1.The initial analysis of iodine nutrition and thyroid function in pregnant women in Foshan

Objective To investigate the status of iodine nutrition and variations of thyroid hormone levels in pregnant women in Foshan and to provide the science theory on the clinical diet supplement rational .Methods The samples were collected from 442 ca‐ses of pregnant women in Chancheng district center hospital from June 2013 to November 2014 .Free triiodothyronine(FT3) ,free thyroxine(FT4) and thyroid stimulating hormone(TSH) were determined by method of electrochemiluminescence .The urinary io‐dine content in pregnant women was measured by using cold digestion method according to iodine catalytic effect of Arsenic‐Ceri‐um .Results The median urinary iodine in 442 cases of pregnant women was 174μg/L .The percentage of midrange iodine deficien‐cy ,mild iodine deficiency ,iodine sufficiency and iodine overdose in 442 cases of pregnant women were respectively 4 .30% ,29 .86% , 39 .59% and 26 .24% .First trimester ,second trimester ,third trimester with abnormal level of urine iodine was respectively 72 .41% ,45 .89% ,62 .91% .The different gestational age with abnormal level of urine iodine and normal level of urine iodine was significant differences(P<0 .05) .The urine iodine sufficient group of FT3 ,TSH were lower than the urine iodine deficiency groups and the urine iodine excess group ,The FT4 of urine iodine sufficient group compare with iodine deficiency groups was significant differences(P<0 .05) .TSH in urine iodine sufficient group to compared urine iodine excess group ,with significant differences(P<0 .05) .Conclusion The abnormal rate of the level of iodine nutrition and the prevalence of thyroid diseases of pregnant women in FoShan are relatively high .The pregnant women should be screening and intervention in time to raise healthier .

By using ATP-ase histochemical and OKT6 colloidal goldsilver immunohi- stochemical methods, we have studied the location and distribution of Langer- han's cells (LCs) in human esophageal epithelium. The results of our study showed that the dendritic LCs scatter in the basal and middle layers of the esophageal epithelium. The morphology and density of LCs are variable as the age grows and the location changes, more LCs are found in the adult esophageal epithelium than in the foetus, and more LCs are found in the lower segment of esophagus than in the upper segment.

4.Expression of VEGF and Survivin in Nasopharyngeal Carcinoma

Objective To explore the expression of vascular endothelial growth factor(VEGF) and Survivin in nasopharyngeal carcinoma.Methods Immunohistochemical method staining for the paraffin sections(SP method) were used to assess the expression of VECF and Survivin expression in 80 cases of nasopharyngeal carcinoma tissues and 24 cases of nasopharyngeal chronic inflammation tissues.Results The overexpression rate of VEGF protein was 75.76% (61/80) in nasopharyngeal carcinoma tissues,and 25.00% (6/80) in nasopharyngeal chronic inflammation tissues(P<0.01) ;The overexpression rate of Survivin protein was 67.50% (54/80) in nasopharyngeal carcinoma tissues,and 16.67% (4/80) in nasopharyngeal chronic inflammation tissues(P<0.01).The expression of VEGF was related to TNM stage,T stage,lymph node metastasis and distant metastasis (P<0.05) ,and there was a close relation between the expression of Survivin and TNM stage,T stage,lymph node metastasis and distant metastasis (P<0.05).Conclusion Detecting the expression levels of VEGF and Survivin proteins simultaneously in nasopharyngeal carcinoma had certain reference value for judging biological behavior and evaluating prognosis of patients with nasopharyngeal carcinoma.
5.Diagnosis and treatment of Nevin Stage V gallbladder cancer

ObjectiveTo summarize the experience of diagnosis and different surgical management for Nevin Stage V gallbladder cancer. Methods The present study was an retrospective analysis of 74 patients who were operated in our hospital and suffered from Nevin Stage V gallbladder cancer proved histopathologically. ResultsThe diagnostic accuracy of color Doppla ultrasound was 75.0%(21/28), abdominal enhanced CT, 82.1%(32/39).23 patients with Nevin Stage V disease received radical or extended radical cholecystectomy, the mean survival time was 1 year and 3 months;11 patients with Nevin Stage V disease without a complete resection had a mean survival of 6 months; 38 patients with Nevin Stage V diesase had a mean survival of 2 months after palliative bypass procedure or biopsy. ConclusionA exploratory operation by laparoscope is useful in the diagnosis of advanced gallbladder cancer. According to local situation of gallbladder cancer and general body state, different management are employed in treatment so that the effect may be improved.
6.Hand hygiene compliance among health care workers in a hospital

Objective To investigate hand hygiene status of health care workers (HCWs)in a hospital,and pro-vide guides for improving hand hygiene compliance.Methods Hand hygiene intervention was adopted in a hospital between 2012 and 2014,hand hygiene compliance among HCWs and healthcare-associated infection in patients be-tween August 1 and August 31 of each year were investigated and analyzed statistically.Results In 2102 -2014, hand hygiene compliance rate of HCWs was 59.55%,62.13%,and 65.16% respectively,which showed a increased trend (χ2 =10.018,P =0.002),HAI rate was 2.13%,1 .48% and 1 .06% respectively,which showed a decreased trend (χ2 =82.377,P <0.001 );hand hygiene compliance rate of doctors was lower than nurses (57.97% vs 65.97%);Of different hand hygiene moments,hand hygiene compliance rate was the lowest before touching a pa-tient(41 .32%),and highest after body fluid exposure (76.47%).Conclusion Comprehensive hand hygiene inter-vention in this hospital has achieved preliminary results,improvement of HCWs’hand compliance is helpful for pre-venting HAI.
7.Clinical application value of IGRA for screening Tibetan patients with pulmonary tuberculosis

