1.Fixed Turbinate Resection in the System of Nasal Endoscopic Surgery in Patients with Refractory Nasal Polyps Sinusitis Curative Effect Analysis

Objective To study the fixed nasal endoscopic surgery combined with turbinate resection system of refractory nasal polyps sinusitis patients curative effect.Methods From December 2014 to June 2016,92 patients in our hospital for treatment of refractory nasal polyps sinusitis which were randomly divided into observation group and control group,with 46 cases in each.The patients in the observation group were treated with modified nasal endoscopic surgery combined with turbinate resection treatment system,and control group patients underwent conventional surgical treatment.The effects of two groups were compared.Results Total efficiency of the observation group was significantly higher than that of the control group.Symptoms after treatment in the two groups,SNOT-20 scores were significantly lower thon these in the treatment before,and symptoms after the treatment group,SNOT-20 scores were significantly lower than these of control group.The complication rate and recurrence rate in the observation group were significantly lower than the control group.The differences were statistically significant (P < 0.05).Conclusion The correction of nasal endoscopic surgery combined with turbinate resection system in the treatment of refractory nasal polyps sinusitis effect significantly,can significantly reduce the pain of patients,and promote an early recovery of patients.
2.Detection of Metallo-?-lactamase and Antibiotics Resistance of Imipenem-resistant Pseudomonas aeruginosa in Qingdao Area

OBJECTIVE To find out the imipenem-resistan metallo-?-lactamase in P.aeruginosa to provide the proof of treatment for clinic.METHODS A multi-disk was used to detect the metallo-?-lactamase of P.aeruginosa and the K-B disk method was used for monitoring of the antibiotic-resistance to 15 antibiotics.RESULTS Fifteen strains producing metallo-?-lactamase were isolated from 93 imipenem-resistant P.aeruginosa strains.The positive rate with metallo-?-lactamase was 16.1%(15/93).The susceptibility tests showed the lower resistance was to cefoperazone sulbactam(Sulperazone),amikacin and piperacillinl tazobactam(Tazocin).their resistance rate respectively was 16.8%,24.8% and 26.6%.The resistance rate to ciprofloxacin and ceftazidime was 43.7% and 33.6%.The resistance rate over 90.0% was to ampicillin,ampicilillin/sulbactam,cefazolin,cefpodoxime,cefuroxime and so on.CONCLUSIONS P.aeruginosa with imipenem-resistance shows seriou multidrug-resistance.The metallo-?-lactamase is the main reason for P.aeruginosa resistance to imipenem and cephalosporins.The doctors should choose antibiotics reasonably according to the susceptibility test when taking effective treatment to P.aeruginosa infection.

Objective: To investigate the influence of early enteral nutrition (EEN) and PN in postoperative patients with liver tumour.Methods: 120 patients with liver tumour were randomized into 2 groups:EEN+PN group or TPN group. Results: The preoperative condition of two groups was not significantly different. ALB, PA, TBIL and PT were better in EEN+PN group than those in TPN group. The time of gut movement recovery in EEN+PN group was shorter than in TPN patients. Conclusion: Early enteral nutrition combined with PN can improve nutritional state and digestive function recovery in patients with hepatectomy.
5.STUDY ON THE EFFECT OF MICRONUTRIENTS SUPPLEMENTATION ON NUTRITION AL STATUS OF CHILDREN

Objective:To study the effect of multi-micronutrients supplementation on nutritional status of children. Method:220 students aged 8-12 years were observed for 6 months and 1 year follow-up. A half of them was given the supplements, another half as placebo. Results:In comparison with the placebo, the supplemented children had a higher urine excretion of vitamin B2 and vitamin C after loading; a higher concentration of serum 1.25(OH)2D3, hydroxyproline and hydroxyproline/creatinine ratio in fasting urine, and a higher bone mineral content and bone mass density in mid-ulna. The supplemented children performed significantly better than the placebo in reading speed, learning capacity and arithmetic examinations. After one year there was still significant difference between intervention and control group in hemoglobin content and marginal vitamin A deficiency. Conclusion:The micronutrients supplementation played a notable rule in resulting better nutritional status and learning ability of school children, and intervention would have a long effect on their health and nutritional status.
6.Quantitative Eschar Shaving with Epidermal Sheet Graft for DeepⅡ Burns in Hands

