powder, 1 agent daily, for 7 days; and oral administration of Bacillus coagulans tablets, 3 tablets one time, 3 times a day, for 14 days, double dose at first. In control group (54 cases), patients were given vaginal placement of Nifuratel nystatin soft capsule, 1 capsule daily, for 6 days;and oral placement of vitamin C tablet, 1 tablet one time, 3 times a day,for 14 days. The short-term effect and the long-term recurrence of were observed 3-7 days, 1 months, 3 months, 6 months after stopping drug treatment. Results 3 days after stopping drugs, the effective rate in the observation group and the control group was 92.59%and 81.48%, respectively, the difference between the two groups was statistically significant (P<0.05) . 6 months after stopping drugs, the recurrence rate was 9.26% in the observation group, and 20.37% in the control group, the difference was statistically significant (P<0.05) . Conclusion For Vaginal candidiasis, on the basis of antifungal therapy, giving Bacillus coagulans tablets for regulation of intestinal micro ecological environment, and traditional Chinese medicine minus Xiaoyao powder for clearing heat and removing dampness, soothing the liver, can significantly improve the short term and long term curative effects,and reduce the recurrence rate.
6.Effect of hypercapnia on lysyl oxidase-dependent collagen cross-linking and hypoxia-induced pulmonary hypertension in rat

AIM: To investigate the effect of hypercapnia on hypoxia-induced pulmonary hypertension and the changes of lysyl oxidase (LOX) and extracellular matrix collagen cross-links in the rat.METHODS: Sprague-Dawley rats were randomly divided into 4 groups: normoxia group, hypoxia group, hypercapnia group and hypoxia+hypercapnia group.LOX activity was detected by fluorescence spectrophotometry.LOX protein expression was detected by immunohistochemistry and Western blot.The mRNA expression of LOX in the pulmonary artery was detected by real-time PCR.RESULTS: The levels of mean pulmonary artery pressure (mPAP), RV/(LV+S) and WA/TA in hypoxia group were significantly higher than those in normoxia group (P<0.01).Moreover, the levels of mPAP and RV/(LV+S) in hypoxia+hypercapnia group were significantly lower than those in hypoxia group (P<0.01).However, no significant difference of mPAP and RV/(LV+S) between hypercapnia group and normoxia group was observed.In hypoxia group, the collagen cross-links in the lung tissue was significantly higher than that in normoxia group and hypercapnia group (P<0.01).Importantly, collagen cross-links in the lung tissue of hypoxia+hypercapnia group was significantly lower than that in hypoxia group (P<0.01).There was no significant difference in collagen cross-links between hypercapnia group and normoxia group.The expression of LOX at mRNA and protein levels and its activity in the pulmonary arteries of hypoxia group were significantly increased as compared with normoxia group (P<0.01).Furthermore, the expression of LOX at mRNA and protein levels and its activity in the pulmonary arteries in hypoxia+hypercapnia group were lower than those in hypoxia group (P<0.01).CONCLUSION: Hypoxia not only up-regulates LOX but also promotes collagen cross-linking in the rat lung, which contributes to the development of pulmonary hypertension.Hypercapnia inhibits hypoxia-induced LOX expression and collagen cross-linking, therefore impairing the progress in hypoxia-induced pulmonary hypertension.
7.Imaging study of maxillary sinus in normal adults using cone-beam CT

[ABSTRACT]OBJECTIVETo estimate different dimensions of maxillary sinus measured on cone-beam computerized tomography (cone-beam CT), and determine the differences in side and gender.METHODScone-beam CT image data from 200 normal adults was selected and analyzed by Invivo Dental software. The craniocaudal diameter, anteroposterior diameter, width and volume of maxillary sinus were measured as well as the thickness of the orbital floor. Statistical analysis was performed with SPSS 11.5.RESULTSThere were no statistically significant differences in the craniocaudal diameter, anteroposterior diameter, width and volume of maxillary sinus between sides (P>0.05, respectively). Significant differences were noted in the craniocaudal diameter and volume of bilateral maxillary sinus and the width of right maxillary sinus based on gender (P<0.05, respectively). There were no significant differences in the width of left maxillary sinus, the anteroposterior diameter of bilateral maxillary sinus and the osseous thickness of the orbital floor (P>0.05, respectively).CONCLUSIONOur findings indicate that cone-beam CT can be used to measure different dimensions of maxillary sinus and provide imaging anatomical data for clinical treatment.
8.A HISTOLOGICAL AND HISTOCHEMICAL STUDY OF SOME PRESERVATING METHODS IN LOCAL ISCHEMIC KIDNEY