Objective To compare the sensitivity and specificity of interferon-gamma release assay(IGRA) in the Tibetan and Han patients with pulmonary tuberculosis(TB),and to investigate its clinical application value for screening Tibetan patients with pulmonary TB.Methods Sixty-four Tibetan cases of pulmonary TB,41 Tibetan cases of non-pulmonary TB,40 Han cases of pulmonary TB and 38 Han cases of non-pulmonary-TB in our hospital from April 2013 to April 2015 were selected and performed IGRA.T cells infected by mycobacterium tuberculosis were detected by ELISA.The difference between Tibetan abd Han nationalities was compared.The clinical value of IGRAfor screening Tibetan patients with pulmonary TB was evaluated by calculating the statistical indicators.Results The sensitivity,specificity,positive predictive value and negative predictive value of IGRA for diagnosing Tibetan pulmonary TB were 90.62%(58/64),80.49%(33/41),87.88%(58/66) and 84.62%(33/39) respectively,which for diagnosing Han patients with pulmonary TB were 92.50%(37/40),84.21%(32/38),86.05%(37/43) and 91.43%(32/35) respectively.The sensitivity and specificity of IGRA for diagnosing Tibetan and Han pulmonary TB had no statistical difference(χ2=0.005,P=0.944;χ2=0.018,P=0.893).Conclusion IGRA has high value for early diagnosis of Tibetan pulmonaty TB and is conducive to screening of early pulmonary TB.
8.Dosimetry of~(125)I seed interstitial brachytherapy

Objective To calibrate and assess the dose distribution of 125 I seed brachytherapy. Methods Twenty 125 I seeds, each with activity of 12.2 MBq, were implanted on the circumference of a circle 15mm across in a phantom. Into a designed prostate model, 70 125 I seeds were implanted in four planes. The absorbed dose rate of the target volume was monitored by Farmar 2570 dosimeter and thermoluminescence dosimeters (TLD) with the isodose curves drawn on Kodak films. Results The central dose rates of the circular target volume assessed by Farmar 2570 and TLD were 8.4, 7.9 cGy/h in the phantom and 12.0, 11.1 cGy/h in the prostate model. For the target volume of the prostate model, the total absorbed dose was 24?219 cGy. The dose rate 4 cm from the prostate cancer as shown by the isodose curves was only 10% of the central dose rate. Conclusion The central dose rate of target volume measured by the two methods are similar.
9.CT evaluation of colorectal carcinoma correlation in operation and histopathology

Purpose:To assess the value and limitation of CT in staging colorectal carcinoma.Methods:CT was performed prior to surgery in 46 patients with 48 colorectal carcinomas for staging the tumor. The results were compared with those of surgical and pathological examinations in all cases in order to study the sensitivity of staging the tumor before operation.Results:(1) CT depicted 31 of 33 pathologically confirmed T4 lesions (beyond the serosa) in all 48 primary carcinomas,appearance of T4 lesion were defined as either strands of soft tissue or nodules into the peri-colorectal fat ,or hyperdensity of pericolorectal fat. (2)Pathologic examination of lymphatic metastases unfirmed 20 of 40 patients who underwent complete resections, and these lymph nodes were not larger than 1cm in 17 patients.The sensitivity is 70%(14/20)and the specificity 85%(17/20)respectively in detection of lymphatic metastases for CT. (3) The accuracy of CT staging was 81.25%,77.50% in T,N respectively in this group of patients,and CT showed all 4 cases of liver metastases and 6 of 7 peritoneal metastases. Conclusions:CT is accurate in detecting T4 stage of colorectal carcinoma. It is effective in depicting local extension and distant metastases. The value of CT for early T staging and N staging in colorectal carcinoma is limited.
10.Dose rate distribution on cultured cell exposed to ~(125)I seed in vitro irradiation

Objective To study the dose rate distribution on cultured cell plane and establish a reference dose rate table of in vitro cell line ~(125)I seed irradiator.Methods Thermoluminescence dosimetry(TLD) was used to measure the irradiation dose rate of a single 6711 model(~(125) I) seed with apparent activity of 10.323 MBq in water at point P.Meanwhile,the theoretic value of the irradiation dose rate at point P was calculated with theoretic formula.The difference between the calculated and observed values within 10% was set as standard to analyze the accuracy of the measurement.The irradiation dose rate of a single 6711 model(~(125) I) seed was measured in 1mm-thick polystyrene + water medium at point P.The value was applied to differential or non-differential proof along with the value from water medium to study the effect of 1 mm thick polystyrene on distribution of irradiation dose in water.Finally,by simulating the(~(125) I) seed plane irradiator with nine(~(125) I) seeds,the distribution table of irradiation dose rate on the cultured cell plane was calculated with theoretical formula.Results The observed value(n= 10) of irradiation dose rate with one(~(125) I) seed in water at point P was(0.359?0.023)cGy/h and the calculated value was 0.347cGy/h,the difference was within 10%.The observed value(n=10) of irradiation dose of one(~(125) I) seed in 1mm-thick polystyrene + water medium at point P was (0.350?0.027)cGy/h,which showed no statistical difference from the observed value in water under differential and non-differential proof.The reference table on dose rate distribution for cells exposed to(~(125) I) seed irradiation in vitro was developed.Conclusions 1mm-thick polystyrene gives no significant effect on irradiation dose rate distribution from(~(125) I) seeds in water.A reference table on the dose rate distribution for cells exposed to(~(125) I) seed irradiation in vitro has been developed,which can be used to determine an optimal irradiating strategy for future work.