Objective Introducing a technique of quantitative eschar shaving with great sheet of epidermal grafting for deep Ⅱdegree burns in hands.Method 148 hands from 112 patients were treated with this methods.From Jan.2001 to Jane 2004,patients who suffered from deep Ⅱdegree burn injuries was quantitatively eschar shaving using electrical or air power dermatome which was fixed at scale mark from 0.012 inc 、0.016 inc to 0.02inc(from 0.3mm 、0.4mm to 0.5mm) according to the burn injury extents,the wounds were divided into three groups just depends on the injury extents,also the wounds was covered with the different thickness of great sheet of epidermal grafting,which was obtained using the electric or air dermatomes.Results There were 148 hands of 112 cases were treated with this technique within 2h.to 72h after injury.The scar thickness was ranged from 0.4mm to 0.8mm,the function of these hands was good,and the shape of hands looks good.Conclusion Good results was obtained with this methods,the wounds covered with the large sheet of epidermal grafting harvested by the quantitative eschar shaving is recovered uneventful.The thickness of the grafting was according to the increasing skin thickness after burns.
7.Selective blockage of abnormal Wnt pathway inhibits the growth and invasion of hepatocarcinoma cells

Objective To construct the recombinant expression vector encoding antisense Tcf fragment for the blockage of abnormal Wnt pathway, and to investigate its effect on the biological behaviors of human hepatocarcinoma cells. Methods Antisense expression vector was transfected into hepatocarcinoma cells SMMC-7721 with GeneJammer. RT-PCR and Western blot were used to detect Tcf expression. Cell proliferation and motility were compared by growth curves and Transwell plate assay. Cell apoptosis was determined by Annexin V and cell cycle was examined by fluorescent staining. Results The stable transfection of antisense Tcf in SMMC-7721 cells significantly reduced Tcf expression at both mRNA and protein levels. Compared with parental and mock-transfected 7721 (7721-vector) cells, antisense Tcf RNA transfected cells 7721-pTas showed much decreased activities of proliferation, migration and invasion in vitro. Furthermore, the apoptosis rate of 7721-pTas cells [(26.34±2.07)%] was significantly higher than that of 7721-vector cells [(6.53±1.02)%] and parental SMMC-7721 cells [(4.33±0.68)%] (P<0.001). The percentages of G0-G1 phase antisense transfected cells were 20.24% and 20.95%, higher than parental SMMC-7721 and 7721-vector cells, and percentages of S phase antisense transfected cells were 11.8% and 11.38%, lower than parental SMMC-7721 and 7721-vector cells, respectively. Conclusions Antisense RNA suppress the growth ability of liver cancer cells by inducing cell apoptosis and impeding the progress of cell cycle, which suggests that selective blockage of abnormal Wnt signal pathway by antisense Tcf RNA may be a potential new gene therapy for liver cancer.
8.Experiment of skeletal muscle regeneration using autologous fascia as scaffold