A comparative observation about histological and histochemical study were made onthe preservating methods of local ischemic kidney of dogs.The methods of this exp-eriment were divided into five groups:A.Local hypothermia;B.Hypothermic perfu-sion solution 500ml,4℃,containing Procaine 450 mg and Heparin 100 mg,infusedinto renal artery;C.Hypothermic perfusion solution 500ml,4℃,containing 20% Ma-nniton 32ml,25% MgSO_4 0.36 ml,Heparin 100 ml,infused into renal artery;D.Localhypothermia and heparin 30 mg injected into the same vessel.E.Local room tem-perature.The specimens were taken from the kidneys of the experimental dogs atvarious period after treated with every method.The results were as follows;A,D and E group presented irreversible histological changes within 4 hoursaftertreatment.Four hours later,the reaction of SDH,ATPase,AlPase werenegative.It suggested that the kidney damage was serious,as well as expressed thatthe effect of these preservating methods for ischemic kidney are not satisfactory.In B group,the histological and histochemical alteration is very light and almostreversible,during 60 days after treatment.In C group,hypothermic perfusionsolution containing Mannitol,MgSO_4 etc either during experiment or follow observa-tion for 60 days,no histological and histochemical alterations were found.It indica-ted that this method is an ideal perservating technique for ischemic kidney.
9.Adult-onset neuronal intranuclear inclusion disease: a case report and review of literature

The clinical, pathological features and diagnostic methods of one case of adult-onset neuronal intranuclear inclusion disease (NIID) were analyzed. The patient was 61-year-old female presented with progressive cognitive impairment, episodic unconsciousness, stroke-like attack and paroxysmal digestive tract symptoms. Diffusion-weighted images showed high signals at the cerebral cortico-medullary junction with lace-type distribution, which persisted. Skin biopsy revealed intranuclear inclusion bodies in adipocytes, fibroblasts, and sweat gland cells. This case suggests that adult neuronal nuclear inclusion disease is a chronically progressive neurodegenerative disease with a highly clinical heterogeneity. The subcortical lace sign and eosinophilic intranuclear inclusion bodies by skin biopsy contribute to the diagnosis.
10.Drug-eluting beads in the treatment of hepatocellular carcinoma with hepatic arterio-portal shunt

Objective To investigate the safety and efficiency of drug-eluting beads (DEB) in the treatment of hepatocellular carcinoma (HCC) with hepatic arterio-portal shunt (HAPS).Methods 26 HCC patients with HAPS who underwent DEB-TACE (transcatheter arterial chemoembolization) were included in this retrospective study.Liver function level included preoperative serum albumin level,ascites,Child-Pugh classification,imaging manifestations of HAPS,follow-up and record survival time and response to cancer treatment.According to the degree of HAPS,all the patients were divided into 4 groups.After performing Kaplan-Meier,survival rate was calculated.Tumor response was measured by mRECIST criteria.Results The median survivals were 310 days,261 days,333 days,and 250 days in the entire study population,group 1,group 2,and group 3,respectively.There was no statistical significance in the survival curve among three groups (P =0.456).In the entire study population,the survival rates were 76.9%,37.3%,and 12.4% at 6 months,12 months,and 24 months,respectively.After performing proportional hazards model,the preoperative serum albumin level,presence or absence of ascites,and preoperative Child-Pugh classification were independent predict factors for prognosis.Conclusions DEB-TACE is a safe and effective treatment for HCC with HAPS.The preoperative liver function is essential for patients'prognosis.