BACKGROUND: There is nearly no muscle tissue with satisfactory function and appearance applying in clinical repair and construction of injured muscles to date. OBJECTIVE: To investigate the feasibility of applying autologous fascia as a scaffold to construct muscle in vivo. DESIGN, TIME AND SETTING: The randomized, self-matched control experiment was carried out between January 2004and June 2006 at Department of Burns & Plastic Surgery, Second Hospital of Shenzhen, Shenzhen, Guangdong Province, China MATERIALS: Twenty-eight healthy New Zealand rabbits, weighing (1.7±0.5) kg, without sex restriction, establishing middle part defect model of anterior tibial muscle of rabbit hind legs. METHODS: One hind leg of each rabbit was randomized to the experimental group (n=28), the other hind leg was assigned to one of 3 control groups, scaffold-connected group (n=10), muscle particle implant group (n=10) and blank control group(n=8). In experimental group, the defect was connected with an autologous fascial scaffold and filled with the mutilated muscle particles, and subcutaneous tissue and skin were sutured in situ. In scaffold-connected group, the treatments were same to the experimental group only except muscle particle implantation. In muscle particle implant group, the defect was filled with muscle particles but without fascial scaffold and other treatments were same to the experimental group. The defect in blank control group received no treatment. MAIN OUTCOME MEASURES: The success rate of muscle transplantation, histological and ultra structural observation,and immunohistochemical identification of desmin were observed at 2, 3, 4, 6 and 9 weeks after operation. The middle parts of samples were also harvested for relative quantitative analysis of α-actin cDNA using reverse transcription-polymerase chain reaction in the experimental group and scaffold-connected group.RESULTS: In experimental group, 1 muscle broke near the proximal junction, the other 27 succeeded and the appearance of healed defects became near normal gradually. In scaffold-connected group, 4 muscles broke, 6 muscles still depressed in defect area; in muscle particle implant group and blank control group, the defects had no change. In experimental group, a large quantity of skeletal muscle satellite cells proliferated, which reached peak at 2-3 weeks, cells attached to the ends of fibrous connective tissue; in scaffold-connected group only fibrous connective tissue was seen. lmmunohistochemistry showed that 85% cells in experimental group were desmin-positive, while the positive rate in scaffold-connected group was < 25%. The relative quantitative analysis of α -actin cDNA showed that there were significant differences between the experimental group and scaffold-connected group at different time points(P < 0.05).CONCLUSION: The success rate of repairing muscle defect with autologous fascial scaffold reached 93.33%, which indicates that it is feasible to promote muscle regeneration with autologous fascial scaffold.
9.Lymphoscintigraphy for lateral lymph node drainage in low rectal cancer

ObjectiveTo assess the diagnostic value of preoperative lymphoscintigraphy for lateral lymph node metastasis in low rectal cancer. MethodsFrom May 1999 to March 2001, pelvic and lower abdominal lymphoscintigraphy with 99mTc sulfur colloid was performed on 32 patients with proven colorectal cancer one day prior to operation. Among these patients were 27 rectal cancers, 3 sigmoid cancers and 2 colon cancers. Fifteen cases of rectal cancer with the tumor located at or below the peritoneal reflection received extended lymphadenectomy with lateral dissection (D3 lymphadenectomy). The result of lymphoscintigrams was correlated with histologic lateral node examination. Results The image ratio of pararectal nodes, obturator nodes, and internal iliac aorti lymph trains was 69%, 91%, and 100% respectively. In 15 patients receiving D3 lymphadenectomy, 10 had negative lymphoscintigrams whereas 5 had positive image, and the lateral pathology positive rate was 13%(2/15). Analysis of these results revealed the sensitivity (100%), specificity (77%) and accuracy (80%) of this diagnostic method. ConclusionPelvic and lower abdominal lymphoscintigraphy with 99mTc sulfur colloid could demonstrate the lateral lymph node drainage of low rectal cancer, and the correlation with postoperative histologic lateral node examination was high enough to enable surgeons to adopt an individualized surgical approach.

Objective To discuss clinical curative effects of using Forehead skin expansion combine with auricular cartilage for repairing nose alar full-thickness defects.Methods From August 2010 to August 2010,36 patients with nose alar full-thickness defects in the second affiliated hospital of kunmin medical university,The defect range exceed 1.5 cm× 1.2 cm.50-80 ml expander was implanted in forehead and injected saline water to expand in order to acquire extra skin.We Turn around the skin of defect as the lining of nose,harvest ipsilateral auricular cartilage for nose ala framework,Expanded forehead pedicle flap was transferred to cover framework.The donor area was sutured directly.The pedicle of flap was cut and trimmed after 2 months.Results Follow-up time of 3-18 months after the operation,All flaps are survive,nose alar defects are repaired successfully.Some cases were performed second surgery,postoperative,nose alar color,thickness,nostril size and shape the same with the contralateral side.Donor site healed with linear scar.Conclusions This method could be easy to obtain excess skin,for repairing large sides nose alar full-thickness defect.Frontal scar is not obvious,It is a practical.
